Abstract:Background: Traumatic brain injury (TBI) is a sudden trauma on the head, in which severe TBI (sTBI) is usually associated with death and long-term disability. MicroRNAs (miRNAs) are potential biomarkers of diverse diseases, including TBI. However, few systematic reviews and meta-analyses have been conducted to determine the clinical value of miRNAs expression in TBI patients. Methods: We conducted this systematic review and meta-analysis study according to Preferred Reporting Items for Systematic Reviews and Meta-Analyses (PRISMA) guidelines. We searched PubMed, Embase, the Cochrane Library, Web of Science, from inception to August 26, 2020. We included articles written in English that have reported on the diagnostic value of miRNAs expression in TBI patients. We excluded studies that did not provided sufficient information to construct the 2×2 contingency table. Results: Eight studies investigating the diagnostic value of miRNA in TBI were analyzed in this study. The overall sensitivity, specificity and area under the curve (AUC) of miRNAs in diagnosis of TBI were 89% [95% confidence interval (CI): 0.84-0.93], 92% (95% CI 0.82-0.97) and 95% (95% CI 0.93-0.97). We found that panels of multiple miRNAs could improve the diagnostic accuracy of TBI. Samples from blood and brain tissue have significantly enhanced diagnostic accuracy, when compared with saliva. The AUC of miRNAs in severe TBI was 0.97, with 91% sensitivity and 92% specificity. Conclusion: This systematic review and meta-analysis demonstrated that miRNAs could be potential diagnostic markers in TBI patients. MiRNAs detected in blood and brain tissue display high accuracy for TBI diagnosis.
The Predictive Role of Systemic Inflammation Response Index in the Prognosis of Traumatic Brain Injury: A Propensity Score Matching Study.

Abstract:BackgroundWe aimed to evaluate the predictive power of systemic inflammation response index (SIRI), a novel biomarker, to predict all-cause mortality in patients with traumatic brain injury (TBI) in the intensive care unit (ICU).MethodsClinical data were retrieved from the Medical Information Mart for Intensive Care-IV (MIMIC-IV) database. Kaplan-Meier (KM) methods and cox proportional hazard models were performed to examine the association between SIRI and all-cause mortality. The predictive power of SIRI was evaluated compared to other leukocyte-related indexes including neutrophils, lymphocytes, monocytes and white blood cells (WBC) by the Receiver Operating Characteristic (ROC)curve for 30-day mortality. In addition, propensity score matching (PSM) was conducted to reduce confounding.ResultsA total of 350 TBI patients were enrolled overall in our study. The optimal cutoff point of SIRI was determined at 11.24 × 109/L. After 1:1 PSM, 66 matched pairs (132 patients) were generated. During the 30-day, in-hospital and 365-day follow-up periods, patients with low SIRI level were associated with improved survival (p < 0.05) compared with patients with high SIRI level. Cox regression analysis identified that higher SIRI values was an independent risk factor for all-cause mortality and results were stable on multiple subgroup analyses. Furthermore, ROC analysis indicated that the area under the curve of SIRI [0.6658 (95% Confidence Interval, 0.5630–0.7687)] was greater than that of neutrophils, monocytes, lymphocytes and WBC. The above results were also observed in the matched cohort.ConclusionIt was suggested that TBI patients with high SIRI level would suffer from a high risk of 30-day, in-hospital and 365-day mortality. SIRI is a promising inflammatory biomarker for predicting TBI patients' prognosis with relatively better predictive power than other single indicators related to peripheral differential leukocyte counts.

Abstract:Objective:To systematically evaluate the diagnostic value of S-100B for intracranial injury in patients with mild traumatic brain injury (mTBI).Methods:PubMed, EMbase, The Cochrane Library, CNKI, CBM, VIP and WanFang Data databases were searched by computer for diagnostic tests about S-100B as predictors of intracranial injury after mTBI from inception to May 27, 2022. Two reviewers independently screened literature according to the inclusion and exclusion criteria, extracted data, and assessed methodological quality of included studies by using the Quality Assessment of Diagnostic Accuracy Studies 2 tool, and the diagnostic value of serum S-100B for mTBI was analyzed by Meta-DiSc software (version 1.4).Results:A total of 21 literature articels involving 8057 patients were included. Meta-analysis showed that diagnostic odds ratio pooled was 5.55 (95%CI: 3.47-8.87), pooled sensitivity was 0.91 (95%CI: 0.88-0.93), pooled specificity was 0.29 (95%CI: 0.28-0.30), pooled positive likelihood ratio was 1.35 (95%CI: 1.25-1.47), pooled negative likelihood ratio was 0.26 (95%CI: 0.16-0.44), and the area under curve of the comprehensive subject working characteristic curve of S-100B diagnosis for intracranial injury with mTBI was 0.760.Conclusion:S-100B has good predictive value in the screening of intracranial injury among mTBI patients, and can be used as a screening tool for identification of traumatic intracranial lesions in patients with mild TBI.
Clinical Evaluation of Prognosis of Severe Posttraumatic Brain Injuries by Using S-100B

