Abstract:The high prevalence of constipation after fracture surgery brings intolerable discomfort to patients on the one hand, and affects post-surgery nutrient absorption on the other hand, resulting in poor prognosis. Given the acknowledged probiotic properties of Lactobacillus rhamnosus, 100 fracture patients with post-surgery constipation were centrally enrolled and administered orally with L. rhamnosus JYLR-127 to assess the efficacy of probiotic-adjuvant therapy in alleviating post-fracture constipation symptoms. The results showed that L. rhamnosus JYLR-127 improved fecal properties, promoted gastrointestinal recovery, and relieved constipation symptoms, which were mainly achieved by elevating Firmicutes (p < 0.01) and descending Bacteroidetes (p < 0.001), hence remodeling the disrupted intestinal microecology. In addition, blood routine presented a decrease in C-reactive protein levels (p < 0.05) and an increase in platelet counts (p < 0.05) after probiotic supplementation, prompting the feasibility of L. rhamnosus JYLR-127 in anti-inflammation, anti-infection and hemorrhagic tendency prevention after fracture surgery. Our study to apply probiotics in ameliorating constipation after fracture surgery is expected to bless the bothered patients, and provide broader application scenarios for L. rhamnosus preparations.

What problem does this paper attempt to address?
The problem that this paper attempts to solve is the high - incidence of postoperative constipation in fracture patients. Specifically, postoperative constipation not only brings unbearable discomfort to patients, but also affects postoperative nutrient absorption, leading to a poor prognosis. Given the known probiotic properties of lactic acid bacteria (especially Lactobacillus rhamnosus), the researchers hope to evaluate the effectiveness of a specific strain, JYLR - 127, in improving the symptoms of constipation after fracture surgery. ### Detailed Explanation 1. **Background Problem**: - After fracture surgery, patients often have constipation problems due to long - term bed rest and limited mobility. - The incidence of constipation is as high as 50% - 70%, which seriously affects the patients' quality of life and rehabilitation process. - Constipation not only affects the patients' comfort, but also interferes with the gut microbiota, thereby affecting the overall health status. 2. **Research Objectives**: - To explore the effect of lactic acid bacteria JYLR - 127 on improving the symptoms of constipation after fracture surgery. - To evaluate the potential of probiotic - assisted treatment in relieving constipation and promoting intestinal recovery. - To study the impact of probiotics on the gut microbial community and its relationship with the inflammatory response. 3. **Specific Methods**: - The study adopted a single - blind randomized controlled trial design and recruited 100 patients with constipation after fracture surgery. - The patients were randomly divided into three groups: the pre - intervention group (FAC), the placebo - control group (CDN), and the probiotic - treatment group (TRE). - Patients in the TRE group took 2 grams of probiotic powder containing Lactobacillus rhamnosus JYLR - 127 orally twice a day for one week. 4. **Expected Results**: - The research results show that JYLR - 127 significantly improved the patients' defecation frequency and fecal characteristics (Bristol score) and reduced the constipation symptom score (PAC - SYM score). - Probiotic intervention also changed the structure of the gut microbial community, increased the abundance of Firmicutes, decreased the abundance of Bacteroidetes, and thus reshaped the dysregulated gut micro - ecology. - Blood routine examination showed that after probiotic supplementation, the C - reactive protein level decreased and the platelet count increased, suggesting its role in anti - inflammation, anti - infection, and prevention of bleeding tendency. Through these studies, the authors hope to provide new ideas for the treatment of constipation after fracture surgery and more clinical evidence and support for the application of Lactobacillus rhamnosus JYLR - 127.
