Abstract:Oral squamous cell carcinoma (OSCC) is one of the most common types of human cancer in the world. Oral leukoplakia (OLK) is the most commonly diagnosed premalignant lesion in the oral cavity and it is also most associated with the development of OSCC. However, the mechanism of malignant transformation from OLK to OSCC is not completely understood. In this study, we attempted to evaluate expression of Aurora A and the main effectors in the Ras/Raf/MEK/ERK pathway in OSCC and OLK clinical tissues. Protein expression was analyzed by immunohistochemical technology in 25 tissue samples of OLK and 21 OSCC clinical tumor specimens. Upon analyzing the levels of Aurora A, phospho-MEK1/2 and phospho-ERK1/2, a remarked increase was observed in tumors compared to OLK tissues. No statistically significant difference between tumor and OLK tissues was found for pRaf-1 S259 and ERK1/2. These results suggest that the increase in Aurora A, phospho-MEK1/2 and phospho-ERK1/2 expression was closely correlated with malignant transformation of OLK. Furthermore, Aurora A, phospho-MEK1/2 and phospho-ERK1/2 might be coordinated for valuable prognostic indicators of malignant transformation of OLK. In addition, more than 50% pERK1/2 immunopositive cells appears to be a good cut off value to predict the malignant transformation of OLK.

Abstract:Purpose: Possible patient and professional factors related to delay in diagnosis of oral squamous cell carcinoma (OSCC) were investigated and analyzed.Materials and Methods: We collected patient data with a survey conducted by medical professionals through a questionnaire at one of the largest hospitals of stomatology in Beijing, China. Multiple logistic regression was applied to look for the correlation between patient delay and predictor variables: age, resident region, lesion site, primary signs and symptoms, and lesion size. A single-factor variance analysis was used to find the correlation between professional delay and other possible related clinical factors.Results: We investigated 102 consecutive patients with OSCC treated at the Department of Oral and Maxillofacial Surgery, Peking University Hospital of Stomatology, Beijing, China. Lesion site (P = .012) and maximal diameter of the lesion (P = .067) were inversely correlated with patient delay. Clinical variables such as whether biopsy was adopted at the initial clinical visit, length of time from the first clinical consultation to cytologic biopsy, the number of clinical departments in which patients were seen, and the necessity for any treatments before definitive diagnosis were significantly correlated with professional delay.Conclusion: Lesions in the tongue and larger maximal diameter of the lesion caused longer patient delay in the diagnosis of OSCC. Whether biopsy was used at the initial management and its delay time were crucial factors to professional delay. Inappropriate clinical treatments before definitive diagnosis and the more referrals that the patient underwent prolonged the delay. Abstract:Background: Oral squamous cell carcinoma (OSCC) is a prevalent malignancy with high morbidity and mortality rates. Timely diagnosis is critical for improving patient outcomes; however, diagnostic delays remain a concern. Understanding the factors that contribute to these delays is essential for developing effective interventions. This cross-sectional study aimed to investigate the demographic, socioeconomic, and clinical determinants of diagnostic delay in patients with OSCC. Methods: This cross-sectional study included 226 patients with OSCC. Demographic data, including age, sex, marital status, education, and monthly household income, were collected. The time from symptom onset to definitive diagnosis was recorded. Stratified analysis and chi-square tests were conducted to assess the association between demographic and socioeconomic factors and diagnostic delays. Results: The mean diagnostic delay was 55.2 days, with 61.9% of the patients experiencing delays exceeding 40 days. Older age (>50 years), single marital status, lower educational level, and lower monthly income were associated with longer diagnostic delays (p < 0.05). Delays in biopsy sample collection also correlated with increased diagnostic delay (p < 0.05). Conclusion: Demographic and socioeconomic factors significantly influence diagnostic delays in patients with OSCC. Targeted interventions to address these disparities are crucial for improving early detection and enhancing patient outcomes.
Is Dental Specialty Referral Associated With Earlier Presentation of Oral Cavity Squamous Cell Carcinoma?

