Abstract:The 2023 Kahramanmaraş earthquakes, with magnitudes of 7.7 and 7.6, caused extensive destruction and psychological distress across southeastern Turkey. This study explores the psychological impact on non-victims, particularly Istanbul residents, focusing on mental health outcomes and coping mechanisms. A cross-sectional survey was conducted from March to May 2024 with 721 participants from various Turkish cities, including a significant portion from Istanbul. Validated psychological scales such as the Beck Depression Inventory-II (BDI-II), Beck Anxiety Inventory (BAI), Warwick-Edinburgh Mental Well-being Scale (WEMWBS), and PTSD Checklist for DSM-5 (PCL-5) measured depression, anxiety, well-being, and PTSD symptoms. Sociodemographic factors like age, gender, occupation, income, education level, and previous earthquake experience were also analyzed. Results showed significant psychological distress among non-victims: 51.9% reported high levels of distress, with 24% meeting PTSD criteria, 30% exhibiting moderate to severe depression, and 28% experiencing significant anxiety. Higher income and education levels correlated with better mental health outcomes. Higher education levels were linked to lower PTSD risk (β = -0.20, p < 0.01) and fewer depression symptoms (β = -0.15, p < 0.05). Higher income was associated with lower depression scores (β = -0.20, p < 0.01) and fewer PTSD symptoms (β = -0.15, p < 0.05). Age positively correlated with well-being (r = 0.68, p < 0.001) and negatively with PTSD symptoms (r = -0.15, p < 0.05). Comparisons with victim studies of major earthquakes, such as the 1995 Great Hanshin-Awaji earthquake, the 1999 Marmara earthquake, the 2008 Wenchuan earthquake, and the 2000 Iceland earthquakes, revealed similar profound psychological impacts. This highlights the need for comprehensive mental health interventions for both direct and indirect exposures. This study underscores the necessity for inclusive mental health strategies to enhance resilience and well-being, ensuring robust recovery after catastrophic events.

Abstract:Nine months after the disaster, menstrual irregularities increased from 14.3% to 44.8%, and PTSD prevalence was 22.7%. Risk factors for menstrual irregularities included post‐traumatic stress symptoms, comorbid chronic illnesses, and smoking. Aim In the aftermath of natural disasters, understanding the intricate links between mental health and physiological responses, such as menstrual cycle patterns, becomes crucial. This study explores the relationship between post‐traumatic stress and menstrual irregularities among women of reproductive age residing in regions affected by the 2023 earthquake in Turkey. Methods 309 women of reproductive age living in 11 provinces affected by the earthquake centered in Kahramanmaraş on February 6, 2023 and declared as disaster areas constituted the study sample. Data were collected online using Google forms nine months after the earthquake. The collected data were obtained using the Participant Information Form, Impact of Events Scale and Post‐traumatic Stress Disorder‐Short Form. Abstract:The earthquake, which affected the south and southeast of Turkey (approximately 10 provinces) on February 6, 2023, brought many devastating consequences. Earthquakes, which occur in all geographies of the world in variable time periods depending on the characteristics of the geological structure, cause many extraordinary situations and consequences for humanity. This situation and results require people defined as disaster victims to struggle with psychological problems arising from the effects of the event. Based on this, this study aims to determine the post-earthquake trauma levels of university students who experienced and were affected by two earthquakes that took place nine hours apart on February 6, 2023, where the districts of Pazarcık (Earthquake intensity 7.7) and Elbistan (Earthquake intensity 7.6) in Kahramanmaraş province of Turkey were the epicenters. intended to be evaluated. In the study, which was carried out with the quantitative research method and the survey model, criterion sampling, one of the purposive sampling methods, was used to determine the research group. Data were collected with the "personal information form" created by the researchers and the "post-earthquake trauma level determination scale" developed by Tanhan & Kayri (2013). Unlike many studies on the extent of the earthquake that took place, it can be said that as a result of this study, which aims to examine the psychological and mental state of earthquake victims, many remarkable results such as the fact that the trauma levels of female earthquake victims are higher than that of men.

