Abstract:Objective To evaluate the clinical effects of the crown lengthening surgery on the restoration of posterior residual roots and crowns with subgingivally involved defects.Methods From December 2009 to December 2011,according to the subgingival depths of the defects,50 teeth from 48 patients with subgingivally involved defects were divided into three groups:group A(depth<3.0 mm),group B(3.0 mm ≤depth≤4.0 mm),group C(depth>4.0 mm).All teeth were treated by the crown lengthening first and then post-crown prosthesis 6 weeks after operation.PD,SDI,and lengths from gingival margin to the frature ends of teeth before operation and 6 weeks after operation were recored and compared.Results The teeth sections of AB groups were exposed preferably and the periodontal health was good.The effective rate was 100%.60% of C group were exposed well and were repaired well.There was significant difference between A and C groups,as well as B and C groups(P<0.05).Postoperative PD,and SBI were better than those preoperative values inthree groups(P<0.05).There was no differences between A and B groups in periodontal index after treatment(P>0.05),while there was significant differences between group C and groups A and B(P<0.05).Conclusions Crown lengthening surgery is a good method for the protection and restoration of posterior residual roots and crowns,but dentists should pay attention to the selection of the indications.
Outcome of Teeth Restored with CAD/CAM Zirconium Dioxide Post-Cores: a Retrospective Study with a Follow-Up Period of 3–6 Years

Abstract:Abstract Statement of problem Computer aided design/computer aided manufacturing (CAD/CAM) zirconia post-cores is one of the options of post crown restoration materials due to their esthetic properties and superior mechanical strength. However, the clinical effect on aesthetics and strength properties is unclear due to the lack of results of their long-term follow-up. Purpose This retrospective clinical study aims to analyze the survival rate, clinical manifestations, and failure factors after CAD/CAM zirconia post-core restoration. Material and methods Clinical and radiographic examinations were performed on 342 patients with 400 teeth for 3–6 years postsurgical follow-up examination. The patients were all received CAD/CAM zirconia post-cores and all-ceramic crowns at the Department of Prosthodontics in the public hospital. The retrospective outcomes were conducted after zirconia post restoration, including survival rate by Kaplan–Meier analysis and findings of manifestations and failure factors. The effects of gender and dental position on survival rate were analyzed by Cox–Mantel Test. Results This study retrospectively evaluated 261 teeth from 229 patients with a 35% drop-out rate. The survival rate was 96.0%, and the success rate was 92.4%. According to the tooth position classification, the survival rate was 100% for 101 anterior teeth, 95.4% for 69 premolars, and 88.3% for 91 molars. According to gender, the survival rate of the male group was 92.3%, while that of the female group was 98.0%, with a significant difference (P < 0.01). The complications included crown fracture (1.9%) periapical inflammation (1.9%), crown debonding (1.1%), percussion abnormal (1.9%) and root fracture (0.8%). Conclusions Within the limitations of this retrospective study, it can be concluded that CAD/CAM zirconia post-cores are clinically promising. Compared with the posterior teeth, CAD/CAM zirconia post-cores are more suitable for anterior teeth.
Peri-implant Marginal Bone Changes with Implant-Supported Metal-Ceramic or Monolithic Zirconia Single Crowns: A Retrospective Clinical Study of 1 to 5 Years.

