Abstract:Simple Summary Diffuse large B-cell lymphoma (DLBCL) frequently occurs in HIV-infected patients. However, the effect of HIV infection on the outcome of the DLBCL population remains controversial. The aim of the present retrospective study was to compare clinical features, phenotypic markers and outcomes of BLBCL between HIV-infected and HIV-uninfected Chinese patients. Our study indicated HIV-infected DLBCL patients displayed high EBER expression but low CD20 and CD79a expression on histopathology. The overall response rate at end of chemotherapy and 1-year overall survival (OS) were low in HIV-infected patients, which may be associated with the higher incidence of leukopenia, neutropenia, thrombocytopenia and hypoalbuminemia, as well as high involvement of the central nervous system (CNS), gastrointestinal tract and bone marrow. Hypoalbuminemia and CNS involvement were independent risk factors for 1-year OS. HIV-infected DLBCL patients without CNS involvement had a favorable outcome if rituximab was included in the chemotherapy regimen. Background: The effect of HIV infection on the clinicopathological characteristics of diffuse large B-cell lymphoma (DLBCL) remains debatable. Methods: Fifty-three HIV-infected and ninety-three HIV-uninfected DLBCL patients were enrolled in the retrospective study by propensity score matching for sex, age, body mass index and international prognostic index (IPI) at a ratio of 1:2. The clinicopathological characteristics were compared between the two groups. Results: HIV-infected DLBCL patients had lower white blood cell counts [x10(9)/L; 4.4 (3.4-5.6) vs. 6.1 (4.2-8.2), p < 0.001], platelet counts (x10(9)/L; 184.7 +/- 89.3 vs. 230.0 +/- 113.9, p = 0.014) and serum albumin (g/L; 37.3 +/- 6.9 vs. 41.3 +/- 6.2, p < 0.001) but higher incidences of central nervous system (CNS) involvement (9.4% vs. 1.1%, p = 0.014), bone marrow involvement (24.5% vs. 11.5%, p = 0.044) and Epstein-Barr viremia (61.1% vs. 26.7%, p = 0.002) than HIV-uninfected patients. In terms of histopathology, HIV-infected patients had higher positivity of Epstein-Barr virus-encoded small RNA (EBER) (41.7% vs. 6.7%, p = 0.002), but lower CD20 (90.2% vs. 98.7%, p= 0.029) and CD79a (23.1% vs. 53.7%, p < 0.001) expression. The overall response rate (ORR) at the end of chemotherapy (70.2% vs. 87.8%, p= 0.012) and 1-year overall survival (OS) (61.7% vs. 84.2%, log-rank p = 0.006) in HIV-infected patients were significantly lower than those in HIV-uninfected patients. Multivariate analysis suggested IPI <= 2.0 [adjusted odds ratio (AOR) (95% confidence interval): 5.0 (1.2-21.2), p = 0.030] was associated with ORR, hypoalbuminemia [AOR: 3.3 (1.3-9.1), p = 0.018] and CNS involvement [AOR: 3.3 (1.0-10.5), p = 0.044] were associated with reduced 1-year OS in HIV-infected patients. Conclusion: HIV-infected DLBCL patients have unique blood profiles and phenotypic markers. Low ORR and 1-year OS were observed in HIV-infected DLBCL patients in our study, even in the HAART era.

Abstract:Diffuse large B-cell lymphoma, not otherwise specified (DLBCL NOS) is subdivided according to the cell-of-origin (COO) classification into germinal centre B-cell (GCB) and activated B-cell (ABC) subtypes, each with different molecular profiles and clinical behaviour. This study aims to describe the pattern of the COO subtypes, the proportion of Epstein-Barr virus (EBV) co-infection, and their influence on survival outcomes in a setting of high HIV prevalence. This retrospective cohort study included patients diagnosed with de novo DLBCL NOS at our tertiary academic centre in Cape Town, South Africa over a 14-year period. Immunohistochemical stains were performed for COO classification, according to the Hans algorithm. Tumour EBV co-infection was established by EBV-encoded ribonucleic acid in situ hybridisation (EBER-ISH) staining. The effect of the COO subtypes and EBV co-infection on overall survival were described by means of univariate, bivariate and multivariate analyses. A total of 181 patients with DLBCL NOS were included, which comprised 131 HIV-uninfected and 50 HIV-infected patients. There was an equal distribution of GCB and ABC subtypes in the HIV-infected and HIV-uninfected groups. EBV co-infection was detected in 16% of the HIV-infected cases and in 7% of the HIV-uninfected cases (p=0.09). There was no significant difference in the incidence of EBV co-infection between the GCB and ABC subtypes (p=0.67). HIV-infected patients with CD4 ≥150 cells/mm3 had similar survival to HIV-uninfected patients (p=0.005). Multivariate regression analysis showed that in the HIV-infected group with marked immunosuppression (CD4 <150 cells/mm3), there was significantly poorer overall survival compared to the HIV-uninfected group (HR 2.4, 95% CI 1.3-4.1). There were no statistically significant differences in overall survival by DLBCL COO subtype. There was no difference in the proportion of DLBCL COO subtypes, regardless of HIV status. EBV co-infection was more common in the HIV-infected group, but less than described in the literature. Unexpectedly, there were no significant differences in survival outcomes between the GCB and ABC subtypes. Higher CD4 counts in the HIV-infected group had good survival outcomes, while lower CD4 counts predicted adverse survival outcomes. Further research is needed to explore the genetic mutational landscape of HIV-associated DLBCL.
Long-Term Survival Rates and Treatment Trends of Burkitt Lymphoma in Patients with HIV—A National Cancer Database (NCDB) Study

