Abstract:Abstract Background The deformation of oral and maxillofacial region leads to not only the damage of morphology and function, but also a series of aesthetic and psychological problems, severely affecting the quality of life of patients. Oral tissue engineering refers to developing biomaterials for repair or regeneration, with the application of tissue engineering technologies. This has become an area of increasing prominence. Current biologically inert materials are insufficient to fulfill clinical requirements. Therefore, tissue-engineered biomaterials with bioactive, even bionic properties are desperately needed. Main body The complexity of the anatomy and the diversity of tissue types of oral and maxillofacial region pose great challenges to the regeneration, in the aspects of both biomaterials and manufacturing technologies. Biomaterials in clinical practice or research have evolved from natural materials to synthetic materials, from homogeneous materials to multiple composite materials. And now composite materials have increasingly demonstrated their advantages in terms of physicochemical and biological properties over conventional materials. In terms of manufacturing, traditional coating, sintering, and milling technologies can no longer satisfy the requirements for high-precision bionic structures of oral-tissue-engineering biomaterials. Scientists have turned to biofabrication technologies such as microfluidics and additive manufacturing. Short conclusion This review aims to summarize the noteworthy advancements made in biomaterials of oral tissue engineering. We outlined the current biomaterials and manufacturing technologies and focused on various applications of these materials that may be connected to clinical treatment and research. We also suggested the future direction of development for biomaterials in oral tissue engineering. In future, biomaterials characterized by precision, functionalization, and individualization will be manufactured through digital, microfluidic, and 3D printing technologies. Graphical abstract

Abstract:Conditions such as congenital anomalies, cancers, and trauma can all result in devastating deficits of bone in the craniofacial skeleton. This can lead to significant alteration in function and appearance that may have significant implications for patients. In addition, large bone defects in this area can pose serious clinical dilemmas, which prove difficult to remedy, even with current gold standard surgical treatments. The craniofacial skeleton is complex and serves important functional demands. The necessity to develop new approaches for craniofacial reconstruction arises from the fact that traditional therapeutic modalities, such as autologous bone grafting, present myriad limitations and carry with them the potential for significant complications. While the optimal bone construct for tissue regeneration remains to be elucidated, much progress has been made in the past decade. Advances in tissue engineering have led to innovative scaffold design, complemented by progress in the understanding of stem cell-based therapy and growth factor enhancement of the healing cascade. This review focuses on the role of biomaterials for craniofacial bone engineering, highlighting key advances in scaffold design and development.

Abstract:Bone defects in the craniomaxillofacial (CMF) region occur as a result of trauma, cancer excisions, congenital defects and deformities. CMF bone reconstruction is challenging due to anatomical complexity, functionality and aesthetic conditions. Bone grafting is the current standard approach to reconstruct CMF injuries. However, it possesses certain limitations to restore the structural and functional defects of CMF bone. To overcome these limitations, tissue engineering and regenerative methodologies have been developed to reconstruct damaged bone tissues. Various biomaterial scaffolds made up of synthetic, natural and ceramics materials have been used to repair craniofacial bone tissues. In this review, we have first discussed the structure of CMF bone, bone tissue engineering and mechanical properties required for CMF bone tissue engineering. Subsequently, a detailed survey on the recent research innovations of tissue engineering with various types of biomaterial scaffolds, stem-cell sources, growth factors and functional scaffold fabrication techniques to repair and remodel the CMF defects has been presented.

Abstract:Reconstruction of craniofacial defects poses a challenging task for craniofacial surgeons. Reconstruction of the underlying craniofacial bone is crucial for successful outcome of these defects. Bone grafting is the standard technique employed for bone reconstruction. However, with the advent of novel biomaterials as bone substitutes for grafting procedures, attractive alternatives have unlocked for surgeons. Since a few decades, the field of craniofacial bone regeneration has expanded enormously with the advent of novel biomaterials and innovative concepts such as bioprinting and drug delivery. At present, several natural and synthetic bone graft materials are available as bone substitutes. In this chapter, the authors will focus on the various biomaterials that are currently used as bone substitutes in craniofacial bone regeneration along with an update of the active research among each class of biomaterials going on in this area. Also, the authors will brief-in the recent concepts for craniofacial bone regeneration.

