Abstract:A retrospective analysis was performed to review 20 patients with left renal vein (LRV) entrapment syndrome. All cases were diagnosed based on ultrasonography, magnetic resonance angiography, and renal venography. Technical success was achieved in all patients. Transposition of superior mesenteric artery was performed in three cases, transposition of LRV in two, and stent implantation in the LRV in 15. Stent migration occurred in one case, and stent reimplantation was performed. One case suffered from hematoma after transposition of the superior mesenteric artery, and reoperation was performed. Follow-up was made from 6 months to 6 years after the operation. Abnormalities were not found in the urine test of 18 patients and their symptoms disappeared. Two cases still had microscopic hematuria after exercise. All surgical operations are effective for nutcracker syndrome except excessive invasiveness. Stent may be one of the mainstream therapies because of its minimal invasiveness.

Abstract:Objective To evaluate the diagnosis and treatment of left renal vein entrapment syndrome. Methods The clinical data of 10 cases of left renal vein entrapment syndrome were retrospectively analyzed.Their mean age was 24 years (range,14-37 years).Of them 7 cases presented with non-glomerular origin hematuria;2,with orthostatic proteinuria;1 with hematuria accompanied by proteinuria.Color Doppler sonography showed that the dilated segment of the left vein was three-fold than the strictured segment in diameter,and magnetic resonance angiography (MRA) showed that the left renal vein was compressed in 9 cases. Results Stent placement across the left renal vein was performed in 7 cases.Superior mesenteric arteriectomy and replantation were performed in 3 cases.The operations were all successful.Follow-up of 1-5 years showed that the clinical symptoms were improved and urine tests were normal. Conclusions Non-glomerular hematuria or orthostatic proteinuria is frequently found in left renal vein entrapment syndrome.Left renal vein entrapment syndrome can be diagnosed by Doppler sonography,MRA and cystoscopy in the presence of hematuria or proteinuria.Generally,conservative treatment and careful follow-up are conducted in these patients.Surgical and radiologic interventional procedures are effective alternatives for those with glomerular dysfunction and complications.
Renal vein thrombosis mimicking urinary calculus: a dilemma of diagnosis

Abstract:Background Renal vein thrombosis (RVT) with flank pain, and hematuria, is often mistaken with renal colic originating from ureteric or renal calculus. Especially in young and otherwise healthy patients, clinicians are easily misled by clinical presentation and calcified RVT. Case presentation A 38-year-old woman presented with flank pain and hematuria suggestive of renal calculus on ultrasound. She underwent extracorporeal shock wave lithotripsy that failed, leading to the recommendation that percutaneous lithotomy was necessary to remove the renal calculus. In preoperative view of the unusual shape of the calculus without hydronephrosis, noncontrast computed tomography was taken and demonstrated left ureteric calculus. However computed tomography angiography revealed, to our surprise, a calcified RVT that was initially thought to be a urinary calculus. Conclusion This case shows that a calcified RVT might mimic a urinary calculus on conventional ultrasonography and ureteric calculus on noncontrast computed tomography. Subsequent computed tomography angiography disclosed that a calcified RVT caused the imaging findings, thus creating a potentially dangerous clinical pitfall. Hence, it is suggested that the possibility of a RVT needs to be considered in the differential diagnosis whenever one detects an uncommon shape for a urinary calculus.
Extravascular Stent Management for Migration of Left Renal Vein Endovascular Stent in Nutcracker Syndrome

Abstract:Background: Nutcracker syndrome is an entity resulting from left renal vein compression by the aorta and the superior mesenteric artery, which leads to symptoms of hematuria or left flank pain. The alternative option of endovascular or extravascular stenting is very appealing because of the minimal invasive procedures. Stents in the renal vein can cause fibromuscular hyperplasia, proximal migration or embolization. Case presentation: A 30-year-old female was diagnosed with nutcracker syndrome for severe left flank pain. After failed conservative approach, she underwent endovascular stenting and subsequently developed recurrent symptom for stent migration one month postoperatively. She underwent successful extravascular stenting with complete symptom resolution. Conclusion: The extravascular stenting is an alternative option after migration of left renal vein endovascular stenting. The computed tomographic imaging was closely correlated to therapeutic interventions and stent migration.

