Abstract:Linezolid is an oxazolidinone antibiotic exhibiting efficacy against multidrug-resistant (MDR) Gram-positive-related infections. However, its population pharmacokinetic (PopPK) profile in Chinese critically ill children has not been characterized. Optimal dosing regimens should be established according to the PopPK/pharmacodynamic(PD) properties of linezolid in the specific population. This work aims to describe the pharmacokinetic (PK) properties of linezolid, assess the factors affecting interpatient variability, and establish an optimized regimen for children in pediatric intensive care unit (PICU). A single-center, prospective, open-labeled PK study was performed. Ultra-performance liquid chromatography-tandem mass spectrometry (UPLC-MS/MS) was applied to measure the plasma levels during linezolid treatment. PopPK analysis was conducted using Phoenix NLME software. Sixty-three critically ill pediatric patients were included. The data showed good fit for a two-compartment model with linear elimination. Body weight and aspartate aminotransferase (AST) were the most significant covariates explaining variabilities in linezolid PK for the pediatric population. Therapeutic target was defined as the ratio of the area under drug plasma concentration-time curve over 24 h to minimum inhibitory concentration (AUC/MIC) of >80. Different dosing regimens were evaluated using Monte Carlo simulation to determine the optimal dosage strategy for linezolid. Although the probability of target attainment (PTA) was high (>96%) for 10 mg/kg every 8 h at MIC≤1 mg/L, it was lower than 70% at MIC>1 mg/L. Thus, the dosing regimen required adjustment. When the dosing regimen was adjusted to 15 mg/kg every 6 h, the PTA increased from 63.6% to 94.6% at MIC=2 mg/L, thereby indicating higher treatment success. Children with AST of >40 U/L had significant higher AUC than those with AST of ≤40 U/L (205.45 vs. 159.96). Therefore, dosage adjustment was required according to the AST levels. The PopPK characteristics of linezolid in critically ill children were evaluated, and an optimal dosage regimen was constructed based on developmental PopPK/PD model and simulation. Abstract:Linezolid is a synthetic antibiotic very effective in the treatment of infections caused by Gram-positive pathogens. Although the clinical application of linezolid in children has increased progressively, data on linezolid pharmacokinetics in pediatric patients are very limited. The aim of this study was to develop a population pharmacokinetic model for linezolid in children and optimize the dosing strategy in order to improve therapeutic efficacy. We performed a prospective pharmacokinetic study of pediatric patients aged 0 to 12 years. The population pharmacokinetic model was developed using the NONMEM program. Goodness-of-fit plots, nonparametric bootstrap analysis, normalized prediction distribution errors, and a visual predictive check were employed to evaluate the final model. The dosing regimen was optimized based on the final model. The pharmacokinetic data from 112 pediatric patients ages 0.03 to 11.9 years were analyzed. The pharmacokinetics could best be described by a one-compartment model with first-order elimination along with body weight and the estimated glomerular filtration rate as significant covariates. Simulations demonstrated that the currently approved dosage of 10 mg/kg of body weight every 8 h (q8h) would lead to a high risk of underdosing for children in the presence of bacteria with MICs of ≥2 mg/liter. To reach the pharmacokinetic target, an elevated dosage of 15 or 20 mg/kg q8h may be required for them. The population pharmacokinetics of linezolid were characterized in pediatric patients, and simulations provide an evidence-based approach for linezolid dosage individualization.
Parametric population pharmacokinetics of linezolid: A systematic review

