Abstract:Objective To explore the safety and feasibility of robotic thyroid surgery via bilateral axillo-breast ap-proach (BABA). Methods From May 2015 to July 2016, 40 patients underwent robotic thyroid surgery via BABA in our hospital. The perioperative parameters, postoperative complications of the patients and the satisfaction of appearance were analyzed. Results Operations were performed successfully on all 40 patients via BARA including 5 cases with unilateral subtotal thyroidectomy, 7 cases with unilateral thyroid lobectomy, 24 cases of unilateral thyroid lobectomy combined with the resection of isthmus and pyramidal lobe and central lymph node dissection, and 4 cases of unilateral thyroid lobectomy with the resection of isthmus and pyramidal lobe and central lymph node dissection and contralateral side subtotal thy-roidectomy. The operation time was (124.1 ±34.6) (70-225) min with the blood loss (7.4 ±6.5) (2-40) mL. The postoperative hospital stay was (2.3±0.7) (1-4) d. It was shown by postoperative pathological examination 28 patients with thyroid papillary carcinoma, 9 patients with nodular goiter and 3 patients with thyroid adenoma. No postoperative hemorrhage was observed. Transient recurrent laryngeal nerve injury occurred in 1 case and transient hypoparathyroidism in 3 cases. There was no both permanent recurrent laryngeal nerve injury and permanent hypoparathyroidism. No tumor recurrence of the cases was found during the period of followed-up from 1 to 14 months with median 7 months. All 40 patients were satisfied with the cosmetic effect of surgery. Conclusions Robotic thyroid surgery via BABA is safe and feasible with good cosmetic results.
Comparison of the robotic bilateral axillo-breast approach and conventional open thyroidectomy in pediatric patients: a retrospective cohort study

Abstract:Background: Operations for pediatric thyroid nodules are more complicated, and usually lead to longer scars, which may impair life quality in the long term. Bilateral axillo-breast approach (BABA) robotic thyroidectomy (BABA RT) may provide a better alternative to conventional open thyroidectomy (COT) for pediatric thyroid nodules. Our study aimed to compare the surgical and oncological outcomes of BABA RT and COT in pediatric patients. Methods: The data of 49 pediatric patients who consecutively underwent BABA RT or COT (20:29) between July 1998 and November 2021 in our center were retrospectively analyzed, including demographics, surgery extent, surgical outcomes, pathological characteristics, and oncological outcomes. Results: All BABA RT procedures were completed successfully without conversion to open operation. The BABA RT group consisted of 5 benign and 15 malignant cases, while the COT group consisted of 19 benign and 10 malignant cases. The operation time, drain removal time, and number of lymph nodes harvested by central lymph node dissection (CND) or lateral lymph node dissection (LND) were equivalent in the BABA RT and COT groups. Notably, the postoperative hospital stay of the BABA RT group was shorter than that of the COT group [8.5 (IQR:3) vs. 11(IQR:8) days, p=0.008]. The aesthetic score of the BABA RT group was much higher than that of the COT group [9 (IQR:1) vs. 6 (IQR: 1), p<0.001]. There was no significant difference between the BABA RT and COT groups in hypoparathyroidism rate (transient, 5 vs. 4; permanent, 1 vs. 0). There was one case of chyle leakage in the COT group and no other complications in any group, such as recurrent laryngeal nerve injury. With a median follow-up of 101 (IQR:189) months, one case of local relapse and one case of pulmonary metastasis in the COT group were documented. Conclusion: In the hands of experienced surgeons, robotic BABA thyroidectomy can be a safe and effective option for both benign and malignant thyroid nodules in children, including those with lymph node metastasis. Robotic BABA thyroidectomy can offer a better and faster postoperative course and a much better cosmetic result, which is crucial for pediatric thyroid patients.