Abstract:Background: Oral cavity squamous cell carcinoma (OSCC) may mimic other more common odontogenic processes such as infection, trauma, and benign pathology. Delays in diagnosis and treatment are known to result in poorer survival outcomes. Purpose: The study purpose was to measure the association between referral type and OSCC stage at time of presentation. Study design, setting, sample: This was a retrospective cohort study composed of newly diagnosed OSCC patients treated with surgical resection from 2014 to 2023 at Roswell Park Comprehensive Cancer Center. Patients who presented with self-referrals or had surgical treatment outside of Roswell Park Comprehensive Cancer Center were excluded. Predictor variable: The primary predictor was referral source (dental vs medical specialty). Main outcome variable: The primary outcome was pathological T stage (pT stage). The secondary outcome was overall survival (OS). Covariates: Covariates included demographic data and cancer-related variables (symptomatology, subsite, and risk factors). Analyses: A multivariate logistical regression model for pT stage was constructed using all significant covariates as well as preoperative patient and tumor characteristics. For OS, Kaplan-Meier survival curves were constructed and compared with the log-rank test. A P < .05 was considered statistically significant. Results: A total of 215 subjects were included in the study sample. The majority were referred by a dental provider (n = 132, 62.3%). Dental referrals presented with significantly earlier T stage disease (RRpT3/T4 0.65, P < .01) and had lower rates of cervical nodal positivity (RRpN1-N3 0.62, P = .01). Dental referral independently increased the odds of early pT stage presentation (odds ratio 5.10, P < .01) after controlling for age, sex, oral pain symptoms, social history (smoking and drinking), head neck cancer history, and tumor subsite. Dental referrals had significantly improved OS (P = .03) and were also associated with lower rates of oral pain symptoms (RRoral pain 0.80, P = .02), lymphovascular invasion (RRLVI 0.50; P = .04), and perineural invasion (RRPNI 0.63, P = .04). Conclusions and relevance: Among subjects with similar risk factors, those referred from dental providers were found to have earlier stage disease. Unfortunately, over half of dental referrals still presented with pain symptoms and more than a third presented with locally advanced disease. Dental providers appear to be positioned to detect earlier OSCC; however, there is room for improvement.

Abstract:Objective: To investigate the diagnostic delay of patients with oral squamous cell carcinoma by self-designed questionnaire to find out the significant influencing factors, so as to identify the influencing factors of the patient's delay and the risk of oral cancer. Methods: A total of 514 patients with primary oral squamous cell carcinoma were enrolled at Peking University School and Hospital of Stomatology from January 2014 to April 2016, including 334 men and 180 women, with a male to female ratio of 1.85:1. The youngest participant was 21 years old, with a maximum of 89 years and a median age of 57.6 years. The position of the disease according to International Classification of Diseases-10 (ICD-10) was divided into lip, buccal, gingival, retromolar, palate, floor of the mouth, tongue, and oropharynx. The age group is divided into 10 years. The level of education was divided into illiteracy, primary education level and advanced education level. Body mass index (BMI) was divided into three levels by 18 kg/m2 and 25 kg/m2. Visual analog scale (VAS) method was to evaluate the patient's preoperative pain. At the same time, the patients were examined by detailed questionnaire to understand the delay factors and delay characteristics, and use SPSS 18.0 software, analysis of variance and chi-square test to explore correlation. Results: Among all patients, the proportion of patients developed in the tongue was the highest (43.3%). Delay was defined by a span of three months or longer from the onset of symptoms until treatment. The delays to diagnosis according to location were as follows (in months and in descending order): lip (6.1 months), buccal (4.1 months), floor of the mouth (3.9 months), tongue (3.6 months), oropharyngeal (2.9 months), retromolar (2.7 months), palatine (2.4 months), and gingival (2.4 months). Different sites and delayed diagnosis were statistically significant (P=0.048). There were no statistically significant differences in gender, education, pain, smoking, drinking and VAS score. Conclusion: The study has found that tumor positions and delay have a significant correlation. The position of the tumor is an obvious factor associated with the findings. The lip is the most likely to delay the diagnosis of oral positions. For the lip of the lesion, more than three months' obvious mass is recommended for timely treatment, while at admission, physicians should take the appropriate diagnostic method as soon as possible.