Abstract:Aims: Linezolid is often used for the infections caused by drug-resistant Gram-positive bacteria. Recent studies suggest that large between-subject variability (BSV) and within-subject variability could alter drug pharmacokinetics (PK) during linezolid therapy due to pathophysiological changes. This review synthesized information on linezolid population PK studies and summarized the significant covariates that influence linezolid PK. Methods: A literature search was performed using PubMed, Web of Science and Embase from their inception to 30 September 2021. Published studies were included if they contained data analysing linezolid PK parameters in humans using a population approach with a nonlinear mixed-effects model. Results: Twenty-five studies conducted in adults and five in paediatrics were included. One- and two-compartment models were the commonly used structural models for linezolid. Body size (weight, lean body weight and body surface area), creatinine clearance (CLcr) and age significantly influenced linezolid PK. The median clearance (CL) values (ranges) in infants (0.128 L/h/kg [0.121-0.135]] and children (0.107 L/h/kg [0.088-0.151]] were higher than in adults (0.098 L/h/kg [0.044-0.237]]. For patients with severe renal impairment (CLcr ≤ 30 mL/min), the CL was 37.2% (15.2-55.3%) lower than in patients with normal renal function. Conclusion: The optimal linezolid dosage should be adjusted based on the patient's body size, renal function and age. More studies are needed to explore the exact mechanism of linezolid elimination and evaluate the PK characteristics in paediatric patients.

Abstract:Aim: To optimize linezolid treatment regimens for Gram-positive bacterial infections based on pharmacokinetic/pharmacodynamic analysis. Materials & methods: The minimum inhibitory concentration (MIC) distribution of 572 Gram-positive strains from patients with clinically confirmed infections was analyzed. Using the Monte Carlo simulation method, the cumulative fraction of response and probability of target attainment were determined for linezolid regimens of 600 mg q.12h and q.8h Results: Linezolid dosage of 600 mg q.12h yielded >90% cumulative fraction of response and probability of target attainment for staphylococcal infections with an MIC of <= 1 mg/l, enterococcal infections with higher MIC values required 600 mg q.8h. Conclusion: Linezolid 600 mg q.12h is still the clinically recommended empirical dosage for Gram-positive bacterial infections. However, as bacterial MICs increase, 600 mg q.8h may be required to achieve better efficacy.

Abstract:Background : Linezolid is a valuable therapeutic option for infections of the central nervous system caused by multi-drug resistant Gram-positive pathogens. Data regarding linezolid pharmacokinetics in cerebrospinal fluid from post-operative neurosurgical patients have revealed wide inter-individual variability. The objectives of this study were to establish a population pharmacokinetic model for linezolid in plasma and cerebrospinal fluid, as well as to optimize dosing strategies in this susceptible population. Methods : This was a prospective pharmacokinetic study in post-operative neurosurgical patients receiving intravenous linezolid. Parallel blood and cerebrospinal fluid samples were collected and analyzed. The population pharmacokinetic modelling and Monte Carlo simulations were performed using the Phoenix NLME software. Results : A two-compartment model (central plasma and cerebrospinal fluid compartments) fit the linezolid data well, with creatinine clearance and serum procalcitonin as significant variables. Linezolid demonstrated highly variable penetration into cerebrospinal fluid, with a mean cerebrospinal fluid/plasma ratio of 0.53. A strong correlation was found between plasma trough concentration and cerebrospinal fluid exposure of linezolid. Based on simulation results, optimal dosage regimens stratified by various renal functions and inflammatory status were proposed. Conclusion : A modeling and simulating strategy was employed in dose individualization to improve the efficacy and safety of linezolid treatment.