Abstract:Thyroglossal duct cysts (TDCs) are the most common congenital midline neck masses. Lingual TDC is a rare variant that emerges as an isolated cyst at the tongue base. Unlike conventional procedures aiming removal of cyst, duct, and hyoid bone via open surgical access, a transoral cystectomy is mostly sufficient in cases with lingual TDC. We present a case describing a 3-month-old infant patient with lingual TDC who was successfully treated with transoral robotic surgery. The cyst wall was completely excised with no complications or obvious bleeding occurred. The operating time was 10 min. He had an uneventful postoperative course. Six months postoperatively, he is free of symptoms with no evidence of recurrence. Surgical treatment of lingual TDC in an infant is possible with transoral robotic approach and minimal risk of complication. Further studies are strongly needed to confirm the safety of robotic surgery in pediatric population.

Abstract:Thyroglossal duct cysts (TDCs) are generally single cyst, multiple TDCs are rare. We describe a case of multiple TDCs, discuss its characteristic features and management, and provide a review of the literature, to improve clinical diagnosis and treatment. We report an extremely rare case of multiple TDCs containing five cysts, together with a review of the relevant English medical literature. To the best of our knowledge, this is the first reported case of TDCs containing more than three cysts in the anterior cervical region. The five cysts were completely excised in a Sistrunk operation. Histological examination of the cystic lesions revealed TDCs. The patient recovered well and no recurrence was found during the 6-year of follow-up. Multiple TDCs are extremely rare, and may be misdiagnosed as a single cyst. Clinicians should be aware of the possibility of multiple thyroglossal duct cysts. Adequate preoperative radiological examinations should be performed, and careful interpretation of the CT or MRI scans is important to diagnosis and surgery.
Bilateral Areolar Approach Endoscopic Thyroidectomy for Low-risk Papillary Thyroid Carcinoma: A Review of 137 Cases in a Single Institute

Abstract:Purpose: The aim of this study was to analyze the clinicopathologic characteristics and postoperative outcomes and to evaluate the feasibility of the bilateral areolar approach (BAA) endoscopic thyroidectomy for low-risk papillary thyroid carcinoma (PTC). Materials and Methods: From January 2012 to February 2013, 137 low-risk PTC patients underwent BAA endoscopic thyroidectomy. Their clinicopathologic characteristics and postoperative outcomes (postoperative cosmetic satisfaction, type of thyroidectomy, number of lymph nodes, postoperative complications, and recurrence of disease) were analyzed. Results: The 137 patients comprised 135 female and 2 male individuals. The average age of patients was 32.02±8.32 years. The mean tumor size was 0.82±0.41 cm. The ratio of minimal extrathyroidal extension patients was 1:19.6. According to the American Joint Committee on Cancer tumor stage, 132 cases were stage I and 5 cases were stage III. The mean follow-up period was 7.80±3.86 months (range, 3 to 15 mo, and median, 7 mo). At 3 months, postoperatively, patients were very satisfied with the cosmetic result as evaluated by a 10-point visual analogue scale (9.14±1.17). After surgery, the mean number of lymph nodes was 5.70±2.92, whereas the mean number of lymph node metastases was 1.06±1.96. Regarding the major postoperative complications, the rates of transient recurrent laryngeal nerve palsy and transient hypoparathyroidism were 4.4% and 27.7%, respectively. None of the patients experienced a thyroid cancer–related death or recurrence. Conclusions: BAA is feasible and safe for the treatment of low-risk PTC patients, with favorable cosmesis. Thus, it is an alternative therapeutic treatment for selected patients with low-risk PTC. However, oncologic safety of BAA thyroidectomy for PTC patients needs to be verified by a large comparative series and long-term follow-up.

