Application of placing the anterior end of endotracheal tube at the glottis in laryngeal mask airway positive ventilation during general anesthesia: A prospective, randomized, controlled trial

Abstract:The laryngeal mask airway (LMA) has many advantages such as easy insertion, minimal stress response, and low incidence of airway complications. [ 1 , 2 ] Although second generation LMA has a good alignment and sealing pressure, and reduces the aspiration risk, it cannot solve the problem caused by LMA end-to-end connection with the trachea. [ 3 ] When there is obstruction in the pathway from the LMA ventilation conduit outlet to the tracheal inlet such as epiglottis downfolding, supraglottic and glottic compression, and laryngospasm, positive pressure ventilation difficulty arises. [ 4 , 5 ] With the attenuation of muscle relaxation effects and increase of laryngeal muscle tension during perioperative period, vocal cord may shift to the middle line. [ 6 ] The reduction of glottic opening increases ventilation resistance. When peak airway pressure (Ppeak) exceeds the oropharyngeal leak pressure (OLP), LMA leakage will occur, which cause inadequate ventilation and increased risk of gas entering the stomach. If surgical stimulation suddenly enhances and the depth of general anesthesia relatively lightens, laryngospasm possibly occurs and glottis closes. [ 7 , 8 ] Glottic closure will cause severe leakage and hypoventilation. In minor surgery, LMA leakage has little impact on ventilation. However, long time LMA leakage leads to hypercapnia in non-minor surgery. In addition, when surgery approaches to complete or has already completed, the LMA leakage can lead to a dilemma where adding sedatives and muscle relaxants inevitably delays recovery and ignoring leads to inadequate ventilation. In order to maintain normal ventilation, LMA is replaced with endotracheal tube (ET) if insufficient ventilation lasts for a long time.