Abstract:In this review, we explore the application of novel biomaterial-based therapies specifically targeted towards craniofacial bone defects. The repair and regeneration of critical sized bone defects in the craniofacial region requires the use of bioactive materials to stabilize and expedite the healing process. However, the existing clinical approaches face challenges in effectively treating complex craniofacial bone defects, including issues such as oxidative stress, inflammation, and soft tissue loss. Given that a significant portion of individuals affected by traumatic bone defects in the craniofacial area belong to the aging population, there is an urgent need for innovative biomaterials to address the declining rate of new bone formation associated with age-related changes in the skeletal system. This article emphasizes the importance of semiconductor industry-derived materials as a potential solution to combat oxidative stress and address the challenges associated with aging bone. Furthermore, we discuss various material and autologous treatment approaches, as well as in vitro and in vivo models used to investigate new therapeutic strategies in the context of craniofacial bone repair. By focusing on these aspects, we aim to shed light on the potential of advanced biomaterials to overcome the limitations of current treatments and pave the way for more effective and efficient therapeutic interventions for craniofacial bone defects.

Abstract:Highlights • Special considerations are needed when designing scaffolds for maxillofacial bone tissue engineering. • Clinically available scaffolds for maxillofacial bone repair need improvements. • Preclinical explorations of novel biomaterials for maxillofacial bone tissue engineering are emerging. Maxillofacial bone defects caused by congenital malformations, trauma, tumors, and inflammation can severely affect functions and aesthetics of maxillofacial region. Despite certain successful clinical applications of biomaterial scaffolds, ideal bone regeneration remains a challenge in maxillofacial region due to its irregular shape, complex structure, and unique biological functions. Scaffolds that address multiple needs of maxillofacial bone regeneration are under development to optimize bone regeneration capacity, costs, operational convenience. etc. In this review, we first highlight the special considerations of bone regeneration in maxillofacial region and provide an overview of the biomaterial scaffolds for maxillofacial bone regeneration under clinical examination and their efficacy, which provide basis and directions for future scaffold design. Latest advances of these scaffolds are then discussed, as well as future perspectives and challenges. Deepening our understanding of these scaffolds will help foster better innovations to improve the outcome of maxillofacial bone tissue engineering. Abstract:The intricate nature of oral-maxillofacial structure and function, coupled with the dynamic oral bacterial environment, presents formidable obstacles in addressing the repair and regeneration of oral-maxillofacial bone defects. Numerous characteristics should be noticed in oral-maxillofacial bone repair, such as irregular morphology of bone defects, homeostasis between hosts and microorganisms in the oral cavity and complex periodontal structures that facilitate epithelial ingrowth. Therefore, oral-maxillofacial bone repair necessitates restoration materials that adhere to stringent and specific demands. This review starts with exploring these particular requirements by introducing the particular characteristics of oral-maxillofacial bones and then summarizes the classifications of current bone repair materials in respect of composition and structure. Additionally, we discuss the modifications in current bone repair materials including improving mechanical properties, optimizing surface topography and pore structure and adding bioactive components such as elements, compounds, cells and their derivatives. Ultimately, we organize a range of potential optimization strategies and future perspectives for enhancing oral-maxillofacial bone repair materials, including physical environment manipulation, oral microbial homeostasis modulation, osteo-immune regulation, smart stimuli-responsive strategies and multifaceted approach for poly-pathic treatment, in the hope of providing some insights for researchers in this field. In summary, this review analyzes the complex demands of oral-maxillofacial bone repair, especially for periodontal and alveolar bone, concludes multifaceted strategies for corresponding biomaterials and aims to inspire future research in the pursuit of more effective treatment outcomes.

