Abstract:Background To determine the impact of dental status, types, and quality of dental prostheses on body composition, masticatory performance and oral health-related quality of life (OHRQoL). Potential associations between body composition, masticatory performance and OHRQoL were also investigated. Methods This cross-sectional study included 110 older adults who received prosthodontic treatment at the Dental Faculty Clinics at Chulalongkorn University. Participants were categorized according to their dental prostheses: complete denture (CD), removable partial denture (RPD) and fixed partial denture (FPD). Retention and stability of the RPD and CD were evaluated using the CU-modified Kapur and the modified NHANES III criteria to classify denture quality into acceptable and unacceptable. Dental status including posterior occluding pairs and number of remaining natural teeth were assessed intraorally. Dependent variables were body composition, masticatory performance and OHRQoL. Body composition, including muscle mass (kg), bone mass (kg), basal metabolic rate (kcal) and visceral fat (%) were determined through a bioelectrical impedance analysis. Masticatory performance was assessed using a multiple sieve method of peanut mastication. OHRQoL was assessed using the validated Thai version of Oral Impacts on Daily Performances (Thai-OIDP) index. After adjusting for covariates, including age and sex, the associations between oral and dental prosthesis status and body composition, masticatory performance as well as OIDP score were analyzed using multivariable linear and negative binomial regression analyses. Spearman's correlation was used to determine the potential associations between body composition, masticatory performance and OHRQoL. Results The presence of fewer natural teeth or wearing an unacceptable removable denture were factors associated with lower bone mass, muscle mass and basal metabolic rate, and with a higher visceral fat. Similar dental and removable denture status were also associated with larger peanut particle size and higher OIDP score. Masticatory performance and OHRQoL variables were moderately correlated (Spearman's rho = 0.44). However, body composition was not correlated with masticatory performance or OHRQoL. Conclusions In individuals wearing dental prostheses, factors such as severity of tooth loss, types, and quality of dental prostheses, particularly retention and stability, negatively impacted not only masticatory function and OHRQoL, but also their overall body composition and health.

Relationship between vertical dimension and bite force as well as electromyography in edentulous people

Abstract:Objective To investigate the relationship between vertical dimension and bite force as well as electromyography (EMG) in elderly edentulous people,and to provide references for determining normal vertical dimension in clinic. Methods 10 edentulous subjects who went to the Affiliated Stomatological Hospital,College of Medicine,Zhejiang University were chosen. The normal vertical dimension was measured by conventional method. Based on the normal vertical dimension (M position),other 12 vertical dimensions were settled at every 0. 5 mm. Bite force at different vertical dimensions was measured using an occlusometer,while electromyography of bilateral masseter and anterior temporal muscles were recorded by multichannel physiologic collection device. Vertical dimension,bite force and muscle activity were statistically analyzed. Results ①Bite force was positively correlated with vertical dimension (P <0. 05). ②Normalized masseter muscle activity (NMM)and normalized anterior temporal muscles activity (NATM)were negatively correlated with vertical dimension (P < 0. 05). ③Masseter muscle activity/ bite force (MM/ BF)and anterior temporal muscles activi-ty / bite force (ATM/ BF)were negatively correlated with vertical dimension. Conclusions ①Bite force increased with the increase of vertical dimension. The normal vertical dimension wasn′t where bite force is maximal. ②Masseter muscle activity decreased with the in-crease of occlusal vertical dimension,but the change of anterior temporal muscle activity wasn′t so apparent compared to that of masseter muscle activity. ③MM/ BF and ATM/ BF decreased with the increase of occlusal vertical dimension. ④There may be a stable area near-by the normal occlusal vertical dimension. Inside the stable area,changes of bite force,masseter muscle and anterior temporal muscle activity were not significant.
Investigation of tooth loss and prosthetic conditions on 986 elderly outpatients