Abstract:Statement of problem. Monolithic zirconia has excellent mechanical and biologic properties. However, evidence of the clinical properties of implant-supported monolithic zirconia prostheses is limited.Purpose. The purpose of this retrospective clinical study was to compare the peri-implant marginal bone changes of metal-ceramic and monolithic zirconia single crowns in the posterior region after prosthetic loading.Material and methods. A total of 224 participants treated with 327 implants restored with either metal-ceramic or monolithic zirconia single crowns in the posterior region between 2012 and 2016 were included in this study. Clinical outcomes, including the plaque index, peri-implant probing depth, and bleeding on probing, were recorded, and the marginal bone level was recorded by using the panoramic radiographs obtained at implant placement, second-stage surgery, and the most recent follow-up visit. The included parameters were analyzed with the nonparametric Mann-Whitney tests (a=.05).Results. The mean follow-up time was 30.4 months, and the cumulative survival rate of implants was 100% and that of the prostheses was 99.1%. The plaque index was 0.46 in the metal -ceramic group, which was significantly higher (P<.05) than 0.37 in the monolithic zirconia group. However, no significant differences (P>.05) were observed in peri-implant probing depth and bleeding on probing between the 2 groups. The marginal bone level at implant placement, second-stage surgery, and the most recent follow-up visit was above the implant platform in both the metal-ceramic and monolithic zirconia groups. The marginal bone changes of the metal-ceramic group was 0.31 mm in the healing period and 0.38 mm in the prosthetic loading period, while in the monolithic zirconia group, it was 0.25 mm in the healing period and 0.43 mm in the prosthetic loading period; no significant differences (P>.05) were observed between the 2 groups.Conclusions. The peri-implant marginal bone level change was comparable after prosthetic loading for metal-ceramic and monolithic zirconia single crowns, although monolithic zirconia was associated with reduced plaque. Abstract:Objectives To study bone healing of two‐wall bone defects after alveolar ridge preservation using mineralized dentin matrix. Materials and Methods After distal roots extraction of second and fourth premolars (P2, P4) on one lateral mandible in 12 beagles, two‐wall bone defects (5 × 5 × 5 mm) were surgically created distally to the remaining mesial roots of P2 and P4. A total of 24 sites were randomly allocated to three groups (implant material‐ time of execution): mineralized dentin matrix (MDM)‐3 m (MDM + collagen membrane; 3 months), MDM‐6 m (MDM particles + collagen membrane; 6 months), and C‐6 m (collagen membrane only; 6 months). Clinical, radiographic, digital, and histological examinations were performed 3 and 6 months after surgery. Results The bone healing in MDM groups were better compared to Control group (volume of bone regenerated in total: 25.12 mm3 vs. 13.30 mm3, p = .046; trabecular volume/total volume: 58.84% vs. 39.18%, p = .001; new bone formation rate: 44.13% vs. 31.88%, p = .047). Vertically, the radiological bone level of bone defect in MDM‐6 m group was higher than that in C‐6 m group (vertical height of bone defect: 1.55 mm vs. 2.74 mm, p = .018). Horizontally, no significant differences in buccolingual bone width were found between MDM and C groups at any time or at any level below the alveolar ridge. The percentages of remaining MDM were <1% in both MDM‐3 m and MDM‐6 m groups. Conclusions MDM improved bone healing of two‐wall bone defects and might be considered as a socket fill material used following tooth extraction.

Abstract:In this study, a three-dimensional evaluation was performed to explore differences between bone-embedded and soft tissue-covered roots after mandibular third molar (M3M) coronectomy. Patients were recruited according to the results of cone-beam computed tomography, 6 months after coronectomy. Completely bone-embedded M3Ms were assigned to group B, while completely soft tissue-covered M3Ms were assigned to group S. Data were recorded using digital software. A total of 213 M3Ms in 181 patients were investigated, of which 170 were assigned to the two study groups. Age was the primary factor influencing root migration (P<0.001). The smaller the degree of angulation of the M3M, the more likely was the root complex to rotate distally (r=−0.37, P<0.001). The depth of the impacted M3M contributed to the regeneration of new bone (P≤0.008). The length of the root complex (odds ratio 0.82, P=0.048) and distance from the root to the alveolar crest (odds ratio 1.23, P=0.026) were two critical factors influencing whether the root complex was bone-embedded. Ensuring that the length of the root complex is <7.6mm and the distance between the root and alveolar crest is ≥5mm were both found to be critical to the remaining root being completely bone-embedded and thus preventing eruption and the need for secondary surgery.
Computer-aided three-dimensional assessment of periodontal healing distal to the mandibular second molar after coronectomy of the mandibular third molar: a prospective study