Abstract:Objective To observe the concentration changes of plasma S-100B following severe brain injuries so as to quantitatively monitor primary and secondary brain injury and evaluate the prognosis after severe brain injury. Methods A total of 40 patients with severe brain injuries were involved in this study. Serial concentration of S-100B in serum was measured in all cases from day 1 to day 4 after severe brain injury in order to evaluate the prognosis in combination with clinical symptom and Glasgow Coma Scale. Results The concentration of S-100B in serum of both groups were higher than that of normal group. S-100B at 2.00 μg/L within 12 hours after brain injuries was set as the evaluating standard that had specificity of 91%,sensitivity of 72% and relative risk of 4.3. After brain injuries,the concentration of S-100B in all the cases decreased but was still higher than the normal value. Furthermore,the concentrtion of S-100B in the group with poor prognosis was continuously and significantly higher than that in the group with sound prognosis,where a significant difference exists ( P 0.01). Conclusions S-100B is a promising predictor for assessing primary and secondary damage of central nervous system and predicting prognosis after severe brain injury and hence provides an important way to assess the clinical treatment result and disease improvement.
Relationship between cerebrospinal fluidα-Synuclein and prognosis of patients with traumatic brain injury

Abstract:Objective To explore the changes and significance of cerebrospinal fluid α-Synuclein in patients with trau-matic brain injury. Methods Cerebrospinal fluid α-Synuclein and serum S-100B level of 60 healthy persons and 92 pa-tients with traumatic brain injury on the first, third, fifth and seventh days were detected by ELISA method, and its rela-tionship with GCS and GOS were analyzed. Results The cerebrospinal fluid α-Synuclein and serum S-100B level of ob-servation group were significantly higher than those of the control group (t=2.18, 2.50, 2.84, 3.97, 2.33, 2.94, 3.41, 4.52,P<0.05). The α-Synuclein level of medium and severe subgroups were significantly higher than that of light sub-group(t=1.97, 2.04, 3.32, 3.71, 2.16, 2.47, 2.87, 3.67, P<0.05). theα-Synuclein level of severe subgroup was higher significantly than that of medium subgroup (t=2.37, 2.62, 3.79, 4.14, P<0.05). The α-Synuclein level on the first day after TBI had negative correlation with GCS on the first day and GOS at the sixth month (r=-0.82,-0.61,P<0.05). Con-clusion Cerebrospinal fluid α-Synuclein level can be as the index to diagnose the condition of patients with traumatic brain injury.
Serum exosomes miR-206 and miR-549a-3p as potential biomarkers of traumatic brain injury

Abstract:Traumatic brain injury (TBI) is one of the leading causes of death and disability worldwide. However, effective diagnostic, therapeutic and prognostic biomarkers are still lacking. Our research group previously revealed through high-throughput sequencing that the serum exosomes miR-133a-3p, miR-206, and miR-549a-3p differ significantly in severe TBI (sTBI), mild or moderate TBI (mTBI), and control groups. However, convincing experimental evidence is lacking. To solve this problem, we used qPCR in this study to further verify the expression levels of serum exosomes miR-133a-3p, miR-206 and miR-549a-3p in TBI patients. The results showed that the serum exosomes miR-206 and miR-549a-3p showed good predictive value as biomarkers of TBI. In addition, in order to further verify whether serum exosomes miR-206 and miR-549a-3p can be used as potential biomarkers in patients with TBI and to understand the mechanism of their possible effects, we further determined the contents of SOD, BDNF, VEGF, VEGI, NSE and S100β in the serum of TBI patients. The results showed that, serum exosomes miR-206 and miR-549a-3p showed good correlation with BDNF, NSE and S100β. In conclusion, serum exosomes miR-206 and miR-549a-3p have the potential to serve as potential biomarkers in patients with TBI.

