Abstract:Information processing speed (IPS) affect quality of life in patients with neurodegenerative diseases. Epidemiologic studies have revealed that apolipoprotein (APOE) ε4 allele is associated with steeper age-related IPS decline (Raz N, 2009; Staehelin HB, 1999). Besides, APOE ε4 allele had a detrimental effect on white matter (WM) or grey matter (GM) structure. In the current study, we aimed to explore the neural substrates underlying APOE related IPS alteration by measuring lobar distribution of white matter hyperintensities (WMH) and regional cortical GM volume. The current study was carried out in 178 cognitively normal elderly (age range, 60-91.4 y, including 34 ε2 carriers, 54 ε4 carriers and 90 ε3 homozygotes). The IPS was determined by Trail Making Tests (TMT). The lobar distribution of WMH was quantitatively assessed. Regional cortical GM was generated by Freesurfer. Based on ANOVA, comparisons of these measures among groups were conducted. Linear regression was used to assess the association between brain measures and TMT score. 1)there was significant difference of TMT-A among groups, and ε4 carriers had the worst performance; 2) across groups, significant difference in frontal and parietal WMH load was found (highest in ε4 carriers), however, no significant difference was found in GM measures (Figure 1); 3) including gender, vascular risk factor, education, and WMH volume in each lobe as variables, the multi-variant general linear regression showed that, in ε4 carriers, parietal WMH volume was independently associated with TMT-A score (β=0.30, p=0.02, Figure 2). Only parietal WMH volumes in ε 4 carriers was significantly associated with log-transformed TMT-A score (r= 0.32, p=0.032). 1) the APOE ε4 allele was linked to more parietal and frontal WMH volume than ε3 homozygotes. The accumulated amyloid deposition in APOE ε4 carriers may affect arterial wall, reduce vessel lumen, and lead to chronic hypoperfusion. Further, anastomoses in parietal and frontal regions are scarce and tortuous, WM in these regions are particularly susceptible to ischemic events resulting from ε4 status. 2) The disruption of parietal WM structure plays a predominant role in ε4 allele-specific IPS decline. Deficits in strategic tracts of parieto-frontal connection may influence efficiency of integrating information and manifested as reduced IPS. Conclusively, this research may provide additional insight into clinical therapy of AD.
