Effect of Fixed and Removable Functional Therapy on Mandibular Anterior Bone Structures: A Fractal Analysis Study

Abstract:(1) Background and aim: The effects of functional therapies on dentoalveolar and skeletal structures have been investigated in orthodontics for many years. The aim of this retrospective study was to evaluate the changes caused by fixed and removable functional therapy in the mandibular anterior trabecular structures using fractal dimension (FD) analysis. (2) Methods: A total of 60 patients with skeletal and dental class II malocclusion were included in the study and three groups were formed: the untreated control group (CG), the Forsus fatigue-resistant device group (FFRDG), and the Monoblock group (MBG). Bone areas of interest determined in the buccoapical of the mandibular incisors and the symphysis in the lateral cephalometric radiographs taken before (T0) and after (T1) functional therapy were evaluated using FD analysis. The relationship between the FD and IMPA (Incisor Mandibular Plane Angle) angles was evaluated. Parametric and nonparametric tests were used in statistical analysis according to normality distribution. The statistical significance level was determined as p < 0.05. (3) Results: There was no statistically significant difference between the FD values of all groups at T0 (p > 0.05). At T1, buccoapical FD values were significantly lower in FFRDG and MBG compared to the control group (p < 0.05), while symphyseal FD values were not found to be significant (p > 0.05). The IMPA angle was significantly lower in the FFRDG and MBG than in the control group at T0, while it was higher at T1 (p < 0.05). While a significant negative correlation was observed between the IMPA angle and buccoapical FD values in both FFRDG and MBG (p < 0.05), it was not observed with the symphysis FD values (p > 0.05). (4) Conclusions: Trabecular changes caused by functional therapy in the mandibular anterior bone can be evaluated on lateral cephalometric radiographs with FD analysis. It was concluded that orthodontists should ensure controlled changes in the IMPA angle during functional therapy, especially for the decreases in FDs seen in the buccoapical alveolar region due to the forward movement of the mandibular incisors.
Evaluating the effects of orthodontic treatment with clear aligners and conventional brackets on mandibular condyle bone quality using fractal dimension analysis of panoramic radiographs

Abstract:Objective: Orthodontic treatment carried out with clear aligners has become a method that is preferred by patients. In this study, the effect following orthodontic treatment with clear aligners and with conventional brackets, on the mandibular condyle trabecular bone formation is aimed to be examined. Materials and methods: Thirty two individuals (16 females and 16 males) with a mean age of 16.80 ± 3,10 years who received orthodontic treatment with clear aligners and conventional brackets were included in the study. Individuals included in the study from the archive data were divided into two groups according to treatment types. The mandibular condyle bone quality in the panoramic images taken from the patients before orthodontic treatment (T0) and after orthodontic treatment (T1) has been compared retrospectively. This examination is carried out via the fractal analysis method. Result: The mandibular condyle bone density in individuals that have undergone orthodontic treatment with clear aligners, has been reduced following orthodontic treatment. In individuals that have undergone orthodontic treatment with conventional brackets however the mandibular condyle bone density has increased following treatment (T0) in comparison to before being treated (T1). The mandibular condyle bone density change according to different treatment groups has been found to be statistically different. Conclusion: Orthodontic treatment with clear aligners provides several advantages to patients. However, besides determining the effectiveness of the treatment, complications must also be examined. Application of clear aligners in orthodontic treatment may cause a decrease in bone density in the mandibular condyle. When evaluating orthodontic treatments carried out with clear aligners, the change of the mandibular condyle head bone should be taken into consideration.

Abstract:A BSTRACT Background: Recognizing the oral needs of people living with rheumatic heart disease (RHD), particularly in relation to their jaws, is of paramount importance. Aims: This study aimed to evaluate potential jaw bone changes in children and adolescents diagnosed with RHD and compare them with a group of healthy children and adolescents. Methods and Material: The dental panoramic radiographs (DPRs) of 32 children and adolescents diagnosed with RHD comprised the case group. In contrast, the control group consisted of DPRs from 32 children and adolescents without any known diseases. Two regions of interest (ROI1 and ROI2) of trabecular bone and a separate region of interest (ROI3) of cortical bone were selected for fractal analysis. The mandibular cortical index (MCI) was evaluated. Results: The ROI1 fractal dimension (FD) value of the individuals in the case group was statistically higher in the right jaw than in the left jaw ( P < 0.01). Significant differences were determined between the case and control groups regarding the left ROI1, and the left and the right ROI2 ( P < 0.05). No significant difference was found between the patient and control groups regarding the MCI value ( P > 0.05). Conclusions: The results of this study showed that bone metabolism was affected unilaterally in the condyle region and bilaterally in the gonial region of patients with RHD. It also revealed that the cortical layer was not affected.