Abstract:This review aimed to summarize existing studies on endoscopy‐assisted thyroglossal duct cyst (TGDC) surgery and discuss the advantages and disadvantages of this novel technique. A systematic review of the databases identified 85 patients. The areola, axillo‐breast, transoral vestibular, and transoral‐sublingual approaches have been reported, and the operative time varies across studies, ranging from 50 to 480 min. All patients completed the endoscopic procedure without conversion to open surgery or postoperative recurrence. Complications were noted in seven patients. Endoscopy‐assisted TGDC surgery is a potential alternative for patients seeking improved cosmetic outcomes. Objectives Thyroglossal duct cysts (TGDCs) are a common congenital mass in the cervical region. As the traditional surgical approach for TGDC removal, the Sistrunk procedure, often leaves a visible neck scar, the demand for improved cosmetic outcomes has increased. Emerging endoscopy‐assisted approaches offer promise for addressing cosmetic concerns. We conducted a scoping review to evaluate the feasibility and safety of endoscopy‐assisted TGDC surgery. Data Sources PubMed, Embase, and Cochrane databases. Methods Electronic databases were searched from their respective inception dates to January 2023. Data on surgical approach, patient demographics, surgical procedure, and postoperative outcomes were extracted and analyzed. The quality of the studies was assessed using the Joanna Briggs Institute Critical Appraisal Checklist. Results The literature search yielded nine articles published between 2011 and 2022. Overall, 85 patients in these studies successfully underwent endoscopy‐assisted TGDC surgery using various approaches, including areolar, axillo‐breast, transoral‐vestibular, and transoral‐sublingual. The operative time varied across the studies, ranging from 50 to 480 min. TGDC sizes ranged from 1 to 3 cm in diameter. Complications, including infection, skin bruising, and dysarthria, were reported in seven patients (8%). No cases of conversion to open surgery or postoperative recurrences were reported. Conclusion Endoscopy‐assisted surgery is a potential alternative for patients seeking TGDC resection with satisfactory aesthetic results while ensuring safety. Abstract:Objective: This study is aim to summarize the experience of robotic thyroidectomy via bilateral axillo-breast approach of our center and also to find out the learning curve of this technique. Methods: In total 220 initial patients who have undergone robotic thyroidectomy via bilateral axillo-breast approach from May 2015 to September 2017 were involved in this study. The data of operation time, clinical characteristics, surgical outcomes and oncological outcomes were collected. The moving average method is use to explore the learning curve. Results: All patients had undergone robotic thyroidectomy successfully without conversion to other surgical approaches. The mean age of the enrolled subjects was 34.4 +/- 7.8 years old, while the sex ratio (male/female) was 38/182. There were 50 benign tumor cases and 170 malignant tumor cases. The mean total operation time was 105.3 +/- 37.6 min. Lymph node metastasis was observed in 61 (35.9%) patients. The mean retrieved lymph node count was 5.1 +/- 3.8 while the mean metastatic lymph node count was 0.7 +/- 1.5. The operation time decreased significantly after about 30-35 cases and formed the plateau. After 80 cases, the operation time significantly decreased again. Conclusion: For skilled endocrine surgeons, robotic thyroidectomy has proved to be safe and feasible, which could be applied extensively in patients strictly selected in high-volume centers, with a relatively short learning curve of about 30-35 cases. Abstract:Submandibular gland (SMG) excision via axillary is a brand new technique and we presented its own unique advantages compared with retroauricular and transoral approaches. The single-port (SP) robotic system offers improved visualization of the view and enhanced flexibility of instruments, ensuring optimal stability and dissection in this surgery. We reported the first case of transaxillary SMG excision using single-port robotic system made in China. A 26-year-old female patient presented with complaint of a painless mass in submandibular region. Ultrasound and computed tomography showed a lesion in submandibular gland. We performed single-port transaxillary robotic SMG excision without any complications. The present case suggests that SMG excision via transaxillary approach is technically feasible and safe with the SP robotic system; however, further exploration and research are needed.

Abstract:We are going to share the experience of robotic-assisted thoracoscopic thymectomy. A 68-year-old patient underwent robotic-assisted thoracic surgery for a primary thymoma. The patient was discharged on postoperative day 3 without any perioperative complications. This case showed the robotic-assisted thoracoscopic thymectomy is a technically sound and safe approach for anterior mediastinal mass.

