Abstract:Toxocariasis, a disease caused by infection with larvae of Toxocara canis, T. cati and/or congeners, represents clinical syndromes in humans including visceral and ocular larva migrans, neurotoxocariasis and covert/common toxocariasis. It is reported to be one of the most widespread public health and economically important zoonotic parasitic infections that humans share with dogs, wild canids, including foxes, and possibly other mammals. Humans become infected by accidental ingestion of embryonated Toxocara eggs, or larvae from tissues from domestic or wild paratenic hosts. Most infections are asymptomatic, and human disease may go unnoticed, as clinical investigation is often not pursued and/or diagnostic testing not conducted. Sometimes toxocariasis can be associated with complications, such as allergic and/or neurological disorders, possibly including cognitive or developmental delays in children. There is no anti-toxocariasis vaccine, and chemotherapy in humans varies, depending on symptoms and location of larvae, and may include the administration of albendazole or mebendazole, together with anti-inflammatory corticosteroids. Some recent studies indicate that toxocariasis is having an increased, adverse impact on human health in some, particularly underprivileged, tropical and subtropical communities around the world. Although tens of millions of people, especially children, are expected to be exposed to, or infected with Toxocara species, there is limited precise epidemiological data or information on the relationship between seropositivity and disease (toxocariasis) on a global scale. To gain an improved insight into this area, the present article reviews salient clinical aspects of human toxocariasis and the epidemiology of this disease, with particular reference to seroprevalence, and discusses future research and approaches/measures to understand and prevent/control this socioeconomically important, yet neglected zoonosis.
Ocular toxocariasis: new diagnostic and therapeutic perspectives

Abstract:Purpose: To provide an updated insight of concepts regarding the overview, epidemiology, risk factors, clinical manifestations, diagnosis, treatment and prevention of ocular toxocariasis. Design: Perspective of literature review. Methods: Review and synthesis of literature about toxocariasis, with interpretation and perspective. A literature search for "ocular toxocariasis" was performed using PubMed, ScienceDirect, Scopus, SciELO and LILACS databases. Results: Mild to moderate infections are frequently reported in ocular toxocariasis which usually occurs in children and typically presents as unilateral vision impairment, blindness is common and could present invasion of the retina. There are three groups of presentation of toxocariasis (according to the physical examination): chronic endophthalmitis, posterior granuloma and peripheral granuloma. Standard diagnosis of ocular toxocariasis is based on the identification of clinical signs, supported by additional diagnostic methods. Regarding treatment, there is no commonly accepted regimen but most of the authors prefer to use steroids and anthelminthic agents, but nowadays there are no standardized parameters in terms of dosage, duration and route of administration. Surgery has been recommended in some cases. Conclusion: Toxocariasis is still a problem of public health, particularly in developing countries with an increasing epidemiological burden in terms of morbidity and mortality and most of the authors agree on the utmost relevance of its prevention. Clinical experience and suspicion of ophthalmologists make an important role in its diagnosis, but always with supportive diagnostic methods. Additional studies should explore new therapeutic options for toxocariasis.
Clinical Course and Optical Coherence Tomography of Ocular Angiostrongyliasis: A Case Report

Abstract:Purpose: To report the clinical course and optical coherence tomography (OCT) findings of ocular angiostrongyliasis. Methods: A 36-year-old female with a history of ingesting regular raw freshwater shrimp and other raw food presented with acute unilateral painless visual loss in the right eye. Her right eye's best-corrected visual acuity (BCVA) was 1 ft of the count finger. Fundus examination showed vitritis, generalized retinal pigment epithelial alteration, and a moving roundworm in the vitreous at the 6 o'clock position. Macular OCT of her right eye showed thinning of the retina, loss of the external limiting membrane and ellipsoid zone, subretinal hyper-reflective material clumping, and hyper-reflective foci at the superficial choroidal layer. Results: The patient was administered oral and topical prednisolone. The roundworm, identified as Angiostrongylus cantonensis, was wholly extracted from the vitreous using a 23G sclerotomy port and pars plana vitrectomy. The final BCVA was 1 ft of the count finger. Conclusion: This case report describes an infrequent presentation and illustrates the clinical course and OCT findings of ocular angiostrongyliasis.

