Abstract:Acupuncture treatment in local areas is commonly used to treat pain or soreness; however, acupuncture around the neck or shoulder may be a risk factor for pneumothorax. Herein, we report two cases of iatrogenic pneumothorax after acupuncture. These points indicate that physicians should be aware of these risk factors through history-taking before acupuncture. Chronic pulmonary diseases, such as chronic bronchitis, emphysema, tuberculosis, lung cancer, pneumonia, and thoracic surgery, may be associated with a higher risk of iatrogenic pneumothorax after acupuncture. Even if the incidence of pneumothorax should be low under caution and fully evaluated, it is still recommended to arrange further imaging examinations to rule out the possibility of iatrogenic pneumothorax.

Abstract:Secondary hemorrhage occurs as a complication of central nervous system (CNS) following acupuncture has been reported in few cases. The present study reports a case of a 70-year-old woman who presents medulla oblongata hemorrhage after acupuncture. The patient suffered severe headache, neck pain, vomiting, and weakness after the neck acupuncture. The purpose of presenting this case report is to raise awareness among clinicians of fateful CNS complications during acupuncture. More importantly, qualified training of acupuncturists is anticipated and standardized processes of clinical acupuncture operations needs to be developed, in order to avoid the occurrence of acupuncture accidents.

Abstract:Purpose: To report a rare case of delayed dislocation of a novel posterior chamber phakic intraocular lens into the vitreous cavity, which was successfully treated by a reformed technique. Case PresentationA 29-year-old female received Ejinn phakic refractory lens (EPRL) implantation to correct her high myopia. Spontaneous dislocation into the vitreous cavity occurred 26-months post-operatively without traumatic history. Pars plana vitrectomy combined with cutting the EPRL into two equal pieces was performed to remove the dislocated EPRL. ConclusionDislocation into the vitreous cavity of EPRL can be successfully and easily removed by our reformed technique. Concerns about zonules-related complications pre-operatively, intraoperatively, and post-operatively must be raised in the practice of EPRL implantation.

Abstract:The orbit is susceptible to traumatic injuries, and surgical management of transorbital penetrating injury with foreign body retention is challenging due to the associated risks of nervous and vascular injury. Application of multiple imaging modalities is helpful for the preoperative surgical planning, and technical advances in percutaneous endoscopy would render the surgical approach even less invasive.

Abstract:Cardiac tamponade is a critical cardiovascular condition where timely diagnosis and treatment are crucial. The formation of an intrapericardial hematoma following acupuncture therapy is clinically rare. This paper reports a case of an elderly female patient who experienced severe chest pain and syncope during acupuncture therapy, subsequently diagnosed with traumatic hemopericardium and acute cardiac tamponade, complicated by cardiogenic shock. Under ultrasound guidance, pericardial puncture and drainage were successfully performed. The patient's symptoms were alleviated, her vital signs stabilized, and follow-up outcomes were favorable. This case provides valuable reference for understanding the pathogenesis, diagnosis, and treatment of pericardial hemorrhage following acupuncture therapy, integrating both clinical practice and literature review.

Abstract:AIM:To investigate the risk factors for eye removal following ocular trauma at a major ophthalmology department in China.METHODS:A retrospective study of patients who underwent eye removal surgery following ocular trauma was completed. Clinical outcomes were consulted in detail through the hospital's computed medical data system. Patients' information including age, gender, cause of ocular trauma, affected eye, and education level was collected and recorded in a standardized database. Chi-squared test, Student's t-test, Fisher's exact test, and bivariate correlation analysis were used for statistical comparisons.RESULTS:The present study included 1675 removal eyes from 1674 patients over the 20-year period. Patients included 80.5% males and 19.5% females, with mean age of 38y. The majority of the patients (70.7%) were blue-collar workers (physical laborers), and 1098 patients (65.6%) did not receive high school education. Work-related injuries were the most common reason for eye removal (n=739, 44.1%), of which 441 cases (59.7%) were related to metal/nail wounds. The most frequent injury type in males was work-related injuries (49.7%), whereas the most frequent injury type in females was home-related injuries (25.8%).CONCLUSION:Work-related injuries are a leading cause of severe ocular injury resulting in eye removal. In addition, men and undereducated patients are more likely to undergo eye removal surgery following ocular trauma. This study identified multiple high-risk factors leading to eye removal following ocular trauma, which is of great importance for preventing severe eye injuries.
Intraocular Foreign Bodies in China: Clinical Characteristics, Prognostic Factors, and Visual Outcomes in 1421 Eyes