Abstract:Thyroglossal duct cyst carcinoma (TGDCa) is a rare cause of neck masses in children. Its pathophysiology stems from two main theories. One suggests that the primary carcinoma originated from ectopic thyroid tissue, whilst the other suggest it is a result of metastasis to the thyroglossal duct cyst from the thyroid gland. A case of a 12-year-old girl who presented with a suspicious neck mass and subsequently diagnosed with TGDCa is reported. A literature review was performed and we conclude that aggressive surgical management of a Sistrunk procedure, total thyroidectomy, bilateral tracheoesophageal groove clearance and neck dissection was justified for this child. We found that if the preoperative diagnosis is TGDCa, a more aggressive surgical approach tends to be taken. However, cases of recurrence remain low and a total thyroidectomy does entail a larger morbidity for the pediatric patient. Hence, we do not recommend an aggressive approach routinely. In this review, we extrapolated AJCC data for thyroid malignancies and staged the TGDCa reported in literature in the last 10 years. It showed us that pT3 tumors tend to have a more aggressive surgical course, whilst a pT1 TGDCa tend to receive a more conservative surgical approach.
Surgical outcomes of robotic thyroidectomy for thyroid tumors over 4 cm via the bilateral axillo-breast approach

Abstract:The study investigated the feasibility of robotic bilateral axillo-breast approach (BABA) thyroidectomy for patients with thyroid tumors larger than 4 cm. BABA thyroidectomy has previously shown safety and effectiveness for thyroid surgeries but lacked extensive data on its application to larger tumors. Between October 2008 and August 2022, there were 74 patients underwent robotic BABA thyroidectomy due to thyroid nodules exceeding 4 cm in size. The mean patient age was 40.3 years. Fine needle aspiration results classified the tumors as benign (50.0%), atypia of undetermined significance (27.0%), follicular neoplasm (16.2%), suspicious for malignancy/malignancy (5.4%), or lymphoma (1.4%). The average tumor size was 4.9 cm, with the majority (85.1%) undergoing thyroid lobectomy, and the rest (14.9%) receiving total thyroidectomy. The mean total operation time was 178.4 min for lobectomy and 207.3 min for total thyroidectomy. Transient vocal cord palsy (VCP) was found in 3 patients (4.1%), and there was no permanent VCP. Among patients who underwent total thyroidectomy, transient hypoparathyroidism was observed in three (27.2%), and permanent hypoparathyroidism was observed in one (9.1%). There were no cases of open conversion, tumor spillage, bleeding, flap injury, or tumor recurrence. In conclusion, robotic BABA thyroidectomy may be a safe treatment option for large-sized thyroid tumors that carries no significant increase in complication rates.

Minimally Invasive Endoscopic Surgery of Thyroglossal Duct Cysts

Abstract:PURPOSE: Thyroglossal duct cysts (TGDCs) may cause cosmetic defects or more serious impairments if left untreated. Our study was designed to examine the potential advantage of endoscopic surgery performed on TGDCs, especially the ability to completely resect the branches and to affect the cosmetic appearance.PATIENTS AND METHODS: Thirty-two patients from Beijing Tongren Hospital, Beijing, China, diagnosed with TGDCs were selected. Seventeen patients with TGDCs underwent traditional Sistrunk's surgery, and 15 patients underwent endoscopic cystectomy.RESULTS: All patients had complete resection and were followed up from 6 months to 4 years. None had a recurrence after endoscopic resection. However, patients in the traditional surgery group had a 5.9% (1/17) rate of recurrence. The incision length was approximately 2.1 cm in the endoscopic group compared with 5.2 cm in the traditional group. The mean time for the endoscopic operation was 97.7 minutes compared with 51.6 minutes in the traditional surgical procedure. The average length of hospital stay was 6.7 days in the endoscopic group compared with 9.7 days in the traditional group.CONCLUSIONS: Endoscope-assisted small-incision thyroglossal duct cystectomy is an efficient method. It causes fewer cosmetic defects and also decreases operative time. It will likely become the new standard procedure for patients with TGDCs.

