Abstract:Aim • Brief description of the three classifications of headaches – Primary, Secondary, Neuropathies/Facial • Focus on Medication Overuse Headaches (M.O.H.) – Secondary type • Learn the evaluation process for determining an M.O.H. • Discuss treatment options – acute, preventative, alternative • Review differential diagnoses and headache red flags • Review & discuss specific patient scenarios Background Medication overuse headache is the term applied to: • Headaches occurring on 15 or more days per month developing as a consequence of regular overuse of acute headache medication for more than three months. • Preceded by episodic headache pattern – migraine or tension – using frequent and/or excessive amounts of acute symptomatic medications. • It usually, but not invariably, resolves after the overuse is stopped. • MOH has also been termed analgesic rebound headache, drug-induced headache, and medication-misuse headache. • Affects 1-2% general population • Accounts for 50-80% patients referred to specialist headache clinic • Women 3:1 men • Risk factors • Genetic predisposition: • Central sensitization after chronic exposure to opiates, triptans, other analgesics • Bio-behavioral factors • Cutaneous allodynia (weakness, numbness, pain) • Frequency of medication use = OVERUSE >> refer to diagnostic criteria • HIGH risk – Intermediate to LOW risk – LOW risk – 10% NSAIDs (data conflicting) Method Treatment Options: Acute treatment Preventative treatment Alternative treatment Results • Understand the three classifications of headaches – Primary, Secondary, Neuropathies/Facial • Better understanding of Medication Overuse Headaches (M.O.H.) – Secondary type • Understand the evaluation process for determining an M.O.H. • Reviewed treatment options – acute, preventative, alternative • Reviewed differential diagnoses and headache red flags • Practiced specific patient scenarios Conclusions Education for patients and families are KEY.
