Effect of 4 weeks vs 8 weeks of acupuncture for knee osteoarthritis in China: protocol for a randomised controlled trial

Abstract:Introduction Knee osteoarthritis represents the prevalent and incapacitating disease. Acupuncture, a widely used clinical treatment for knee osteoarthritis, has been shown to ameliorate pain and enhance joint function in affected individuals. However, there is a lack of evidence comparing different courses of acupuncture for knee osteoarthritis. In this trial, we will assess the effect of 4 weeks vs 8 weeks of acupuncture in patients with knee osteoarthritis. Methods and analysis The protocol is a pragmatic, parallel, two-arm randomised controlled trial, with the data analyst and assessor being blinded. 148 eligible patients with knee osteoarthritis will be randomly allocated in a 1:1 ratio to receive 4-week or 8-week acupuncture. Electroacupuncture will be administered three times per week for 4 or 8 weeks, respectively. Patients with knee osteoarthritis in both groups will be followed up to 26 weeks. The primary outcome is the response rate at week 26, and secondary outcomes include knee joint pain, knee joint function, knee joint stiffness, quality of life, patient global assessment, the Osteoarthritis Research Society International response rate and rescue medicine. A cost-effectiveness analysis will be carried out over 26 weeks. Ethics and dissemination The protocol has been approved by the Medical Ethical Committee of Beijing University of Chinese Medicine (2023BZYL0506). The study findings will be disseminated through presentation in a medical journal. Additionally, we plan to present them at selected conferences and scientific meetings. Acupuncture, as a widely used clinical treatment method for knee osteoarthritis, has been proven to relieve pain and improve joint function. However, there is currently insufficient evidence regarding the comparison of the effects of different acupuncture courses. ### Research Objectives The main objective of this trial is to evaluate the difference in efficacy between 4 - week and 8 - week acupuncture treatments for patients with knee osteoarthritis. Specific goals include: 1. **Primary Outcome**: The response rate at week 26, defined as the proportion of patients achieving the minimal clinically important improvement (MCII). MCII is defined as at least a 2 - point increase in the NRS score and at least a 6 - point increase in the WOMAC function score. 2. **Secondary Outcomes**: Including knee pain, knee joint function, knee joint stiffness, quality of life, overall patient assessment, OARSI response rate, and rescue drug use. ### Research Design This is a practical, parallel, two - group randomized controlled trial. 148 eligible patients with knee osteoarthritis will be randomly assigned in a 1:1 ratio to either the 4 - week or 8 - week acupuncture treatment group. Electro - acupuncture treatment will be carried out 3 times a week for 4 weeks or 8 weeks. All patients will be followed up within 26 weeks. ### Methods - **Inclusion Criteria**: Age 45 - 75 years, diagnosed with knee osteoarthritis according to the ACR criteria, unilateral or bilateral chronic knee pain for more than 3 months, radiologically confirmed Kellgren - Lawrence grade II or III within the past 6 months, average knee pain score ≥ 4 when walking on flat ground within the past week, and signing a written informed consent form. - **Exclusion Criteria**: Having a history of knee surgery or waiting for surgery, knee pain caused by other diseases, having received arthroscopic examination within the past 1 year or having received intra - articular drug injection within the past 6 months, having received acupuncture treatment within the past 6 months, having severe acute or chronic organ diseases or mental disorders, having coagulation disorders, being pregnant or in the lactation period, having a pacemaker installed or having epilepsy, having participated in other clinical trials within the past 1 month. - **Randomization**: Generate a random number sequence through SAS V.9.3 software and perform stratified randomization by center. The random number sequence and treatment plan will be placed in numbered opaque envelopes and distributed to acupuncturists by an independent research assistant. - **Blinding**: Due to the nature of the intervention, acupuncturists and patients cannot be blinded, but outcome assessors and data analysts will be blinded. - **Interventions**: The 4 - week group will receive 12 acupuncture treatments, and the 8 - week group will receive 24 acupuncture treatments, with each treatment lasting 30 minutes. Acupuncture points include 5 main points and 3 auxiliary points, and the specific operation methods are shown in Table 2. ### Data Analysis - **Sample Size**: Based on the team's previous research, the expected response rates in the 4 - week group and the 8 - week group are 35% and 60% respectively. Each group requires 59 patients. Considering a 20% dropout rate, each group actually requires 74 patients. - **Statistical Analysis**: Use IBM SPSS V.26.0 software for statistical analysis, and p < 0.05 is considered statistically significant. Data will be tested for normality. For data that conforms to the normal distribution, the mean and standard deviation will be calculated; for data that does not conform to the normal distribution, the median (interquartile range) will be calculated; and for count data, the percentage will be calculated. All randomized cases will be included in the intention - to - treat analysis, and missing data will be filled using the multiple imputation method. Sensitivity analysis will use a mixed - effects model for comparison at multiple measurement points. ### Ethics and Dissemination This study has been approved by the Medical Ethics Committee of Beijing University of Chinese Medicine (2023BZYL0506) and registered in the Chinese Clinical Trial Registry (ChiCTR2300073383). The research results will be published in medical journals and presented at selected conferences and scientific meetings.