Abstract:This study investigated the potentials of hsa_circ_0018401 and miR-127-5p in traumatic brain injury (TBI) diagnosis, stratification and outcome prediction. A retrospective analysis of clinical data and blood samples of n = 109 TBI patients was performed. Expression levels of hsa_circ_0018401 and miR-127-5p were measured using Real-time PCR. The diagnostic values, as well as the values in TBI stratification, of hsa_circ_0018401 and miR-127-5p were assessed by receiver operating characteristic analyses. The prognostic impacts were investigated for one-year endpoint events using multivariable Cox regression analyses and receiver operating characteristic analysis. The target genes for miR-127-5p were predicted. An upregulation of hsa_circ_0018401 and a downregulation of miR-127-5p expression was detected in patients with TBI, and the highest or lowest levels were found in moderate/severe TBI. A negative correlation between miR-423-3p level and Dual luciferase reporter assay verified the binding relationship between hsa_circ_0018401 and miR-127-5p. Hsa_circ_0018401 and miR-127-5p, used alone or combinedly, showed clinical values for TBI diagnosis and stratification, as well as outcome prediction. The proteins for target genes covered TBI-related functions and pathways. Therefore, hsa_circ_0018401 and miR-127-5p could represent promising new biomarkers to identify TBI from healthy, moderate/severe TBI from mild TBI, as well as to predict the TBI outcome.

Serum Biomarkers and Disease Progression in CT-negative Mild Traumatic Brain Injury.

Abstract:Blood proteins are emerging as potential biomarkers for mild traumatic brain injury (mTBI). Molecular pathology of mTBI underscores the critical roles of neuronal injury, neuroinflammation, and vascular health in disease progression. However, the temporal profile of blood biomarkers associated with the aforementioned molecular pathology after CT-negative mTBI, their diagnostic and prognostic potential, and their utility in monitoring white matter integrity and progressive brain atrophy remain unclear. Thus, we investigated serum biomarkers and neuroimaging in a longitudinal cohort, including 103 CT-negative mTBI patients and 66 matched healthy controls (HCs). Angiogenic biomarker vascular endothelial growth factor (VEGF) exhibited the highest area under the curve of 0.88 in identifying patients from HCs. Inflammatory biomarker interleukin-1 beta and neuronal cell body injury biomarker ubiquitin carboxyl-terminal hydrolase L1 were elevated in acute-stage patients and associated with deterioration of cognitive function from acute-stage to 6-12 mo post-injury period. Notably, axonal injury biomarker neurofilament light (NfL) was elevated in acute-stage patients, with higher levels associated with impaired white matter integrity in acute-stage and progressive gray and white matter atrophy from 3- to 6-12 mo post-injury period. Collectively, our findings emphasized the potential clinical value of serum biomarkers, particularly NfL and VEGF, in diagnosing mTBI and monitoring disease progression.
The roles of diffusion tensor imaging and serum biochemical indexes in the evaluation of acute mild traumatic brain injury