Abstract:Background: Burkitt lymphoma (BL) accounts for 10–35% of AIDS-defining lymphoma in people with HIV (PWH). Previous research consisting of smaller cohorts has shown decreased survival for HIV-associated BL. This study aims to compare overall mortality in BL patients with and without HIV, while investigating impact of treatment modalities in HIV-associated BL. Methods: Using the 2004–2019 NCDB, we identified 4312 patients with stage 3 or 4 BL who had a known HIV status and received either chemotherapy alone or chemotherapy and immunotherapy. Time to death was evaluated using Kaplan–Meier survival estimates. Risk of death was evaluated using an extended multivariable Cox model adjusted for multiple factors and with a Heaviside function for HIV status by time period (0–3 month vs. 3–60 month). Results: Of the 4312 patients included, 1514 (35%) had HIV. For months 0–3 from time of diagnosis, HIV status was not associated with a statistically significant increase in risk of death (HR = 1.04, 95% CI: 0.86, 1.26, p = 0.6648). From month 3to 60, positive HIV status was associated with a 55% increase in risk of death compared to those without HIV (95% CI: 1.38, 1.75, p < 0.0001). Further, this difference in hazard rates (0–3 vs. 3–60) was statistically significant (HR = 1.49, 95% CI: 1.22–1.82, p < 0.001). Conclusions: There is an increased mortality rate from months 3 to 60 in BL patients with HIV compared to patients without HIV. Additionally, risk of death in the first 3 months is significantly decreased by 45% in patients with HIV treated with combination chemotherapy and immunotherapy compared to patients without HIV receiving combination chemotherapy and immunotherapy, providing valuable clinical insight into treatment decision making in the care of HIV-associated BL.

Abstract:Abstract There are no studies comparing the prognosis for mature T-cell lymphoma (TCL) in people with HIV (PWH) to people without HIV (PWoH) and to AIDS-defining B-cell lymphomas (A-BCLs) in the modern antiretroviral therapy era. North American AIDS Cohort Collaboration on Research and Design and Comprehensive Oncology Measures for Peripheral T-cell Lymphoma Treatment are cohorts that enroll patients diagnosed with HIV and TCL, respectively. In our study, 52, 64, 101, 500, and 246 PWH with histologic confirmation of TCL, primary central nervous system lymphoma, Burkitt’s lymphoma, diffuse large B-cell lymphoma (DLBCL), and Hodgkin’s lymphoma (HL), respectively, and 450 TCLs without HIV were eligible for analysis. At the time of TCL diagnosis, anaplastic large-cell lymphoma (ALCL) was the most common TCL subtype within PWH. Although PWH with TCL diagnosed between 1996 and 2009 experienced a low 5-year survival probability at 0.23 (95% confidence interval [CI]: 0.13, 0.41), we observed a marked improvement in their survival when diagnosed between 2010 and 2016 (0.69; 95% CI: 0.48, 1; P = .04) in contrast to TCLs among PWoH (0.45; 95% CI: 0.41, 0.51; P = .53). Similarly, PWH with ALCLs diagnosed between 1996 and 2009 were associated with a conspicuously inferior 5-year survival probability (0.17; 95% CI: 0.07, 0.42) and consistently lagged behind A-BCL subtypes such as Burkitt’s (0.43; 95% CI:0.33, 0.57; P = .09) and DLBCL (0.17; 95% CI: 0.06, 0.46; P = .11) and behind HL (0.57; 95% CI: 0.50, 0.65; P < .0001). Despite a small number, those diagnosed between 2010 and 2016 experienced a remarkable improvement in survival (0.67; 95% CI: 0.3, 1) in comparison with PWoH (0.76; 95% CI: 0.66, 0.87; P = .58). Thus, our analysis confirms improved overall survival for aggressive B- and T-cell malignancies among PWH in the last decade.