Abstract:Background: Evidence suggests that natural disasters, such as earthquakes, can lead to post-traumatic stress disorder (PTSD), with women being particularly vulnerable. However, there is a dearth of knowledge about the underlying mechanisms linking traumatic experiences to PTSD as well as gender differences in the operation of these mechanisms. This study examined the mediating effect of intrusive rumination and the moderating effect of gender on the relationship between earthquake exposure level and PTSD symptoms. Methods: This cross-sectional study was conducted 12 months after the devastating earthquake in Kahramanmaraş, Turkey. The sample included 632 adult survivors of the earthquake, including 374 females (59.2%) and 258 males (40.8%). Participants completed a set of questionnaires assessing their demographic information, level of earthquake exposure, intrusive rumination, and PTSD symptoms. Correlation analysis, mediation analysis, and conditional process analysis were employed to evaluate the research hypotheses. Results: Findings revealed significant associations between the level of earthquake exposure, intrusive rumination and PTSD symptoms. The level of earthquake exposure significantly predicted PTSD symptoms. Moreover, intrusive rumination partially mediated the relationship between earthquake exposure level and PTSD symptoms. Additionally, the mediating effect of intrusive rumination was more pronounced among females than males. Conclusion: This study revealed the significant mediating role of intrusive rumination in the development of post-earthquake PTSD symptoms. By uncovering gender differences in this mechanism, it also emphasized the importance of designing interventions that are sensitive to the varying needs of different demographic groups affected by the disaster in post-disaster mental health interventions.

Abstract:On February 6, 2023, an earthquake registering 7.8 on the Richter scale struck southeast Turkey, and this was followed by a second earthquake of magnitude 7.5. The earthquake resulted in widespread loss, devastation, and destruction, and is anticipated to bear a significant psychological toll on survivors. This study aimed to investigate the prevalence and correlates of ICD-11 posttraumatic stress disorder (PTSD) in those living in earthquake-affected areas of Turkey. The sample was a convenience sample of 527 adults living in earthquake-affected areas of Turkey. Prevalence rates and gender differences in exposure to earthquake-related traumatic events were investigated. Moreover, prevalence rates of ICD-11 PTSD were computed, and demographic and trauma-related correlates of ICD-11 PTSD were assessed using logistic regression analyses. Most of the sample (99.2%; n = 523) experienced at least one earthquake-related traumatic event, with the majority experiencing multiple events. Findings demonstrated high endorsement of the ICD-11 PTSD symptom clusters, with 54.1% meeting criteria for diagnosis of probable ICD-11 PTSD. Correlates associated with increased risk of PTSD include female gender, physical injury in the earthquake, physical injury of family or loved ones in the earthquake and being in the higher earthquake-related traumatic events exposure quartiles. These findings indicate high levels of trauma exposure and PTSD among survivors of the recent earthquakes which occurred in Turkey. The identification of correlates associated with increased risk of PTSD is useful for identifying Turkish earthquake survivors who are most likely to need psychological support.

Abstract:It is known that disasters can have long-term effects on the mental health of individuals. In particular, healthcare workers may be under greater stress in a time of disaster, as they are not only affected by the disaster, but they also take part in rescue efforts. This study was conducted to investigate the psychological effects of the Kahramanmaraş earthquakes on healthcare workers employed at the Adıyaman Training and Research Hospital. The sample in the cross-sectional study consisted of 299 healthcare personnel working in the Adıyaman Training and Research Hospital. The study data were collected through a questionnaire consisting of five sections. The data were analysed using SPSS 22 software. It was found that among healthcare workers, those who were women, married, individuals whose homes were damaged, injured, or lost a relative in the earthquake had experienced mental health problems such as post-traumatic stress disorder (PTSD), anxiety, depression, and poor sleep quality the most. The earthquakes that occurred on 6 February negatively affected the mental health of healthcare workers. In order to reduce these negative effects experienced by healthcare workers, various types of mental health screening should be performed, and supportive psychological services should be provided urgently.

Abstract:On February 6, 2023, two major earthquakes with epicenters in southeastern Türkiye, measuring 7.7 and 7.6 magnitudes respectively, devastated widespread areas. Research suggests that survivors of natural disasters including earthquakes have increased trauma and anxiety responses. Due to the negative psychological effects of the earthquake, it is necessary to examine the factors that improve mental health. This study aimed to assess the mediating role of coping flexibility and emotion dysregulation in the relationship between post-traumatic stress symptoms (PTSS) and anxiety responses (GAD). Within the framework of the study, 596 people affected by the earthquake completed the scales (298 Females and 298 Men; age mean: 32.09). Data were collected online from the convenience sampling method. Reliability-measured scales were used to measure the level of being affected by the earthquake, severity of anxiety, coping flexibility, and emotional dysregulation. The structural equation modeling indicated that the mediating role of coping flexibility and emotion dysregulation was significant in the relationship between post-traumatic stress symptoms (PTSS) and general anxiety disorder (GAD). The presence of posttraumatic symptoms was associated with lower coping flexibility, greater difficulties in emotion regulation, and higher levels of anxiety. After two catastrophic earthquakes, there is a risk of long-term deterioration of individuals' mental health. This study may be considered when implementing interventions for the mental health of individuals affected by the earthquake.