Abstract:Objective To investigate the mild traumatic brain injury (mTBI) using diffusion tensor imaging (DTI)in MRI and the serum neuron specific enolase (NSE) and S100B protein expression changes.Methods A total of 84 patients with mild injury and 70 age and sex matched normal volunteers as controls were prospectively admitted to Department of Neurosurgery,Tianjin First Center Hospital between October 2013 and January 2016.All participants underwent Rivermead post-concussion symptoms questionnaire (RPQ) within 7 days of laboratory examination and evaluation.Among them,all patients with mTBI and 50 normal volunteers were included in this study to undergo MRI scanning,including susceptibility weighted imaging (SWI) and diffusion tensor imaging (DTI),within 7 days to compare the imaging characteristics.The patients were divided into 2 groups depending on SWI appearances.The mTBI group 1 consisted of those with positive findings in SWI and the mTBI group 2 consisted of the other patients.The differences in RPQ scores and DTI data,including fractional anisotropy (FA) and apparent diffusion coefficient (ADC),was analyzed between those 2 groups.The expression levels of serum NSE and S100B protein in the 34 patients with mTBI and 20 normal volunteers.Results Among the 84 patients,there were 31 patients with abnormal SWI at admission and 53 patients with normal SWI,and their median RPQ scores were 17 (1-31) and 15 (22-27) respectively.There was no significant difference between the 2 groups (P =0.116).At 6 months post discharge,20 cases in abnormal SWI group and 33 cases in normal SWI group were followed up,and their RPQ scores were 6 (1-15) and 3 (1-11) respectively(P =0.177).DTI analysis showed that there were 32 abnormal FA areas in SWI abnormal group and 7 in SWI normal group.In addition,there were 34 ADC abnormal areas in SWI abnormal group and 7 in SWI normal group.Those differences were statistically significant (P =0.001,P ＜0.001).Serum NSE levels were 10.01 ±2.82 μg/ L,30.51 ± 12.32 and 12.35 ± 3.67 μg/L,S100B protein levels were 0.17 ± 0.06 μg/L,0.52 ± 0.26 μg/L and 0.19 ±0.01 μg/L in healthy control group,abnormal SWI group and normal SWI group,respectively.There were significant differences in NSE and S100B protein levels among the 3 groups (P =0.006,P =0.025),while there was no significant difference between the healthy control group and the normal SWI group (all P ＞ 0.05).Conclusion The combined detection of serum NSE and S100B protein by cranial DTI may have important clinical value and significance in accurate diagnosis and treatment of acute mTBI.
Association between Blood and Computed Tomographic Imaging Biomarkers in a Cohort of Mild Traumatic Brain Injury Patients

Abstract:The objective of this work was to analyze the relationships between traumatic brain injury (TBI) on computed tomographic (CT) imaging and blood concentration of glial fibrillary acidic protein (GFAP), ubiquitin C-terminal hydrolase-L1 (UCH-L1), and S100B. This prospective cohort study involved 644 TBI patients referred to Stanford Hospital's Emergency Department between November 2015 and April 2017. Plasma and serum samples of 462 patients were analyzed for levels of GFAP, UCH-L1, and S100B. Glial neuronal ratio (GNR) was calculated as the ratio between GFAP and UCH-L1 concentrations. Admission head CT scans were reviewed for TBI imaging common data elements, and performance of biomarkers for identifying TBI was assessed via area under the receiver operating characteristic curve (ROC). We also dichotomized biomarkers at established thresholds and estimated standard measures of classification accuracy. We assessed the ability of GFAP, UCH-L1, and GNR to discriminate small and large/diffuse lesions based on CT imaging using an ROC analysis. In our cohort of mostly mild TBI patients, GFAP was significantly more accurate in detecting all types of acute brain injuries than UCH-L1 in terms of area under the curve (AUC) values (p < 0.001), and also compared with S100B (p < 0.001). UCH-L1 and S100B had similar performance (comparable AUC values, p = 0.342). Sensitivity exceeded 0.8 for each biomarker across all different types of TBI injuries, and no significant differences were observed by type of injury. There was a significant difference between GFAP and GNR in distinguishing between small lesions and large/diffuse lesions in all injuries (p = 0.004, p = 0.007). In conclusion, GFAP, UCH-L1, and S100B show high sensitivity and negative predictive values for all types of TBI lesions on head CT. A combination of negative blood biomarkers (GFAP and UCH-L1) in a patient suspected of TBI may be used to safely obviate the need for a head CT scan. GFAP is a promising indicator to discriminate between small and large/diffuse TBI lesions.
Role of Diffusion Tensor Imaging Combined with Neuron-Specific Enolase and S100 Calcium-binding Protein B Detection in Predicting the Prognosis of Moderate and Severe Traumatic Brain Injury