Abstract:Toxoplasma gondii, an obligate intracellular parasite, is a globally prevalent pathogen capable of infecting a wide range of warm-blooded animals, including humans. Ocular toxoplasmosis (OT), a severe manifestation of T. gondii infection, can lead to potentially blinding complications. This comprehensive review delves into the current understanding of T. gondii biology, exploring its complex life cycle, diverse transmission routes, and strain diversity. This article provides an in-depth analysis of the clinical manifestations of OT, which can result from both congenital and acquired infections, presenting a spectrum of signs and symptoms. The review examines various diagnostic strategies employed for OT, including clinical examination, multimodal imaging techniques such as fundus fluorescein angiography (FFA), indocyanine green angiography (ICGA), optical coherence tomography (OCT), and optical coherence tomography angiography (OCTA), as well as laboratory tests including serology and molecular methods. Despite extensive research, the specific mechanisms underlying ocular involvement in T. gondii infection remain elusive, and current diagnostic options have limitations. Moreover, the treatment of active and recurrent OT remains a challenge. While existing therapies, such as antimicrobial agents and immunosuppressants, can control active infections, they do not offer a definitive cure or completely prevent recurrence. The clinical endpoints for the management of active and recurrent OT are also not yet well-established, and the available treatment methods carry the potential for adverse effects. This article highlights the need for future research to elucidate the pathogenesis of OT, investigate genetic factors influencing susceptibility to infection, and develop more sensitive and specific diagnostic tools. Enhancing global surveillance, implementing robust prevention strategies, and fostering multidisciplinary collaborations will be crucial in reducing the burden of OT and improving patient outcomes. This comprehensive review aims to provide a valuable resource for clinicians, researchers, and policymakers, contributing to a better understanding of T. gondii infection and its impact on ocular health.

Abstract:Ocular toxocariasis is a clearly defined disease. However, much remains to be learned concerning the migratory route, ocular changes, diagnosis and treatment. Studies in paratenic hosts have contributed to our understanding and will yield more information. Various experimental animals have been used, such as mice, rabbits, guinea pigs, primates, hamsters and gerbils. Of these, the last appear to be the most appropriate model due to their high susceptibility to ocular infection. Results obtained from different animal models are often not comparable due to the fact that dose and routes of inoculation are diverse. Early stages in the pathogenesis of ocular toxocariasis are manifested by haemorrhages in the anterior chamber and iris, replaced in time by accumulations of white cells. Ocular migration produces an early cell reaction, formed by an infiltration of neutrophils accompanied by vasculitis and retinal microinfarcts. Over a period of time, an increase of macrophages and the distribution of the infiltrates is observed. Later, granulomatous lesions are formed. These do not necessarily contain a larva and their appearance varies in different animal models. Local production of IgE and the presence of specific IgG have been described.

Echographic characteristics of ocular toxocariasis

Abstract:Ocular toxocariasis most commonly presents as one of three distinct clinical syndromes: endophthalmitis, localized posterior granuloma, or peripheral granuloma. Using standardized echography, the authors studied 11 toxocariasis patients presenting with one of these syndromes. Common echographic findings were noted in ten of these patients, and included: (1) a solid, high-reflective peripheral mass, (2) a vitreous band or membranes extending between the posterior pole and the mass, and (3) a traction retinal detachment or fold from the posterior pole to the mass. These findings were present in patients presenting with leukocoria or endophthalmitis, as well as in patients in whom the peripheral nematode lesion could be visualized clinically. In combination with the history, clinical examination, and enzyme-linked immunosorbent assay (ELISA), standardized echography may be useful in establishing the diagnosis of ocular toxocariasis in cases of leukocoria in which nematode endophthalmitis is suspected.
Human ocular angiostrongyliasis: a literature review

Abstract:Human ocular angiostrongyliasis caused by Angiostrongylus cantonensis infection in the eye is a very rare condition. Until now, there has been no comprehensive analysis of this disease. We searched and analysed the references found on the Internet that refer to human ocular angiostrongyliasis and reviewed the aetiology, clinical manifestations, diagnosis, epidemiology and treatment of the condition. Twenty-six references were found reporting 35 patients from 10 countries that were diagnosed with human ocular angiostrongyliasis. People are usually infected by eating raw or undercooked intermediate hosts of the parasite such as snails or contaminated vegetables. The most common symptom was visual loss. Although several treatments have been used, ocular angiostrongyliasis can still result in permanent visual impairment and may even cause blindness. As the eye is the site of infection and direct visualization is possible, ocular examination is crucial for diagnosis. The therapeutic success depended on early and complete surgical removal.

