Abstract:Objectives: Cough is one of the most common and defining symptoms of asthma, and is also associated with poor quality of life, and, yet it has not been well characterized in previous clinical trials or cohort studies. The Cough Questionnaire in Asthma (CQA) was developed as an useful and simple tool for use in clinics to assess the severity, frequency, and impact of cough, where the items were generated by specialists’ opinions, literature review, and patient’s interviews. We developed the CQA and examined the questionnaire’s validity. Methods: Adult patients with specialist-diagnosed asthma were consecutively recruited at the University of Hong KongShenzhen Hospital asthma clinic. The questionnaire was assessed using Cronbach’s alpha coefficients. The correlations of CQA scores with the 6-item Asthma Control Questionnaire (ACQ6), Cough Hypersensitivity Questionnaire (CHQ) and subjective rating of cough by patients and physicians, respectively. For longitudinal validaity, Spearman correlations was used to detect the changes of CQA scores with the changes of other measures. Results: 152 asthmatic patients completed the study, the preliminary CQA was a 8-item tool, after assessing the items scores and the item-total Correlations ranged from 0.135-0.859. Three items were therefore deleted, so the final CQA5 was changed to 5-item tools, the final Cronbaha’s alpha coefficients was 0.926, the item-total Correlations ranged from 0.734 to 0.873. The test-retest correlation of the CAQ5 was 0.96. The Spearman correlation coefficients (r) of the CQA5 score were 0.601 with ACQ6, 0.842 with patient assessment of cough control, and 0.749 with physician assessment of cough control (all p<0.01). The CHQ score did weak correlation with CQA5 and ACQ6 score. The correlation between ΔCQA5 and ΔACQ6 was well in the longitudinal validity, r=0.767, p<0.0001. Conclusions: CQA5 is a simple and useful patient-reported tool for assessing cough in asthmatic patients. Cough might be a distinct symptom in asthmatics and the mechanisms are likely heterogeneous and cough in asthma should be measured in detail.