Abstract:Objective We aimed to evaluate the intervention effect of clinical pharmacist-mediated optimization of a linezolid regimen using a population pharmacokinetics (PPK) model. Methods Patients treated with linezolid in two medical centers from January 2020 to June 2021 were retrospectively included in the control group; in contrast, those treated from July 2021 to June 2022 were prospectively enrolled in the intervention group. Clinical pharmacists optimized the dosage regimen according to the published linezolid PPK model in the intervention group. The interrupted times series approach was used to analyze the data. The incidence of linezolid-induced thrombocytopenia (LIT), target attainment of pharmacokinetic/pharmacodynamic parameters, and other adverse drug reactions (ADRs) were compared between the two groups. Results We enrolled 77 and 103 patients in the control and intervention groups. The intervention group had a lower incidence of LIT and other ADRs than the control group (10.7% vs . 23.4%, P =0.002; 1.0% vs. 7.8%, P =0.027). The intervention group exhibited a considerably lower trough concentration (C min ) and area under the curve divided by the minimum inhibitory concentration (AUC 24 /MIC) than the control group ( P =0.001; P <0.001). C min and AUC 24 /MIC rates within the target range were substantially higher in the intervention group than in the control group (49.6% vs. 20.0%, adjusted P <0.05; 48.1% vs. 25.6%, adjusted P <0.05). Conclusion Interventions by clinical pharmacists reduced the incidence of LIT and other ADRs. Implementation of model-informed precision dosing for linezolid markedly increased the C min and AUC 24 /MIC rates within the target range. We recommend MIPD-guided linezolid dose reduction for patients with renal impairment.

Abstract:This study aimed to evaluate the pharmacokinetics (PKs) and safety of a generic drug, linezolid, compared to those of a reference drug in healthy Chinese subjects under both fasting and fed conditions. This was a randomized, open‐label, 2‐period, 2‐sequence crossover study. The subjects received a single dose of the test or reference drug, linezolid (600 mg), in each period. The PK parameters were calculated using a non‐compartmental method and compared between the 2 drugs. Bioequivalence was analyzed using geometric mean ratios (GMRs) of the 2 formulations and their corresponding 90% confidence intervals (CIs). The safety of the 2 formulations was assessed under both fasting and fed conditions. Forty‐eight subjects completed the study, 24 each in the fasting and feeding groups. The average plasma concentration‐time patterns of linezolid were similar for both medications under both conditions. The GMR and 90% CIs of the maximum plasma concentration and the area under the plasma concentration‐time curve of linezolid were ranged from 0.80 to 1.25. Both drugs were well tolerated with a similar incidence of adverse drug reactions. In conclusion, the PK and safety profiles of the 2 formulations were comparable. Food intake did not influence the PK profiles of linezolid. These results suggest that the test drug can be used as an alternative to reference drugs.

Abstract:A pilot‐scale study investigating the use of low‐pressure, high‐intensity UV radiation for disinfection of urban wastewater was conducted. The inactivation of coliform bacteria, wastewater‐indigenous enteric viruses, seeded poliovirus, and seeded F‐specific coliphage was studied. During the course of the pilot study, infectious human adenoviruses were isolated from 15 of 16 large‐volume samples of UV‐disinfected secondary‐ and tertiary‐treated wastewater. Half of the tertiary‐treated, UV‐disinfected effluent samples from which the adenoviruses were isolated had total coliform concentrations that complied with California's Water Recycling Criteria. To determine the relative UV resistance of the adenovirus isolates, purified viruses were seeded into tertiary‐treated wastewater and exposed to low‐pressure, high‐intensity, collimated UV radiation. A dose of approximately 170 mW‐s/cm2 was required to achieve 99.99% inactivation. These findings suggest that UV doses effective at meeting certain wastewater regulations for total coliform bacteria may not provide suitable inactivation of the UV‐resistant human adenoviruses.

Abstract:ABSTRACT This study aimed to develop a pharmacokinetic model of linezolid in premature neonates and evaluate and optimize the administration regimen. Liquid chromatography-tandem mass spectrometry (LC-MS/MS) was used to detect the blood concentration data of 54 premature neonates after intravenous administration of linezolid, and the relevant clinical data were collected. The population pharmacokinetic (PPK) model was established by nonlinear mixed effects modeling. Based on the final model parameters, the optimal administration regimen of linezolid in premature neonates with different body surface areas (BSA) was simulated and evaluated. The pharmacokinetic properties of linezolid in premature neonates are best described by a single-compartment model with primary elimination. The population typical values for apparent volume of distribution and clearance were 0.783 L and 0.154 L/h, respectively. BSA was a statistically significant covariate with clearance (CL) and volume of distribution (V d ). Monte Carlo simulations showed that the optimal administration regimen for linezolid in premature neonates was 6 mg/kg q8h for BSA 0.11 m 2 , 7 mg/kg q8h for BSA 0.13 m 2 , and 9 mg/kg q8h for BSA 0.15 m 2 with minimum inhibitory concentration (MIC) ≤1 mg/L, 7 mg/kg q8h for BSA 0.11 m 2 , 8 mg/kg q8h for BSA 0.13 m 2 , and 10 mg/kg q8h for BSA 0.15 m 2 with MIC = 2 mg/L. A pharmacokinetic model was developed to predict the blood concentration on linezolid in premature neonates. Based on this model, the optimal administration regimen of linezolid in premature neonates needs to be individualized according to different BSA levels.

