Wuling San Ameliorates Cisplatin-Induced Nephrotoxicity in Mice by Regulating Oxidative Stress, Inflammation, and Apoptosis Pathways

Abstract:Ethnopharmacological relevance: As cis-diamminedichloroplatinum [cisplatin (CDDP)] may induce nephrotoxicity, its clinical application as an anticancer drug is seriously limited. Currently, the prevention and treatment of CDDP-induced nephrotoxicity are inadequate. In China, patients often opt for traditional Chinese medicine to mitigate CDDP-induced nephrotoxicity. In clinical trials, Wuling San has been a significantly effective treatment for CDDP-induced nephrotoxicity. However, its mechanisms of action remain unclear.Aim of the studyThe objectives of this investigation were to: (a) establish whether Wuling San exerts nephroprotective effects in mice presenting with oxidative stress, inflammation, and apoptosis after CDDP exposure and (b) elucidate its therapeutic mechanisms.Materials and methods: Mice were randomly allocated into the following groups: control, CDDP, CDDP + Wuling San (13 g/kg), CDDP + Wuling San (26 g/kg). The treatments were administered by gavage for 10 consecutive days. On day 7, 22 mg/kg CDDP was intraperitoneally administered to elicit nephrotoxicity. Serum blood urea nitrogen (BUN) and creatinine (CRE) levels were measured. Pathological changes, kidney index, and renal tubular injury were observed. The following oxidative stress markers were analyzed, malondialdehyde (MDA), glutathione (GSH), superoxide dismutase (SOD), and catalase (CAT). Dihydroethidium (DHE), cytochrome P450 E1 (CYP2E1), 4-hydroxynonenal (4-HNE), and heme oxygenase-1 (HO-1) fluorescence intensities were detected in the kidney tissues. Inducible nitric oxide synthase (iNOS) levels were analyzed by immunofluorescence staining. Cyclooxygenase-2 (COX-2), nuclear factor kappa-B (NF-κB) p65, B-cell lymphoma-2 (Bcl-2), and Bcl-2-associated X (Bax) levels were measured using immunohistochemistry and western blotting analyses. The mRNA levels of tumor necrosis factor-α (TNF-α) and interleukin-1β (IL-1β) were measured using qPCR. Apoptosis was confirmed and quantified by TUNEL staining. Cytosol CytC and caspase-9/7/3 levels were determined using western blotting.Results: Wuling San pretreatment reduced serum BUN and CRE levels, alleviated histopathological injury, and lowered the kidney indices and tubular injury scores in the mice exposed to CDDP. In the renal tissues, Wuling San pretreatment reduced MDA, 4-HNE, DHE, HO-1, and CYP2E1 levels and increased GSH, SOD, and CAT levels. Furthermore, Wuling San pretreatment downregulated the protein levels of iNOS, COX-2, and NF-κB p65 and mRNA levels of TNF-α and IL-1β. It also significantly reduced TUNEL density; diminished the levels of Bax, caspase-9/7/3, cleaved caspase-9/7/3, and cytosolic CytC; and augmented Bcl-2 expression.Conclusions: Wuling San ameliorated CDDP-induced nephrotoxicity in mice by mitigating oxidative stress, NF-κB-mediated inflammation, and apoptosis. This research provides a theoretical and empirical basis for the clinical application of Wuling San in patients with cancer undergoing CDDP chemotherapy.