Abstract:Objective: Fractal analysis (FA) is a non-invasive method that quantitatively measures complex patterned geometric structures present throughout the image. Trabecular morphology of the alveolar bone and the changes occurring in the trabeculae in case of periodontitis can be detected with this method. To examine the periodontal defects in human skull bones using the FA, to compare them with healthy alveolar bone regions. Methods: Furcation and intrabony defects were artificially created in the mandible alveolar bones (n:24). Periapical X-ray images of alveolar bone regions containing teeth with defects were taken using the parallel technique. Fractal analysis was performed by box-counting method using Image J software on images from areas containing healthy and defective trabecular bone. Results: No statistically significant difference was found between the fractal values of healthy tissue and bifurcation defetcs and between the healthy tissue and intrabony defects (p>0.05). Conclusion: Many factors may have affected the outcomes; patient selection, imaging methods, sample size, Region of interest (ROI) selection-location and size, individual and anatomical variations. These variables need to be standardized as much as possible and the limitations of the method need to be improved.

Abstract:Objectives: Primary hyperparathyroidism (HPT) is a condition caused by an overproduction of parathormone, in excess of the amount required by the body. Its most common cause is a parathyroid gland adenoma and parathyroidectomy is currently the only curative treatment for primary HPT. We present a case history of a 65-year-old patient who was diagnosed with primary HPT after the recognition of dental problems. Methods: Dental complaints of the patient alerted the dentists, and the patient was referred for further medical evaluation. In addition to his current medical status, his medical records including the biochemical parameters of bone metabolism recorded between 2001 and 2006 were reviewed. The panoramic films of the patient obtained between 1997 and 2008 were also assessed with the fractal analysis method. Results: After consideration of the radiographical, biochemical and clinical evaluations of the patient, the final diagnosis was made as hyperparathyroid adenoma and surgical removal was scheduled. Soon after surgery, alkaline phosphatase, calcium and intact parathormone levels returned to normal. The fractal analysis value of the mandibular alveolar bone also increased. Conclusions: Dentoalveolar changes observed in HPT include alveolar bone demineralization. The fractal dimension (FD) analysis of the bone tissue has been introduced as an alternative method to investigate the quality of the alveolar bone. FD values of the patient showed osteoporotic bone characteristics between 1997 and 2006 until the date of parathyroidectomy. Mandibular bone FD analyses revealed a prominent development, which was also observed in dual energy X-ray absorptiometry values.

Abstract:Background: Bruxism may cause increased alveolar bone thickness and density and irregular enlargement of the periodontal space. Aim: This study aimed to evaluate the mandibular bone quality using radio-morphometric indices and Fractal Dimension (FD) analysis in orthopantomography (OPG). Material and Methods: OPGs of 100 patients, 50 bruxers and 50 non-bruxers, were included in this study. Values, such as mental index (MI), panoramic mandibular index (PMI), gonial index (GI), antegonial notch depth (AND), mandibular cortical index (MCI), and antegonial index (AI), were calculated in OPG. Eight bilateral areas of interest (ROI) were selected on ort for FD analysis: ROI 1, mandibular condyle; ROI 2, mandibular ramus; ROI 3, mandibular angulus; and ROI 4, mandibular mental area. Results: MI, PMI, and AND values were higher in bruxers than in the control group (p 0.05). As a result of the calculations, the FD values of the left condyle (p=0.02) and left angulus (p=0.03) areas showed a statistically significant difference between individuals with and without bruxer. No statistically significant difference was found in the FD measurements calculated from the ramus and mental areas on the right and left sides (p>0.05). The relationship between FD values and gender in these areas was examined, and no statistically significant difference was found (p<0.05). Conclusion: In dentistry, bruxism can be diagnosed and treated by measuring MI, PMI, and AND values. No difference was found in mandibular cortical bone thickness in bruxers and non-bruxers, according to AI and MCI. The mean GI measured on the right side differed between groups. FD values of the mandibular trabecular bone were affected by bruxism in the right condyle and right angulus areas.