Abstract:Oral Cancer (Oral Squamous Cell Carcinoma; OSCC) contributes to one of the important causes of mortality in human beings. These impending malignancies however may present clinically with warning signs and symptoms which is often neglected due to the lack of awareness in the population. Though there are developments in diagnosing early stage of oral cancer,the detection rate is continued to be minimal. Research proved a strong association between early diagnosis and better prognosis in these malignancies. Hence, knowledge regarding these warning indicators will certainly facilitate the clinicians to discover early cancer and provide an overall benefit pertaining to treatment in these patients.This review aims to highlight the early signs and symptoms of OSCC and their significance in improved prognosis of the patients. A Medline-PubMed search was conducted of the literature over the past years using the keywords: “oral squamous cell carcinoma”, “warning signs”, “Early symptom”, “clinical presentation”, “white lesion”, “ulcer”, “erythroplakia”, and “abnormal growth”. A total of 58 articles were reviewed, of which 11 were literature reviews, 15 were original studies and 32 were case reports. Appropriate Assesment and analysis were performed to identify prompt warning signs and symptoms of OSCC.The key findings of all the early presentation were Eloborated with its clinical significance. In addition,this paper also helps to identify particular type of oral malignancies with their common clinical presentation reaching its emphasis based on the warning signs and symptoms.The importance of re-evaluation and biopsy to diagnose/eliminate malignant lesion in doubtful clinical situation is also highlighted. Abstract:Research suggests having an oral and pharyngeal cancer (OPC) examination for early diagnosis can increase survival rate. However, the OPC screening rate is low in certain groups. The goal of this study is to identify factors that are associated with having an OPC examination. Participants with landlines and aged 25 years and older were recruited from six northern Florida counties. Bivariate and logistic regressions were used to predict the outcome of whether the participants had ever had an OPC examination as well as whether participants had ever heard of an OPC examination. Of 2,260 participants with a mean age of 55.9±15 years, the majority of participants never smoked (53.4%), self-identified as Whites (70.6%), and had some college or 2-year degree education (30.3%). Smokers were significantly less likely to have ever heard of an OPC examination than those who never smoked. Significant smoking status by race interaction and smoking status by social support interaction were found. Whites who never smoked were more likely to have had an OPC examination than non-whites who never smoked. Former and current smokers with greater social support were more likely to have had an OPC examination than those with lower social support. These findings from this study inform the need to enhance the awareness of having an OPC examination among smokers and reduce barriers for racial minority populations to receive an OPC examination. Future research is warranted to develop interventions to target certain populations to improve the rate of OPC examination.

Abstract:Background: Oral squamous cell carcinoma (OSCC) is the sixth most common cancer with high morbidity and mortality rates. Late diagnosis and high incidence rate of OSCC have become global healthcare issues. The purpose of this review was to provide an overview of the diagnostic and prognostic biomarkers in OSCC. Methods: A literature search in PubMed, ScienceDirect, and Google Scholar was performed. The keywords "early detection", "oral cancer", and "oral squamous cell carcinoma" were searched in the title and abstract of the articles published in English from 2000 to mid-2021. The full texts of 250 articles were retrieved and only 63 articles met the inclusion criteria. Results: In summary, all selected papers discussed the importance of early detection along with different factors and techniques to detect oral cancer. The biomarkers were divided into three groups as follows: salivary biomarkers, circulating biomarkers, and tissue biomarkers. Conclusions: In this review article, salivary biomarkers along with the circulating and tissue biomarkers were reviewed. Besides, some detection techniques were explained.

Abstract:OBJECTIVE:To investigate the clinical iatrogenic factors related to delay in diagnosis of oral squamous cell carcinoma.METHODS:A total of 102 consecutive patients with squamous cell carcinoma of the oral cavity, treated in Department of Oral and Maxillofacial Surgery, Peking University Hospital of Stomatology were included in the study. For iatrogenic (professional) delay, stepwise multiple logistic regression and monofactor analysis, chi-square, test with 95% confidence interval (CI), were applied to analyze the correlation between the professional delay in diagnosis and the possible related clinical factors.RESULTS:Whether adopting biopsy at the first clinical management, length of time from the first clinical visit to adopting biopsy, number of clinical departments visited and whether taking any treatment before definitive diagnosis had significant influence on the delay.CONCLUSIONS:Taking biopsy at the first clinical visit, shortening the time from the first visit to biopsy and less undirected treatment can reduce the iatrogenic delay.