Abstract:Vascular, Ahead of Print. ObjectivesRenal artery aneurysm (RAA) is a rare condition among visceral aneurysms, often asymptomatic and incidentally discovered through imaging. Surgical or interventional procedures are utilized for treatment, depending on the aneurysm's size, morphology, and the patient's physical condition, to prevent rupture.MethodsA patient was admitted with a giant asymptomatic RAA, measuring a maximum diameter of 11 cm on a serendipitous occasion.ResultsWe successfully accomplished a full embolization treatment for the aneurysm, exclusively utilizing inflow tract embolization techniques. Six months post-treatment, a follow-up abdominal ultrasound examination demonstrated a notable decrease in the aneurysm's diameter, signifying the positive outcome of our intervention.ConclusionsThis approach offers a feasible alternative in challenging cases, especially when traditional surgical methods are not feasible or carry significant risks.

Associations of left renal vein entrapment with IgA nephropathy and Henoch-Schönlein purpura nephritis

Abstract:Objectives: The aims of the study were to identify whether left renal vein (LRV) entrapment was more prevalent in IgA nephropathy (IgAN) and Henoch-Schönlein purpura nephritis (HSPN) compared with other types of renal diseases, and the association of LRV entrapment with glomerular incidental IgA and galactose-deficient-IgA1 (Gd-IgA1) deposition. Methods: A total of 797 patients with biopsy-proven kidney diseases have been screened for LRV entrapment by color Doppler ultrasound, and the prevalence of LRV entrapment in different types of renal diseases were then analyzed. Propensity score matching analysis was used to adjust for age, gender, and body mass index. Immunostaining of Gd-IgA1 with KM55 was performed in paraffin-embedded sections of renal biopsy specimens. Results: LRV entrapment was diagnosed in 47 patients (6%) with several kinds of renal diseases in our cohort. A total of 32 (68%) LRV entrapments were combined with expanded IgAN (idiopathic IgAN and HSPN). The prevalence of LRV entrapment in expanded IgAN was significantly higher than that in non-expanded IgAN (17 vs. 2%, p < 0.001), even after adjustment for age, gender, and body mass index by propensity score matching analysis (13 vs. 2%, p < 0.001). Removing expanded IgAN and LN, glomerular incidental IgA deposition was observed to be significantly more common in patients with LRV entrapment compared with patients without it (43 vs. 9%, p < 0.001). Furthermore, in glomerular diseases with incidental IgA deposits, significantly much larger proportion of patients with LRV entrapment were positive for glomerular Gd-IgA1 in contrast to patients without LRV entrapment (5/5 vs. 5/17, p = 0.01). Conclusions: LRV entrapment coexisted with several kinds of renal diseases, with a significantly higher prevalence in patients with idiopathic IgAN and HSPN. In patients of LN and IgAN-unrelated disease with LRV entrapment, glomerular IgA and Gd-IgA1 deposition was more common compared with patients without LRV entrapment.

Abstract:Background: Common etiologies of renovascular occlusive disease include atherosclerosis disease, developmental fibrotic conditions such as fibromuscular dysplasia, and vasculitis. Extrinsic compression of the renal artery is a rarely reported phenomenon but can lead to similar clinical manifestations. Methods: We report recent experience with 2 patients who presented with extrinsic renal artery compression due to entrapment. Diagnosis was made with a constellation of findings on computed tomography angiography, dynamic duplex sonography, and catheter angiography. Both patients had hypertension and 1 had downstream subsegmental renal infarcts. The patients, both with right-sided renal artery entrapment, were treated with open surgical decompression. Exposure was achieved via extended Kocher maneuver followed by mobilization of the right kidney and, in 1 patient, detachment of the right lobe of liver to allow circumferential exposure of the proximal right renal artery to the aorta. All entrapping tissue was circumferentially released. Results: Both operations were uncomplicated. Intraoperative sonography was used to confirm luminal patency of the released segments. Follow-up of renal artery duplex in both patients demonstrated resolution of dynamic compression. Renal artery peak systolic velocity and accelerations indices were all within normal limits. In both patients, improvement in blood pressure control was noted and discontinuation of anticoagulation was possible in the patient who had recurrent episodes of renal infarct. Conclusions: Extrinsic compression of renal artery by diaphragmatic crura is rare but should be considered in younger patients or otherwise any patients with no vascular risk factors when renovascular hypertension workup yields no demonstrable intrinsic disease. A high index of suspicion should be raised when an anomalously high origin of the renal artery or proximity to the diaphragmatic crura is seen on cross-sectional imaging. Work-up should include dynamic imaging to assess compression of renal arteries during expiration. Open surgical or laparoscopic decompression of the involved renal arteries can be curative.