Abstract:PurposeTo explore the clinical characteristics of patients with ocular toxocariasis in eastern China.MethodsThe medical records of 35 cases of ocular toxocariasis in Fudan University Eye & ENT Hospital between May 2009 and April 2011 were retrospectively reviewed and analyzed. UBM, RetCam or fundus imaging, and high-frequency or conventional ultrasonography were performed in these patients.ResultsThe mean patient age in our series of ocular toxocariasis was 11.86 ± 8.80 years. There were 28 children and seven adults, the majority residing in a rural area (88.57%). All cases were classified into three clinical subtypes: granuloma in the peripheral retina (60% of cases); granuloma in the posterior pole (28.57% of cases); and vitreous inflammation mimicking chronic endophthalmitis (5.71% of cases). We also identified an additional subtype with unique clinical features that we termed “combined type” presenting in 5.71% of our patients. This subtype exhibited granulomas in both the posterior pole and peripheral retina. RetCam fundus imaging was able to identify granulomas in the posterior and peripheral subtypes, 100% and 80.95% of cases respectively. Moreover, UBM combined with conventional ultrasonography identified granulomas in 95% of the peripheral subtype cases and in 100% of the posterior pole subtype cases.ConclusionsThis is the first ocular toxocariasis series described in Chinese patients. Compared with reports from developed countries, the ocular features in our series were more severe and complicated, presenting with poorer visual acuity and a high rate of retinal detachment (45.7%). The application of RetCam and UBM during examination in ocular toxocariasis can provide valuable information in determining the severity of disease and features important in considering surgical procedures in such patients.
Pediatric Ocular Toxocariasis in Costa Rica: 1998-2018 Experience

Abstract:Purpose: To report the epidemiology, clinical characteristics, treatment and outcomes of patients with ocular toxocariasis. Methods: Retrospective review of patients diagnosed with ocular toxocariasis at a pediatric referral center from 1998 until 2018 in Costa Rica. Results: 157 patients were diagnosed with ocular toxocariasis with a mean follow-up of 3.1 years. The mean age at presentation was 6.7 years old. The most common causes of consultation included decreased vision (29.9%), strabismus (26.7%), and leukocoria (19.7%). The most common findings included peripheral granuloma, posterior pole granuloma, and chronic endophthalmitis. Sixty-nine (43.9%) eyes had retinal detachment. Patients were managed with a variety of treatments including pars plana vitrectomy, systemic corticosteroids, anti-helminthic drugs or a combination of these. Most eyes did not exhibit functional improvement despite treatment. Conclusions: Most patients with pediatric ocular toxocariasis in Costa Rica often present late and despite treatment, most remain with poor vision in the affected eye.
[The ocular form of larval toxocariasis in the Czech Republic]

Abstract:The damage of the eye is one of the clinical syndromes of the Toxocara infection, caused by the migration activity of the Toxocara canis and Toxocara cati larvae. Ocular larva migrans (OLM) lesions mostly occur unilaterally and frequently are diagnosed as retinoblastoma. The typical symptoms of OLM are presented in our publication. The first signs of the ocular infection most often include diminished vision, leukocoria, red eye and strabismus. Inflammatory intraocular reaction, caused as a response to the antigens, released from dead or dying larvae is often diagnosed. The living larvae are observed very rarely. The reason and a way of the ocular invasion are still not sufficiently cleared. The supposition that the ocular syndrome is caused by migration of single larvae, when the immunological response is light, was not reliably verified. Sometimes if the infection size was heavy, the eye can be invaded with systemic complications as well. The most common laboratory findings include an elevation of the immunoglobulins in the serum of the patient and the presence of Toxocara specific antibodies response in the vitreous and/or aqueous humor. An analysis of the humor in cases of presumptive OLM was made only in 8 patients because not all oculists are ready to take this material, mainly in children. Highly sensitive assay--ELISA reaction with specific excretory-secretory antigen (TES) was used. For discrimination between chronic and recent infection the method of measuring IgG avidity was applied.

Abstract:BACKGROUND:Echinococcosis is a dangerous zoonotic parasitic disease. Ocular echinococcosis is very rare, especially the hydatid cysts in subretinal space. We present a case of subretinal echinococcosis and management.CASE PRESENTATION:A 37-year-old man with subretinal echinococcosis who developed panuveitis and visual impairment. The patient lives on agriculture and animal husbandry, which made him susceptible to parasitic infection. He had severe panuveitis and blurred vision on arrival at hospital. According to his ocular examination and systemic review, the subretinal echinococcosis diagnosis was made. The patient received pars plana lensectomy and pars plana vitrectomy. The lesion underneath his retina was removed, and histopathology examination confirmed the subretinal echinococcosis diagnosis.CONCLUSIONS:Echinococcosis is a dangerous zoonotic parasitic disease in pastoral areas. Ocular echinococcosis is usually secondary to systemic infection. Although the incidence is rare, the disease could lead to destructive visual function impairment.