Abstract:Cancer of the oral cavity (Oral cancer) is the 11th most common malignancy in the world, despite the general global trend of a slight decrease in the incidence of oral cancer, tongue cancer incidence is increasing. About 90% of tumors are subtyped to oral Squamous cell carcinoma (OSCC). The incidence and mortality of this tumor shows variability according to the geographic location in which it is diagnosed, however in the last decade an increase was seen in the percentage of young patients, especially patients with tongue cancer. The overall prognosis of this cancer is roughly 55-65%, this is probably due to late diagnosis. Early diagnosis of oral cancer is the most important factor affecting the overall survival and prognosis, thus several diagnosis methods have been developed in the past few years. Still, the prognosis did not improve as expected. Oral cancer biomarkers in saliva is as easy body fluid, for noninvasive detection. Several researches identified several possible biomarkers, but none was specific. In our review, the incidence and mortality of oral tumors pose a main health problem in many aspects all around the world, as well as differences in behavior of these tumors. We witnessed more cases of anterior tongue cancers affecting mainly the young age patient group, a two decades younger than the normal risk group of oral cancer. Several countries in Europe showed a significant increase of oral cancer prevalence, such as Germany, especially in men. Similar behavior was also reported in the United States, which showed a change in the risk groups. Studies have reported an alarming lack of awareness about oral cancer, its symptoms and early diagnosis. These gaps in knowledge need to be addressed by further public education, possibly targeted at high-risk groups. With the knowledge of possible, specific, early biomarkers, primary detection could improve the prognosis tremendously. Research on the salivary biomarkers of the disease would help to develop screening programs tailored more to the risk groups.

Abstract:Oral squamous cell carcinomas (SCC) may present with similar signs and symptoms of common conditions of the oral cavity. Even for experienced clinicians, this can make diagnosis challenging and delayed. With a reported 2.5% increase in death for every week that diagnosis, and therefore treatment, is delayed, early diagnosis is imperative. We report a diagnostically challenging case of a 47-year-old fit and healthy male patient who presented with a non-healing socket following the extraction of a misdiagnosed periodontally involved mandibular third molar. Following multiple visits to both primary and secondary care, alongside various investigations, eventually a diagnosis of SCC of the mandibular alveolus was established. CPD/Clinical Relevance: This case raises awareness of the varied presentations of oral SCCs and the importance of considering a malignant cause despite the presentation being similar to that of common oral conditions, such as a non-healing socket.

Abstract:Oral cancer (OC) is an uncommon malignancy in Western countries, being one of the most common cancers in some high-risk areas of the world. It is a largely preventable cancer, since most of the different risk factors identified, such as tobacco use, alcohol consumption, and betel nut chewing, are behaviors that increase the likelihood of the disease. Given its high mortality, early diagnosis is of utmost importance. Prevention and the anticipation of diagnosis begin with identification of potentially malignant lesions of the oral mucosa and with local conditions promoting chronic inflammation. Therefore, every lesion must be recognized promptly and treated adequately. The clinical recognition and evaluation of oral mucosal lesions can detect up to 99% of oral cancers/premalignancies. As stated by the World Health Organization, any suspicious lesion that does not subside within two weeks from detection and removal of local causes of irritation must be biopsied. Surgical biopsy remains the gold standard for diagnosis of oral cancer. Adjunctive tools have been developed and studied to help clinicians in the diagnostic pathway, such as toluidine blue vital staining and autofluorescence imaging. In the near future other methods, i.e., identification of salivary markers of progression may help in reducing mortality due to oral cancer.