Abstract:Abstract Background The periodontal healing distal to the mandibular second molar (M2M) after coronectomy of the M3M was controversary. This study aimed to combine a digital method with cone-beam computed tomography (CBCT) and estimate periodontal healing of M2M after M3M coronectomy. An accurate and stable indicator in three dimensions was also tentatively explored. Methods Patients with a M3M in contact with the inferior alveolar canal were included. CBCT was applied immediately after coronectomy (baseline) and 6-months later. Data were investigated with digital software for registration. Previously reported and coronectomy-related factors were included for univariate and multivariate analyses. Results A total of 181 patients (213 M3Ms) completed 6-month follow-up. Significant reduction in the distal intra-bony defect (DBD) depth of the M2M was shown (1.28 ± 1.24 mm, P < 0.001). DBD depth of the M2M at baseline was the most influential factor (r = 0.59), followed by preoperative M3M condition, age, rotation and eruption of the root complex. Remaining enamel (OR = 6.93) and small retromolar space (0.67) contributed to re-contact of the root complex and M2M. Bone volume regenerated in the distal 2 mm was associated significantly with DBD-depth reduction (r = 0.74, P < 0.001). Conclusions Bone volume regenerated in the distal 2 mm of the M2M denoted stability of distal periodontal healing of the M2M. DBD depth at baseline was the most influential factor for healing of a DBD of the M2M after M3M coronectomy. The remaining enamel and a small retromolar space could contribute to re-contact of the root complex and the M2M. Trial registration: China Clinical Trial Center, ChiCTR1800014862. The use of enamel matrix derivative alone versus in combination with bone grafts to treat patients with periodontal intrabony defects: a meta-analysis.

Abstract:The authors performed a meta-analysis to compare the clinical outcomes of enamel matrix derivative (EMD) used in combination with various bone grafts with EMD alone in patients with intrabony defects.The authors retrieved relevant studies through Sept. 30, 2011, from MEDLINE, PubMed, Embase and Cochrane Central Register of Controlled Trials. The main clinical outcomes were probing pocket depth (PPD) reduction, clinical attachment level (CAL) gain, gingival recession (REC) increase and defect fill gain. The authors performed two separate meta-analyses, according to the length of follow-up. They also conducted subgroup analyses regarding the study designs and surgical procedures used.The authors included 11 studies in their meta-analysis. At six to eight months' follow-up, pooled estimates showed that there was no significant difference regarding PPD reduction (P = .62) and CAL gain (P = .23) among the treatment groups, but there was a significant difference regarding defect fill gain and REC increase. At 12 months' follow-up, pooled estimates revealed no significant differences regarding PPD reduction (P = .29), CAL gain (P = .15) and REC increase (P = .30) between the groups, but the authors still detected a significant difference for defect fill gain.In trials with a short-term follow-up, the combination therapies yielded better clinical outcomes regarding defect fill gain and REC increase compared with EMD alone, whereas most clinical outcomes were not significantly different between the two groups in the long run. The additional benefits from using combination therapies to promote periodontal tissue regeneration need to be confirmed.
Comparison of the biomechanical effects of the post-core crown, endocrown and inlay crown after deep margin elevation and its clinical significance

Abstract:Background: The purpose of this in vitro study was to compare and evaluate the stress distribution of maxillary first premolar residual crowns restored with post-core crowns, endocrowns and inlay crowns after deep margin elevation, to explore the fitting restoration for residual crowns using finite element analysis. Methods: A healthy complete right maxillary first premolar from a male adult was scanned by cone beam computed tomography (CBCT). The finite element model of the tooth was established by reverse engineering software such as Mimics, Geomagic and Hypermesh. On this basis, the residual crown model after deep margin elevation was made, and the experimental group models were divided into three groups, those restored with post core crowns, endocrowns and inlay crowns. Vertical and oblique static loads were applied to the experimental models to simulate the force on the tooth during mastication (the loading position was located in the central fossa of the occipital surface, and the load was 100 N) using Abaqus software. Results: The peak value and distribution of von Mises stress in each part of the experimental model were observed. After deep margin elevation, the peak dentin von Mises stresses were lower than the tensile strength of normal dentin in the post-core crown, endocrown, and inlay crown groups; the lowest stress results were found in the post-core crown group for the dentin, restoration, enamel, and deep margin elevation (DME) layers under vertical and oblique loading. In terms of stress distribution clouds, the peak stresses in the dentin tissue were located in the apical 1/3 of the root after postcore crown restorations for both loads, while stress concentrations were evident in the cervical and root areas after endocrown and inlay crown restorations; regardless of the load and restoration method, the corresponding stress concentration areas appeared at the junction of the DME and dentin tissue at the loading site of the restorations; CONCLUSIONS: Post-core crowns, endocrowns and inlay crowns can be used to restore residual crowns after deep margin elevation, and post-core crowns can better protect the residual tooth tissue.
[A Prospective Cohort Study on the Effect of Implant Restoration Following Micro Crestal Flap-Alveolar Ridge Preservation at Molar Extraction Sockets with Severe Periodontitis].