Abstract:Background: Traumatic brain injury (TBI) seriously affects the quality of life of patients. The present study evaluated the role of diffusion tensor imaging (DTI) combined with Neuron-Specific Enolase (NSE) and 5100 calcium-binding protein B (S100B) protein in predicting the prognosis of moderate and severe TBI. Methods: The TBI patients were divided into moderate TBI (TBIm) and severe TBI (TBIs) groups according to the Glasgow Coma Scale (GCS) after admission. The patients were then divided into good and poor prognosis groups according to the Glasgow Outcome Scale (GOS); moreover, their follow-ups were recorded at 3 and 6 months after injury. This study also included 65 healthy individuals with matched age and gender as the control group. The fractional anisotropy (FA) values of DTI, serum neuron-specific enolase (NSE), and S100B protein levels were detected in this study. The data were analyzed in SPSS software (version 22.0) to evaluate the role of DTI combined with NSE and S100B protein in predicting the prognosis in TBIm and TBIs. Results: After TBI, the FA values of DTI in the TBI group were lower than those in the control group (P<0.05); moreover, the serum NSE and S100B values in the TBI group were higher than those in the control group (P<0.05). In the TBIm patients, the FA values of the corpus callosum in the good prognosis group were higher than that in the poor prognosis group (P<0.05); however, there was no significant difference between the two groups regarding the FA values of the internal capsule and the cerebral peduncle (P>0.05). The serum levels of NSE and S100B in the good prognosis group were significantly lower than those in the poor prognosis group (P<0.05). In the TBIs patients, the FA value of all areas in the good prognosis group was significantly higher than that in the poor prognosis group (P<0.05). However, there was no significant difference between the two prognosis groups regarding the serum levels of NSE and S100B (P>0.05). Conclusion: Although DTI combined with NSE and S100B protein can effectively predict the prognosis of patients with moderate and severe TBI in the early stages, various other measures have been used in the studies to predict the prognosis of TBI patients. Accordingly, comparison with other measures is essential in further studies.
Study on Serum Mir-185-5p in Assessing the Injury Severity and Prognosis of Patients with Traumatic Brain Injury.

Abstract:Background:This study aims to explore whether serum miR-185-5p levels are related to the injury severity and prognosis of traumatic brain injury patients.Methods:Serum miR-185-5p level was quantified in 120 TBI patients. The Glasgow Coma Scale (GCS) was used to grade the damage, and the Glasgow Outcome Scale (GOS) was used to evaluate the prognosis 3 months after TBI. Pearson correlation analysis was performed to determine the relationship between serum miR-185-5p level and injury severity and prognosis, and the value of serum miR-185-5p level to assess injury severity and prognosis was evaluated by receiver operating characteristic (ROC) curve.Results:Serum miR-185-5p level in moderate and severe TBI patients was higher than in mild TBI patients, and serum miR-185-5p was closely related to GCS score and GOS score. Serum miR-185-5p level higher than 0.36 could distinguish patients with mild to moderate TBI injury, with 72.97% sensitivity and 97.62% specificity, while that higher than 0.43 had 46.34% sensitivity and 91.89% specificity to distinguish moderate to severe TBI patients. Moreover, serum miR-185-5p levels higher than 0.36, with a sensitivity of 96.30% and a specificity of 60.24%, distinguished the poor prognosis of TBI patients. Serum miR185-5p level was an independent predictor of poor prognosis in TBI patients after 3 months and was effective in discriminating adverse outcomes at 3 months.Conclusions:Serum miR-185-5p level was significantly correlated with 3-month injury and adverse prognosis in TBI patients, suggesting that serum miR-185-5p level may be a biomarker that provides supplementary prognostic information and can be used to identify the risk of adverse prognosis in TBI patients.

Abstract:In forensic pathology, traumatic brain injury (TBI) is a frequently encountered cause of death. Unfortunately, the statistic autopsy data, risk investigation about injury patterns, and circumstances of TBI are still sparse. Estimates of survival time post-TBI and postmortem diagnosis of TBI are especially important implications in forensic medicine. Neurogranin (Ng) and myelin basic protein (MBP) represent potential biomarkers of TBI. The present study analyzed retrospectively the forensic autopsy records of TBI cases at a university center of medico-legal investigation from 2008 to 2020. Immunohistochemistry and enzyme-linked immunosorbent assays (ELISA) were used to investigate the expression changes of Ng and MBP in the cortical brain injury adjacent tissues and serum, respectively, from cases of TBI at autopsy with different survival times post-TBI. The results show that the major mechanism of death of TBI is assault, and accident was the major manner of death. Ng and MBP are mainly expressed in the cortical nerve cells and the myelin sheath, respectively. The serum levels of Ng and MBP in each TBI group were higher compared with those in the controls. The brain cortical levels of Ng and MBP decreased at first and then steadily increased with extended survival time post-TBI. The immunopositive ratios and serum concentration of Ng and MBP have shown significant differences among control group and all TBI group (p < 0.001). Collectively, the immunohistochemical analyses of Ng and MBP in human brain tissues may be useful to determine the survival time after TBI, and Ng and MBP level in the human blood specimens could be considered as a postmortem diagnostic tools of TBI in forensic practice.
Effect of Non-Cerebral Trauma on the Value of Serum S100B

