Cost Analysis of Robotic versus Standard Laparoscopic Hysterectomies: A Preliminary Analysis

Abstract:Objective: The purpose of this study was to compare the costs and net revenue associated with robotic versus traditional laparoscopic hysterectomies. Design: The study was designed as a retrospective chart review. Methods: The study group consisted of patients who underwent laparoscopic hysterectomies using the robotic versus standard laparoscopy from January 2005 to March 2009. Data were collected as the mean for the cohort and included total charges, total collected, total direct costs, and operative margin (OM). Total costs were divided into operating room costs (labor, supplies, and equipment), hospital costs (anesthesia, pharmacy, room, and board) and additional costs. Statistical analysis was performed comparing the two groups by the Wilcoxon rank sum test, two sided t test, and Fisher exact test. Results: Twenty-eight robotic cases were matched to 48 laparoscopic cases. Demographics were similar between groups. Total charges where higher with the robot, ($30,725 vs. $22,526, p<0.001) as was operating room cost ($4,767 vs. $2,874, p<0.001), labor ($1,830 vs. $1,386, p<0.001), and supplies ($2,637 vs. $1,387, p<0.001). Total collections ($8,651 vs. $7,946, p=0.6), hospital cost ($912 vs. $913, p=0.4), and anesthesia costs ($275 vs. $276, p=0.8) were not different. Total direct costs were higher in the robotic group ($6,630 vs. $4,491, p<0.001). The median operating margin was $3,646 less for robotics (p=0.2). Conclusions: In Carilion Clinic, even though charges are higher for robotic hysterectomies, the collections are similar to those for laparoscopic hysterectomies. Operating room costs are higher for the robotic hysterectomy group, which has a negative impact on the operative margin. These preliminary data have potential implications for those establishing gynecologic robotic programs and budgets involving surgical productivity. (J GYNECOL SURG 28:255)
