Airway management in children with COVID-19

Abstract:The novel coronavirus disease (COVID-19) mayresult in acute respiratory distress syndromeand respiratory failure, necessitating mechanicalrespiratory support. Healthcare professionals areexposed to a particularly high risk of contracting thevirus while providing resuscitation and respiratorysupport, which may in turn result in graveconsequences and even death. Although COVID-19has been shown to cause milder disease in children,paediatricians and intensivists who provide carefor children must be prepared to provide optimalrespiratory support without putting themselvesor other medical, nursing, and paramedical staffat undue risk. We propose an airway managementapproach that is especially relevant in the currentCOVID-19 pandemic and provides instructions for:(1) Elective intubation for respiratory failure; and(2) Emergency intubation during cardiopulmonaryresuscitation. To minimise risk, intubation methodsmust be kept as straightforward as possible andshould include the provision of appropriate personalprotection and equipment to healthcare workers.We identify two key considerations: that bag-maskventilation should be avoided if possible and thatbacterial and viral filters should be placed in the respiratory circuit. Our novel approach providesa framework for airway management that couldbenefit paediatric critical care practitioners whoprovide care for any children with a novel viralillness, with a focus on infection prevention duringhigh-risk airway management procedures.