Abstract:Objectives: This study aimed to analyze the clinicopathological prognostic factors affecting the survival of patients with oral squamous cell carcinoma (OSCC).Materials and Methods: A retrospective study was conducted on patients with OSCC who received treatment at the Oral Oncology Clinic of the National Cancer Center (NCC) from June 2001 to December 2020. The patients' sex, age, primary site, T stage, node metastasis, TNM staging, perineural invasion (PNI), lymphovascular invasion (LVI), differentiation, surgical resection margin, smoking, and drinking habits were investigated to analyze risk factors. For the univariate analysis, a Kaplan-Meier survival analysis and log-rank test were used. Additionally, for the multivariable analysis, a Cox proportional hazard model analysis was used. For both analyses, statistical significance was considered when <i>P</i><0.05.Results: During the investigation period, 407 patients were received surgical treatment at the NCC. Their overall survival rate (OS) for five years was 70.7%, and the disease-free survival rate (DFS) was 60.6%. The multivariable analysis revealed that node metastasis, PNI, and differentiation were significantly associated with poor OS. For DFS, PNI and differentiation were associated with poor survival rates.Conclusion: In patients with OSCC, cervical node metastasis, PNI, and differentiation should be considered important prognostic factors for postoperative survival.

Abstract:Despite advances in surgical and oncological treatments, the incidence and five-year survival rates (~50 percent) of oral cancers (OC) have not improved over the last decades and remain a major public health problem. Seventy percent are still diagnosed at a late stage (T3 or T4), with an average delay in diagnosis of two to five months. As the cure and survival of patients are directly related to the development stage of the tumor at the time of diagnosis, the objective of this work was to analyze all the determinants related to oral cancer and to propose new clinical approaches for diagnosis and screening. A proposal for new models of screening, training, and concrete action to improve public awareness of the major global problem of OC is made. The strengths and weaknesses of OC screening studies need to be objectively understood to effectively guide and energize testing in primary care settings, with the prospect of using new and emerging technologies that can help improve the discriminatory accuracy of case detection. Most national organizations have not, to date, recommended population-based mass screening, due to a lack of sufficient scientific evidence of associated mortality reduction. Where health care resources are high, opportunistic individual screening is recommended, although the low diagnostic capacity of front-line clinicians is alarming.

Abstract:BACKGROUND: Graft-versus-host disease (GVHD) is an immune system reaction that occurs in patients with a history of hematopoietic stem cell transplantation (HSCT), in which the grafted donor's cells attack those of the host. The objective of this systematic review was to present a study on oral squamous cell carcinoma (OSSC) that developed from GVHD areas in patients undergoing HSCT.MATERIAL AND METHODS: An electronic search was conducted in the databases PUBMED, WEB OF SCIENCE, SCOPUS, MEDLINE and SCIENCE DIRECT, according to PRISMA guidelines.RESULTS: Of the 1582 results, 23 articles were included, resulting in 81 cases. The most common underlying disease for performing the transplant was Myeloid Leukemia (55.6%). The mean age was 39 years, with a predilection for males (64.2%). The tongue was the site of GVHD that most frequently underwent transformation to SCC (59.3%). The average time between transplantation and the development of GVHD was of approximately of 8 months, while the average period of development between transplantation and the development of OSCC was of approximately of 111 months. The most common treatment to GVHD was cyclosporine associated with corticosteroids.CONCLUSIONS: OSCCs arising from areas of GVHD present a different evolution from conventional oral carcinomas, since they affect younger patients, smoking and alcohol are not important etiological factors and finally because they present good prognosis, but further studies with larger number cases followed are needed to confirm our findings.

Abstract:The purpose of this review was to identify and describe the causes that influence the time-intervals in the pathway of diagnosis and treatment of oral cancer and to assess its impact on prognosis and survival. The review was structured according to the recommendations of the Aarhus statement, considering original data from individual studies and systematic reviews that reported outcomes related to the patient, diagnostic and pre-treatment intervals. The patient interval is the major contributor to the total time-interval. Unawareness of signs and/or symptoms, denial and lack of knowledge about oral cancer are the major contributors to the process of seeking medical attention. The diagnostic interval is influenced by tumor factors, delays in referral due to higher number of consultations and previous treatment with different medicines or dental procedures and by professional factors such as experience and lack of knowledge related to the disease and diagnostic procedures. Patients with advanced stage disease, primary treatment with radiotherapy, treatment at an academic facility and transitions in care are associated with prolonged pre-treatment intervals. An emerging body of evidence supports the impact of prolonged pre-treatment and treatment intervals with poorer survival from oral cancer.