Abstract:Background : This study aimed to evaluate the mandibular bone structure of patients using oral anticoagulants (OACs) vitamin K antagonist drugs (warfarin) and other OACs including direct oral anticoagulants [(DOACs) apixaban, rivaroxaban, dabigatran, edoxaban]. Analyses were based upon the fractal dimension (FD), the panoramic mandibular index (PMI) and the Klemetti index (KI), which is also known as the mandibular cortical index (MCI). Methodology : Ninety participants were divided into three groups: group 1: 30 systemically healthy individuals who had not used any anticoagulants before, group 2: 30 individuals using warfarin, and group 3: 30 individuals using DOACs. FD was used to analyze trabecular bone architecture in the condyle, angle, and two sites in the alveolar bone. PMI was used to evaluate the quantity of cortical bone and KI was used to evaluate the cortical bone quality. Results : There was no difference between the groups regarding FD analysis and KI; however, a difference was found between groups 1, 2, and 3 in the PMI ( P ≤ 0.001). The PMI in group 1 was higher than in groups 2 and 3. Conclusion : Mandibular radiomorphometric indices can be used on panoramic radiographs to evaluate the quantity of mandibular cortical bone in patients using oral anticoagulants.

Abstract:Abstract Purpose The present study aimed at evaluating the effectiveness of fractal analysis on determining the osseointegration of dental implants. Material and methods In a single center, retrospective clinical trial, patients with dental implants in the mandibular premolar/molar region, ASA I–II and < 65-year-old patients were included. Orthopantomograph (OPG) were taken before implant surgery (t0), within a week of surgery (t1), and 1 (t2) and 2 (t3) months after surgery, respectively. Three regions of interest (ROIs) from mesial, distal, and apical sites of the implants were chosen and fractal analysis (FA) was conducted with the box-counting algorithm using White and Rudolph’s method. Results A total of 39 patients 19 women and 20 men, with a mean age of 52.2 years (52.3 and 52.1 years, respectively) were included. The mean, minimum and maximum values of mesial (roi1), distal (roi2), and apical (roi3) surfaces were compared. The fractal dimension (FD) values of t1 were significantly lower compared with t0 as they decreased during the first week. FD values gradually increased after the first week although never exceeded the FD values of t0. Also, difference between mean FD values of t0 and t3 were found statistically significant ( p < 0.05). Discussion FA is a promising and noninvasive method to predict osseointegration of a dental implant based on dental radiographs, and it can help shorten the total treatment time.

Abstract:Objectives: To assess the impact of the presence or position (buccal/palatal) of impacted canines on trabecular bone density using fractal analysis (FA) on cone-beam computed tomography (CBCT) images, and to compare the results with a control group without impacted canines. Methods: This retrospective study included 41 patients with unilateral impacted canines (30 palatal, 11 buccal) and a control group of 39 patients who underwent surgically assisted rapid maxillary expansion. All patients had CBCT images recorded for diagnostic and treatment purposes. Cross-sectional CBCT images were obtained between the first and second premolars on both sides of the patients' maxilla. From these images, fractal dimension (FD) was measured in a 20 × 20 pixel region of interest in the trabecular bone using the ImageJ software. Results: The FD values were significantly higher on the impacted side in the impacted canine group (p = 0.02). Within the impacted canine group, a significant increase in FD was observed on the impacted side in the buccal-impacted subgroup (p = 0.02), while no significant difference was observed in the palatal-impacted subgroup (p 0.05). Conclusions: According to the results of our study, there is an association between the position of the impacted canine and trabecular bone density. An increased trabecular bone density may play a role in the etiology of buccally impacted canines. Clinicians should consider anchorage planning, and appropriate force level, during the forced eruption of buccally impacted canines with high surrounding bone density, to minimize undesirable movements and achieve optimal treatment outcomes.