Abstract:Objective: To evaluate the survival rate, success rate, soft tissue conditions and marginal bone level changes of implants following micro crestal flap-alveolar ridge preservation at molar extraction sockets with severe periodontitis, compared to natural healing. Methods: From March 2015 to January 2017, patients scheduled for molar extraction as a consequence of severe periodontitis and planned implant-retained prostheses from Department of Periodontology Peking University School and Hospital of Stomatology were selected. A total of 40 molar extraction sockets from 40 patients received implant placement following micro crestal flap-alveolar ridge preservation or natural healing. The front consecutive 20 teeth were assigned to the natural healing group, and the back ones were assigned to the micro crestal flap-alveolar ridge preservation (MCF-ARP) group. The superstructures were placed 6 months later. Within 2 weeks (baseline) and 1, 2 and 3 years after implant crown restoration, modified plaque index, probing depth, modified bleeding index and keratinized tissue width were recorded every six months. Parallel periapical radiographs were taken to evaluate the peri-implant marginal bone level and to calculate marginal bone loss. Independent sample t test or Mann-Whitney U test were used to compare the differences in the above clinical and imaging indicators between the two groups. Results: The implant survival rate and success rate of the two groups were both 100% (20/20). There were no significant differences in the modified plaque index, probing depth, modified bleeding index, buccal keratinized tissue width and marginal bone loss between two groups at 1, 2 and 3 years after implant crown restoration (all P>0.05). Marginal bone loss was 0.22 (0.14, 0.34) mm in the natural healing group and 0.21 (0.12, 0.30) mm in the MCF-ARP group at a 3-year post-loading evaluation. Conclusions: Within the limitations of the present study, implants placed at ridge preserved and naturally healed molar extraction sockets with severe periodontitis demonstrate comparable clinical outcomes at a 3-year post-loading evaluation.
Demineralized dentin matrix promotes gingival healing in alveolar ridge preservation of premolars extracted for orthodontic reason: a split-mouth study

Abstract:Objective: The purpose of this study was to prospectively evaluate the efficacy of a demineralized dentin matrix (DDM) in decreasing the initial inflammatory response of the gingiva and facilitating the repair and regeneration of soft tissue in alveolar ridge preservation. Methods: This clinical study employed a split-mouth design. Fourteen patients with a total of forty-four sites underwent extraction and alveolar ridge preservation (ARP) procedures. A Bilaterally symmetrical extraction operation were conducted on the premolars of each patient. The experimental group received DDM as a graft material for ARP, while the control group underwent natural healing. Within the first month postoperatively, the pain condition, color, and swelling status of the extraction sites were initially assessed at different time points Subsequently, measurements were taken for buccal gingival margin height, buccal-lingual width, extraction socket contour, and the extraction socket area and healing rate were digitally measured. Additionally, Alcian Blue staining was used for histological evaluation of the content during alveolar socket healing. Results: Both groups experienced uneventful healing, with no adverse reactions observed at any of the extraction sites. The differences in VAS pain scores between the two groups postoperatively were not statistically significant. In the early stage of gingival tissue healing (3 days postoperatively), there were statistically significant differences in gingival condition and buccal gingival margin height between the two groups. In the later stage of gingival tissue healing (7, 14, and 30 days postoperatively), there were statistically significant differences in buccal-lingual width, extraction socket healing area, and healing rate between the two groups. Furthermore, the histological results from Alcian Blue staining suggested that the experimental group may play a significant role in promoting gingival tissue healing, possibly by regulating inflammatory responses when compared to the control group. Conclusion: The application of DDM in alveolar ridge preservation has been found to diminish initial gingival inflammation after tooth extraction. Additionally, it has shown the ability to accelerate early gingival soft tissue healing and preserve its anatomical contour. Clinical trial registration: chictr.org.cn, identifier ChiCTR2100050650.

