The Outcome Of Acute Heart Failure In Patients With History Of Heart Transplant: A Nationwide Analysis

Abstract:Objective Acute Heart Failure is one of the most common causes of hospital admission and is associated with increased mortality. However, there is scarce data on the outcome of acute heart failure in patients with a history of a heart transplant. Methods Using the National Inpatient Database of 2020, retrospective data of patients with a history of a heart transplant, who were admitted for acute heart failure, were identified using the ICD-10 code. The outcome was in-hospital mortality, cardiogenic shock, acute kidney injury (AKI), ventricular fibrillation, ventricular tachycardia, intra- aortic pump use, ventricular assist devise use, acute hypoxic respiratory failure, length of hospital stays, and total hospital charges. This study compared the outcomes of patients admitted for acute heart failure who did or did not have a heart transplant. Results 95,604 patients admitted for acute heart failure (AHF). Of those, 0.3% had a history of a heart transplant. Patients with Heart transplants were younger (mean age 49, 95% CI 44 - 53 VS 66, 95% CI 66 - 67). There was no difference in in-hospital mortality (p=0.5023), ventricular assist device use(p=0.7677), and acute hypoxic respiratory failure (p=0.1089). There were increased rates of acute kidney injury (p<0.0001), cardiogenic shock (p<0.0001), ventricular fibrillation (p<0.0001), ventricular tachycardia (p< 0.0001), and intra- aortic pump use(p<0.0001). Patients with a history of heart transplant also had an increase in length of hospital stay (47 days, 95% CI 36 - 57 VS 6, 95% 5 - 6) and total hospital charges ($ 1197928, 95 % CI $987,268.5 - $1,408588 VS $65306.13, 95% $59161.7 - $71450.55) Conclusion Per this study, there are no statistical differences in in-hospital mortality between patients with a history of a heart transplant when compared to the general population admitted for acute heart failure. However, patients with Heart transplants had increased acute kidney injury, cardiogenic shock, intra-aortic pump use, length of hospital stay, and charges.