Fractal analysis of mandibular trabecular pattern following protraction facemask therapy : A pilot study

Abstract:Objective: To assess the remodeling effects of protraction facemask therapy on the trabecular pattern of the mandible and identify quantitatively the regions of the mandible undergoing maximum changes in the trabecular pattern. Materials and methods: The retrospective study was conducted in 30 subjects divided into two groups. The treatment group (group 1) consisted of 15 subjects with skeletal class III malocclusion treated with a facemask (mean age: 9.1 ± 2.1 years). Digital panoramic radiographs (OPG) were obtained before treatment (T0) and at the end of active facemask therapy (T1) with a mean duration of 16.1 ± 2.5 months. In all, 15 skeletal class I subjects who did not undergo any orthodontic treatment (mean age 9.1 ± 2.2 years) comprised the control group (group 2) who had two OPGs at a mean interval of 13.6 ± 2.2 months. The fractal dimensional (FD) value was calculated in three regions: angular, condylar, and corpus region. Results: Intragroup comparisons revealed a significant increase in the FD values in the right condylar (T0 = 1.2 ± 0.2; T1 = 1.4 ± 0.1) and left condylar (T0 = 1.2 ± 0.1; T1 = 1.4 ± 0.1) region (p < 0.05) and in the corpus region (FD values: right T0 = 1.2 ± 0.1, T1 = 1.4 ± 0.1; left T0 = 1.2 ± 0.1, T1 = 1.4 ± 0.1; p < 0.05). The intergroup comparison demonstrated that the treatment group showed greater changes compared with the controls in the condylar process (mean difference: right = 0.19, left = 0.20; p < 0.05) and the corpus region (0.172; p < 0.05). Conclusion: FD analysis demonstrated that the condylar process and the corpus region underwent the most significant change in the trabecular pattern in subjects treated with protraction facemask therapy.
New methodology for evaluating osteoclastic activity induced by orthodontic load

Abstract:Orthodontic tooth movement (OTM) is a dynamic process of bone modeling involving osteoclast-driven resorption on the compression side. Consequently, to estimate the influence of various situations on tooth movement, experimental studies need to analyze this cell. Objectives The aim of this study was to test and validate a new method for evaluating osteoclastic activity stimulated by mechanical loading based on the fractal analysis of the periodontal ligament (PDL)-bone interface. Material and Methods The mandibular right first molars of 14 rabbits were tipped mesially by a coil spring exerting a constant force of 85 cN. To evaluate the actual influence of osteoclasts on fractal dimension of bone surface, alendronate (3 mg/Kg) was injected weekly in seven of those rabbits. After 21 days, the animals were killed and their jaws were processed for histological evaluation. Osteoclast counts and fractal analysis (by the box counting method) of the PDL-bone interface were performed in histological sections of the right and left sides of the mandible. Results An increase in the number of osteoclasts and in fractal dimension after OTM only happened when alendronate was not administered. Strong correlation was found between the number of osteoclasts and fractal dimension. Conclusions Our results suggest that osteoclastic activity leads to an increase in bone surface irregularity, which can be quantified by its fractal dimension. This makes fractal analysis by the box counting method a potential tool for the assessment of osteoclastic activity on bone surfaces in microscopic examination.
Effect of Image Resolution and Compression on Fractal Analysis of the Periapical Bone

Abstract:INTRODUCTION: Fractal dimension (FD) on periapical radiographs is used as a simple descriptor of the complex architecture of the trabecular bone surrounding the dentition. It is used on periapical and panoramic radiographs as a descriptor of the complex architecture of trabecular bone surrounding teeth.AIM: The aim of this study was to evaluate the effect of image resolution and different compression levels on fractal dimension of alveolar bone with images obtained using storage phosphor plate (SPP) system.METHODS: Periapical images of premolar and molar teeth on both sides of three dry human mandibles were obtained with Digora Optime (Soredex Corp., Helsinki, Finland) SPP system. The SPPs were exposed equally and scanned immediately after exposure with standard, high and very high resolutions. All the images then were compressed and saved by degrees of 0%, 25%, 50%, 75% and 90%. FD was calculated using public domain software (ImageJ with FracLac plug-in) on two non-overlapping regions of interest (ROIs) on premolar and molar periapical bone areas of each radiograph using differential box-counting method. The ROIs on corresponding images were of the same size and position. FDs were compared using two-way ANOVA and Tukey-Kramer multiple comparison tests (p=0.05).RESULTS: There was no significant difference in FD calculations in different levels of compression for all the resolutions. Images obtained with high resolution scans showed significantly lower variation in FD values compared to very high and standard resolutions for all compressions (p<0.0001).CONCLUSION: The high resolution demonstrated the lowest variation in FD values in all levels of compression which makes it the most reliable and consistent resolution for measuring the FD values. The level of compression does not make a significant difference in FD values for all the scan resolutions. Scanning resolution of SPPs should be carefully chosen when evaluating the change in FD of alveolar bone for various bone disorders.