Abstract:Variations of the left renal vein can be in the form of circumaortic vein or renal collar, retro-aortic vein, additional renal vein, or multiple primary tributaries. We report a unique complex venous pattern of concomitant circumaortic and retro-aortic left renal veins associated with a fenestrated left renal artery. Two renal veins, anterior and posterior to the renal artery, originated from the renal hilum. The anterior vein was further divided into two branches. One branch passed through the fenestrated renal artery to continue as the anterior limb of the circumaortic vein. It received the suprarenal and gonadal veins and drained into the inferior vena cava. The other branch coursed posterior to the fenestrated renal artery and joined the posterior renal vein. The posterior renal vein was divided into two branches: one forming the posterior limb of the circumaortic vein, and the other continued obliquely downwards as the retro-aortic vein. Variations in the left renal vein have been implicated in several clinical conditions, such as varicocele and pelvic varices. It also plays a crucial role in renal transplantation, as the left kidney is often used as the donor organ. Even though many reports have been published on circumaortic and retro-aortic veins, the complex venous pattern associated with a fenestrated renal artery has not been reported previously.

Abstract:This case report describes the discovery of a type 1 retroaortic left renal vein (RLRV) in an 83-year-old male cadaveric donor with multiple comorbidities. RLRV is an anatomical variant with an estimated incidence of 0.5-3.6%, with type 1 RLRV being the most common subtype. RLRV is typically asymptomatic, which aligns with the benign nature of the anatomical variation seen in this case. However, it is important to recognize that RLRV can be symptomatic. The hallmark manifestations of renal vein entrapment, colloquially known as the "nutcracker syndrome," include hematuria, proteinuria, flank pain, and varicocele, which are concurrent with the encasement of the renal vein between the aorta and surrounding anatomical structures. RLRV is typically diagnosed using multidetector CT (MDCT) or Doppler ultrasound (DUS). The therapeutic approach to symptomatic RLRV primarily encompasses conservative strategies, such as the administration of angiotensin-converting enzyme (ACE) inhibitors and aspirin, whereas surgical interventions are generally reserved for refractory cases, i.e., when conservative measures fail to alleviate the symptoms.

Abstract:Introduction: This study reports a patient who developed a secondary renal artery aneurysm (RAA) after occlusion of the main renal artery. Methods: A 25-year-old woman was hospitalized due to an enlarged renal artery aneurysm (RAA). Computed tomography angiography revealed a 2.2 mm left renal aneurysm and the absence of the left renal artery trunk, with collateral blood supply from the branch arteries of the aorta. The left kidney function remained normal, allowing successful aneurysm embolization. Three years after embolization, the patient's hypertension improved and became more manageable. Conclusion: Compensation through other abdominal aorta branches after renal artery trunk occlusion is rare, and these branches may also lead to the development of aneurysms. Regular monitoring of these patients is essential.

Abstract:Objective:Discuss the clinical features and treatment methods of the retroaortic left renal venous, improve understanding this diseases.Methods:A 61—year—old male manifested abdominal and waist pain for 3 months with got fever 5 for days.Physical examination showed knock pain of the left renal area.Lab examination revealed a 22×10~9/L of the blood serum leukocytes and normal urine.Computed tomography showed retroaortic left renal vein be compressed and narrowed with left kidney enlarged,perinephric effusion.Results:After conservative therapy of anti—inflammatory for 5 days,the symptoms of waist pain remitted,blood and urine examination turned to normal level.The patients discharged from hospital.Conclusions;Retroaortic left renal vein is a rarely congenital anomalies of the renal vein.The renal vein located between the abdominal aortic artery and the spine.Many cases were found due to the physical examination with no clinical symptoms.Conservative therapy re suits good effects to most patients.But to some cases operation is needed.

Abstract:We present a case of migration of the left renal vein (LRV) stent into the right ventricle in a patient with nutcracker syndrome. After a failed attempt of percutaneous endovascular removal of the stent, the stent in the heart was removed by open cardiac surgery, another stent was placed into the LRV via a puncture in the inferior vena cava (IVC) under direct vision, and the patient had an uneventful postoperative recovery. At 3 years follow-up, the patient was asymptomatic and Doppler ultrasonography demonstrated the stent to be patent and well-positioned.