Maternal Serum CA 125 Levels in the Diagnosis of Abruptio Placentae

Abstract:Objective: To evaluate measurements of maternal serum CA 125 for the diagnosis of abruptio placentae. Methods: This investigation was conducted at Swedish Medical Center/Seattle from December 1991 through April 1992. During the study period, we identified 21 consecutive patients with a clinical diagnosis of abruptio placentae. In addition, we enrolled two groups of patients whose pregnancies were not complicated by placental abruption (28 preterm controls and 53 term controls). Results: Mean maternal serum levels of CA 125 were significantly higher among women with abruptio placentae (40.6 ± 29.6 U/mL) than among preterm controls (26.6 ± 17.2 U/mL) or term controls (22.0 ± 11.4 U/mL). Using a positive threshold of 35 U/mL, the sensitivity and specificity of maternal serum CA 125 for abruptio placentae were 42.9% (95% confidence interval [CII 21.7-64.1) and 92.5% (95% CI 85.4-99.6), respectively. Conclusions: These findings support an earlier report documenting higher mean maternal serum levels of CA 125 among women with pregnancies complicated by abruptio placentae than in control subjects. However, measurement of maternal serum CA 125 does not appear to be a useful marker for the diagnosis of abruptio placentae. At present, clinical diagnosis of abruptio placentae, with the aid of ultrasonography to rule out other causes of late gestational hemorrhage, should be considered the most sensitive and specific method of detecting this pregnancy complication.

Population Pharmacokinetics of Vancomycin and Prediction of Pharmacodynamics in the Chinese People

Abstract:Population pharmacokinetics of vancomycin (VAN) in the Chinese patients was described by using nonlinear mixed-effects modeling (NONMEM). 619 VAN serum concentrations data from 260 patients including 177 males and 83 females were collected separately from two centers. A one-compartment model was used to describe this sparse data. No significant difference was observed between two center datasets by introducing SID covariate. The final model was as CL= (θ (base0+ θ(max) x(1 -e(-θ(Age)(Age/72) and V = θ x θ (Age)(Age/72). The creatinine clearance (CL(Cr)) and Age were identified as the most significant covariate in the final model. Typical values of clearance (CL) and volume of distribution (V) in the final model were 2.91 L x h(-1) and 54.76 L, respectively. Internal model validation by Bootstrap and NPDE were performed to evaluate the robustness and prediction of the final model. The median and 95% confidence intervals for the final model parameters were based on 1000 Bootstraps. External model evaluation was conducted using an independent dataset that consisted of 34 patients to predict model performance. Pharmacodynamic assessment for VAN by AUC (0-24 h) to MIC ratios of over 400 was considered to be the best to predict treatment outcomes for patients. AUC (0-24 h) was calculated by clearance based on the above population model. The results indicate that the conventional dosing regimen probably being suboptimal concentrations in aged patients. The approach via population pharmacokinetic of VAN combined with the relationship of MIC, Age, CL(Cr) and AUC(0-24 h)/MIC can predict the rational dose for attaining efficacy.
Evaluation and application of population pharmacokinetic models for optimising linezolid treatment in non-adherence multidrug-resistant tuberculosis patients