Abstract:International Journal of STD &AIDS, Ahead of Print. ObjectiveEvaluate the clinical features in people with Hodgkin's lymphoma living with HIV (HIV-HL) during the combination ART (cART) era.DesignSystematic review and meta-analysis.MethodsThe study was conducted in accordance with the recommendations of 2020 PRISMA and MOOSE statements. The protocol was prospectively registered through the PROSPERO (CRD42021289520). Manuscripts published until July 2023 were systematically searched in the PubMed, EMBASE, Cochrane Library, and Web of Science databases, with no language and year of publication restriction. Meta-analysis was performed to estimate a pooled proportion of each outcome using a random-effect analysis. Quality assessment was performed by using New-Castle Ottawa scale. Certainty of evidence was graded using the GRADE.ResultsSixteen cohorts, representing 3.882 HIV-HL patients, were included in this review. Our findings indicate that HIV-HL patients showed a 2-year overall survival (OS) of 92% (95% CI 0.87, 0.95). However, the 5-year overall survival decreased to 79% (95% CI 0.74, 0.83), with a high certainty of evidence according to GRADE. Additionally, the 5-year progression-free survival declined to 79% and complete remission rate increased to 81%. Our meta-analysis indicates an increase for B symptoms (80%, 95% CI 0.75, 0.84) and extranodal involvement in bone marrow (43%, 95% CI 0.30, 0.47) among HIV-HL patients.ConclusionThe HIV-HL patients showed a 2-year OS of 92%. However, the 5-year OS decreased to 79%. The reported main cause of mortality among HIV-HL patients was progression of HL. Our systematic review and meta-analysis suggest that cART is associated with improved short-term survival of HIV-HL patients.

Abstract:Background: HIV-associated lymphoma is the most common subtype of HIV-associated cancers, with a very low incidence. To date, there are only few retrospective studies analyzing the clinical features and prognosis of HIV-associated lymphoma. Therefore, we performed a study to analyze the clinical characteristics and outcomes of newly diagnosed HIV-associated lymphoma patients in our center, as the largest HIV-associated lymphoma cohort in China so far. Aims: Herein, we analyzed the clinical characteristics and prognosis of 107 newly diagnosed HIV-associated lymphoma patients, as the largest cohort reported in China to date. Methods: Totally 107 newly diagnosed HIV-associated lymphoma patients in Chongqing University Cancer Hospital, Southwest China, from July 2008 to August 2021, were analyzed. Results: In the 107 HIV-associated lymphoma cases, median age was 49 years (range: 23–87 years), and more patients were male (87.9%). Most patients had elevated lactate dehydrogenase (LDH) (78.5%), elevated β2-MG (91.6%), advanced Ann Arbor stage (73.8%) and high IPI score (IPI score: 3-5) (62.7%) at diagnosis. Totally 46 patients (43%) had one or more sites of extranodal involvement, of whom 24 (22.4%) possessed two or more extranodal sites. Median CD4+ T cell count at diagnosis was 191/μl (range: 4-1022); 84 patients (78.5%) were on combination antiretroviral therapy (cART) at lymphoma diagnosis. Of the 72 patients evaluated at the end of treatment, the overall response rate (ORR) was 81.9% (59/72); 29 (40.3%) cases achieved complete response (CR), 30 (41.7%) had partial response (PR) and 9 (12.5%) experienced progressive disease (PD). In 99 HIV-associated lymphoma cases with complete follow-up data (median follow-up of 44.5 months [range: 2-160 months]), median PFS and OS were 36.0 months and not reached, respectively. The overall 5-year PFS and OS rates were 46.9% and 53.4%, respectively. Compared with individuals not administered anti-lymphoma treatment or administered less than 3 cycles anti-lymphoma therapy, those who received standardized anti-lymphoma therapy (≥3 cycles) showed improved PFS (p=0.0006) and OS (p<0.0001). In patients administered systemic anti-lymphoma therapy, the overall 2-year PFS rates for chemotherapy and R-chemotherapy were 29.5% and 67.1%, respectively (HR=0.44, 95%CI 0.210-0.927; p=0.020). The overall 2-year OS rates for chemotherapy and R-chemotherapy were 51.9% and 73.1%, respectively (HR=0.42, 95%CI 0.183-0.969; p=0.042). Summary/Conclusion: Overall, these data indicated that HIV-associated lymphoma has aggressive characteristics, including advanced Ann Arbor stage, high IPI score, more extranodal involvement and elevated LDH. Survival is significantly improved due to the wide use of cART and chemoimmunotherapy. Standardized anti-lymphoma therapy is significantly associated with superior outcomes.
HIV and prior exposure to antiretroviral therapy alter tumour composition and tumour: T‐cell associations in diffuse large B‐cell lymphoma

Abstract:Diffuse large B‐cell lymphoma (DLBCL) is the most common subtype of lymphoma in both HIV+ and HIV− patients. Data from a cohort of patients in Malawi, where HIV is endemic, revealed that both HIV and exposure to antiretroviral therapy (ART) prior to DLBCL diagnosis significantly alter the tumour microenvironment and T‐cell–tumour associations. While HIV‐tumours in our cohort are characterized by Th2‐driven T cell expansions, HIV+/ART‐experienced tumours also reflect a Th1 response. Further, HIV+/ART‐naïve patients had the highest overall survival, and while these tumours exhibit increased CD8+ T‐cell presence, they lack tumour‐driven clonal expansions. Finally, cell of origin and morphological classifications failed to associate with or predict outcome, while HIV and ART exposure, MYC expression and MYC/BCL2 co‐expression by IHC remained negatively prognostic. Summary Diffuse large B‐cell lymphoma (DLBCL) is the most common subtype of lymphoma worldwide, accounting for up to 40% of new non‐Hodgkin Lymphoma (NHL) globally. People living with HIV are up to 17 times more likely to develop NHL, and as such, DLBCL is the leading cause of cancer death in this high‐risk population. While histologically indistinguishable, HIV‐associated (HIV+) and HIV‐negative (HIV−) DLBCL are molecularly distinct, and biological differences may have implications for the development of future therapeutic interventions. Further, the impact of immunologic differences in people with HIV, including preceding ART, remains largely unknown. Here, we investigate the impact of HIV infection and ART exposure on the clinical features of DLBCL and T‐cell immune response by performing imaging mass cytometry on our unique patient cohort in Malawi. In this cohort, HIV infection is positively prognostic, and HIV+/ART‐naïve patients have the best outcomes. No established biomarkers other than Ki67 are associated with HIV or ART status, and the only tumour‐intrinsic biomarkers that remain prognostic are MYC and MYC/BCL2 protein co‐expression. Finally, TCR clonality is associated with distinct tumour‐T cell interactions by HIV/ART status, indicating differential anti‐tumour immune responses. We demonstrate previously undescribed HIV and ART‐related differences in the DLBCL tumour microenvironment.