Abstract:Calvarial defects are common reconstructive dilemmas secondary to a variety of etiologies including traumatic brain injury, cerebrovascular disease, oncologic resection, and congenital anomalies. Reconstruction of the calvarium is generally undertaken for the purposes of cerebral protection, contour restoration for psychosocial well-being, and normalization of neurological dysfunction frequently found in patients with massive cranial defects. Current methods for reconstruction using autologous grafts, allogeneic grafts, or alloplastic materials have significant drawbacks that are unique to each material. The combination of wide medical relevance and the need for a better clinical solution render defects of the cranial skeleton an ideal target for development of regenerative strategies focused on calvarial bone. With the improved understanding of the instructive properties of tissue-specific extracellular matrices and the advent of precise nanoscale modulation in materials science, strategies in regenerative medicine have shifted in paradigm. Previously considered to be simple carriers of stem cells and growth factors, increasing evidence exists for differential materials directing lineage specific differentiation of progenitor cells and tissue regeneration. In this work, we review the clinical challenges for calvarial reconstruction, the anatomy and physiology of bone, and extracellular matrix-inspired, collagen-based materials that have been tested for in vivo cranial defect healing.

Abstract:Biomimetics is the science of imitating nature's designs and processes to create innovative solutions for various fields, including dentistry and craniofacial reconstruction. In these areas, biomimetics involves drawing inspiration from living organisms/systems to develop new materials, techniques, and devices that closely resemble natural tissue structures and enhance functionality. This field has successfully demonstrated its potential to revolutionize craniofacial procedures, significantly improving patient outcomes. In dentistry, biomimetics offers exciting possibilities for the advancement of new dental materials, restorative techniques, and regenerative potential. By analyzing the structure/composition of natural teeth and the surrounding tissues, researchers have developed restorative materials that mimic the properties of teeth, as well as regenerative techniques that might assist in repairing enamel, dentin, pulp, cementum, periodontal ligament, and bone. In craniofacial reconstruction, biomimetics plays a vital role in developing innovative solutions for facial trauma, congenital defects, and various conditions affecting the maxillofacial region. By studying the intricate composition and mechanical properties of the skull and facial bones, clinicians and engineers have been able to replicate natural structures leveraging computer-aided design and manufacturing (CAD/CAM) and 3D printing. This has allowed for the creation of patient-specific scaffolds, implants, and prostheses that accurately fit a patient's anatomy. This review highlights the current evidence on the application of biomimetics in the fields of dentistry and craniofacial reconstruction.

Abstract:Abstract Issues caused by maxillofacial tumours involve not only dealing with tumours but also repairing jaw bone defects. In traditional tumour therapy, the systemic toxicity of chemotherapeutic drugs, invasive surgical resection, intractable tumour recurrence, and metastasis are major threats to the patients’ lives in the clinic. Fortunately, biomaterial-based intervention can improve the efficiency of tumour treatment and decrease the possibility of recurrence and metastasis, suggesting new promising antitumour therapies. In addition, maxillofacial bone tissue defects caused by tumours and their treatment can negatively affect the physiological and psychological health of patients, and investment in treatment can result in a multitude of burdens to society. Biomaterials are promising options because they have good biocompatibility and bioactive properties for stimulation of bone regeneration. More interestingly, an integrated material regimen that combines tumour therapy with bone repair is a promising treatment option. Herein, we summarized traditional and biomaterial-mediated maxillofacial tumour treatments and analysed biomaterials for bone defect repair. Furthermore, we proposed a promising and superior design of dual-functional biomaterials for simultaneous tumour therapy and bone regeneration to provide a new strategy for managing maxillofacial tumours and improve the quality of life of patients in the future.

Abstract:Craniofacial fractures and bony defects are common causes of morbidity and contribute to increasing health care costs. Successful regeneration of bone requires the concomitant processes of osteogenesis and neovascularization. Current methods of repair and reconstruction include rigid fixation, grafting, and free tissue transfer. However, these methods carry innate complications, including plate extrusion, nonunion, graft/flap failure, and donor site morbidity. Recent research efforts have focused on using stem cells and synthetic scaffolds to heal critical-sized bone defects similar to those sustained from traumatic injury or ablative oncologic surgery. Growth factors can be used to augment both osteogenesis and neovascularization across these defects. Many different growth factor delivery techniques and scaffold compositions have been explored yet none have emerged as the universally accepted standard. In this review, we will discuss the recent literature regarding the use of stem cells, growth factors, and synthetic scaffolds as alternative methods of craniofacial fracture repair.

