It never rains but it pours: Austerity and mortality rate in peripheral areas

Abstract:Since 2007, Italian regions experiencing a financial deficit, as arising from healthcare expenditure, larger than 5% of their overall funding, have been compelled to implement deficit-reducing interventions, by means of a three-year Recovery Plan (RP), whose realization is monitored by the central government. We use a difference-in-differences approach, which exploits timing and regional variations in exposure to the RP policy, to assess the effects of this austerity policy on monthly mortality rates, at the municipal level for the period 2003-2018. Employing a method introduced by Imaiet al. (2021) within a spatial non-stationarity framework, we deal with the non-random selection of the regions under treatment and, therefore, we enhance the confidence in the satisfaction of the parallel trend assumption under staggered adoption. We find that austerity has had negative effects on the mortality rate, particularly in peripheral areas and for the most vulnerable population.