Abstract:Objective To investigate the tooth loss,prosthesis hygiene and abutment conditions as well. Methods 986 elderly outpatients,aged from 60 to 88 years old, were interviewed and clinically examined by the same two dentists following the unified criteria. Results The tooth loss rate was 93.5% . Statistical analyses indicated the tooth loss percentage of male outpatients(92.0%) was significant different from that of female ones(95.6%)(P<0.05).60.3% of elderly outpatients were found to have dental prostheses. It had positive correlation with age. In over denture cases, the percentages of root cavities and gingivitis were 27.3% and 38.4%, respectively. Conclusion It is important to give preventive oral care and health education to the public. The prosthetic design should accord with the tooth loss characteristics of elderly people.
Relationship between skeletal bone mineral density and subjective masticatory difficulty

Abstract:Background: Masticatory ability is an essential factor for sustaining quality of life and social and systemic well-being, particularly in elderly. This study aimed to reveal the association between subjective masticatory difficulty and skeletal bone mineral density (BMD). Methods: Data from the Korean National Health and Nutrition Examination Survey, which was conducted from 2008 to 2011 were analyzed. This study included 13,092 Koreans (5656 males, 7436 females) over 50 years of age. Masticatory difficulty was evaluated based on a self-reported questionnaire. Areal BMD of the total hip, femoral neck, and lumbar spine as well as lean body mass were determined using dual-energy X-ray absorptiometry. Data about the sociodemographic characteristics, physical activity, number of teeth present, sum of decayed, missing, and filled permanent teeth (DMFT) index and Community Periodontal Index (CPI) were collected. Multivariate logistic regression analysis was conducted to analyze associations between subjective masticatory difficulty and BMD, adjusting for the confounding covariates. Results: Significant differences were observed in the areal BMD of the total hip, femoral neck, and lumbar spine as well as lean body mass accordance with the presence of subjective masticatory difficulty in both males and females. The number of teeth, DMFT, and CPI score did not show significant differences based on the presence of self-reported satisfaction of chewing performance in both males and females. Results from multivariate logistic regression demonstrated that the subjective masticatory difficulty showed significant interactions with skeletal BMD and the associations between masticatory satisfaction and BMD of the total hip and femoral neck were more prominent in females compared to those in males. Conclusions: The skeletal BMD, particularly areal BMD of the femoral neck was significantly associated with subjective masticatory difficulty in elderly, especially in elder females.
The Association of Bite Instability and Comorbidities in Elderly People

Abstract:Objective The purpose was to evaluate the association between bite instability and comorbidities, comprehensive geriatric evaluations, or disabilities in elderly people. Methods A dentist examined the oral function, such as the bite stability, number of teeth, and the use of dentures, in 119 patients (93 women, mean age: 86.7±7.8) in 2 nursing homes for the elderly. The association between the oral function and the prevalence of diseases, including hypertension, diabetes mellitus, and dementia, was analyzed. Results The median number of teeth was 0 [0, 4]. The patients were divided into a bite-stable group (n=78, 66%) and bite-unstable group (n=41, 34%). The prevalence of hypertension was significantly higher in the bite-stable group than in the bite-unstable group (83% vs. 63%, respectively; p=0.0149), whereas the prevalence of diabetes mellitus was significantly lower in the bite-stable group than in the bite-unstable group (10% vs. 27%, respectively; p=0.0190). The prevalence of a cognitive function decline was significantly lower in the bite-stable group as well (59% vs. 83%, p=0.0082). According to the simplified comprehensive geriatric assessment 7, the bite-stable group scored significantly higher for instrumental activities of daily living (ADL) than the bite-unstable group (54% vs. 24%, respectively; p=0.0021). A multivariate logistic regression analysis demonstrated that bite instability was independently correlated with hypertension, diabetes mellitus, and instrumental activities of daily living. Conclusion Bite instability was independently associated with a decreased prevalence of hypertension or increased prevalence of diabetes mellitus and low levels of instrumental ADL in the elderly.