Abstract:Objective The level of serum S100B protein is an important serum marker to evaluate traumatic brain injury,however,whether extracerebral trauma can affect serum S100B protein remains controversial,the aim of this study is to assess whether extracerebral trauma can affect the value of serum S100B protein measurements.Methods 325 patients of trauma from the emergency department and emergency observation of our hospital as the objects of this study,basic information of patients and blood samples were collected at the corresponding time points.According to has brain injury or not,the patients were divided into IHI group,NHI group,MTHI group.We analysis serum S100B protein content of the three groups,and analysis the effects of long bone fractures,hemorrhagic shock,ISS score of serum S100B protein in NHI group.Results Three hours after the injury,of patients from MTHI group are highter than the IHI group and the the NHI group(P0.01),and there is no statistically difference(P=0.126) between IHI group and the NHI group about the serum S100B protein,while in 6 hours after the trauma,the serum S100B protein level are elevated(P0.001),and also found that the serum S100B protein of MIHI group are highter than the IHI group and the the NHI group(P0.01),while IHI group the NHI group compared no statistically difference(P=0.217).In NHI group,the serum S100B protein level of patients with long bone fractures is higher than the non-fracture patients,the serum S100B protein level of patients with hemorrhagic shock is higher than the non-shock patients,the serum S100B protein level of patients with ISS score16 points is higher than ISS score 16 points.Conclusion Extracerebral trauma can affect the value of serum S100B protein measurements.Hemorrhagic shock,long bone fractures,and injury severity scores can affect the level of serum S100B of extracerebral trauma.
Role of S-100B,NSE and MBP in Prediction of Prognosis in Patients with Severe Head Injury

Abstract:Objective To elucidate the role of S-100B, NSE and MBP in predicting the prognosis in patients with severe head injury. Methods The concentrations of S-100B, NSE and MBP in serum were measured within 12 hours after the injury in 40 patients with severe head injury who were treated in our department from January to December 2002. And outcomes of all the patients were evaluated by GOS 2 months after the injury. Results The serum concentrations of S-100B, NSE and MBP after the injury in 40 patients with severe head injury were higher than those in the normal control group. There were significant difference in the serum concentrations of s-100B, NSE and MBP between the good prognosis group and the poor prognosis group. The their concentrations within 12 hours after head injuries were closely related with the prognosis. Furthermore, according to the ROC curves of S-100B, NSE and MBP, S-100B was more valuable for predicting prognosis in the patients with severe head injury. Conclusions The present results suggest that S-100B, NSE and MBP may play important roles in predicting the prognosis in the patients with severe head injury. But S-100B may be a more promising predictor of the prognosis in the patients with severe head injury.

Abstract:Background: Decreased serum netrin-1 concentrations have been found after acute brain injury. We investigated the role of serum netrin in prognosis of traumatic brain injury (TBI). Methods: In this prospective and observational study, enzyme-linked immunosorbent assay was used to detect serum netrin-1 concentrations in 50 mild TBI patients (Glasgow coma scale (GCS) score, 13-15), 83 moderate TBI patients (GCS score, 9-12), 69 severe TBI patients (GCS score, 3-8) and 50 healthy controls. Glasgow outcome scale score of 1-3 at 6 months after trauma was defined as poor outcome. Results: Serum netrin-1 concentrations were significantly lower in moderate or severe TBI patients than in controls and in severe TBI patients than in moderate TBI patients, while not in mild TBI patients than in controls. GCS score and Rotterdam computed tomography classification were closely correlated with serum netrin-1 concentrations among TBI patients. Forty-two (20.8%) patients had poor outcome. Receiver operating characteristic curve analysis revealed that serum netrin-1 concentrations could distinguish patients with poor outcome from the other remainders significantly. In addition, serum netrin-1 concentrations were independently associated with poor outcome. Conclusions: Serum netrin-1 might serve as a potential biomarker for prognosis of TBI.

