Valvar bypass surgery to ameliorate persistent lower limb edema caused by post-thrombotic syndrome: a case report and literature review

Abstract:Obstruction and/or reflux compromise during venous emptying can facilitate different pathophysiologies in chronic venous insufficiency (CVI). We present a patient with persistent lower limb CVI edema caused by post-thrombotic syndrome (PTS), who responded well to femoral vein valve therapy via axillary vein bypass after unsuccessful valvuloplasty, and led a normal life. During a 12 month observation period, bridging vessels completely restored original anatomical structures. In a literature study, no similar surgeries were reported, but we show that this operation may be feasible in selected patients.

What problem does this paper attempt to address?
The problem that this paper attempts to solve is lower - limb edema caused by chronic venous insufficiency (CVI), especially the persistent lower - limb edema caused by post - thrombotic syndrome (PTS). Specifically, after experiencing deep vein thrombosis (DVT), patients have severe symptoms of chronic venous insufficiency in the lower limbs, such as edema, a feeling of heaviness, pain, varicose veins and pigmentation. These symptoms seriously affect the patient's quality of life, and conservative treatment is ineffective, so surgical intervention is required to improve the symptoms. The paper describes an innovative surgical method - femoral vein bypass surgery through axillary vein valve transplantation, which has successfully alleviated the patient's symptoms and enabled the patient to return to normal life. There are no similar reports in the literature for this method, but this case shows that this surgery may be feasible in selected patients.