Abstract:Weakened capability of eating and swallowing is common among elderly population, affecting negatively on their health and well-being. This work measured occlusal force and tongue muscle strength of elderly patients and then assessed whether correlations existed between oral physiological properties and the eating and swallowing capability. A total of 26 elderly patients with potential swallow disorder were recruited in this work. Participants’ denture status and daily diet texture were determined by a pre-questionnaire analysis. Their swallowing capability was assessed and rated by water drinking test. Maximum tongue pressure, incisor occlusal force and hand gripping force were measured for each subject. It was observed that participating subjects have on average a significantly lower biting force and maximum tongue pressure than that of normal adults. The swallowing grades assessed by water drinking test showed significant correlations with both the biting force (r=−0.498, P<0.05) and the tongue muscle strength (r=−0.544, P<0.05). The biting force of dysphagia participants correlates significantly with dental conditions (P<0.05). The observed correlation between tongue pressure and diet texture indicates that tongue muscle strength could be used as a reliable indicator of the eating and swallowing capability for elderly people.
Oral function is associated with the body and muscle mass indices of middle-aged dental patients

Abstract:Objectives: Undernutrition is associated with frailty, one of the common reasons for seeking long-term care. In older adults, nutritional status is known to be associated with oral function; however, it is not yet known if there is a similar association in middle-aged adults. The present study determined the association between nutritional status and oral function in middle-aged adults. Material and methods: This cross-sectional study was conducted in 117 outpatients aged 40-64 years who underwent dental check-up at a private dental clinic. Factors associated with oral function, including oral moisture, number of teeth present, oral diadochokinesis, tongue pressure, lip-seal strength, and masticatory performance, were evaluated. Body mass index (BMI), fat-free mass index (FFMI), and skeletal muscle mass index (SMI) were analyzed as nutritional statuses. Pearson's correlation analysis was conducted to determine the correlation between nutritional statuses and other variables. Multiple linear regression analysis was performed, with adjustment for age and sex, using BMI, FFMI, or SMI as response variables and oral moisture, maximum tongue pressure, lip-seal strength, oral diadochokinetic rate, and masticatory performance as explanatory variables. Results: All nutritional statuses showed significant correlation with maximum tongue pressure and lip-seal strength. Multiple linear regression analysis revealed that BMI was associated with maximum tongue pressure and lip-seal strength, whereas FFMI and SMI showed significant association with sex, maximum tongue pressure, and lip-seal strength. Conclusions: In middle-aged outpatients at a regional dental clinic, lip-seal strength and tongue pressure were positively associated with BMI, FFMI, and SMI.
Associations Between Abdominal Obesity, Chewing Difficulty and Cognitive Impairment in Dementia-Free Chinese Elderly

Abstract:Objective To investigate the independent and synergistic associations between abdominal obesity, chewing difficulty and cognitive impairment in a community-dwelling older adults sample in China. Methods Cognitive function was measured by the 5 min- Montreal Cognitive Assessment (5 min-MoCA) and abdominal obesity was measured by A Body Shape Index (ABSI) in 572 participants recruited from local communities. Chewing difficulty was assessed via a self-report questionnaire. Linear regression and general logistic regression were performed to investigate the association of chewing difficulty and abdominal obesity with cognition. Results Chewing difficulty score [β (95% CI) = −.30 (−.49, −.11)] and ABSI [β (95%CI) = −.30 (−.55, −.05)] were independently associated with worse performance on the 5 min-MoCA. Whilst ABSI was not associated with cognitive impairment, the co-existence of chewing difficulty and abdominal obesity [OR (95% CI) = 2.22 (1.18, 4.17)] was found associated with the presence of cognitive impairment. Conclusion Chewing difficulty and abdominal obesity were independently associated with cognition. Abdominal obesity and chewing may have an additive effect on cognitive function.
Association between oral health status and occlusal bite force in young adults

Abstract:Background/purpose Oral health is related to general health and a person's overall well-being. The aim of the present study was to explore the association between oral health status and bite force among young adults. Materials and methods Maximum bite force (MBF) was measured using Dental Prescale II in conjunction with a pressure-sensitive film and bite force analyzer in 40 young adults aged 20 to 40. Supragingival dental plaque was collected and cultured. Plaque weight, pH, and colony counts were assessed. The decayed, missing, and filled teeth index (DMFT) and body mass index (BMI) were recorded. Results Bite force was negatively correlated with the number of missing teeth and the sum of missing and filled teeth. When the filled-to-remaining-teeth ratio (F/R ratio) was less than 8%, the bite force was significantly higher compared to an F/R ratio of 8–25%. Additionally, the amount of total bacteria was positively correlated with total bite force, and the quantity of Streptococcus mutans ( S. mutans ) along with total bacteria was positively correlated with bite force in the molar region (*P < 0.05). The molar region predominantly contributed to bite force. Conclusion Elevated levels of cariogenic bacteria may increase the risk of tooth loss, subsequently leading to reduced bite force. This reduction in bite force can further impact the efficiency of chewing function and, consequently, the quality of life. An F/R ratio above 8% could be easily calculated clinically and could serve as a guide to identify patients, particularly young adults, at risk of reduced bite force.