Abstract:Oral cancer is a major public health problem, and there is an increasing trend for oral cancer to affect young men and women. Public awareness is poor, and many patients present with late-stage disease, contributing to high mortality. Oral cancer is often preceded by a clinical premalignant phase accessible to visual inspection, and thus there are opportunities for earlier detection and to reduce morbidity and mortality. Screening asymptomatic individuals by systematic visual oral examinations to detect the disease has been shown to be feasible. A positive screen includes both oral cancer and oral potentially malignant disorders. We review key screening studies undertaken, including 1 randomized clinical trial. Screening of high-risk groups is cost-effective. Strengths and weaknesses of oral cancer screening studies are presented to help guide new research in primary care settings and invigorated by the prospect of using emerging new technologies that may help to improve discriminatory accuracy of case detection. Most national organizations, including the US Preventive Services Task Force, have so far not recommended population-based screening due a lack of sufficient evidence that screening leads to a reduction in oral cancer mortality. Where health care resources are high, opportunistic screening in dental practices is recommended, although the paucity of research in primary care is alarming. The results of surveys suggest that dentists do perform oral cancer screenings, but there is only weak evidence that screening in dental practices leads to downstaging of disease. Where health care resources are low, the feasibility of using primary health care workers for oral cancer screening has been tested, and measures indicate good outcomes. Most studies reported in the literature are based on 1 round of screening, whereas screening should be a continuous process. This review identifies a huge potential for new research directions on screening for oral cancer.

A paradigm shift in the prevention and diagnosis of oral squamous cell carcinoma

Abstract:Background: Oral squamous cell carcinoma (OSCC) is a widespread disease with only 50%-60% 5-year survival. Individuals with potentially malignant precursor lesions are at high risk. Methods: Survival could be increased by effective, affordable, and simple screening methods, along with a shift from incisional tissue biopsies to non-invasive brush biopsies for cytology diagnosis, which are easy to perform in primary care. Along with the explainable, fast, and objective artificial intelligence characterisation of cells through deep learning, an easy-to-use, rapid, and cost-effective methodology for finding high-risk lesions is achievable. The collection of cytology samples offers the further opportunity of explorative genomic analysis. Results: Our prospective multicentre study of patients with leukoplakia yields a vast number of oral keratinocytes. In addition to cytopathological analysis, whole-slide imaging and the training of deep neural networks, samples are analysed according to a single-cell RNA sequencing protocol, enabling mapping of the entire keratinocyte transcriptome. Mapping the changes in the genetic profile, based on mRNA expression, facilitates the identification of biomarkers that predict cancer transformation. Conclusion: This position paper highlights non-invasive methods for identifying patients with oral mucosal lesions at risk of malignant transformation. Reliable non-invasive methods for screening at-risk individuals bring the early diagnosis of OSCC within reach. The use of biomarkers to decide on a targeted therapy is most likely to improve the outcome. With the large-scale collection of samples following patients over time, combined with genomic analysis and modern machine-learning-based approaches for finding patterns in data, this path holds great promise.
Analysis of primary oral squamous cell carcinoma in Beijing inhabitants: A 5-year continuous study of a single center

Abstract:Objective:To analyze the incidence,treatment and resource consumption of primary oral squamous cell carcinoma (OSCC) in Beijing inhabitants.Methods:Consecutive cases of primary OSCC of Beijing inhabitants admitted to the Stomatology Hospital of Peking University between 2009-2013 were selected from the medical record department.A retrospective analysis was performed on the clinical data which including age,sex,sites,TNM stage,habits of smoking and drinking,treatment,fees,etc.Results:415 cases were included in this analysis.The male-to-female ratio was 1.2∶ 1.Tongue was the most common affected site(41.0％).The difference of the distribution of the affected sites between male and female patients was statistically significant(P ＜0.01).96.6％ patients were diagnosed above the age of 40 years with the median age of 64(56,73).The difference of the distribution of age between male and female patients was statistically significant(P ＜ 0.01).The patients with stage Ⅲ-Ⅳ accounted for 54.9％.With the development of T stage of the primary tumor,the average hospital fee,surgery fee and hospitolization time increased correspondingly.Conclusion:Late stage OSCC accounts for over half of all OSCC patients.The consumption of medical resources increases significantly with the development of tumor,thus more preventive measures are required.