Abstract:PURPOSE: To investigate epidemiologic and clinical characteristics, prognostic factors, and visual outcomes of posttraumatic intraocular foreign body (IOFB). DESIGN: Retrospective, consecutive case series. METHODS: Information from 1421 eyes (1299 inpatients) with IOFB selected from all open-globe injury cases (4968 eyes of 4865 inpatients) in 15 tertiary hospitals in China over a 5-year period were collected from a standardized preformulated database of eye injuries. Factors affecting retinal lesions or detachment, development of endophthalmitis, and visual outcome were analyzed statistically. RESULTS: IOFBs represented 28.60% of all open-globe injuries. Of the 1421 eyes, endophthalmitis developed in 232 (16.76%), and independent protective factors included primary repair within 24 hours (odds ratio [OR], 0.59; P = .006) and self-sealing wounds (OR, 0.69; P = .013). At discharge or follow-up, the enucleation rate was 7.53%, best-corrected visual acuity (VA) improved in 54.33%, and 4.85% had no light perception. Wound length of at least 3 mm and IOFB located in the posterior segment increased the risk of retinal lesion or detachment, and both (OR, 1.66; P < .001; OR, 4.04; P < .001) were significant negative predictors for visual outcome, along with wound larger than IOFB in largest length (OR, 2.38; P = .002) and endophthalmitis (OR, 2.01; P = .0003). Better initial VA (OR, 0.76; P < .001) was a protective factor for final VA. CONCLUSIONS: For IOFBs, primary wound closure by repair within 24 hours or self-sealing independently reduces the risk of endophthalmitis. Worse presenting VA, larger wounds, IOFB in posterior segment, wound larger than IOFB in largest length, and endophthalmitis predict worse visual outcome. (Am J Ophthalmol 2011;152: 66-73. Abstract:A case history is reported of a patient who presented with acute pedicle torsion in a left ovarian cyst, which had been diagnosed seven months previously. The diagnosis was established by sonography. She declined surgery for personal reasons, and was treated with acupuncture. Her pain was successfully relieved, and she continued to be observed. The acupuncture was repeated daily for 15 days, after which time repeat sonography showed reduction in size of the cyst, and no torsion of the pedicle. No relapse had occurred at 16 months follow up.

Abstract:Graves' ophthalmopathy is the most common and quite often severe autoimmune disorder that impairs appearance and visual function, primarily affecting retrobulbar fat, connective tissue, and extraocular muscles. [ 1 ] It is the most common orbital disease globally, which has a peak incidence between fourth and sixth decade. [ 2 , 3 ] Its incidence is related to thyroid function, with most patients exhibiting hyperthyroidism, while a minority present with hypothyroidism or normal thyroid function. [ 4 , 5 ] In the course of Graves' disease, approximately 25% to 50% of patients will develop Graves' ophthalmopathy, exhibiting symptoms such as lid lag and proptosis, ectropion, exposure keratitis, strabismus, diplopia, compressive optic neuropathy, alterations in visual acuity and potentially blindness. [ 6 , 7 ] Corneal ulcer perforation is one of the most serious complications of Graves' ophthalmopathy which represents an ophthalmological emergency due to its devastating consequences. They are an important cause of ocular morbidity, leading to a decrease in vision and, in the worst-case scenario, even blindness and loss of the eye. Emergency treatment is mandatory to try to restore the anatomical integrity of the globe, to salvage useful vision as much as possible, and to reduce the possible complications to a minimum.

Abstract:Oculomotor nerve paralysis (ONP) is a common disease discovered in the ophthalmology department, mainly manifested as ptosis with the same side, eyeball in the fixed external exhibition space, the movement of inward, upward, and downward are limited, which can lead to some complications such as exotropia, ptosis, diplopia, and others. [ 1 , 2 ] The etiology of ONP is complex and diverse, and microcirculation disorder is considered to be the leading cause. [ 3 , 4 ] In various publications, some researchers have established the fact that acupuncture therapy is a safe and feasible method for ONP. [ 5 , 6 ] However, the effect of electroacupuncture (EA) for breast cancer patients with ONP after chemotherapy was rarely reported. Here, we described a case of a 56-year-old breast cancer patient with ONP, who was successfully treated by using EA therapy as choosing the acupoints of Jingming (BL1), Cuanzhu (BL2), Yuyao (EX-HN4), Sizhukong (TE23), Tongziliao (GB1), Taiyang (EX-HN5), Chengqi (ST1) on the lesion side.