Abstract:Background/purpose The relationship among chewing ability, food texture selection, and nutritional status in the elderly is well-known. This study tried to find the reliable biomarkers to predict the chewing ability and nutritional status of the elderly people. Materials and methods Sixty-eight elderly subjects (mean age, 80.38 ± 7.32 years) were enrolled in this study. Oral examinations assessed the number of total teeth, pair of antagonistic teeth, and type of dentures in participants. Sociodemographic data, dietary habits, and medical history were collected via self-reported questionnaires. Nutritional status was assessed based on the calf circumference and serum albumin level measurements. Results The elderly individuals with fewer number of total teeth, fewer pairs of antagonistic teeth, or the removable dentures preferred minced and moist food textures. These preferences were significantly associated with the smaller calf circumference (31.29 ± 4.08 cm, P < 0.05) and lower serum albumin levels (3.52 ± 0.63 g/dL, P < 0.001) compared to those with better chewing ability. The elderly participants with <20 total teeth, <10 pairs of antagonistic teeth, or the removable dentures showed significantly lower calf circumference and serum albumin levels than those with ≥20 total teeth, ≥10 pairs of antagonistic teeth, or the fixed dentures, respectively (all P -values <0.05). Conclusion Optimal chewing ability is crucial for maintaining an adequate nutritional status in the elderly people. The calf circumference and serum albumin level are found to be the reliable biomarkers for predicting the chewing ability and nutritional status of the elderly people.

Denture acceptance among newly rehabilitated elderly population in old age homes in South India

Abstract:Introduction: Elders in old age homes are mainly those who have been abandoned by their family and have one or more physical or mental disabilities. It is a major challenge for the dental professional to plan oral health strategy for this group of patients. Aim of the study: Aim of the present study is to observe and determine the acceptance of new dental prosthesis among the elderly residing as inmates of 3 old age homes in Mangalore City. Materials and methods: This cross-sectional study was conducted using a survey proforma. Information regarding their experience with the new denture, 1-month, 3 months and 6 months after denture insertion was gathered. Statistical analysis of the data was done using the Chi-square test with the P < 0.05 considered significant. Results: One hundred and eighty-three residents out of a total of 400 residents in 3 old age homes were denture wearers. Among them, 101 (55.2%) were females, and 82 (44.8%) were males. Dental prosthesis whether worn regularly, discomfort, retention, cleansing of denture during a period of 1-month, 3 months and 6 months was found to be significant satisfaction with the prosthesis, denture adhesives used, food accumulation during a period of 1-month, 3 months and 6 months was found to be nonsignificant. Conclusion: The emotional makeup of the patient must be taken into consideration during the entire procedure. We must understand that the success of the prosthesis depends mainly on the patient's confidence in the dentist rather than the quality of the prosthesis.

Abstract:Background: The elderly are at risk of experiencing oral health problems and chewing due to tooth-loss, decreased occlusal teeth support, and decreased salivary flow rate. Objective: This study aimed to compare distribution and comparison tooth loss, the proportion of occlusal teeth support, salivary flow rate, and the choice of food texture between the normal and cognitive impairment groups. Methods: Preliminary study and cross-sectional designed method on community-dwelling elderly in Bandung City, Indonesia. The inclusion criteria were no complaint of orofacial pain, not wearing denture prostheses, good communication skills, reading and writing skills, and good physical health. Demographic data include age, gender, education, and occupation. Tooth loss distribution, the proportion of occlusal contact zone, salivary flow rate, and choice of food texture were determined. Cognitive function was measured using the mini mental short examination (MMSE) form. A Mann-Whitney test was used to compare normal and cognitive impairment groups with a significance level of p-value <0.05. Results: 25 cognitive normals with MMSE score ≥25 and 10 cognitive impairment with MMSE score <25. Mean anterior tooth-loss was 2.16±3.89 vs. 4.90± 4.28 (p 0.05 under normal condition compared with the cognitive impairment groups. Fewer occlusal support zone was found more frequently in cognitive impairment by groups B2, B3, B4, and C according to the Eichner Index classification. Salivary flow rate was 1.2942± 0.5768 vs 1.2755± 0.9811, p> 0.05. The participants in both groups preferred a hard-solid food portion. Conclusion: The loss of anterior teeth is significantly different. The loss of posterior teeth appeared to be different between the elderly with cognitive impairment compared with the normal groups. The occlusal support zone, salivary flow rate, and the choice of food texture as a meal served were similar between both groups in this study.
Dentition and weight status in community‐dwelling older adults