Abstract:Little is known about the outcome for HIV-associated Hodgkin lymphoma (HIV-HL) as this is less common than HIV-negative lymphoma. Therefore, we performed a multi-center study to analyze the clinical characteristics and outcomes of HIV-HL patients in China. Nineteen cases of HIV-HL were diagnosed and treated at three center and including the sixth people's hospital of Zhengzhou, Peking union medical college hospital, and Chongqing university cancer hospital, between December 2013 and June 2022. Data on the clinical features, laboratory results, response, and prognosis were collected and analyzed. The median age at diagnosis was 43(22–74) years. All patients were infected with HIV through sexual transmission, with ten cases transmitted through man having sex with man (MSM) and nine cases transmitted through heterosexual transmission. Seven patients were diagnosed with lymphoma and found to be infected with HIV. Four cases were in stage III, and fifteen cases were in stage IV. After a median follow up of 46.8(4.0-112.9) months, 17 cases were alive after ABVD regimen chemotherapy combined with combination antiretroviral therapy (cART). The 5-year progression-free survival (PFS) and overall survival (OS) rate were 83.9% and 89.5%,respectively. HIV-HL exhibits an invasive process in clinical practice, and cART combined with ABVD regimen chemotherapy can achieve long-term survival for patients.

Abstract:Introduction: Diffuse large B-cell lymphoma (DLBCL) is an aggressive malignancy of B-cells frequently encountered among people living with HIV. Immunological abnormalities are common in immunocompetent individuals with DLBCL, and are often associated with poorer outcomes. Currently, data on derangements of immunological proteins, such as cytokines and acute phase reactants, and their impact on outcomes in HIV-associated DLBCL (HIV-DLBCL) is lacking. This study assessed the levels and prognostic relevance of interleukin (IL)-6, IL-10 and Transforming Growth Factor Beta (TGFβ), the acute phase proteins C-reactive protein (CRP) and ferritin; serum free light chains (SFLC) (elevation of which reflects a prolonged pro-inflammatory state); and the activity of the immunosuppressive enzyme Indoleamine 2,3-dioxygenase (IDO)in South African patients with DLBCL. Methods: Seventy-six patients with incident DLBCL were enrolled, and peripheral blood IL-6, IL-10, TGFβ, SFLC and IDO-activity measured in selected patients. Additional clinical and laboratory findings (including ferritin and CRP) were recorded from the hospital records. Results: Sixty-one (80.3%) of the included patients were people living with HIV (median CD4-count = 148 cells/ul), and survival rates were poor (12-month survival rate 30.0%). The majority of the immunological proteins, except for TGFβ and ferritin, were significantly higher among the people living with HIV. Elevation of IL-6, SFLC and IDO-activity were not associated with survival in HIV-DLBCL, while raised IL-10, CRP, ferritin and TGFβ were. On multivariate analysis, immunological proteins associated with survival independently from the International Prognostic Index (IPI) included TGFβ, ferritin and IL-10. Conclusion: Derangements of immunological proteins are common in HIV-DLBCL, and have a differential association with survival compared to that reported elsewhere. Elevation of TGFβ, IL-10 and ferritin were associated with survival independently from the IPI. In view of the poor survival rates in this cohort, investigation of the directed targeting of these cytokines would be of interest in our setting.