Abstract:Objectives: The objectives of the investigation were to describe changes in mandibular bone structure with aging and to compare the usefulness of cortical and trabecular bone for fracture prediction. Materials and methods: From 1968 to 1993, 1,003 women were examined. With the help of panoramic radiographs, cortex thickness was measured and cortex was categorized as: normal, moderately, or severely eroded. The trabeculation was assessed as sparse, mixed, or dense. Results: Visually, the mandibular compact and trabecular bone transformed gradually during the 24 years. The compact bone became more porous, the intertrabecular spaces increased, and the radiographic image of the trabeculae seemed less mineralized. Cortex thickness increased up to the age of 50 and decreased significantly thereafter. At all examinations, the sparse trabeculation group had more fractures (71-78 %) than the non-sparse group (27-31 %), whereas the severely eroded compact group showed more fractures than the less eroded groups only in 1992/1993, 24 years later. Sparse trabecular pattern was associated with future fractures both in perimenopausal and older women (relative risk (RR), 1.47-4.37) and cortical erosion in older women (RR, 1.35-1.55). RR for future fracture associated with a severely eroded cortex increased to 4.98 for cohort 1930 in 1992/1993. RR for future fracture associated with sparse trabeculation increased to 11.43 for cohort 1922 in 1992/1993. Conclusion: Dental radiographs contain enough information to identify women most at risk of future fracture. Clinical relevance: When observing sparse mandibular trabeculation, dentists can identify 40-69 % of women at risk for future fractures, depending on participant age at examination.
Fractal analysis of radiographic trabecular bone texture and bone mineral density: two complementary parameters related to osteoporotic fractures

Abstract:Trabecular bone microarchitecture and bone mineral density (BMD) are two main factors related to osteoporotic fractures. Currently, however, microarchitecture is not evaluated. We have developed and validated a trabecular bone texture analysis from radiographic images. The objective was to determine if the fractal analysis of texture was able to distinguish osteoporotic fracture groups from control groups, either in vertebrae, hip, or wrist fractures, and to determine if this indicator and BMD were independent and complementary. In this cross-sectional unicenter case-control population study in postmenopausal women, 107 fracture cases were enrolled and age-matched with 197 control cases. This population comprised 40 vertebral fractures (with 70 controls), 30 hip fractures (55 controls), and 37 wrist fractures (62 controls). Hip and lumbar spine BMD were measured by double-energy X-ray absorptiometry. Fractal analysis of texture was performed on calcaneus radiographs and the result was expressed as the H parameter (H = 2-fractal dimension). The H parameter showed a lower value (0.679 +/- 0.053 SD) in fracture cases versus control cases (0.696 +/- 0.030; p = 0.007), the statistical significance persisting after adjustment for age and for lumbar spine (LS) or hip BMD. This result was confirmed in vertebral fractures (p = 0.0001) and hip fractures (p = 0.003) but not wrist fractures (p = 0.07). We determined the threshold between high and low H values and then the odds ratios (OR) of fracture for low H for BMD < or = -2.5 SD in T score and for the combinations of both parameters. The OR of fracture for low H was 1.6 (95% CI, 1.1-2.6). For LS BMD < or = -2.5 SD the OR of 6.1 (3.4-10.8) shifted to 9.0 (4.0-20.4) when we added low H and for hip BMD it shifted from 5.6 (3.3-9.4) to 8.1 (4.0-16.8). In vertebral, hip, and wrist fracture cases the results were also significant. These data have shown that the fractal analysis of texture on calcaneus radiographs can distinguish osteoporotic fracture groups from control groups. This analysis and BMD provide independent and complementary information. These data suggest that we can improve the fracture risk evaluation by adding information related to microarchitecture, derived from analysis of conventional radiographic images.
