Bronchial Rheoplasty for Chronic Bronchitis

Abstract:Introduction: Patients with chronic bronchitis (CB) usually experiences excessive mucus production, which causes increased exacerbation risk, and quality-of-life (QoL) impairment. Bronchial Rheoplasty (BR) is a novel treatment for patients with CB, it involves an endobronchial catheter that applies electrical impulses to the goblet cells in the airways to reduce mucus production. Method: In Denmark we treated 16 patients. Work up included lung plethysmography and COPD Assessment Test (CAT) Score. One lung was treated at a time, each lung treated four weeks apart. Follow up was done 3 months after the last treatment, where a lung plethysmography and CAT-score were done as well. Results: 16 patients were treated in the period from April 2022 to September 2023. All patients completed the whole treatment, having the procedure done in both lungs, except one patient, which was due to complications not related to the treatment, therefor that patient isn't included in the data analysis. Also, the last patient who received the RheOx treatment got treated bilaterally on the same day. No patients experienced procedure related complications. One patient died within a year after second treatment due to pneumonia. Conclusion: BR is a safe procedure, which reduces the patients' symptoms. The results are overall promising but large scale studies are desired to assess the effect and duration of this treatment more effectively.