Abstract:Background: Population pharmacokinetic (popPK) models can optimise linezolid dosage regimens in patients with multidrug-resistant tuberculosis (MDR-TB); however, unknown cross-centre precision and poor adherence remain problematic. This study aimed to assess the predictive ability of published models and use the most suitable model to optimise dosage regimens and manage compliance. Methods: One hundred fifty-eight linezolid plasma concentrations from 27 patients with MDR-TB were used to assess the predictive performance of published models. Prediction-based metrics and simulation-based visual predictive checks were conducted to evaluate predictive ability. Individualised remedial dosing regimens for various delayed scenarios were optimised using the most suitable model and Monte Carlo simulations. The influence of covariates, scheduled dosing intervals, and patient compliance were assessed. Results: Seven popPK models were identified. Body weight and creatinine clearance were the most frequently identified covariates influencing linezolid clearance. The model with the best performance had a median prediction error (PE%) of -1.62 %, median absolute PE of 29.50 %, and percentages of PE within 20 % (F20, 36.97 %) and 30 % (F30, 51.26 %). Monte Carlo simulations indicated that a twice-daily 300 mg linezolid dose may be more efficient than 600 mg once daily. For the 'typical' patient treated with 300 mg twice daily, half the dosage should be taken after a delay of ≥ 3 h. Conclusions: Monte Carlo simulations based on popPK models can propose remedial regimens for delayed doses of linezolid in patients with MDR-TB. Model-based compliance management patterns are useful for balancing efficacy, adverse reactions, and resistance suppression.

Abstract:Aims Levofloxacin is a quinolone antibiotic with a broad antibacterial spectrum. It is frequently used in elderly patients with pneumonia. The pharmacokinetic profile of elderly patients changes with age, but data on the pharmacokinetics of levofloxacin in these patients are limited. The aim of this study was to establish a population pharmacokinetic model of levofloxacin in elderly patients with pneumonia and to optimize individualized dosing regimens based on this newly developed model. Methods This is a prospective, open‐label pharmacokinetic study in elderly patients with pneumonia. Blood samples were collected using an opportunistic approach. The plasma concentrations of levofloxacin were determined by high‐performance liquid chromatography. A population pharmacokinetic model was established using nonlinear mixed‐effect model software. Monte Carlo simulations were used for dose simulation and dose optimization. Results Data from 51 elderly patients with pneumonia were used for the population pharmacokinetic analysis. A one‐compartment model with first‐order elimination was most suitable for describing the data, and the estimated glomerular filtration rate was the only covariate that had a significant impact on the model. The final model estimated that the mean clearance of levofloxacin in elderly patients with pneumonia was 5.26 L/h. Monte Carlo simulation results showed that the optimal dosing regimen for levofloxacin was 750 mg once a day in elderly patients with pneumonia, with a minimum inhibitory concentration of 2 mg/L. Conclusions The population pharmacokinetic model of levofloxacin in elderly patients with pneumonia was established, and the dose optimization of levofloxacin was completed through Monte Carlo simulation.