Abstract:Rationale: Thyroglossal duct carcinoma, a rare clinical condition characterized by ectopic thyroid adenocarcinoma within thyroglossal duct cysts (TGDCs), typically confirmed through intraoperative rapid pathology, this condition generally has a favorable prognosis. Nevertheless, comprehensive treatment guidelines across all disease stages are lacking, the purpose of this study is to report 1 case of the disease and propose the treatment plan for each stage of the disease. Patient concerns: A patient presented with thyroid swelling, classified as C-TIRADS 4A following a physical examination. Preoperative thyroid puncture identified papillary thyroid carcinoma, and genetic testing revealed a BRAF gene exon 15-point mutation. Ancillary tests showed a slightly decreased thyroid stimulating hormone (TSH) level (0.172) with no other significant abnormalities. Diagnoses: Preoperative fine-needle aspiration cytology (FNAC) confirmed right-side thyroid cancer. Intraoperative exploration uncovered a TGDC and intraoperative rapid pathology confirmed thyroglossal duct carcinoma. Interventions: A Sistrunk operation and ipsilateral thyroidectomy were performed. Outcomes: Postoperative recovery was satisfactory. Lessons: Thyroglossal duct carcinoma is a rare disease affecting the neck. Due to limited clinical cases and the favorable prognosis associated with this condition, there is currently no established set of diagnostic and treatment guidelines. According to tumor size, lymph node metastasis, thyroid status and other factors, the corresponding treatment methods were established for each stage of thyroglossal duct cancer, which laid the foundation for the subsequent treatment development of this disease.

Comprehensive analysis and treatment of intraoral thyroglossal duct cysts with intraoperative techniques: a 7-case series review in a tertiary care center

Abstract:Objective: Intraoral thyroglossal duct cyst is a relatively rare clinical disease. This article reviews the diagnosis and treatment process of 7 patients and explores the clinical characteristics of diagnosis and treatment of intraoral thyroglossal duct cyst in combination with past literature reports. Method: A retrospective analysis was conducted on 7 cases of intraoral thyroglossal duct cyst admitted to the Otolaryngology ward of Dalian Municipal Central Hospital from January 2017 to January 2024. The cases were recorded in terms of gender, age, symptoms, physical signs, radiological examinations, surgical methods, and postoperative complications. All cases were followed up, and the latest follow-up results were recorded. Results: Among the 7 cases, 6 patients underwent laryngoscopic and radiological examinations before surgery, and 1 child was found to have a cyst during surgery. All cases were diagnosed with intraoral thyroglossal duct cyst and treated with plasma radiofrequency surgery. None of the patients had postoperative complications, and no recurrence was found in the six-month follow-up after discharge. Conclusion: Intraoral thyroglossal duct cyst is rare in clinical practice. It is important to pay attention to its differential diagnosis clinically, and careful review of images is required before surgery. Cryoablation with low-temperature plasma radiofrequency is not only minimally invasive and has a quick recovery but also has few complications and a low recurrence rate. It is a safe and effective treatment method that is worthy of clinical promotion. Level of evidence: Level 3.

Abstract:A thyroglossal duct cyst (TGDC) is one of the most commonly encountered congenital anomalies of the neck. However, it is difficult to diagnose as differentiating it from other cysts like brachial cysts, lymphangiomas, epidermoid cysts, dermoid cysts, and hydatid cysts, is challenging. In this paper, we systematically reviewed the literature of 47 patients—25 males (53.1%) and 21 females (44.7%)—about their TGDC to assess the clinical picture, therapy, and prognosis of the disease. Most of the patients were children under the age of ten (63.8%). All patients had a history of a painless swelling in the anterior midline of the neck that moved in response to deglutition and tongue protrusion, thus interfering with their daily activity. Post-resection recurrence was unusual, with only 3 of 47 patients (6.4%) experiencing recurrence.
Robot-Assisted Resection of Mesenteric Cysts in Pediatric Patients: a single-centered Retrospective study