Acupuncture for Perimenopausal Symptoms in Women Who Underwent Oophorectomy - A Comparative Study

Abstract:Objective: To explore the effects of acupuncture on perimenopausal symptoms in women who underwent oophorectomy. Methods: 67 women who had undergone oophorectomy were divided into an acupuncture group (n = 36) and a comparison group ( n = 31) according to their wishes. The first group was treated by acupuncture and the latter group by Livial. Clinical symptoms were assessed by the modified Kupperman index. The levels of venous blood serum beta-endorphin (beta-EP), follicle stimulating hormone (FSH), luteinizing hormone (LH), estradiol (E-2) and the maturation index (MI) of vaginal epithelial cells were assessed. Results: There were no significant group differences for the MI of vaginal exfoliative cells nor for the levels of FSH, LH and E-2 after treatment (p > 0.05), but Kupperman scoring and the levels of beta-EP differed significantly between the acupuncture and the Livial group (p < 0.05). No side-effects were reported in either group. Conclusion: Acupuncture results in a significant improvement in perimenopausal symptoms in women who have had an oophorectomy. Acupuncture performs as well or better than Livial. Yet, this result may have been influenced by a potential bias and the small sample size.
Inverted ILM Flap for a Needle Injury to the Macula after Peribulbar Anaesthesia: A Case Report and Literature Review

Abstract:Globe perforation following peribulbar anesthetic injection is a rare but dreaded complication that often results in poor visual outcomes. This case report is on a female patient who sustained vitreous hemorrhage, retinal detachment, and macular breaks due to a peribulbar block administered in the setting of cataract extraction. The retina was repaired with pars plana vitrectomy, endolaser of the peripheral retinal break only, and an internal limiting membrane inverted flap for the macular breaks to avoid the endolaser on the macular area, achieving stable visual outcomes. The authors discussed various modes of local anesthesia for vitreoretinal surgery, risks for globe perforations, and how to approach retinal detachment secondary to needle perforations, which are complex cases at high risk for proliferative vitreoretinopathy. Early recognition and intervention in eyes with an inadvertent perforation can lead to a good outcome. Eyes with a longer axial length, superior, and multiple perforations are at higher risk of developing complications such as retinal detachment and vitreous hemorrhage. Complications such as retinal detachment, macular injury, and vascular occlusion are risk factors for poor prognosis.
Treatment of corneal perforation with lenticules from small incision lenticule extraction surgery: a preliminary study of 6 patients.

Abstract:Purpose: To evaluate the use of lenticules extracted using small incision lenticule extraction (SMILE) surgery as a surgical alternative for wound closure in corneal perforation. Methods: Corneal lenticules obtained through SMILE surgery with central thickness >100 mu m were fixed over corneal perforation sites using 10-0 nylon interrupted stitches. Patients were monitored for a minimum of 1 year and were assessed using slit-lamp microscopy, fluorescein, digital tonometry, and best spectacle-corrected visual acuity (BSCVA) measurements. Postoperative complications throughout the study period were recorded. Results: Corneal perforations were successfully sealed in all 6 patients; 3 patients (50%) exhibited improved postoperative BSCVA. Between 3 and 4 weeks after the operation, part of the lenticules became incorporated into the corneal stroma and complete reepithelialization was achieved. During the follow-up period of 12 months, no evidence of infection, relapse, or perforation was detected in any patient. Conclusions: These preliminary findings suggest that the use of corneal lenticules may be a safe and effective surgical alternative for corneal perforation closure, with potential clinical application as relatively simple and inexpensive temporary measures to improve the condition of the cornea for further definitive interventions.
Rarely Reported Ocular Complications Following Surgery to Correct Chronic Suppurative Otitis Media

Abstract:The objective of this study was to describe the rarely reported ocular complications following surgery to correct chronic suppurative otitis media. A retrospective analysis was made of 2,318 cases of chronic suppurative otitis media treated with surgery between January 2004 and December 2009 at West China Hospital, Sichuan University, a medical center in Western China. Ninety-two cases of ocular complications were identified, giving an incidence of 3.97%. In 63 of the 92 cases (68.48%), the patients complained of blurred vision and the condition was classified as grade 1. In 24 cases (26.09%), the patients complained of mild periorbital edema and the condition was classified as grade 2. Four cases (4.35%) were classified as grade 3 because of severe edema with erythema on eyelids. One case (1.09%) was classified as grade 4 because of periorbital ecchymosis in addition to edema. The clinical features and the possible etiology were analyzed and are discussed. Ocular complications following chronic suppurative otitis media surgery are rare. Preauricular incision, disturbance of venous and lymphatic drainage, and preseptal cellulitis were the possible etiological factors. Eye injury during surgery and use of an anticholinergic agent might be risk factors for blurred vision after this surgery.