Abstract:Background: HIV infection has been associated with survival disparities among persons with hepatocellular carcinoma (HCC). However, most studies examining survival do not control for provider (e.g. type of HCC treatment given) or individual-level factors (e.g. homelessness, substance use) that could impact survival. In this study, we evaluate the effect of HIV status on survival among persons with HCC, in a comprehensive model that accounts for key individual, provider, and systems-level factors. Methods: We conducted a retrospective cohort study of persons with HIV (PWH) matched 1 : 1 to HIV-negative controls based on age and year of HCC diagnosis in the national Veterans Administration (VA) health system. The primary outcome was survival. We used Cox regression models to evaluate the effect of HIV status on risk of death. Results: This cohort included 200 matched pairs diagnosed with HCC between 2009 and 2016. A total of 114 PWH (57.0%) and 115 HIV-negative patients (57.5%) received guideline-concordant therapy ( P = 0.92). Median survival was 13.4 months [95% confidence interval (CI) 8.7-18.1] among PWH and 19.1 months (95% CI 14.6-24.9) for HIV-negative patients. In adjusted models, older age, homelessness, advanced Barcelona Clinic Liver Cancer (BCLC) stage, and not receiving any HCC treatment predicted risk of death. HIV status was not associated with risk of death [adjusted hazard ratio (aHR) 0.95; 95% CI 0.75-1.20; P = 0.65]. Conclusion: HIV status was not associated with worse survival among HCC patients, in a single-payer, equal access healthcare system. These results suggest that HIV infection alone should not exclude PWH from receiving standard therapy.

Abstract:Background: Limited real-world evidence is available for patients with diffuse large B-cell lymphoma (DLBCL) who received an autologous stem cell transplantation (ASCT) in Germany. Objectives: This study aims to describe the real-world survival outcomes of patients with DLBCL who received ASCT in Germany after diagnosis. Design: This study is a retrospective database analysis covering the period between 2010 and 2019. Methods: Unadjusted overall survival (OS) was plotted using the Kaplan–Meier estimator for the overall population and stratified by relapse status. A Cox regression was run to identify factors that influence OS. Results: A total of 112 patients received an ASCT, with the average time from first-line treatment to ASCT being 11.7 months. The median OS estimated by Kaplan–Meier was 83.4 months for the entire cohort. The only variable that significantly reduced the OS was the presence of subsequent treatment after ASCT in a time-dependent model. Conclusion: OS after ASCT for DLBCL patients in Germany is higher than previously reported and may still be considered a valid option for carefully selected patients with relapsed/refractory DLBCL.

Abstract:Despite the widespread use of combined antiretroviral therapy (cART) and the subsequent decrease in AIDS-defining cancers, HIV-related lymphomas remain a leading cause of morbidity and mortality in people with HIV (PWH). Diffuse large B-cell lymphoma (DLBCL) is the most common non-Hodgkin lymphoma (NHL) subtype in PWH. This lymphoma is a heterogeneous disease including morphological variants and molecular subtypes according to the cell of origin or the mutation profile. In the pre-cART era, treatment with standard-dose chemotherapy induced high rates of toxicity and outcomes were very poor. The introduction of cART and the incorporation of infection prophylaxis allowed the use of conventional intensive chemotherapy regimens used in the general population, such as R-CHOP or R-EPOCH. The use of cART during chemotherapy treatment was initially controversial due to the potential risk of adverse drug–drug interactions. However, the availability of current cART regimens with less potential to cause drug interactions and evidence that cART improves survival rates in NHL strongly support the use of cART in PWH with DLBCL. Consequently, interdisciplinary collaboration between HIV specialists and hemato-oncologists for the management of potential interactions and overlapping toxicities between antiretroviral and antineoplastic drugs is crucial for the optimal treatment of PWH with NHL.

Abstract:The year 2024 marks the 20th year since when free cART was introduced in India. This study reports the clinicopathologic profile and outcomes of people living with HIV (PLHIV) who were diagnosed with lymphoma from Northern India. We also review the outcomes of HIV-associated lymphoma in India reported over the past 2 decades. This study included all PLHIV of age ≥ 12 years who were diagnosed with lymphoma and treated at our center between June 2009 and June 2022. Demographic profile, lymphoma subtype, stage and CD4 count at lymphoma diagnosis, and treatment outcomes were analysed. Forty-eight PLHIV patients were diagnosed with lymphoma during the study period of which 31(64.5%) were diagnosed with AIDS Defining Lymphoma (ADL) while 17 (35.4%) were diagnosed with Non-AIDS Defining Lymphoma. Extra-nodal disease was more common in ADLs as compared to NADLs (87% vs. 23%; p < 0.001). 58% ( n = 28) patients were previously known HIV, 42% ( n = 20) were diagnosed to have HIV concurrent with lymphoma diagnosis. DLBCL was the most common subtype, diagnosed in 45.8% patients. 79% (38/48) patients received a definitive treatment regimen for lymphoma. The median overall survival of the cohort was 26 months. 5-year OS for patients who received definitive lymphoma therapy was 55.2 ± 9%. On multivariate analysis, the receipt of lymphoma directed therapy (HR:5.6 {95% CI:1.34–23.37}, p value:0.018) and a baseline CD4 count of ≥ 200cells/μL (HR:3.5 {95% CI:1.09–11.26}, p value: 0.035) were predictors of overall survival. The lack of lymphoma-directed therapy and a CD 4 count < 200 cells/μL continue to be a significant factors negatively impacting survival in the contemporary cART era in India.