Abstract:Abstract Background Mesenteric cysts (MCs) are rare intra-abdominal masses in childhood. Laparoscopic-assisted surgery has become the main procedure for their resection, but robot-assisted surgery has rarely been reported. The purpose of this study was to retrospectively analyze and present our experience with robot-assisted resection of MCs using the da Vinci Xi surgical system and to discuss the technical points. Methods Children diagnosed with abdominal MCs who underwent surgical treatment at the Children's Hospital of Zhejiang University School of Medicine between January 2020 and November 2023 were retrospectively analyzed, and clinical data, surgical details, and prognosis of the patients were systematically collected. Results Among 40 patients, a total of 18 underwent robot-assisted surgery and 22 underwent laparoscopic-assisted surgery. In the robot-assisted surgery group, the entire procedure was performed endoscopically, whereas in the laparoscopic surgery group, eight procedures were converted to laparotomy. The operation time of 134.5 minutes for the robot-assisted group was not significantly longer than the 104 minutes for the laparoscopic-assisted surgery group (P > 0.05), but the average length of postoperative hospital stay was significantly shorter (P < 0.05). Conclusions Robotic-assisted resection of MCs is safe and feasible and the refinement of the Da Vinci Xi surgical system was much better than that of the conventional laparoscopic equipment, resulting in significantly improved intraoperative and postoperative outcomes.

Abstract:Abstract This article reports an older woman with a 2-year history of enlarging submental neck mass. Head and neck imaging displayed a group with a solid central component. Additionally, a fine-needle aspiration suggested neoplasia. Thyroglossal duct cyst cancer is sporadic, accounting for ˂1% of all thyroid malignancies. A Sistrunk procedure was followed by a total thyroidectomy and unilateral neck dissections. From histological and immunohistochemical examinations, the incidental finding of carcinoma indicates the diagnosis of thyroid tissue thyroglossal duct cyst carcinoma, as it is the most common site for malignancy in ectopic thyroid tissue. The patient had an excellent recovery without additional treatments after surgery.

Abstract:Abstract Thyroglossal duct cysts (TGDC) are the most common type of congenital neck masses, which generally present in young adults. We present a rare case of a giant TGDC in a 77-year-old patient who required atypical perioperative management. The patient presented with a large soft mass on his anterior neck. Computed tomography showed a lobulated cystic mass measuring 18 × 16 cm, extending from the tongue base to the inferior level of the clavicle. Because difficult intubation was expected, the cyst was punctured and most of the fluid was drained prior to surgery. The swelling of the tongue base was remarkably reduced, and intubation was performed safely. The cyst was extracted using the Sistrunk procedure and tracheotomy was performed. Histopathological examination confirmed the diagnosis of TGDC. Preoperative volume reduction of the cyst and tracheotomy should be considered for oral intubation and postoperative airway management, respectively, in patients with large TGDC.

Abstract:Thyroglossal duct cyst (TGDC) is one of the most prevalent congenital midline cervical anomalies which commonly appear during childhood. It has a typically slow-growing presentation with small- or moderate-sized swelling. Here, we described our experience in managing a case of TGDC with an extraordinary clinical course and size, with further evidence from a literature review. A 35-year-old gentleman presented later in adult life with a large midline neck mass of 9.3 x 9.0 x 10.7 cm reaching up to the level of mandible superiorly and sternal notch inferiorly. His history and physical examination, along with fine needle aspiration cytology (FNAC) and CT scan, were suggestive of TGDC. He successfully underwent a Sistrunk operation for TGDC without any morbidity. Postoperatively, he recovered well without evidence of recurrence during follow-up. Large TGDC is always a challenge to clinicians. Correlating the clinical features and imaging is crucial to decide the appropriate surgical treatment.
