Is d-dimer a Useful Diagnostic Tool for Identifying Multisystem Inflammatory Syndrome in Children in Critically Unwell Children?

Abstract:d -Dimer levels are known to be raised in critically ill children in pediatric intensive care units (PICUs) and are associated with mortality in this cohort. 1 During the emergence of SARS-CoV-2, higher d -dimer levels were associated with a greater risk of thromboembolic disease in severe adult COVID-19 cases 2 and were shown to be significantly elevated in pediatric patients with multisystem inflammatory syndrome in children (MIS-C) associated with SARS-CoV-2 exposure. 3 Meta-analysis of inflammatory markers in MIS-C found d -dimer levels to be significantly higher than those reported in severe pediatric COVID-19. 4 , 5 However, as d -dimer levels are raised among critically unwell children, it is unknown whether d -dimer levels are differentially elevated in PICU patients with MIS-C compared with other primary diagnoses.