Abstract:This study aimed to examine the relationship between earthquake-related trauma severity and smoking status in individuals affected by earthquakes. The study sample consisted of 488 earthquake survivors (57.1% women) older than 18 years old who were placed in dormitories in an earthquake-safe area after the Kahramanmaraş-centered earthquakes in 2023. The study data were collected two months after the earthquakes using an information form and the Scale that Determines the Level of the Trauma after the Earthquake (SDLTAE). The data were analyzed using descriptive statistics and independent sample t tests, one-way ANOVA, and binary logistic regression at the significance level ( p <.05). The findings showed that 75.2% of the individuals experienced severe trauma. According to the results of the present study, gender, the need for psychological support, earthquake-related losses, and smoking related to trauma. Being a woman, seeking psychological support, experiencing earthquake-related health problems, facing earthquake-caused material losses, and increased smoking after the earthquake were significant predictors of earthquake-related trauma. This study, conducted two months after the earthquake, aimed to measure the early effects of the earthquakes. Studies are also needed to analyze the long-term impact of earthquakes.

Abstract:On February 6, 2023, two major earthquakes with magnitudes of 7,7 and 7,6 struck the Kahramanmaraş region in Turkey. Over 50.000 people lost their lives and more than half a million buildings were damaged as a result of these earthquakes. Earthquake survivors may develop post-traumatic stress disorder and sleep hygiene problems. This study aims to evaluate the correlation between post-traumatic stress and sleep hygiene among surviving adults six months after the February 6th Kahramanmaraş Earthquakes in Turkey. This study was conducted with 1004 earthquake survivors in 11 provinces influenced by the February 6th Kahramanmaraş earthquakes in Turkey. The data was collected with Personal Information Form, Impact of Events Scale-Revised, and the Sleep Hygiene Index. It has been determined that 54.1% of earthquake survivors scored ≥ 33 on the Impact of Events Scale-Revised. The level of post-traumatic stress disorder was high and their sleep hygiene index level was above average among earthquake survivors. A strongly significant positive correlation was observed between the Impact of Events Scale-Revised and the Sleep Hygiene Index. It is recommended to plan and implement community-based psychological interventions to reduce post-traumatic stress disorder and improve sleep hygiene among survivors after earthquakes.

Abstract:Objective: It is well known that natural disasters such as earthquakes negatively affect physical and mental health by exposing people to excessive stress. The aim of this study was to investigate determinants of psychosocial health status among the pregnant and postpartum women who experienced earthquake in Türkiye. Methods: Pregnant and postpartum women (n = 125) living in tent cities in the Kahramanmaraş region formed the study sample. Data were collected between February 20 and 26, 2023, through face-to-face interviews. The instruments used for data collection were the Introductory Form, the Depression Anxiety Stress Scale, the Traumatic Childbirth Perception Scale, and the Post-Traumatic Stress Disorder–Short Scale. Results: A moderate positive relationship was found between stress and posttraumatic stress and traumatic childbirth perception in pregnant and postpartum women, and a high positive relationship was found between anxiety and depression. A high level of relationship was found between anxiety and stress and depression. Conclusions: It is seen that the psychosocial health of pregnant and postpartum women, who belong to the risk group in the earthquake zone, is at high risk. Psychological support is urgently needed to preserve and improve their psychosocial health.

Abstract:Given the profound impact of natural disasters on mental health, particularly in vulnerable groups such as adolescent and young adult, understanding their psychological results is crucial for providing necessary support and designing effective interventions. The aim of this study was to investigate the relationships between earthquake exposure, posttraumatic stress symptoms, coping mechanisms, and social support in adolescents and young adults who survived the 2023 earthquake in Turkey. The research was conducted in Kırıkhan district of Hatay province, which was severely damaged after the earthquake that occurred in Turkey in 2023. The sample for the descriptive and cross-sectional study consisted of 204 participants. Convenience sampling method was used for data collection. Data were collected using the Personal Information Form (PIF), Impact of Events Scale (IES), Stress Coping Strategies Inventory (CSSI), and Multidimensional Perceived Social Support Scale (MPSSS). Hierarchical multiple regression analysis was used to determine the factors associated with IES-R scores. Three models were tested to understand the factors predicting levels of posttraumatic stress (IES-R scores). The findings suggest that exposure to earthquake, problem-focused coping and seeking support play an important role in understanding posttraumatic stress levels. This study provides valuable insights into the psychological impact of the earthquake on adolescents and young adults, as well as the development of effective intervention strategies and support programs for young earthquake survivors.

