Abstract:Every year >15 million individuals globally affect with a stroke and is considered the second leading cause of mortality among all neurological disorders. Though numerous neuroprotective interventions available have been investigated, several hurdles limit their clinical translation including the existence of blood-brain barrier (BBB), low bioavailability, efflux by the P-glycoprotein (P-gp) pumps, shorter half-life of drugs and narrow therapeutic window. The complex post-stroke neurobiology involving oxidative stress, inflammation, excitotoxicity and apoptosis emphasizes the need for innovative and targeted therapeutic approaches for stroke management. In recent years, lipid nanocarriers have gained tremendous attention as they facilitate the enhanced permeation across the BBB, and improve the pharmacokinetic (PK) and pharmacodynamic (PD) properties of the therapeutic moieties. In this review, the stroke pathophysiology, various conventional treatment strategies and their challenges have been addressed. The delivery of various therapeutic moieties including synthetic drugs, phytoconstituents, nucleic acids and peptides via the potential lipidic nanosystems has been deliberated. We also emphasized on the regulatory perspectives and clinical trials employed for the management of post-stroke complications.
