Abstract:This report describes a case of unusual deep skin ulcers with tortuous sinus tract formation in an immunocompetent woman. She was initially diagnosed with a Staphylococcus aureus skin infection and histopathologically diagnosed with pyoderma gangrenosum. However, culture from the deep end of ribbon gauze inserted into the subcutaneous sinus tract revealed shiny, light-yellow mucoid colonies, which were identified as Cryptococcus neoformans var. grubii. She was treated with fluconazole for nine months and completely healed. Cryptococcosis is an opportunistic infection caused by variants of C. neoformans species. Cutaneous manifestations of cryptococcosis are quite divergent, rarely occurring as deep skin ulcers with sinus formation.

Abstract:OBJECTIVES: The objective of this study is to explore the clinical diagnosis and treatment of the congenital sternoclavicular sinus in the region of sternoclavicular joint, providing valuable insights for clinical practice in children.METHODOLOGY: A retrospective review of case notes was conducted for all patients treated for congenital sternoclavicular sinus at a tertiary care pediatric otolaryngology practice from January 2022 to September 2023.This review analyzed the clinical presentations, imaging examination, pathological examination, and treatment.RESULTS: A total of 56 patients with congenital sternoclavicular sinus were included in the study, with a mean age of 4.10 ± 2.57 years old. Among these patients, the sinuses were located on the left side in 43 cases,on the right side in 12 cases,and bilaterally in one case.While two patients never experienced inflammation, 54 patients did, and 45 of them underwent incision and drainage before excision. Imaging examination, including ultrasonography and computerized tomography (CT) scans, revealed characteristics such as thickening of subcutaneous tissue,subcutaneous tubular structures or tubercles in front of the clavicle,and irregularly shaped lesions with clear borders and uneven internal low echo, Pathological examination confirmed that the sinuses were lined by squamous epithelium in all patients.CONCLUSIONS: The congenital sternoclavicular sinus may be a remnant of the fourth branchial cleft on the skin side.Complete resection of the sinus during the quiescent phase of inflammation is the recommended curative treatment to prevent recurrent infection.

Abstract:CSCS, though uncommon, presents distinct regional involvement along with pathognomonic clinical and pathological features. CSCS is marked by a pale skin streak extending superiorly from the orifice—a diagnostic "skin streak sign." Complete surgical excision constitutes an effective treatment modality for CSCS. Objectives We encountered patients with a congenital cutaneous sinus tract in the sternoclavicular joint region, which we designate as "congenital sternoclavicular sinus (CSCS)." The aim of this investigation is to enhance recognition of this subtle yet noteworthy entity and develop standardized protocols for its management. Patients and Methods Between 2013 and 2023, 172 patients, including 78 males and 94 females, were referred to our institution for the management of CSCS. Clinical charts were retrospectively reviewed. Results The majority of patients (60.5%) were young children below 3 years of age, with only six adult patients and a median age of 27.5 months. The left side was implicated in 157 cases (91.3%). In 146 cases (84.9%), a faint skin streak was noted above the orifice. Yet, no pharyngeal sinus tracts were detected, either through barium swallow studies or direct laryngoscopy. All skin lesions featured a diminutive orifice near the sternoclavicular joint, with the tract extending deeply into the subcutaneous tissue and terminating blindly, short of entering the joint, after a distance of 10 mm (ranging from 5 to 21 mm). Histopathological analysis revealed that the epithelial lining predominantly consisted of stratified squamous epithelium (87.8%), with ciliated columnar epithelium accounting for the remaining 12.2%. Conclusions CSCS, though infrequent, presents with distinctive pathological and clinical features. The condition predominantly affects the left sternoclavicular joint region, with the notable "skin streak sign" aiding in diagnosis. We considered CSCS as one disease entity of branchial arch anomalies. Complete surgical excision offers a definitive cure. Level of Evidence 4 Laryngoscope, 2024

Abstract:Sj(o)gren's syndrome is a chronic autoimmune disease,characterized by hypofunction of salivary and lacrimal glands and multisystem involvement.During the past decades,the most commonly used classification criteria is 2002 AECG criteria,and 2012 ACR criteria and 2016 ACR/EULAR new classification criteria had different characteristics.Sj(o)gren's syndrome has a wide spectrum of clinical manifestations.It needs to be differentiated from various diseases,including history of head and neck radiation treatment,active hepatitis C infection,AIDS,sarcoidosis,graft-versus-host disease,anti-acetylcholine drugs application,IgG4-related disease and lymphoma etc,in order to avoid misdiagnosis or missed diagnosis.
A case of primary Sj, gren’s syndrome with progressive multiple cysts of lungs