Abstract:Objective This cross-sectional study investigates the immediate psychological effects of disaster relief operations on team members following 2 consecutive major earthquakes in Turkey. Methods A total of 170 participants, including professional firefighters, search and rescue (SAR) workers, and volunteers, were surveyed approximately 1 month after the conclusion of active SAR operations. The study utilizes the DSM-V criteria and the Posttraumatic Stress Disorder Checklist (PCL-5) to assess symptoms of post-traumatic stress disorder (PTSD) among participants. Results The findings reveal a point prevalence of 35.3% for probable PTSD, highlighting the substantial psychological impact on disaster relief teams. Factors such as age, residency in affected areas, and active SAR involvement significantly influenced probable PTSD rates. Interestingly, actively engaged SAR members had lower probable PTSD rates, possibly due to their training. Those who directly witnessed the earthquakes had higher scores, highlighting the impact of firsthand exposure. Additionally, individuals aged 50 and above displayed a higher mean total severity score compared to younger participants. Conclusions This research contributes to understanding the mental well-being of disaster relief professionals. The study's findings underscore the importance of timely mental health support and training for these responders, emphasizing the need for preparedness in disaster relief teams.

Abstract:This study aimed to explore the experiences of earthquake victims who experienced the Kahramanmaras earthquakes and individuals who actively served in search and rescue teams in the field. In this qualitative study, data were collected through semi-structured, in-depth interviews and focus group discussions (FGDs) forms from 47 earthquake victims living in various tent cities and 46 members of search and rescue teams in the Kahramanmaras and Hatay provinces. In-depth individual and focus group interviews were conducted using the maximum variation sampling method, which is a purposive sampling method, to identify the experiences of the participants. Data were analyzed using the content analysis method. The results revealed that search and rescue teams faced problems in coordination and transportation, communication, debris safety, and multidisciplinary reinforcement of their teams, especially during the intervention phase. Earthquake victims, on the other hand, were affected psychosocially and faced many problems such as access to the allocation of resources and aids and lack of information before, during, and after the earthquakes. In this context, these themes on the experiences of large-scale earthquakes that caused great psychosocial, economic, and social devastation from the perspectives of earthquake victims and search and rescue teams can contribute to the planning of early disaster prevention and response strategies for policymakers to improve disaster management at national and international levels.

Abstract:To evaluate the relationship between post-earthquake trauma levels and fear of happiness with sexual quality of life in women who had survived the earthquake. The study revealed that as the level of post-earthquake trauma and fear of happiness increased, so too did the quality of sexual life. These findings indicate that traumatic events strengthen the bonds between couples and positively affect the quality of sexual life. Nevertheless, providing support to individuals following the earthquake and informing them about sexual life and sexual health issues can contribute to individual health and harmony within the family, thereby positively impacting public health.

Abstract:This study investigated the prevalence and risk factors of probable posttraumatic stress disorder (PTSD), depression, and complicated grief among 529 adults residing in a container city, 4 months after the 2023 earthquakes in Türkiye. Participants were assessed using the PTSD Checklist for Diagnostic and Statistical Manual of Mental Disorders, 5th Edition , the Hamilton Depression Scale, and the Inventory of Complicated Grief. The prevalence rates of probable PTSD, depression, and complicated grief were 21.4%, 15.9%, and 64.7%, respectively. Logistic regression analyses indicated that PTSD was significantly associated with the loss of a loved one, fear of future earthquakes, and decreased perceived life control. Similarly, depression was associated with being trapped under debris, experiencing fear during the earthquake, and reduced perceived life control. These findings suggest that individuals presenting these risk factors warrant closer follow-up for potential psychological issues. Additionally, the impact of fear related to the earthquake and the role of perceived life control emerge as crucial considerations for psychological interventions.