Evaluation of deproteinised bovine bone matrix combined with absorbable biofilm for the preservation of extraction sites of mandibular impacted wisdom teeth

Abstract:Background: Bone defects and deep periodontal pockets often exist distal to the second molar after mandibular third molar extraction, seriously threatening the periodontal health of the second molar. Objective: To evaluate the effect of socket preservation with bone substitute materials on alveolar bone resorption and prevention of the distal periodontal defect of the adjacent tooth after mandibular impacted third molar extraction compared with natural healing. Methods: Ninety-nine patients with mandibular impacted teeth, treated in our hospital from January 2018 to December 2020, were randomly divided into the control and experimental groups. The experimental group underwent minimally invasive tooth extraction and socket preservation using the deproteinised bovine bone mineral, Bio-Oss and the bioabsorbable collagen membrane, Bio-Gide. The control group healed naturally after minimally invasive tooth extraction. The alveolar ridge dimension of the extraction sites, the probing depth, tooth mobility and gingival index on the distal aspect of the mandibular second molars were examined and recorded before and six months after the operations. Results: There was a significant difference between the experimental group and the control group in the alveolar bone width (P< 0.05) and height (P< 0.05) before and after surgery. The probing depth of the extraction sites in both groups was reduced. Conclusion: Using Bio-Oss and Bio-Gide to preserve extraction sites of impacted teeth can promote recovery more effectively than natural healing on the height of the distal alveolar bone and the width of the alveolar crest of the second molar and thus improve the periodontal status of the adjacent second molar.
[Evaluation of the efficacy of combined application of concentrated growth factors in ridge preservation of teeth with severe periodontitis]

Abstract:Purpose: To evaluate the efficacy of combination of deproteinized bovine bone matrix and concentrated growth factors in maintaining the three-dimensional contour of alveolar bone in ridge preservation of teeth with severe periodontitis. Methods: Thirty patients with posterior teeth suffering from severe periodontitis requiring extraction and with the intention of implant restoration were selected and randomly divided into the experimental group and control group, with 15 cases in each group. In the experimental group, DBBM combined with CGF fluid was used as bone graft material and placed in the extraction socket of the patient after minimally invasive tooth extraction and thorough debridement,while DBBM was used in the control group mixed with normal saline as the bone graft material.The extraction wounds of both groups were covered with absorbable biofilm and free gingival tissue. CBCT was performed at the initial diagnosis, immediately after operation, and 6 months after operation, and the CT images were imported into Mimics 20.0 software package to measure the difference after fitting to obtain data, SPSS 26.0 software package was used for statistical analysis of the data. Results: The vertical height of the alveolar bone in the experimental group and the control group was significantly increased 6 months after operation compared with the initial diagnosis (P＜0.05), but there was no significant difference between the 2 groups(P＞0.05). There was significant difference in the change of the alveolar bone width at 1 mm below the alveolar crest between the two groups at the initial diagnosis and 6 months after operation(P＜0.05), and the horizontal width absorption in the experimental group was smaller than that in the control group. The horizontal absorption rate of bone graft material in the two groups at 1 mm below the alveolar crest showed that the experimental group was lower than the control group, and the difference was statistically significant(P＜0.05). Conclusions: Compared with DBBM alone, combined application of DBBM and CGF can better maintain the alveolar bone contour of the extraction socket with severe periodontitis.

Abstract:The purpose of this study was to explore the different effects between biomimetic mineralized collagen (MC) and ordinary physically blended hydroxyapatite/collagen (HA/Col) composite in evaluating new bone formation and regenerated bone height in human extraction sockets. Thirty-four patients who cannot retain teeth caused by trauma or decay were randomly selected from Department of Stomatology of Dongzhimen Hospital from December 2013 to December 2014. The patients were randomly divided into two groups. After the operation of tooth extraction, 17 patients were implanted with biomimetic MC (MC group), and other 17 patients were implanted with ordinary physically blended nHA/Col composite (nHA/Col group). X-ray positioning projection by auto-photographing was taken to test the distance between the lowest position and the neighboring CEJm-CEJd immediately, 1 month and 3 months after the operation. The height of new bone formation of the MC group was significantly higher than the nHA/Col group. Biomimetic MC showed better clinical outcomes in the bone formation for extraction site preservation and would have broad application prospect in the field of oral and maxillofacial surgeries.