Abstract:Purpose: Ocular toxocariasis (OT) is a zoonotic infection caused by larval stages of Toxocara canis and T. cati. The current review and meta-analysis aimed to evaluate the global prevalence of OT. Methods: Five English (PubMed, Scopus, Science Direct, Web of Science, and Google Scholar) databases were explored and 101 articles met the inclusion criteria. Results: The pooled prevalence (95% confidence interval) of OT was higher in immunological studies (9%. 6-12%) than in studies that applied ophthalmic examination (1%. 1-2%). The lower middle-income level countries had the highest prevalence (6%. 2-12%) as well as the African region (10%. 7-13%). The highest infection rate (4%. 2-7%) was detected in the 1-25 mean age group. Conclusion: Regular anthelminthic treatment of cats and dogs, and removal of animal feces from public places must be considered.
[Clinical features, the effectiveness of treatment and function of vision organ in children and adolescents with ocular toxocariasis]

Abstract:Purpose: Toxocariasis is caused by Toxocara canis, the intestinal nematode commonly found in dogs. Patients are mostly young children, with an average age of 7.5 years. Patients have unilateral progressive painless vision loss and occasional leukocoria. Ocular toxocariasis has a variable presentation, from localized peripheral or macular granulomas to chronic endophthalmitis. Material and methods: Clinical features, the effectiveness of treatment and function of vision organ in 19 children and adolescents with ocular toxocariasis were analyzed retrospectively. Results: The mean patient age was 10.3 years. Inflammation was unilateral in all patients. In 11 cases (57.9%) there was peripheral granuloma, in 3 cases (15.8%) posterior pole granuloma and endophthalmitis in 5 cases (26.3%). Zentel was applied in 14 patients, Hetrazan in 3 patients and Mintezol in 2 patients. Improvement of visual acuity was achieved in 11 eyes (57.9%), no improvement in 7 eyes (36.8%), deterioration in 1 eye (5.3%). Conclusions: Ocular toxocariasis is the disease with differentiated clinical presentation. Toxocara uveitis can cause significant vision loss. In some cases quick diagnosis and effective treatment with corticosteroids and antihelminthics allowed to obtain clinical improvement.
Parasitic Eye Diseases: Nuances in Rapid Identification and Advanced Microscopy

Abstract:Aim: To study parasitic eye diseases in a tertiary institute of North-east India by live examination of parasites, rapid staining, and scanning electron microscopy (SEM). Methods: A 12-year retrospective analysis was performed and all patients diagnosed with ocular parasitic diseases were identified. Examination under a compound microscope, fluorescein staining, and scanning electron microscopy were done. Results: A total of 160 ocular parasitosis cases were identified. The cases for which rapid staining and SEM studies were done included Cysticercosis (n = 18, 11.25%), Hydatidosis (n = 5, 3.13%), Dirofilariasis (n = 5, 3.13%), Thelaziasis (n = 3, 1.87%), and Gnathostomiasis (n = 2, 1.25%). Live examination was performed in 11 cases (6.63%) and 8 cases (4.82%) underwent scanning electron microscopy. . Conclusion: Fluorescein staining for identification of parasites and SEM study helped in detailing microscopic and ultrastructural findings.
Study of onchocerciasis-related visual impairment in North Kivu province of the Democratic Republic of Congo in Africa

Abstract:Purpose: The Objective of this study is to determine baseline data regarding onchocercal eye lesions and associated visual loss in the Nord Kivu province, an onchocerciasis hyperendemic tropical rain forest area in the Democratic Republic of Congo (DRC). Methods: A cross-sectional study was conducted in the Nord Kivu province of the DRC during which 2150 subjects were examined ophthalmologically. The eye examination included visual acuity (VA), slit-lamp examination, ophthalmoscopy, intraocular pressure, and visual field assessment by the confrontation test. Patients with suspicion of glaucoma were further evaluated by Humphreys automated perimeter. Results: 39 (1.81%) out of 2150 subjects had onchocerciasis-related eye lesions and 4 (0.19%) were blind (VA <3/60). Chorioretinitis (0.88%) was the most frequent onchocerciasis lesion followed by keratitis (0.46%), microfilaria in the anterior chamber (0.28%), iridocyclitis (0.28%), secondary glaucoma (0.19%), complicated cataract (0.19%), and optic atrophy (0.19%). Visual impairment was discovered in 114 (5.3%) out of 2150 subjects, of whom 39 (0.19%) had blindness and 75 (3.4%) had low vision. Visual impairment was mostly caused by nononchocerciasis-related diseases like cataract (27.2%), retinal diseases (19.3%), glaucoma (15.8%), and iridocyclitis (15.8%) rather than because of onchocerciasis (9.6%) among all causes of visual impairment. Conclusion: Features of ocular onchocerciasis usually described in forest and savanna areas were relatively less common than expected in and around Goma, the capital of the Nord Kivu province of the DRC.
[Ocular manifestations of toxocariasis in schoolchildren from the state from the Anzoátegui state in Venezuela]