Abstract:A 32- year- old woman complained of petechiae of the lower limbs with pain of knee joints for two months. About ten years ago, the patient had been shortage of saliva and tear. For recent five years, she felt the drying of skin all over the body. Laboratory exam showed that rhumatoid factor was 1∶ 2000, ANA titers 1∶ 320, IgG 22.38g/L, IgA 7.23g/L and La(SSB) positive .Pathological exam of lip area tissue supported the diagnosis of Sj, gren s syndrome. ECT exam for dynamic function of salivary gland showed no function of bilateral submaxillary salivary glands， decreased concentration and drainage function of bilateral parotid salivary glands.Chest X ray exam for several times confirmed that there were progressive multiple cysts with thin wall in both lungs.

Abstract:Non-Hodgkin's lymphoma (NHL) involving the paranasal sinuses is rare in human immunodeficiency virus (HIV)-infected patients. We report a case of diffuse large B-cell lymphoma of the left maxillary sinus in a patient with acquired immunodeficiency syndrome (AIDS). A 58 year-old man presented with 30 years history of repeated nasal obstruction that worsened with a runny nose for a week, and his nasal mucus was off-flavor. Computed tomography (CT) imaging and Magnetic resonance imaging (MRI) revealed an infiltrating mass of the left maxillary sinus, which extended along the postero-medial-lateral-superior bone wall and invaded the left nasal cavity, the left infratemporal fossae and pterygomaxillary fossae. The patient was treated with chemotherapy which resulted in partial regression of the disease and alleviation of local symptoms.

Abstract:Patients with unilateral ear discharge and hearing loss often have external or middle ear diseases. We present a 55–year–old man who suffered from persistent ear discharge and hearing loss in the left ear. Local findings showed that his left ear canal was filled with a large amount of granulation tissue, with purulent, foul–smelling discharge. Computed tomography indicated left middle ear cholesteatoma and mass shadow in the left external auditory canal. Modified radical mastoidectomy was performed. A piece of white plastic stick was found in the middle ear during the operation. Foreign body–induced cholesteatoma and external auditory canal granuloma in adults are very rare. We present this rare case so that these conditions can be better recognized and understood.

Abstract:Objective:Pediatric chronic sinusitis （CRS） is a common disease within the field of otolaryngology-head and neck surgery. Due to the immaturity of sinus development and immune competence in children, its etiology and pathophysiology are complex, and its clinical features and outcomes differ significantly from those in adult patients. Currently, there are issues in the diagnosis and treatment of pediatric CRS, particularly in areas such as antibiotic use and surgical interventions, owing to a lack of sufficient attention. In recognition of this, the Chinese Rhinopathy Research Cooperation Group developed this expert consensus based on a systematic review of the latest literatures from both domestic and international sources, with reference to the latest evidence-based medical evidence worldwide, and in combination with their own clinical experience. The consensus covers various aspects including epidemiology, predisposing factors, pathophysiology, diagnosis and differential diagnosis, as well as treatment strategies such as medical therapy and surgical intervention. It aims to standardize the clinical diagnosis and treatment of pediatric CRS, improve clinical efficacy and patient satisfaction, reduce clinical expenditures, and decrease the occurrence of adverse reactions.
A case of an odontogenic cutaneous sinus tract

Abstract:A 22-year-old female presented with a draining sinus tract on her left cheek. The tooth responsible was examined clinicopathologically. On clinical examination, the mandibular left first molar tooth was restored with an amalgam filling. Radiographic examination revealed base or pulp capping material below the restoration and a radiolucent periapical lesion surrounding the distal root apex. Conservative non-surgical root canal treatment was performed; 10 months later, the sinus had healed completely and the periapical lesion had resolved. Histopathological examination of pulp tissue recovered during treatment revealed foreign bodies, made up of capping material and amalgam, associated with chronic inflammation. These findings suggested that chronic inflammation in the pulp tissue had resulted in a draining sinus tract.