Abstract:Mandibles are the largest and strongest bone in the human face and are often severely compromised by mandibular defects, compromising the quality of life of patients. Mandibular defects may result from trauma, inflammatory disease and benign or malignant tumours. The reconstruction of mandibular defect has been a research hotspot in oral and maxillofacial surgery. Although the principles and techniques of mandibular reconstruction have made great progress in recent years, the development of biomedical materials is still facing technical bottleneck, and new materials directly affect technological breakthroughs in this field. This paper reviews the current status of research and application of various biomaterials in mandibular defects and systematically elaborates different allogeneic biomaterial-based approaches. It is expected that various biomaterials, in combination with new technologies such as digital navigation and 3D printing, could be tuned to build new types of scaffold with more precise structure and components, addressing needs of surgery and post-reconstruction. With the illustration and systematization of different solutions, aims to inspire the development of reconstruction biomaterials.

Abstract:Three-dimensional printed natural and synthetic biomaterials have evolved as gold standards for tissue engineering scaffolds in recent trends owing to their superior role in hard tissue regeneration. The major drawback of these scaffolds is their relatively poor mechanical strength. Another key consideration in the design of the scaffolds is the difficulty in replicating the complex structural composition of hard tissues such as bones, and its structure cannot be reproduced with a single material that provides a limited range of properties. Sufficient mechanical strength is provided by the structure required for the replacement tissue. The mechanical properties of the scaffold play an important role in many applications of tissue engineering. Therefore, is it sufficient to withstand the force with only a single material used for the scaffold? There are many materials such as natural resin, synthetic resin, and polymers. They are used in combination to fulfill the function and to act as a kingpin by solving their drawbacks. The added material is not only superior in mechanical strength but also compatible with the tissues surrounding the implant, promoting cell adhesion and gradually degrading rather than intoxicating the patient. This review focuses on the various biomaterials used as scaffolds for critical size defects and the aftermath in their mechanical properties.

Abstract:Computer-aided design/computer-aided manufacturing and 3-dimensional (3D) printing techniques have revolutionized the approach to bone tissue engineering for the repair of craniomaxillofacial skeletal defects. Ample research has been performed to gain a fundamental understanding of the optimal 3D-printed scaffold design and composition to facilitate appropriate bone formation and healing. Benchtop and preclinical, small animal model testing of 3D-printed bioactive ceramic scaffolds augmented with pharmacological/biological agents have yielded promising results given their potential combined osteogenic and osteoinductive capacity. However, other factors must be evaluated before newly developed constructs may be considered analogous alternatives to the "gold standard" autologous graft for defect repair. More specifically, the 3D-printed bioactive ceramic scaffold's long-term safety profile, biocompatibility, and resorption kinetics must be studied. The ultimate goal is to successfully regenerate bone that is comparable in volume, density, histologic composition, and mechanical strength to that of native bone. In vivo studies of these newly developed bone tissue engineering in translational animal models continue to make strides toward addressing regulatory and clinically relevant topics. These include the use of skeletally immature animal models to address the challenges posed by craniomaxillofacial defect repair in pediatric patients. This manuscript reviews the most recent preclinical animal studies seeking to assess 3D-printed ceramic scaffolds for improved repair of critical-sized craniofacial bony defects.

Abstract:Abstract Millions of craniofacial surgeries are performed annually worldwide for craniofacial bones’ replacement and augmentation. This represents a significant economic burden as well as aesthetic expectations. Autografts and allografts are the first choice for treatment of craniofacial defects; however, their limited availability and difficulty to shape have led to investigation for alternative strategies. Biomaterial-based approaches have been used for implantation as they have ample supply but their processing through conventional technologies present several drawbacks; the major one relates to the poor versatility towards the production of patient-specific implants. Additive manufacturing has gained considerable attention during the last decade, as it allows the manufacturing of implants according to patient need. Biomaterial implants can be additively manufactured but have one or more limitations of stress shielding, radiopacity, high strength to weight ratio and limited bone integration. Over the last few decades, composites are investigated to surmount the limitations with traditional implants and also improve their bone integration. This review provides an overview of the most recent polymeric composite-based biomaterials that have been used in combination with 3D printing technology for the development of patient-specific craniofacial implants. Starting with the conventional treatments, biomaterials available for the craniofacial implants, the additive manufacturing rationale are discussed. Also, the main challenges still associated with 3D printing of polymer-based composites are critically reviewed and the future perspective presented.