The objective of this study was to compare the outcomes of linezolid versus teicoplanin in HAP patients with renal insufficiency and to explore optimal dosage strategy for linezolid. Methods: The retrospective study enrolled adult patients treated with intravenous linezolid or teicoplanin at Suzhou Municipal Hospital between July 2018 and August 2023. For the comparative pharmacodynamic study, effectiveness, safety and target attainment of trough concentration (C min ) for teicoplanin versus linezolid treatment in HAP patients with document Gram-positive bacteria and renal insufficiency were compared. For the population pharmacokinetics (PPK) analyses, linezolid concentrations collected exclusively from HAP patients with renal insufficiency were used and the optimal dosage strategy was investigated using Monte Carlo simulations. Results: Linezolid-treated patients had a higher bacterial eradication rate than teicoplanin-treated patients (88.5% vs 63.4%, P < 0.001). A higher proportion of patients in the linezolid group experienced at least one adverse reaction (42.0% vs 25.0%, P = 0.025). Significantly more supratherapeutic C min , less therapeutic C min were achieved in the linezolid group (adjusted P < 0.05). A total of 207 linezolid concentrations from 166 patients with renal insufficiency were available for the PPK analysis. Age and creatinine clearance (CrCL) were identified as significant covariates that influenced clearance. Simulations show that 300 mg q12h provide the optimal exposure in patients with a CrCL of 60 or 45 mL/min, and 200 mg q12h was recommended for patients with a CrCL of 30 or 15 mL/min. Conclusion: Linezolid-treated patients with HAP and renal insufficiency had higher bacterial eradication rates, supratherapeutic exposure and adverse reactions than teicoplanin-treated patients. Linezolid dose reduction in patients with renal insufficiency improved the probability of achieving optimal exposure. Keywords: linezolid, population pharmacokinetic, pharmacodynamic, renal insufficiency, hospital-acquired pneumonia Hospital-acquired pneumonia (HAP) is one of the most common healthcare-associated infections, which is associated with increased hospital length of stay, healthcare costs and high mortality. 1,2 Gram-positive bacteria are responsible for a substantial proportion of HAP infections. 3 The isolation of multidrug-resistant Gram-positive bacteria, particularly methicillin-resistant Staphylococcus aureus (MRSA), has increased steadily in the past decade. 4 Vancomycin, linezolid and teicoplanin are usually the preferred antibiotics for treating HAP caused by multidrug-resistant Gram-positive bacteria. 5–7 The dose-dependent nephrotoxicity of vancomycin has been well recognized, which occurred in up to 40% of patients. 8 Given the risk of nephrotoxicity associated with vancomycin, linezolid and teicoplanin may be considered as preferred alternative choices in patients with renal insufficiency. 9,10 Two randomized clinical trials have already been performed to compare the efficacy and safety outcomes between linezolid and teicoplanin for the treatment -Abstract Truncated-

Development and Validation of a Risk Prediction Model of Linezolid-induced Thrombocytopenia in Elderly Patients

Abstract:Abstract Background Linezolid is an oxazolidinone antimicrobial agent developed for treating multi-drug-resistant gram-positive bacterial infections. Objective This study aimed at investigating risk factors of linezolid (LI)-induced thrombocytopenia (LI-TP) and establishing a risk predictive model for LI-TP. Setting ZhongShan Hospital, FuDan University, China. Method A retrospective study was performed in patients aged ≥ 65 years receiving linezolid therapy from January 2015 to April 2021. Clinical characteristics and demographic data were collected and compared between patients with LI-TP and those without. Main outcome measures Incidence and risk factors of LI-TP in elderly patients. Results A total of 343 inpatients were included as the train set from January 2015 to August 2020. Among them, 67 (19.5%) developed LI-TP. Multivariate logistic regression analysis revealed that baseline platelet counts < 150×10 9 ·L -1 (OR=3.576; P < 0.001), age ≥ 75 years (OR=2.258; P =0.009), eGFR< 60 mL·(min·1.73m 2 ) -1 (OR=2.553; P =0.002), duration of linezolid therapy ≥ 10 d (OR=3.218; P <0.001), ICU admittance (OR=2.682; P =0.004), and concomitant with piperacillin-tazobactam (PTZ) (OR=3.863; P =0.006) were independent risk factors for LI-TP. The risk predictive model was established and exhibited a moderate discriminative power, with an AUC of 0.795 [95%CI 0.740-0.851] and 0.849 [95%CI 0.760-0.939] in train set (n=343) and validation set (n=90), respectively. Conclusion The risk factors of LI-TP in elderly patients were duration of linezolid therapy, age, eGFR, ICU admittance, baseline platelet counts, and concomitant with PTZ. A risk predictive model based on these risk factors may be useful to identify patients with high risk of LI-TP.