Abstract:Background/Objective Tooth loss is common among older adults and can affect dietary intake and weight status. This study investigated associations between dentition status and body mass index (BMI) in older adults. Materials and Methods This was a cross‐sectional study of data from a convenience sample of older adults (65–89 years) treated at an urban U.S. dental school clinic. Clinical and demographic data were obtained from electronic health records. Dentition status was determined based on data from odontograms. Multinomial logistic regression was used to estimate the odds ratio (OR) and 95% confidence interval (CI) of having a non‐normal weight status for each measure of dentition status, after adjusting for covariates. Results Patients (n = 1765) were 54.1% female, 51.5% White, 41.6% African American and 22.5% Hispanic/Latino. The median (interquartile range [IQR]) age was 71 (67.0–75.0) years; the mean (±SD) BMI was 28.5 (±5.7) kg/m2; 72.5% were overweight or obese. The median (IQR) number of remaining teeth was 20.0 (13.0–24.0); the median numbers of anterior and posterior occluding pairs of teeth were 5.0 (2.0–6.0) and 2.0 (0.0–5.0), respectively; and 44.9% had a functional dentition (≥21 teeth). Having a higher number of remaining teeth and more posterior occluding pairs were associated with lower odds of obesity (OR = 0.980, 95% CI = 0.964, 0.997, p = .022 and OR = 0.931, 95% CI = 0.885, 0.980, p = .006, respectively). Lack of a functional dentition was associated with higher odds of obesity (OR = 1.400, 95% CI = 1.078, 1.818, p = .012), after controlling for covariates. Conclusion Older adults with tooth loss ‐ especially loss of posterior occlusion and lack of a functional dentition ‐ were more likely to be obese than of normal weight.

Masticatory efficiency in patients with partially dentate dentitions

Abstract:Objectives To assess associations amongst masticatory performance, dental functional status, and perceived chewing problems with common soft and hard foods. Materials and methods 387 adult participants with reduced natural dentitions were included and underwent an oral examination, completed a written structured questionnaire and performed a two-color chewing gum test for masticatory efficiency. A multivariate regression model was used to analyze associations between masticatory efficiency and a number of dental conditions, with age, gender and perceived chewing difficulties as background variables. Likelihood ratios were calculated to analyze the relationship between masticatory efficiency and dental conditions within a validated hierarchical dental functional classification system expressed by (1) number of teeth in upper and lower jaw, (2) completeness of anterior regions, (3) number of premolar occluding pairs, and (4) number of molar occluding pairs. Results The regression model showed a significant positive association between unmixed ratio and self-reported problems with chewing (p = .004), and age (p < 0.001), and negative associations with having a sufficient premolar region (p < 0.001). In the hierarchical dental functional classification system, the likelihoods of having a high unmixed ratio and chewing problems with hard foods were highest for the predictor ‘less than 10 teeth in each jaw’ (LRs 4.01, respectively 13.06). Conclusions In partially dentates people, perceived chewing difficulties were associated with lower objective masticatory efficiency. Having fewer than 10 teeth in each jaw and having an impaired premolar region were best predictors for low masticatory efficiency. Clinical significance This study shows that having fewer than 10 teeth in each jaw and having an impaired premolar region are associated with low masticatory efficiency and chewing problems. Preservation of at least 10 teeth in each jaw and 3 to 4 premolars is important for preventing chewing difficulties.
Periodontal status is associated with oral function in community-dwelling older adults, independent of dentition status

