Abstract:T he medical library on our campus is a beautiful, scholarly, rich, resourceful, and tasteful place. It has hundreds of computer stations, almost 400,000 print volumes, and close to 4,000 journals. It has historical collections, spooky paintings of deceased academics, and comfortable seating. However, at any midmorning visit it is apparent that it is very nearly empty. It is a clean, well-lighted place but as in Hemingway’s story, it is mostly attractive for an old man who avoids the dark, dusty, noisy, and disorderly in favor of a late-night café. Even more he avoids the solitude of home. Is the library at our medical center defunct? The famous book emporium, ‘‘A Clean Well Lighted Place for Books’’ in San Francisco, has closed. Here, a large sign announces the permanent closing of the library coffee shop with a recommendation to a commercial facility across campus. What would the old waiter in Hemingway’s story think? The very nice and underemployed clerk promptly gets my books checked out and tells me they will not be due back for 6 months since there is little demand for books. Where is everybody? Is no one reading? Ha!!! If you could hear the movement of light in the fiberoptics, the hum of the software, the clatter of a thousand keyboards around campus and beyond, this would not be a very quiet place. The library is of course a virtual function in the main. This library articulates with 128 databases, and offers over 2,000 e-journals and 4,000 e-books. The media collection is huge and online assistance can get pretty much anything you might need to support scholarship. If you are not very clever at using the resources, there are online programs and courses in real classrooms to bring you, even if kicking, into the information age. If you are a professor lost in the romance of chalk dust, the library staff will help you to design an electronic course and join the remainder of the faculty in software activities to support better courses. Your course materials and handouts will be online and the students will correspond with you with great facility by e-mail or whiteboards. So this clean, well-lighted place that seems almost moribund is in fact a frenzy of information activity. We should not reflexively mourn the loss of the old if the new is so much better. However, a visit to the sanctuary of the library can be heartily recommended to find peace, quiet, enough people to prevent loneliness, and that sweet smell of the stacks, the essence of ink and binding glues that has made the library a special place for hundreds of years. The walk back to the office passed through the valet parking area of the ambulatory care building. Cars were backed up for a block, and young people were running hither and yon moving cars out and in. The clinic lobby was very crowded, and there was a long line at the clinic pharmacy. Upstairs schedules were being devastated by emergencies, conflicts, and staffing issues. This medical center hires thousands of people and is in fact the largest employer in the city, like many academic health centers. Getting here is a challenge of traffic and inept process and there never seem to be enough appointments. However, many of the patients have been coming here for years to follow issues such as hypertension, cancer, rare skin conditions, heart problems, neurological pathology, etc., that have required in-person examination by experts on a regular basis to adjust medication, anticipate clinical changes, and assure the best in care. The vast majority of the work done in this ambulatory building could have been done in a virtual mode. If that care was effective, the need for even the inpatient side of the house of health would implode as a consequence of prevention. It is pleasant to imagine the patients at their home console describing a problem, collecting transcutaneous fluid samples to be analyzed either on the spot or after sending through mail. Patients at home might be using their own peripherals for blood pressure, pulse oximetry, etc., or they may be doing some less frequent studies that required sending out a loaner from the clinic for ultrasound or some other probe.