Abstract:Objective To improve the recognition of intramedullary spinal abscess by a case of congenital dermal sinus with intramedullary spinal abscess and reduco the incidence of congenital dermal sinus with intramedullary spinal abscess.Methods Clinical,laboratory data and image of a confirmed case about one infant of congenital dermal sinus with multiple intramedullary spinal abscess were investigated,the related literature was reviewed.Results In this case,when the infant with congential dermal sinus had infection,he failed to gain antibiotic therapy, timely surgical treatment,his infection had diffused, and multiple intramedullary spinal abscess flared up.Conclusions Intramedullary spinal abscess is a rare disease.If treatment is delayed, the prognosis is poor and the mortality rate is high.MRI is the ideal investigation for diagnosis.Intramedullary spinal abscess can happen subsequent to congenital dermal sinus with infection, and cause neurological sequela. So an infant with congenital dermal sinus should be offered to avoid complication caused by infection.
Cholesterol granuloma of the maxillary sinus: a case report

Abstract:Background: Cholesterol granuloma is not a common entity in the paranasal sinuses. It is a foreign body reaction to the cholesterol crystal deposition. Mostly associated with chronic middle ear diseases. Case presentation: This article reports a case of cholesterol granuloma in the maxillary sinus. A 23-year-old Asian man presented with cough, nasal obstruction, and postnasal discharge. On endoscopy, there was a mass protruding from the right maxillary ostium. On computed tomograpy imaging, there was a polypoidal mass in the right maxillary sinus. Endoscopic sinus surgery was performed, the cholesterol granuloma was removed from the right maxillary sinus, and the tissue was submitted for histopathological examination, which showed foreign body giant cell reaction to cholesterol crystals. Conclusion: Cholesterol granuloma of maxillary sinus is not common and often missed by clinicians. It is necessary to consider the cholesterol granuloma of maxillary sinus in the differential diagnosis sinonasal lesions. Histopathological analysis is required for confirmation and should be removed surgically. This case may help as a reference for clinician to approach these kinds of cases.

Abstract:The purpose of this case report is to present a 62-year-old female patient who sought dental treatment. Her medical history included xenophthalmia, diabetes, glaucoma, and vision loss due to macular degeneration. During the clinical examination, a cutaneous rash was observed in the frontal and bilateral malar regions. Intraoral examination revealed hyposalivation, and the patient complained of xerostomia. Simultaneously, she was undergoing an investigation for a pulmonary nodule. A salivary gland biopsy was suggested with the initial suspicion of Sjögren's Syndrome.

Isolated sphenoid sinus disease: an analysis of 122 cases.

Abstract:Isolated sphenoid sinus disease (ISSD) is a relatively uncommon disease. The present study is a retrospective review of 122 patients with ISSD who were treated at the Department of Otolaryngology, Eye, Ear, Nose and Throat Hospital at Shanghai Medical University over a 25-year period. The diagnosis of ISSD was made on the basis of history and physical examination, signs and symptoms, nasal endoscopy, and computed tomography (CT) and magnetic resonance imaging (MRI). The final diagnosis of ISSD was confirmed by histopathologic and microbiological examinations of the surgical specimens. The pathological findings in this study included sphenoid cyst (47 cases), sphenoid sinusitis (31 cases), fungal disease (19 cases), inverted papilloma (4 cases), sphenochoanal polyp (1 case), foreign body (8 cases), malignant tumors (8 cases), and others (4 cases). The most common initial symptom was headache, followed in decreasing order by visual changes, cranial nerve palsies, and nasal symptoms. The more frequent use of routine CT and MRI scanning, as well as endoscopy, in the diagnosis of sinus disease has led to an increase in the early diagnosis of ISSD. The recent advances in endoscopic sphenoidotomy has allowed for relatively safe and immediate treatment of ISSD, preventing late extension into adjacent vital structures, which is commonly fatal. Endoscopic surgery also enables the surgeon to make a precise pathological diagnosis.