Abstract:Türkiye has a long history of devastating earthquakes, and on February 6, 2023, the region experienced two major earthquakes with magnitudes of 7.7 and 7.6, striking Pazarcık and Elbistan, Kahramanmaraş, respectively, on the East Anatolian Fault Zone. These earthquakes resulted in significant loss of life and property, impacting multiple cities across 11 cities, and leaving a lasting impact on the country. The 2023 Kahramanmaraş Earthquakes rank among the deadliest and most damaging earthquakes in Türkiye, alongside the historical significance of the 1939 Erzincan Earthquake and the 1999 Marmara Earthquake. Despite reforms following the 1999 Marmara Earthquake in disaster policy and preparedness, the scale of damage from the February 6 earthquakes has been shocking, necessitating further insights and lessons for future earthquake management. This paper presents the outcomes of immediate response efforts organized after the 2023 Kahramanmaraş earthquakes to elucidate emergency response activities and their impacts on communities, considering the substantial size and severity of the damages. The study focuses on evaluating the emergency response provided within the first 24 h, 3 days, and 2 weeks after the earthquakes, aiming to promptly identify the nature and effectiveness of these responses, as well as the conditions that hindered their efficacy. By shedding light on the specific experiences and challenges faced during these crucial timeframes, the research aims to offer valuable insights and lessons learned. These findings contribute to improved preparedness strategies and more efficient emergency response measures needed in responding to future disaster scenarios. Ultimately, this study provides a useful resource for all stakeholders involved in emergency response and disaster management, offering valuable guidance to enhance resilience and preparedness in the face of seismic hazards.

Effects of peritraumatic reactions on post-traumatic stress among Kahramanmaras earthquake survivors

Abstract:PURPOSE: Peritraumatic reactions play a crucial role in the development of mental health problems, including depression and post-traumatic stress disorder. Therefore, this study sought to examine the influence of the peritraumatic reactions, including peritraumatic dissociation, peritraumatic distress, mental defeat, and tonic immobility, on post-traumatic stress disorder and major depressive disorder in earthquake survivors.MATERIALS AND METHODS: A total of 261 adult participants aged between 18 and 65 (M<sub>age</sub>=29.20, SD = 28.06, 162 were female, and 99 were male) who were exposed to the Kahramanmaras earthquake in February 2023 were recruited in the study. Data were collected between April 10 and 18 2023, two months after the earthquake. Participants completed questionnaires, including The International Trauma Questionnaire, The International Depression Questionnaire, The Mental Defeat Questionnaire, The Tonic Immobility Scale, and The Peritraumatic Dissociative Experiences Questionnaire.RESULTS: Two-step multiple linear regression analyses indicated all peritraumatic reactions predicted both post-traumatic stress disorder and depression. Dominance analysis results showed that the contribution of peritraumatic dissociation in predicting PTSD and depression was higher among other peritraumatic reactions.CONCLUSION: The findings of the study revealed a robust association between peritraumatic reactions and both depression and PTSD, shedding light on the underlying processes in the development of trauma-related disorders. Early assessment of peritraumatic reactions may be useful in identifying individuals at risk of developing PTSD and depression.

Psychological Assessment of Health Care Workers in the Aftermath of the February 2023 Earthquakes in Turkey

Abstract:Objective: The goal of this study was to examine the psychological and physical effects experienced by health care workers (HCWs) participating in the response to the February 2023 earthquakes in Turkey and to identify any associated factors. Methods: An online survey was used to collect data from HCWs on duty in earthquake-stricken areas. The following assessment tools were utilized: Posttraumatic Stress Disorder (PTSD) Checklist for DSM-5, Patient Health Questionnaire-9, Generalized Anxiety Disorder-7, Maslach Burnout Inventory, Posttraumatic Growth Inventory, and Short Form-12. Results: A total of 175 HCWs with a mean age of 37.27 years participated in the study. Of these, 39.4% suffered from PTSD, 30.3% experienced depression, and 31.4% experienced anxiety. Female gender, loss of significant others, and previous psychiatric treatment were found to be associated with worse mental health. Nurses tended to have higher levels of PTSD than the medical doctors; the medical doctors had significantly lower scores on the Posttraumatic Growth Inventory compared with the nurses and the other HCWs and lower mental component summary scores on the Short Form-12 compared with the other HCWs. Meeting basic needs and subjective evaluation of teamwork were also linked to mental health. The study also found that marital status, age, and length of time spent in earthquake-stricken areas were associated with scores on the Maslach Burnout Inventory. Conclusions: After the earthquake in Turkey, HCWs experienced a significant amount of various adverse mental health outcomes related to certain demographic variables such as gender, profession, previous psychiatric treatment, loss of relatives, and evaluation of living conditions and teamwork. Since HCWs play an essential role in reducing the harmful effects of disasters, recognizing groups at risk and planning tailored interventions may help prevent mental health issues.