Abstract:Engineering complex tissues requires a precisely formulated combination of cells, spatiotemporally released bioactive factors, and a specialized scaffold support system. Injectable materials, particularly those delivered in aqueous solution, are considered ideal delivery vehicles for cells and bioactive factors and can also be delivered through minimally invasive methods and fill complex 3D shapes. In this review, we examine injectable materials that form scaffolds or networks capable of both replacing tissue function early after delivery and supporting tissue regeneration over a time period of weeks to months. The use of these materials for tissue engineering within the craniofacial complex is challenging but ideal as many highly specialized and functional tissues reside within a small volume in the craniofacial structures and the need for minimally invasive interventions is desirable due to aesthetic considerations. Current biomaterials and strategies used to treat craniofacial defects are examined, followed by a review of craniofacial tissue engineering, and finally an examination of current technologies used for injectable scaffold development and drug and cell delivery using these materials.

Abstract:Segmental mandibular defects are a common problem in head and neck surgery. Osseous free flap transfer has revolutionized mandibular reconstruction; however, donor site morbidity remains a significant problem. Bone tissue engineering (BTE) employs strategies for reconstructing critical-sized bone defects, improving the patients' quality of life. Scaffolds employed for BTE must be mechanically stable, osseointegrate with neighboring bone, and promote osteogenesis. The need for modified, hybrid constructs with a diversity of favorable characteristics is essential. Abstract:Craniofacial reconstruction requires robust bone of specified geometry for the repair to be both functional and aesthetic. While native bone from elsewhere in the body can be harvested, shaped, and implanted within a defect, using either an in vitro or in vivo bioreactors eliminates donor site morbidity while increasing the customizability of the generated tissue. In vitro bioreactors utilize cells harvested from the patient, a scaffold, and a device to increase mass transfer of nutrients, oxygen, and waste, allowing for generation of larger viable tissues. In vivo bioreactors utilize the patient’s own body as a source of cells and of nutrient transfer and involve the implantation of a scaffold with or without growth factors adjacent to vasculature, followed by the eventual transfer of vascularized, mineralized tissue to the defect site. Several different models of in vitro bioreactors exist, and several different implantation sites have been successfully utilized for in vivo tissue generation and defect repair in humans. In this review, we discuss the specifics of each bioreactor strategy, as well as the advantages and disadvantages of each and the future directions for the engineering of bony tissues for craniofacial defect repair.

Abstract:Reconstruction of cranial defect is commonly performed in neurosurgical operations. Many materials have been employed for repairing cranial defects. In this paper, materials used for cranioplasty, including autografts, allografts, and synthetic biomaterials are comprehensively reviewed. This paper also gives future perspective of the materials and development trend of manufacturing process for cranioplasty implants.

Abstract:Musculoskeletal injuries including bone defects continue to present a significant challenge in orthopedic surgery due to suboptimal healing. Bone reconstruction strategies focused on the use of biological grafts and bone graft substitutes in the form of biomaterials-based 3D structures in fracture repair. Recent advances in biomaterials science and engineering have resulted in the creation of intricate 3D bone-mimicking structures that are mechanically stable, biodegradable, and bioactive to support bone regeneration. Current efforts are focused on improving the biomaterial and implant physicochemical properties to promote interactions with the host tissue and osteogenesis. The "smart" biomaterials and their 3D structures are designed to actively interact with stem/progenitor cells and the extracellular matrix (ECM) to influence the local environment towards osteogenesis and de novo tissue formation. This article will summarize such smart biomaterials and the methodologies to apply either internal or external stimuli to control the tissue healing microenvironment. A particular emphasis is also made on the use of smart biomaterials and strategies to create functional bioactive implants for bone defect repair and regeneration.