Abstract:Annals of Otology, Rhinology &Laryngology, Ahead of Print. ObjectivesSilent sinus syndrome (SSS) is a rare condition involving idiopathic maxillary sinus underdevelopment, orbital floor resorption, enophthalmos, and hypoglobus. SSS is more commonly described in adults, with only very limited literature in the pediatric population. Given the rarity of the condition, there is a lack of consensus regarding proper diagnostic criteria and treatment strategies. We present a case of pediatric SSS and a review of other cases in the literature.MethodsCase report and systematic review.ResultsA 12-year-old female patient was referred for incidental MRI findings suggestive of SSS. Examination revealed minimal enophthalmos with some hypoglobus. A CT scan of the sinuses confirmed the diagnosis of SSS, and the patient underwent a right sided endoscopic modified medial maxillectomy/mega-antrostomy with image guidance given the significant maxillary hypoplasia. A literature review of CINAHL, PubMed, and Scopus identified 15 studies that referenced pediatric SSS cases.ConclusionsThis article reports a case of pediatric SSS and compares the presenting symptoms, diagnostic criteria, and surgical intervention to other cases of pediatric SSS. As pediatric SSS is a very rare disease, this case offers a possible treatment strategy for a severely atelectatic maxillary sinus.

Diagnosis and Treatment of Congenital Dilatation of Stensen's Duct

Abstract:OBJECTIVES/HYPOTHESIS:This study aimed to describe the diagnosis and management of congenital dilation of Stensen's duct (CDSD) in seven cases.STUDY DESIGN:Retrospective study.METHODS:We collected data including medical records, radiology, and histopathology findings and follow-up for seven patients (four males) with CDSD. The mean age was 23.9 years (range, 2-72 years).RESULTS:The clinical features of CDSD were the primary presentation of painless swelling in the cheek without an obvious cause that was unilateral or bilateral and occurred at any age, and a swelling along Stensen's duct. In patients without a history of inflammation, aggressive massage of the swelling could produce abundant intraoral salivary flow. Parotid sialography demonstrated a dilated Stensen's duct with a smooth margin but no obvious obstruction. All seven patients underwent superficial parotidectomy including the intact Stensen's duct. No patient showed recurrent swelling after a follow-up of 6 to 65 months.CONCLUSIONS:CDSD is an uncommon congenital disorder of the parotid gland. Management with parotidectomy is effective.

Abstract:Dermatomyositis (DM) is an inflammatory myopathy of unknown cause which affects the skeletal muscles. DM is a rare disease, and the complication of calcinosis is even rarer. In this study, a case of calcinosis with sinuses caused by DM is reported, and the epidemiology, diagnosis, and treatment of calcinosis caused by DM were analyzed.

Abstract:Ear, Nose &Throat Journal, Ahead of Print. This case report describes rare concomitant allergic fungal rhinosinusitis (AFRS) and chronic granulomatous invasive fungal sinusitis (CGIFS) in a 34-year-old woman with acute lymphoblastic leukemia and graft-versus-host disease (GVHD) post bone marrow transplantation. Initially presenting with rhinorrhea and nasal obstruction, the patient was diagnosed with AFRS in the right maxillary sinus, followed by a postoperative course of CGIFS in the left nasal cavity, showcasing the unique occurrence. She was not immunocompromised during diagnosis. CGIFS may have occurred because of surgery; however, voriconazole led to significant improvement. This case highlights noninvasive and invasive fungal infections in patients with chronic rhinosinusitis and a history of GVHD and underscores the complexity of diagnosing and managing such cases.

Abstract:Solitary or isolated involvement of the sphenoid sinus (Isolated sphenoid sinus disease; ISSD ) is a relatively uncommon disease. The present study reviews retrospectively 125 patients with ISSD treated at the Departments of Otolaryngology, Eye Ear Nose and Throat Hospital, Shanghai Medical University (109), and Hyogo College of Medicine (16) over a period of 25 years. Diagnosis was made on the basis of history, signs, nasal endoscopy, imaging techniques with CT/MRI. The final diagnosis of ISSD was established after histopathological and microbiological examinations of the excisional specimen. The pathology was sphenoid sinusitis (40), sphenoid cysts (35), fungal diseases (19), malignant tumors (9), and others (20). The most common initial symptom was headache, followed by visual changes and cranial nerve palsies due to the nearby involvements. Increasing use of routine imaging techniques with CT/MRI and diagnostic nasal endoscopy resulted in the increased numbers of early ISSD. Recent nasal endoscopic ostial sphenoidotomy warrants for the precise pathological diagnosis, and at the same time for the safe and immediate treatment, prior to the sequential extension to adjacent vital structures even to the serious or fatal stage.