Abstract:Objective To investigate the changes in the serum levels of neuron specific enolase (NSE) and matrix metalloproteinases-2 (MMP-2) and matrix metalloproteinases-9 (MMP-9) and their clinical significance in the patients with severe traumatic brain injury.Methods A prospective control study was carried out.The serum from 62 (including 6 patients who died in 6 months) TBI patients and 20 normal persons were collected to measure the levels of NSE,MMP-2 and MMP-9 by using ELISA and analyzed the change trend of them together with Glasgow Outcome Scale.Results The serum levels of NSE,MMP-2 and MMP-9 were significantly higher in the brain trauma group than those in the control group(P ＜0.0l).The serum levels of NSE peaked on 5th day,although dropping on the 7th day after brain trauma,was significantly higher in the brain trauma group than that in the control group.The serum levels of MMP-2,MMP-9 peaked on 1 st day and the second peak appeared on 5th day.The serum levels of NSE,MMP-2 and MMP-9 were significantly higher in the patients with poor prognoses group than those with good prognoses group(P ＜ 0.01).Conclusions NSE,MMP-2 and MMP-9 of the serum were involved in the pathological process of acute brain injury.Although there are several serious limitations of the use of NSE,MMP-2 and MMP-9 as a biomarker in traumatic brain injury,their concentrations may be useful as a practical and helpful screening test to identify neurotrauma patients who are at increased risk.
Diagnostic and Prognostic Values of S100B versus Neuron Specific Enolase for Traumatic Brain Injury; a Systematic Review and Meta-analysis

Abstract:Introduction: Traumataic brain injury (TBI) represents a significant global health burden. This systematic review delves into the comparison of S100B and Neuron-Specific Enolase (NSE) regarding their diagnostic and prognostic accuracy in TBI within the adult population. Methods: Conducted on October 21, 2023, the search identified 24 studies encompassing 6454 adult patients. QUADAS-2 and QUAPAS tools were employed to assess the risk of bias. The analyses aimed to evaluate the diagnostic and prognostic performance of S100B and NSE based on sensitivity, specificity, and area under the curve (AUC). The outcomes were detecting intracranial injury, mortality, and unfavorable outcome. Results: Pooled data analysis tended towards favoring S100B for diagnostic and prognostic purposes. S100B exhibited a diagnostic AUC of 0.74 (95% confidence interval (CI): 0.70-0.78), sensitivity of 80% (95% CI: 63%-90%), and specificity of 59% (95% CI: 45%-72%), outperforming NSE with an AUC of 0.66 (95% CI: 0.61-0.70), sensitivity of 74% (95% CI: 53%-88%), and specificity of 46% (95% CI: 24%-69%). Notably, both biomarkers demonstrated enhanced diagnostic value when blood samples were collected within 12 hours post-injury. The analyses also revealed the excellent diagnostic ability of S100B with a sensitivity of 99% (95% CI: 4%-100%) and a specificity of 76% (95% CI: 51%-91%) in mild TBI patients (AUC = 0.89 [0.86-0.91]). In predicting mortality, S100B showed a sensitivity of 90% (95% CI: 65%-98%) and specificity of 61% (95% CI: 39%-79%), slightly surpassing NSE's performance with a sensitivity of 88% (95% CI: 76%-95%) and specificity of 56% (95% CI: 47%-65%). For predicting unfavorable outcomes, S100B exhibited a sensitivity of 83% (95% CI: 74%-90%) and specificity of 51% (95% CI: 30%-72%), while NSE had a sensitivity of 80% (95% CI: 64%-90%) and specificity of 59% (95% CI: 46%-71%). Conclusion: Although neither biomarker has shown promising diagnostic performance in detecting abnormal computed tomography (CT) findings, they have displayed acceptable outcome prediction capabilities, particularly with regard to mortality.

Abstract:The present study aimed at the predictive value of early release patterns of protein S-100B and neuron specific enolase (NSE) in patients with traumatic brain injury. We investigated 69 patients who were admitted to the Department of Neurosurgery following traumatic brain injury. Both NSE and S-100B serum concentrations during the first three days after admission were highly and significantly correlated with Glasgow Coma and Coma Remission Scale scores at the respective blood sampling times as well as 2 weeks later. Signs of intracranial pathology as evaluated by CCT or MRI scans showed no association with NSE or S-100B release patterns. Our data support the hypothesis that NSE and protein S-100B are useful and sensitive neurobiochemical markers for the early clinical outcome of traumatic brain injury.