Abstract:Burkitt lymphoma (BL) and diffuse large B cell lymphoma (DLBCL) are aggressive forms of lymphoma in adults and demonstrate overlapping morphology, immunophenotype and clinical behavior. The risk of developing these tumors increases ten to hundred-fold in the setting of HIV infection. The genetic causes and the role of specific mutations, especially in the setting of HIV, are largely unknown. The decoding of the human genome and the advent of high-throughput sequencing have provided rich opportunities for the comprehensive identification of the genetic causes of cancer. In order to comprehensively identify genes that are recurrently mutated in immune-competent DLBCL and BL, we obtained a total of 92 cases of DLBCLs and 40 cases of BL. These cases were compared to a set of 5 DLBCLs and BL tumors derived from patients with HIV. The DLBCL cases were divided into a discovery set (N=34) and a prevalence set (N=61). The Burkitt cases were also divided into discovery and prevalence sets (N=15, N=45 respectively). For each of the discovery set cases we also obtained paired normal tissue. We performed whole-exome sequencing for all of these using the Agilent solution-based system of exon capture, which uses RNA baits to target all protein coding genes (CCDS database), as well as ~700 human miRNAs from miRBase (v13). In all, we generated over 6 GB of sequencing data using high throughput sequencing on the Illumina platform. We identified a total of 432 genes that were recurrently mutated in DLBCL and BL. We found that each tumor had an average of 20 gene alterations, which is fewer than most other solid tumors sequenced to date. Commonly implicated biological processes comprising these genes included signal transduction (e.g. PIK3CD, PDGFRA), immune response (e.g. B2M, CD83, IRF8) and chromatin modification (e.g. MLL3, SETD2). We found that lymphomas that arose in the setting of HIV had fewer mutations overall and had a paucity of mutations related to immune response. These data implicate the depressed immune response by HIV as a contributing risk factor for the development of lymphomas and suggest that HIV lymphomas are genetically less complex than their immune competent counterparts. This study represents one of the largest applications of exome sequencing in cancer, and provides early clues to the genetic causes of HIV-lymphomas.

Abstract:Abstract Background: Primary effusion lymphoma (PEL) is a rare and aggressive non-Hodgkin lymphoma associated with HIV and human herpesvirus 8 (HHV-8). (Blood PMID: 8695812). Without treatment, patients typically survive only two to three months (Oncol Res Treat PMID: 28253516) Limited research has been conducted on PEL, particularly in minorities in the United States. Limited research, especially in minority populations (Blood PMID: 30154110), has prompted a nationwide study comparing demographics, clinical characteristics, treatment patterns, and survival outcomes between Hispanic (HI) and Non-Hispanic (NH) PEL patients. Methods: Data on PEL patients from the National Cancer Database (NCDB) between 2004 and 2019 were analyzed in the US. Demographics and treatment characteristics were compared across ethnic groups. Kaplan-Meier and Cox regression analyses were used to compare overall survival (OS) between HI and NH populations. Multivariate analysis and propensity score matching were conducted, adjusting for age, stage, co-morbidity score, insurance status, type of facility, and great circle distance. Results: Of 459 PEL patients, 18% were HI, and 79% were NH. Both groups had mostly male patients (87% vs. 86%). HI patients were younger at diagnosis (mean 49 years vs. 57 years, p=0.163). Diagnoses were mostly from 2014-2019, and the majority were white (87% vs. 69%). Government-sponsored insurance was more prevalent in both groups (62% vs. 66%). HI had a higher percentage of uninsured individuals (8% vs. 5% NH) (p-value=0.01). The median income bracket was over $46,000 for both cohorts, HI 34% and NH 37%. Advanced comorbidities were similar between HI 50% and NH 43% based on Charlson-Deyo Score. The survival probabilities at 2, 5, and 10 years for HI vs NH were (40% vs 40%), (34% vs 289%), and (34% vs 24%), respectively. The median survival time (MS) was 0.8 years for HI and 0.9 years for NH. There was no overall survival (OS) difference (p-value=0.51). On multivariate analysis, there were no independent variables associated with better or worse OS. The propensity-matched analysis showed no MS difference between HI and NH (0.80 vs 0.71 years). Conclusion: This hospital-based analysis confirmed low survival rates and revealed no statistical differences in MS or OS among HI and NH individuals with PEL. Interestingly, the disease was more prevalent in high-income subjects in both cohorts, despite HI individuals having lower insurance rates. A better understanding of intrinsic disease characteristics and biological variables can offer valuable insights into how to unveil potential targets of better and hopefully meaningful therapies that could improve survival outcomes. Abstract:Introduction: Advances in the treatment of diffuse large B-cell lymphoma (DLBCL) have improved survival, with Surveillance, Epidemiology, and End Results (SEER) Program data showing 5-year overall survival (OS) of 64.7% (SEER*Explorer, 2023). However, insurance status in the US is related to DLBCL survival (Han et al., Cancer 2014), and real-world data for patients who are uninsured or who may have socioeconomic barriers to care remain limited. We performed a retrospective cohort study to examine outcomes for patients with newly diagnosed DLBCL treated in a large safety-net hospital system in the third most populous county in the US. Methods: Patients aged 18 years and older diagnosed with DLBCL between January 2011 and June 2022 and treated at Harris Health System, Houston, TX, were included in the analysis. Demographic, disease, treatment, response, and follow-up data were abstracted from the electronic medical record. US Census Block Group FIPS codes derived from geocoded home address at the time of initial diagnosis were used to compute the Area Deprivation Index (ADI). Eligibility for cellular therapies including hematopoietic stem cell transplantation (HSCT) and chimeric antigen receptor T-cell therapy (CAR-T) was determined by the abstractors. Time-to-event analysis was performed using the Kaplan-Meier method, and Cox proportional hazards regression was used to examine factors associated with event-free survival (EFS) and OS. Results: Among 461 patients with newly diagnosed DLBCL (Table 1), the median age was 53 years (range, 18-93), 75% were uninsured, and 80% were in the most disadvantaged ADI national quartiles, defined as at or above the fiftieth percentile. Most patients had stage III or IV disease and an Eastern Cooperative Oncology Group Performance Status (ECOG) of 0 to 1 at the time of diagnosis. One hundred ninety-eight patients (44%) had poor-risk disease by the Revised International Prognostic Index (R-IPI). Seventy-five (16%) had a history of HIV infection. Common first-line treatments included R-CHOP ( n = 210, 46%) and R-EPOCH ( n = 183, 40%); 18 received no therapy (3.9%). The median time from diagnosis to treatment initiation (DTI) was 18 days (range, 0-246); 76% completed therapy. Of 425 evaluable patients, 288 (68%) achieved a complete response (CR) and 44 (10%) had a partial response (PR); of these, 74 (22%) relapsed. Twenty-five (58%) of 43 patients eligible for cellular therapies at relapse received them: 14 (56%) underwent autologous HSCT, 8 (32%) received CAR-T, and 3 (12%) had both. Median follow-up time for the cohort was 48.8 months (range, 0.2-147). By the time of analysis, 167 patients (36%) had developed refractory/relapsed disease, and 148 (32%) had died. At 1, 2, and 5 years, EFS was 72%, 64%, and 58%, and OS was 82%, 76%, and 69% (Figure 1). At 1, 2 and 5 years, EFS was 77%, 69%, and 64% and OS was 85%, 79%, and 75% for patients with germinal center B-cell-like (GCB) DLBCL, whereas EFS was 68%, 56%, and 50% and OS was 80%, 73%, and 62% for those with non-GCB DLBCL. In multivariable Cox regression models, predictors of OS were poor-risk disease by R-IPI (HR 6.08 compared to very-good-risk disease, 95% CI 1.02-36.09), ECOG (HR 2.10 for ECOG ≥2 compared to 0-1, 95% CI 1.12-3.95), and normalized LDH (HR 1.07 per fold increase, 95% CI 1.03-1.11). Race/ethnicity, insurance status, and DTI were not associated with significant differences in OS. However, receiving fewer than planned cycles of first-line therapy was associated with higher mortality (HR 2.00, 95% CI 1.12-3.56), as was not attaining CR by the end of first-line therapy (HR 7.90, 95% CI 3.65-17.12). Conclusion: This is one of the largest real-world studies to date examining outcomes of patients with newly diagnosed DLBCL at a safety-net hospital system. Overall, patients had shorter EFS and OS compared to historical clinical trials but had similar OS to SEER estimates. In this health system with robust financial assistance programs and access to cellular therapies for some patients, uninsured status was not associated with inferior outcomes. Addressing barriers to care may improve outcomes for uninsured patients in other settings.