Abstract:The objective of this study was to determine ocular manifestations of toxocariasis in schoolchildren. A study was conducted in two schools in the Anzoátegui state in Venezuela in 2019. The ELISA test was used to determine IgG antibodies against Toxocara spp. The families completed a questionnaire, and the children were clinically evaluated by pediatricians and ophthalmologists. 118 children participated, 18.6% presented anti-Toxocara spp. The clinical manifestations with a statistically significant association were allergic reactions, epiphora, and decreased visual acuity. The ophthalmological evaluation found keratitis, uveitis, iritis, retinal granuloma, endophthalmitis, amaurosis, leukocoria, retinal detachment and endotropia. The findings show a high frequency of eye disease in children with toxocariasis from a state of Venezuela.

Abstract:Parasitic diseases are relatively rarely diagnosed and treated in Europe. Therefore, European clinicians are not familiar with their clinical and imaging features. In an era of increased human migration, it is fundamental for clinicians to be able to identify such diseases. We have recently described the features of cystic echinococcosis, schistosomiasis, fascioliasis and ascariasis. Here, we report on the clinical and imaging features as well as on the current therapy options of infections by the small liver flukes: Clonorchis sinensis, Opisthorchis viverrini (Southeast Asian liver fluke) and Opisthorchis felineus (cat liver fluke) and other Opisthorchis species prevalent in South Asia.

Abstract:Ocular toxocariasis damages vision and may cause blindness. It is a relatively 'new' disease, the histological changes having been described in 1950 and the causative organism identified in 1956. Many aspects of the epidemiology of toxocariasis are unclear. It is generally accepted that the usual route of entry to the eye is via the blood stream. Once the eye is invaded characteristic changes are produced with comparative sparing of the anterior segment. Species differences in susceptibility can be very marked. In Mongolian gerbils 55% of infected animals exhibit ocular lesions. Humans are fortunate in that they are far less susceptible to ocular infection. While it has been established that ocular toxocariasis is caused by Toxocara canis, the role of T. cati in this condition is still unclear. It is perhaps surprising that the prevalence of such a disease in humans should remain in doubt. However, the wide variation in the few estimates of prevalence may well be a reflection of the wide variation in exposure as evidenced by the wide variation in seroprevalence. A further factor mitigating against an identification of prevalence rates is the fact that the condition is usually unilateral. Most surveys of blindness stipulate a vision of 6/60 or less in the better eye and so toxocariasis cases go unrecorded. There is a need for surveys which identify the level of vision in both eyes.

Abstract:Ocular toxoplasmosis (OT) is an intraocular infection caused by the parasite Toxoplasma gondii. OT is manifested as retinal choroiditis and is the most common infectious cause of posterior uveitis. Invasion of the retina by T. gondii leads to disruption of the blood-ocular barrier and promotes the migration of immune cells to the ocular tissues. Cytokines such as IFN-γ and IL-1β are effective for controlling parasite growth, but excessive inflammatory responses can cause damage to the host. In this review, we will discuss in detail the latest advances in the immunopathology and treatment of OT.

Abstract:Purpose of review: This review highlights recent studies that have increasingly implicated cytomegalovirus (CMV) as a significant cause of keratouveitis and retinitis in immunocompetent hosts. Recent findings: Molecular testing has identified that CMV infection is frequently present in cases of Posner-Schlossman and Fuchs, keratouveitis syndromes previously presumed to be idiopathic conditions. Ocular hypertension and endothelial cell loss are important complications of CMV keratouveitis and are likely mediated by viral invasion of the trabecular meshwork and corneal endothelium. Topical ganciclovir is a well tolerated, effective, and economical therapy. CMV retinitis is possible in the absence of HIV/AIDS. Summary: CMV has long been considered an innocuous infection in the general population, though recent studies have found otherwise. Intraocular reactivation, replication, and invasion of the trabecular meshwork and endothelium lead to recurrent bouts of ocular hypertension and endothelial cell loss, the complications of which may be tempered with initiation of antivirals. Topical ganciclovir is a promising therapy that needs investigation. CMV retinitis, an entity previously believed isolated to the severely immunosuppressed population, has been reported on numerous occasions in presumably immunocompetent individuals, particularly following local steroid injections. Further studies may elucidate the pathogenesis of CMV in immunocompetent populations.