Abstract:Objective: To determine whether periodontal status is associated with oral function, including masticatory performance and occlusal force, among community-dwelling older adults. Background: Although a potential association between periodontal status and oral function has been reported, variations in the root surface area (RSA) of each tooth have not been considered. Methods: We used data from a population-based study involving community-dwelling older adults. The RSA with periodontal ligament (RSA-PL), which quantifies the RSA attached to the periodontal ligament and alveolar bone, was calculated based on full-mouth periodontal examination. Masticatory performance was assessed using spectrophotometric measurement of the color (a* value) of color-changing chewing gum. The bilateral maximal occlusal force (MOF) was measured using a pressure-sensitive sheet and dedicated software. The associations of the RSA-PL, a quantitative marker of periodontal tissue support, with the a* value and MOF were assessed using linear regression models. Results: The analyses included data from 250 adults [60.8% women; age, mean (standard deviation) 82.5 (5.0) years]. On average, the study participants had an RSA-PL of 26.3 cm2 , a* value of 25.0, and an MOF value of 555.1 N. After adjustments for potential confounders, including dentition status, age, sex, dental visit regularity, smoking status, physical activity level, depressive symptoms, a history of stroke or diabetes mellitus, and body mass index, the RSA-PL was found to be associated with the a* value [coefficient (per 1 cm2 increase): 0.16, 95% confidence interval (CI) 0.10-0.22)] and MOF (coefficient: 9.2, 95% CI 5.3-13.1). Conclusion: This study demonstrated that greater amounts of remaining tooth-supporting structures, indicated by higher RSA-PL values, were associated with better masticatory performance and a higher occlusal force among community-dwelling older adults.

Abstract:Objective This systematic review aimed to provide an overview of the most recent evidence on the association between measured masticatory function and cognitive status. Materials and Methods Literature and manual searches were conducted using three electronic databases (PubMed, Web of Science and CINAHL). Observational studies published between 2011 and 2021 investigating the association between masticatory function, dementia and cognitive status in adult humans were abstracted and reviewed by three reviewers. Studies that assessed participants' masticatory function using objective and subjective measurements and that individually examined its association with cognitive function were included. The included studies were divided into cross‐sectional and cohort studies, and the quality of each study was analysed using critical appraisal skills checklists. Additionally, the main conclusions and strength of the evidence were assessed for each article. Results A total of 21 studies (11 cross‐sectional studies that objectively evaluated masticatory function, 9 cross‐sectional studies that subjectively evaluated masticatory function and 1 prospective cohort study) were evaluated. The poorer masticatory function was associated with lower cognitive status even after adjusting for potential risk factors of dementia in four of 11 and six of nine cross‐sectional studies where the masticatory function was respectively evaluated objectively and subjectively. One prospective cohort study also demonstrated that masticatory function, as evaluated based on measurements of occlusal force, predicted cognitive decline during the follow‐up period. Conclusion Several studies demonstrated a positive association between masticatory function and cognitive status. However, further studies, particularly longitudinal studies, are required to determine whether the association is causal.

What Is the Relationship between the Chewing Ability and Nutritional Status of the Elderly in Korea?

Abstract:This study aims to determine the relationship between chewing ability and the nutritional status of the elderly in Korea. This study utilized the data from the Korea National Health and Nutrition Examination Survey (KNHANES) conducted from 2013–2018 for persons who were ≥65 years of age. Of the 7835 subjects, 43.2% had chewing difficulty. Compared to the normal group, the chewing difficulty group had more stress, lower exercise frequency, less snack intake, a lower frequency of eating out, and a higher proportion of food insecurity. The chewing difficulty group had significantly lower food intake compared to the normal group, including various food groups such as cereals and grain, potatoes, fruits, meat, and milks and dairy products. The intake of fresh fruits was 24.5% lower and the intake of plant food (fresh fruits and nonstarchy vegetables) was 17.8% lower in the chewing difficulty group compared to the normal group. In addition, the intake of most nutrients (carbohydrates, fat, calcium, phosphorus, sodium, potassium, vitamin A, riboflavin, niacin, and vitamin C) was significantly lower in the chewing difficulty group than in the normal group. The chewing difficulty was significantly associated with undernutrition (OR = 1.63). In conclusion, chewing ability is closely related to food and nutrient intake among the elderly, which can decrease the quantity and quality of diet and is also related to undernutrition. Therefore, it is necessary to develop customized nutrition programs and aging-friendly food products that consider the chewing ability of the elderly.