Abstract:7507 Background: Diffuse Large B-cell Lymphoma (DLBCL) is the most common subtype of Non-Hodgkin Lymphoma, with the majority of patients (pts) achieving long-term survival due to the curative nature of frontline therapy. However, limited data exists regarding presentation and clinical outcomes for underrepresented minorities. In this study, we aimed to evaluate the influence of racial identity on overall survival (OS) outcomes for DLBCL pts. Methods: The National Cancer Database was used to identify DLBCL pts diagnosed from 2004-2018. Demographic and treatment characteristics were compared between racial groups. Kaplan-Meier and Cox regression analyses were used to compare OS between Black and white populations. Multivariate analysis and propensity score matching was performed with adjustment for age, stage, co-morbidity score, median income, and insurance status. Results: Of 223,709 DLBCL pts, 87% were white, 8% Black, and 5% other. As compared to white pts at diagnosis, Black pts were younger (mean 56 years [SD ± 16] vs. 66 years [SD ± 15]), more likely to have ≥1 co-morbidity (33% vs. 27%), be HIV-positive (26% vs. 5%), and have both B-symptoms (40% vs. 30%) and stage IV disease (42% vs. 37%) (All p<0.001). In terms of socioeconomic demographics, Black pts were more likely to be uninsured (8% vs. 3%) and be in the lowest median income quartile (43% vs. 15%), but were more likely to receive treatment at academic centers (50% vs. 36%) (all p<0.001). Both Black and white pts had similar IPI scores at diagnosis and were equally likely to receive multi-agent chemotherapy (77% vs. 77%, p<0.001). With a median follow up of 44.9 months, median OS for all treated HIV-negative DLBCL pts was 109 months (range 21-197). Compared to age-matched white pts, Black pts age ≤ 60 had worse median OS (46 vs. 76 months) along with 5- (73% vs. 75%) and 10-year OS (65% vs. 69%) (all p <0.001). Similar results were seen for Black and white pts between the ages of 61-79, but these differences were not demonstrated for pts ≥80 years old (Table). On multivariate analysis, Black race was independently associated with worse OS (HR 1.06, CI 1.01-1.10, p=0.02). Interestingly, the propensity matched analysis demonstrated no significant OS difference between Black and white pts (median 127 vs. 117 months; HR 1.0, CI 0.94-1.06; p=0.90). Conclusions: We present the largest study to date examining racial disparities in DLBCL. This data demonstrates that Black patients have significantly shorter OS compared to white patients, which persists on multivariate analysis. However, this disparity in survival became nonsignificant when patients are equally matched on surrogate markers of healthcare access such as insurance status and median income. Further studies into examining these racial differences are warranted to optimize care for all DLBCL patients. [Table: see text]

