Lifestyle behaviors and risk of cardiovascular disease and prognosis among individuals with cardiovascular disease: a systematic review and meta-analysis of 71 prospective cohort studies

Abstract:Healthy lifestyle behaviors (LBs) have been widely recommended for the prevention and management of cardiovascular disease (CVD). Despite a large number of studies exploring the association between combined LBs and CVD, a notable gap exists in integration of relevant literatures. We conducted a systematic review and meta-analysis of prospective cohort studies to analyze the correlation between combined LBs and the occurrence of CVD, as well as to estimate the risk of various health complications in individuals already diagnosed with CVD.

Therefore, the main objectives of this systematic review and meta-analysis include: 1. **Analyze the relationship between healthy lifestyle behaviors and the incidence of cardiovascular diseases**: - Whether a combination of healthy lifestyle behaviors can significantly reduce the incidence of cardiovascular diseases in the general population. - Evaluate the specific impact of different combinations of healthy lifestyle behaviors on the incidence of cardiovascular diseases. 2. **Assess the impact of healthy lifestyle behaviors on patients diagnosed with cardiovascular diseases**: - Whether a combination of healthy lifestyle behaviors can significantly reduce the recurrence rate and all-cause mortality in patients diagnosed with cardiovascular diseases. - Quantify the extent to which each additional healthy lifestyle behavior reduces the risk of cardiovascular diseases. 3. **Provide high-level evidence supporting the positive role of healthy lifestyle behaviors in the prevention and management of cardiovascular diseases**: - Through systematic review and meta-analysis, provide scientific evidence for health policymakers and clinicians, emphasizing the importance of promoting healthy lifestyle behaviors. ### Research Methods - **Data Sources and Search Strategy**: The research team searched relevant prospective cohort studies in the PubMed, EMBASE, and Web of Science databases up to February 10, 2023. - **Inclusion Criteria**: Study design as prospective cohort studies; study subjects as adults, including the general population or individuals diagnosed with cardiovascular diseases; follow-up time exceeding 1 year; studies focusing on three or more healthy lifestyle behaviors; reported predefined outcome indicators such as the incidence and mortality of cardiovascular diseases. - **Data Analysis**: Random-effects models were used to estimate pooled relative risks (RRs) and 95% confidence intervals (CIs), and subgroup analysis, meta-regression, publication bias, and sensitivity analysis were conducted. ### Main Results - **General Population**: Individuals with the healthiest combination of lifestyle behaviors had a 58% reduction in the incidence of cardiovascular diseases and a 55% reduction in cardiovascular disease mortality. - **Patients Diagnosed with Cardiovascular Diseases**: Individuals with the healthiest combination of lifestyle behaviors had a 62% reduction in the recurrence rate of cardiovascular diseases and a 67% reduction in all-cause mortality. - **Dose-Response Relationship**: Each additional healthy lifestyle behavior reduced the risk of cardiovascular diseases by 17% and cardiovascular disease mortality by 19%. ### Conclusion Adopting healthy lifestyle behaviors can significantly reduce the incidence and mortality of cardiovascular diseases, as well as adverse prognosis in patients diagnosed with cardiovascular diseases. It is recommended to not only focus on single healthy lifestyle behaviors but to advocate for a combination of multiple healthy lifestyle behaviors to more effectively prevent and manage cardiovascular diseases.