Abstract:Ear filling injection of hyaluronic acid (HA) is emerging as a new application of HA filling in clinical practice. However, its risks and complications have not been sufficiently investigated. Herein, we report a case of 25-year-old female with embolization of ophthalmic artery, a severe complication caused by ear filling of HA. Injection of HA at the triangular fossa immediately induced amaurosis and dizziness, and complete loss of light sensation in the right eye 10 min after injection. These symptoms did not resolve after emergency treatment, and she was sent to our hospital for treatment. A diagnosis of central retinal arterial occlusion (CRAO) was made, for which the patient received intravascular interventional therapy as an emergency treatment. Following surgery, the patient received a multifaceted treatment approach to promote nerve health, improve blood circulation, reduce edema, and enhance oxygen delivery through hyperbaric oxygen therapy. This treatment regimen restored light perception and resolved mottled skin discoloration. This case report expands our understanding of the potential mechanisms and anatomical factors involved in embolization associated with ear filler injections. Furthermore, it highlights the importance of prompt intervention, providing valuable insights for reducing the complication rate and improving patient outcomes following such procedures.

Abstract:<h3 class="Heading">Background</h3><p class="Para">Acupuncture is a famous traditional medicine in China, but the complications caused by broken acupuncture needles have been rarely reported. It seems easy to remove the foreign matters usually, but things become difficulty in special issues. Here, we reported a recently encountered case to provide an important teaching point of treating a chronically retained broken needle in retroperitoneum.</p><h3 class="Heading">Case presentation</h3><p class="Para">A 42-year-old man presented with a chronically retained broken needle in his body after acupuncture therapy two years ago. However, due to the discomfort at the left back recently and ordinary inconvenience such as security check, he came to our hospital for minimally invasive surgery. He was introduced to our department because the broken needle had migrated from subcutaneous to adipose tissue in retroperitoneum during the two years. Considering the position of the broken needle, the patient was performed by laparoscopy in general anesthesia. The operation time was about 31 min and there were only three 7 mm incisions in the left lateral abdominal wall. The X-ray exam was performed to confirm that the broken needle was removed integrally. The patients begun normal activity at 6 h after surgery and was discharged on the second day after surgery.</p><h3 class="Heading">Conclusions</h3><p class="Para">Acupuncture is widely used for pain treatment in China, but how to handle the complication of acupuncture needle broken in body are rarely reported. Laparoscopy will be the reasonable choice for treating needles broken in retroperitoneum.</p>

Acupuncture and Traditional Chinese Medicine in the Treatment of Peripheral Facial Palsy Caused by Wisdom Tooth Extraction: A Case Report