Nutritional status of older adults in India: An exploration of the role of oral health and food insecurity factors

Abstract:Background: In this study, we attempted to generate insights into the determinants of nutritional status among older adults in India by exploring the role of factors including oral health, food insecurity, socioeconomic, demographic and health-related variables. The study also examined the moderating role of tooth loss with the association between chewing ability and nutritional status. Methods: The data was obtained from Longitudinal Ageing Study India (LASI) - Wave 1 (2017-18). The sample consisted of 27,411 older adults (Male = 13, 232; Female = 14, 179) aged 60 years and above. In addition to descriptive and bivariate analysis, we employed multinominal logistic regression analysis. Result: There was a significant association between the ability to chew solid food and tooth loss in the nutritional status of older adults. It was further revealed that tooth loss acted as a moderator (aRRR = 1.50, 95 % CI: 1.08-2.08) between the chewing ability and the risk of being underweight. The weight loss due to food insecurity increased the relative risk ratio of being underweight (aRRR = 1.58, 95 % CI: 1.25-2.00). Further evidence showed that self-rated health (SRH), Instrumental Activities of Daily Living (IADL), morbidity status and socioeconomic factors had a significant role in determining the nutritional status among older adults. Conclusion: Chewing ability, tooth loss, and food insecurity determine nutritional status among older adults. It is suggested to care for oral and general physical health as these factors increase the risk of nutritional deficits. Also, policy measures should be strengthened to address the specific health and food security needs of older adults from vulnerable socioeconomic circumstances.

Abstract:Background and objectives: In an aging society, the maintenance of the oral function of the elderly is of importance for the delay or prevention of frailty and long-term care. In the present study, we focused on the maximum tongue pressure (MTP) value and analyzed the relationship between MTP and age, occlusal status, or body mass index (BMI). Materials and Methods: This one-center observatory study was conducted using a cohort consisting of 205 community-dwelling outpatients over 65 years old. The MTP values of all subjects were measured using a commercially available tongue pressure measurement device and statistically analyzed. In addition, the correlation between MTP value and BMI was analyzed. Results: The MTP value decreased with age, especially in subjects classified as Eichner B and C. The difference in occlusal status did not show any statistically significant influence on MTP value. The correlation between BMI and MTP value was indicated in the tested groups other than an age of 65-74 and Eichner A groups. Conclusions: Although MTP value decreased with age, the difference in occlusal status did not have an impact on MTP value. The correlation between BMI and MTP value was not shown in the youngest group or a group with sufficient occlusal units. The results presented in the present study may imply that, even if MTP is low, younger age and/or better occlusal status compensate for the inferior MTP value in the cohort studied.

Abstract:Chewing ability is also related to activities of daily living (ADLs) and nutritional status; however, these associations have not been firmly established. We examined chewing ability as a predictor variable and explored its relationship with cognitive functioning as mediated by ADLs and nutritional status data were collected by face-to-face interviews. Patients were receiving home healthcare service in Mun-gyeong city, Gyung-buk, Korea. Participants comprised 295 patients aged 81.35 ± 6.70 years. Structural equation modeling (SEM) was performed using AMOS 18.0 (SPSS Inc., Chicago, IL, USA). The model fit was based on absolute fit index and incremental fit index. Data were collected to assess cognitive functioning (using the Korean version of the Mini-Mental Status Examination for dementia screening (MMSE-DS)), ADL, a mini-nutritional assessment (MNA) questionnaire, and a chewing ability test. Participants with better chewing ability had significantly better cognitive functioning, ADLs, and nutritional status (p < 0.001). Chewing ability directly affected cognitive functioning and indirectly affected how ADLs and MNA affected MMSE-DS. Chewing ability is an important factor influencing the cognitive functioning of elderly adults in Korea, both directly and indirectly through mediating variables such as nutritional status and ADLs. Efforts to help older adults maintain their chewing ability are necessary for preventing cognitive impairment.