Abstract:ObjectiveThe primary aim of this study was to determine the effect of implant-supported porcelain-fused-to-metal (PFM) and indirect-composite-resin (ICR) fixed dental prostheses on peri-implant marginal bone resorption (MBR) in custom-made anatomic modified zirconia dental implants.MethodsA prospective randomized controlled clinical trial was conducted. Participants with premolars indicated for dental extractions were recruited into this study to receive a single-unit implant-supported fixed dental prosthesis. Modified anatomic zirconia implants with thorny-retentive surfaces were placed and loaded randomly after 3 months with either PFM or ICR crowns. Participants were recalled after 12 and 18 months for radiographic evaluation of peri-implant MBR. Implants survival was also reported.Results18 out of 20 zirconia implants were included in all study phases. 18-month survival rate was 90%. After 12 months of implant placement, the mean MBR values were 0.53 (±0.21) mm and 0.60 (±0.14) mm in the ICR group compared to 0.67 (±0.16) mm and 0.61 (±0.27) mm in the PFM group. In the 18-month follow-up, the mean MBR values were 0.61 (±0.27) and 0.67 (±0.16) mm in the ICR group compared to 0.77 (±0.29) and 0.77 (±0.27) mm in the PFM group. No significant differences were found in MBR mean values between study groups at 12- and 18-month follow-up points.ConclusionThis study showed that PFM and ICR crowns were viable zirconia-implant-supported restorations with no preference regarding MBR after 18 months. Nevertheless, long-term evaluations are warranted.
[Effect of Regenerative Therapy for the Furcation Involvements of Mandibular Molars Evaluated by Cone-Beam Computed Tomography].

Abstract:OBJECTIVE:To evaluate the effect of regenerative therapy for the treatment of furcation involvements of mandibular molars with cone-beam computed tomography (CBCT).METHODS:In the study, 38 furcation involvements of mandibular molars were included and randomly divided into two groups. The experimental group accepted guided tissue regeneration and bone graft therapy, and the control group only flap surgery. The clinical examination and CBCT examination were performed at baseline and 1 year post-surgery.RESULTS:The clinical and CBCT data of both groups were not statistically different at baseline (P>0.05). At the end of 1 year post-surgery, except gingival recession, the clinical parameters of both groups were significantly improved (P<0.001). The vertical and horizontal attachment gains of the experimental group were (3.20 ± 1.82) mm and (2.05 ± 1.27) mm, respectively, and significantly higher than the changes of the control group (P<0.001). And at the end of 1 year post-surgery, the experimental group showed significantly higher bone gain at the vertical and horizontal directions compared with those of the control group: (2.82 ± 0.97) mm and (2.24 ± 0.92) mm, respectively (P<0.001).CONCLUSION:With the limitation of this study, the effect of guided tissue regeneration surgery and bone graft therapy for the treatment of furcation involvements of mandibular molars is significantly better than that of the flap surgery. CBCT can reflect the horizontal and vertical bone changes of furcation area, which is more comprehensive than traditional periapicals.

Abstract:An urgent issue is the preservation or reconstruction of the volume of bone tissue in planning and surgical treatment in the fields of medicine, such as traumatology, orthopedics, maxillofacial surgery and dentistry. After tooth extraction, resorption of the bone tissue of the alveolar crest of the jaws occurs, which must either be further eliminated by performing additional operations or using osteoplastic material for socket preservation at the extraction stage. Background and Objectives: The aim of the study was a comparative analysis of various osteoplastic materials used to preserve the volume of bone tissue in the preimplantation period. Materials and Methods: As part of the study, 80 patients were treated, who underwent socket preservation using xenografts, plasma enriched with growth factors, an autologous dentin matrix (ADM) and hydroxyapatite. Results: The results of the treatment 16 weeks after removal were comprehensively analyzed using a morphometric analysis of the bone's volume, cone beam tomography and morphological examination of burr biopsy specimens, as well as by determining the stability of the installed implant at different stages of treatment. Conclusions: The lowest level of bone tissue resorption according to the CBCT data was noted in the ADM and xenograft groups. It should be noted that the use of osteoplastic material in jaw surgery when reconstructing alveolar defects is an essential procedure for preventing the atrophy of bone tissue.
Effects of periodontal tissue in the gingival retraction before the cementation of zirconia ceramic crowns on an-terior teeth