Survival of patients with CD20-negative variants of large B-cell lymphoma: an analysis of the National Cancer Data Base

Abstract:Using records from the National Cancer Data Base, we studied overall survival of CD20-negative variants of diffuse large B-cell lymphoma (DLBCL): primary effusion (PEL, N = 228), plasmablastic (PBL, N = 481), ALK-positive large B-cell (ALK + LBLC, N = 15), and human herpesvirus-8-positive DLBCL (HHV8 + DLBCL, N = 77). Three-year survival was 27% for PEL, 40% for PBL, 34% for ALK + LBCL, and 63% for HHV8 + DLBCL. Compared with unspecified DLBCL, and adjusting for clinical characteristics (including the HIV status), survival was significantly worse for PEL (hazard ratio [HR], 1.58; 95% confidence interval [CI], 1.31-1.90), PBL (HR 1.66; 95% CI, 1.41-1.95), and ALK + LBCL (HR, 2.70; 95% CI, 1.27-5.75), but not for HHV8 + DLBCL (HR, 0.89; 95% CI, 0.54-1.45). The HIV status was not an independent prognostic factor in PEL, PBL, or HHV8 + DLBCL. Advanced stage was prognostic for PBL (p = .0002), but not for ALK + LBCL (p = .96), or HHV8 + DLBCL (p = .28). In PEL and PBL survival significantly differed according to primary site. Novel therapeutic approaches are urgently needed for these rare diseases.

Abstract:Abstract Lymphoma, a leading cancer cause of death among people living with HIV (PLHIV) globally, poses a pressing public health challenge in Sub-Saharan Africa (SSA), the epicentre of the HIV epidemic. Despite its prevalence, the genetic underpinnings of lymphoma in Black Africans and PLHIV remain poorly defined, contributing to regional disparities. This study aims to elucidate the impact of HIV status and EBV on the genetic landscape, molecular subtypes and survival outcomes of diffuse large B-cell lymphoma (DLBCL) in ethnically diverse SSA patients. We established a cohort of 117 newly diagnosed DBLCL cases (55 HIV+, 21 EBV+, 62 Black Africans), with histopathologic and clinical annotation from a public teaching hospital in Cape Town, South Africa (2010-22). Tumor specimens underwent histologic review, whole exome sequencing (WES) and bulk whole transcriptome sequencing. Germline variants were filtered by paired normal WES (when available) and normal references augmented by supervised machine learning in tumor-only specimens. Aberrant somatic hypermutation (ASHM) was assessed in curated genes within paired samples. Microsatellite instability (MSI) was assessed via MSI-sensor. Canonical genetic aberrations reproduced in this cohort included recurrent SNVs in TP53, CARD11, TET2, SOCS1, MYD88, and KMT2D with differences in relative proportion compared to the NCI consortium cohort being more attributable to HIV or EBV status than ethnicity. HIV+ cases harbored frequent mutations in TET2, BRAF, and KRAS, with fewer in NFKB pathway genes including CARD11, IKBK, and BTK. ABC-type HIV+ cases manifested higher expression of genes in the JAK-STAT pathway. EBV+ cases exhibited distinct genomic features including amplifications in NOTCH1, CARD11, NFKBIE, MYC, CD274, and JAK2, as well as frequent gain-of-function (GOF) mutations in the NOTCH1 PEST domain (40% vs 3.8% q=0.04). Tumor mutation burden was consistent across ethnic and virological strata (median mutations 139-167, p>0.05). Notably, ASHM was not identified as a predominant mutational signature in either HIV-positive or negative cases, and 4 HIV+ DLBCL cases exhibited MSI, corroborated by protein-affecting mutations in mismatch repair genes. The 2-year overall survival (OS) was less than 40%, markedly inferior to published Western cohorts, even when controlled for clinical prognostic factors and treatment regimen. This study highlights genetic and biological distinctions in DLBCL due to HIV and EBV. Key findings include the absence of ASHM correlates of pathogenic driver mutations, challenging a prevailing hypothesis of HIV-associated lymphomagenesis, and the identification of MSI in HIV+DLBCL. The dismal survival of this cohort and distinct genomic features, especially in EBV+DLBCL, highlight the urgency of further biological interrogation and inclusive clinical trials for these demonstrably underserved populations.