Abstract:Background: Peripheral facial palsy is characterized by acute peripheral facial nerve paralysis. While most cases have no identifiable cause, some are linked to infectious or noninfectious factors. The potential connection between herpes simplex virus-1 and peripheral facial palsy has been studied since the 1970s, with a small number of cases reported following tooth extraction. Patients who have treatment delayed for over a year and still exhibit no signs of recovery have a bleak prognosis. In this case, factors contributing to facial paralysis in this patient are facial nerve injury as a result of wisdom teeth extraction, improper nursing, and delayed treatment. Case presentation: A 23-year-old female presented with numbness and stiffness on the right side of the outer skin around the lips. These symptoms persisted for about 4 years after the extraction of the right lower wisdom tooth. Physical examination revealed House-Brackmann grade II peripheral facial paralysis. Acupuncture and traditional Chinese medicine treatments were initiated with an aim to facilitate nerve repair and neurofacilitation in the affected area. Following a 4-week course of treatment, the patient experienced a decrease in numbness and stiffness in the area, and treatment was discontinued. Conclusion: The combination of acupuncture and traditional Chinese medicine is a safe and promising supplementary therapy for peripheral facial palsy caused by wisdom tooth extraction. Nevertheless, it is imperative to conduct larger scale and randomized controlled studies to determine whether these complementary interventions have a significant additive or synergistic effect toward achieving complete recovery in the patients. <title>Hintergrund</title>Die periphere Fazialisparese ist durch eine akute periphere Gesichtsnervenlähmung gekennzeichnet. Meist ist keine erkennbare Ursache feststellbar, doch besteht bei einigen Fällen ein Zusammenhang mit infektiösen oder nicht-infektiösen Faktoren. Ein möglicher Zusammenhang zwischen dem Herpes-simplex-Virus-1 (HSV-1) und der peripheren Fazialisparese wird seit den 1970er Jahren untersucht, und es wurde über eine kleine Zahl von Fällen nach einer Zahnextraktion berichtet. Patienten, bei denen die Behandlung erst nach einjähriger Verzögerung oder noch später beginnt und die keine Anzeichen für eine Besserung zeigen, haben eine ungünstige Prognose. In diesem Fall sind die Faktoren, die zur Gesichtslähmung des Patienten beitragen, eine Verletzung des Nervus facialis infolge der Weisheitszahnextraktion, unsachgemäße Pflege und ein verzögerter Behandlungsbeginn.<title>Fallbericht</title>Eine 23-jährige Frau stellte sich mit Taubheitsgefühl und Steifigkeit auf der rechten Seite im Bereich der äußeren Haut um die Lippen herum vor. Die Symptome bestanden seit der Extraktion des rechten unteren Weisheitszahns etwa 4 Jahre zuvor. Die körperliche Untersuchung ergab eine periphere Fazialisparese Grad II nach House-Brackmann. Es wurde eine Behandlung mit Akupunktur und traditioneller chinesischer Medizin begonnen, um die Nervenreparatur und die Neurofazilitation im betroffenen Bereich zu fördern. Nach vierwöchiger Behandlung kam es bei der Patientin zu einer Abnahme des Taubheitsgefühls und der Steifigkeit im betroffenen Bereich und die Behandlung wurde beendet.<title>Schlussfolgerung</title>Die Kombination von Akupunktur und traditioneller chinesischer Medizin ist eine sichere und erfolgversprechende ergänzende Therapie für die periphere Fazialisparese nach Weisheitszahnextraktion. Allerdings müssen unbedingt größere und randomisierte kontrollierte Studien durchgeführt werden, um festzustellen, ob diese komplementärmedizinischen Methoden in Hinblick auf das Ziel einer vollständiger Genesung der Patienten einen signifikanten additiven oder synergistischen Effekt haben.
Corneal perforation after corneal foreign body - Case Report

Abstract:Purpose: To present the case of a patient with a history of trauma and corneal foreign body in the right eye, followed by decreased visual acuity in the right eye, corneal perforation with good recovery after surgical treatment. Material and method: We report a case of a patient who presented to our clinic with a sudden decrease of visual acuity in the right eye, two months after an incident resulting in a corneal foreign body in the right eye. In the case presented, the patient applied a local medical self-treatment, an antibiotic and a topical corticosteroid. After a few weeks, the patient presented to the ophthalmologist, a foreign body was extracted from the cornea of the right eye and a topical treatment with a non-steroidal anti-inflammatory drug, a cycloplegic and an antibiotic were indicated. However, corneal perforation occurred and the patient was urgently sent to our service, where a corneal anaesthesia was also found. Results: Corneal perforation healed with a minor paracentral opacification. Discussions: Corneal perforation in our patient was due to corneal melting because of topical steroid anti-inflammatory autotherapy, late corneal foreign body extraction and topical treatment with non-steroidal anti-inflammatory drugs. Corneal anesthesia is also an important factor that enhances corneal melting and perforation. The surgical intervention performed healed the corneal perforation. Conclusions: Corneal anaesthesia and topical anti-inflammatory administration led to corneal perforation. Corneal sensitivity should be tested in patients with corneal foreign body. Corneal patching proved to be an adequate solution in this patient.

Abstract:A case of perforating ocular injury with a retrobulbar foreign body and a large full-thickness posterior pole defect near the optic disc was scheduled for vitrectomy after primary corneal suturing. Because it was difficult to remove the retrobulbar foreign body by orbitotomy and perform the outside suture, the retrobulbar foreign body was removed through the posterior hole by a transocular approach, and an autologous Tenon capsule flap was used to internally patch the large full-thickness posterior pole defect, thus enabling silicon tamponade. After 3 months of follow-up, there was no immune response around the patch. The retina remained mostly attached with a maintained peripheral visual field, normal intraocular pressure, and good cosmetic appearance. This surgical technique may be valuable in patients with a perforating retrobulbar foreign body and a large full-thickness posterior pole defect. Arch Ophthalmol. 2012; 130(4): 493-496