Abstract:Objective:To compare the effects of periodontal tissue between gingival retraction or not before the ce?mentation of zirconia ceramic crowns on anterior teeth. Methods:Follow up and measure the meliorated plaque index (mPLI),meliorated sulcus bleecling index(mSBI) and probing depths of two groups that each group with 50 patients,who were division by gingival retraction or not before the cementation of zirconia ceramic crown. Results:Reexamined all the pa?tients after cementation in 1,3 and 6 months. There were no statistically significant difference between two groups in the mPLI and probing depths after 1 month and 3 months respectively. But there was statistically significant difference after 6 months( P<0. 05). There was no statistically significant difference between two groups in the mSBI after 1 month,but there were statis?tically significant difference after 3 and 6 months,respectively(P<0. 05). Conclusion:The technology of gingival retraction before the cementation of zirconia ceramic crowns on anterior teeth,which can prevent periondontal disease,and release the damage of periondontal tissue.
Modified alveolar ridge preservation to promote the osteogenesis of the distal surface of second molar after mandibular impacted third molar extraction: a preliminary study

Abstract:Objectives: The objective of the retrospective study was to explore whether the use of a modified alveolar ridge preservation technique impacts osteogenesis on the distal surface of the second molar after mandibular third molar (M3) extraction. Materials and methods: A total of 54 patients were enrolled in this study and divided into three different groups, including modified alveolar ridge preservation (MARP) group, traditional tooth extraction (TRA) group, and classical guided bone regeneration (GBR) group. In this study, MARP was designed with the highlights of the preservation of the alveolar bone superior and lingual to M3. These patients chose different surgical methods according to their own wishes for past infection or in order to prevent pericoronitis, and the operation time and surgical cost of each group were recorded. The periodontal conditions of the ipsilateral mandibular second molar (M2) and the height of its distal alveolar bone were measured during the postoperative follow-up. Results: The probing depth, clinical attachment level, and osseous defect depth on the distal surface of the ipsilateral M2 in the MARP group were better than those of the TRA group at any time of the follow-up (P < 0.05 for all), but there was no statistical difference in the measurements when compared to the GBR group at 6 months after operation (P > 0.05 for all). Conclusions: Thus, MARP therapy not only improves the regeneration of periodontal osseous defects distal to the M2 after M3 extraction but also reduces the operation time and surgical cost. Clinical relevance: This paper introduces a modified surgical method that can not only economically and effectively remove the impacted mandibular third molar but also obtain stable osteogenesis.
Outcomes of Endodontic Microsurgery Using a Microscope and Mineral Trioxide Aggregate: A Prospective Cohort Study

Abstract:Introduction: The aim of this study was to investigate the outcome of endodontic microsurgery and analyze the prognostic factors. Methods: Our prospective cohort study included 98 teeth in 81 patients. An endodontist performed all surgical procedures using endodontic microsurgical approaches. The treated teeth were recalled and examined clinically and radiographically at least 1 year after surgical treatment. The outcome was determined based on clinical and radiographic results. Radiographic healing was classified into 4 categories: complete, incomplete, uncertain, and unsatisfactory healing. An analysis of predictors was performed using multi-variate logistic regression. Results: At recall, 74 of the 98 teeth (75.5%) were examined 12 to 30 months after surgery; 71 of the 74 teeth were analyzed clinically and radiographically, and 3 teeth had been extracted. On periapical radiographs, 55 (74.3%) of the 74 teeth showed complete healing, whereas 12 (16.2%) demonstrated incomplete healing. Together the percentage of complete and incomplete healing was 90.5% (67/74), and all 67 teeth were clinically normal. Uncertain healing was observed in 3 teeth (4.1%), one of which was symptomatic with swelling and sinus tract involvement and the other 2 were asymptomatic. The remaining 1 tooth (1.4%) showed unsatisfactory healing and was asymptomatic. The use as an abutment was found to be a negative factor associated with patient outcome (P<.05; odds ratio = 22; confidence interval, 20.47-23.53). Conclusions: The combined rate of complete and incomplete healing of teeth 12 to 30 months after endodontic microsurgery was 90.5%. The use as an abutment may have a negative effect on treatment outcome. (J Endod 2017;43:694-698)