Abstract:Laparoscopic partial nephrectomy has recently emerged as a minimally invasive treatment for small- to moderate-sized renal tumors. Renal artery pseudoaneurysms (RAPs) have been well-reported in patients with renal trauma or who have undergone percutaneous urological procedures, including biopsy, nephrostomy and percutaneous nephroureterolithotomy. However, RAP following laparoscopic partial nephrectomy for central renal tumor is a rare but serious, potentially life-threatening complication. In total, two patients underwent laparoscopic partial nephrectomy at The First Affiliated Hospital of Zhejiang University School of Medicine (Hangzhou, China) for central renal tumors that had developed gross hematuria several weeks following the surgical procedures. The formation of RAPs was confirmed by contrast-enhanced computed tomography scans. Superselective embolizations of the renal artery branches were successfully performed to treat these two patients. In the current report, the etiology, diagnosis and management of RAPs are discussed.
A case of left testicular artery with high origin passing through a left renal vein fenestration

Abstract:BACKGROUND: Fenestrations of are extremely rare in the venous system, especially renal veins. This paper aims to present a case of left renal vein fenestration where a high origin testicular artery passes through it. Materials and Methods The variation was observed incidentally in a 74-year-old Caucasian male cadaver during routine retroperitoneal dissections for second year medical students.RESULTS: A fenestration in the mid portion of the left renal vein was observed. The length and height of the fenestration was 23 and 3.6 millimeters, respectively. The left testicular artery passed through the fenestration and followed a normal course distal to the fenestration. Posterior to the left renal vein, the testicular artery originated from the lateral aspect of abdominal aorta, just caudal to the left renal artery. On the right side, the testicular artery had a similar high origin, and two renal arteries were present. No venous variations were observed on the right side.CONCLUSIONS: The long course of the left renal vein is a factor of preference for donor kidney selection. Uncommon variations of the left renal veins, such as fenestrations, might result in a change in surgical technique and would put the left donor kidney at risk of prolonged anastomosis time and lower survival rates.

Renal arterial angiography and embolization in patients with iatrogenic renal artery injury

Abstract:Objetctive To study the finding of renal arterial angiography and the effect of segmental renal arterial embolization(SRAE) in patients with iatrogenic renal artery injury.Methods Between Augest 2005 and November 2008,14 patients with persistent or intermittent gross hematuria after renal operations in our hospital were collected.Renal artery injuries were confirmed by renal artery angiography in all patients,of which 11 cases were secondary to percutaneous nephrolithotomy(PCNL),and 3 cases were secondary to renal biopsy,renal suture of penetrating wound,ureteroscopy combined with double-J tube implantation respectively.The gross hematuria emerged in 3 to 346 days after renal operations(Median time,8 days).All patients underwent SARE after renal artery angiography.The embolic agents included coils,PVA,NBCA,ethanol.Fourteen patients were followed up for 10 to 49 months after SRAE.Results In 14 patients,the lesions were located in lower pole of the kidney in 10 cases,and in upper pole of kidney in 4 cases.The findings of angiography showed that renal pseudoaneurysm(RAP) in 1 case,renal arteriovenous fistula(RAVF) in 6 cases,and RAVF combined with RAP in 7 cases.Total occlusions were accomplished and hematuria ceased in 2-12 days after SRAE in all cases.During the follow-up periods,hematuria emerged again in one patient with RAVF in the lower pole of the right kidney caused by blunt trauma in the same kidney after 13 months.Renal arterial angiography demonstrated that the former lesion had no recurred.But a pseudoaneurysm feed by another interlobar artery was found in the lower pole of the right kidney.The patient was treated with SRAE by using coils again,and no hematuria recurred within 14 months.None of 14 patients had severe complications after SRAE.Conclusions RAVF and RAP are dominating forms in iatrogenic renal artery injury.Segmental renal arterial embolization by using coils is safe and effective for iatrogenic renal artery injury.
Endovascular Treatment of Renal Artery Aneurysm

Abstract:Objective To assess the safety and efficacy of endovascular treatment for renal artery aneurysms(RAA).Methods A total of 11 patients who underwent endovascular treatment were analyzed retrospectively.The endovascular techniques included simple coil filling the sac(n=6),coil occlusion of the parent artery(n=3) and stent-graft implantation(n=2).All patients were followed up from 3 to 68 months.Results Thirteen true saccular RAAs were discovered in 11 patients,9 in the right,4 in the left side.Eleven patients(11 RAA) accepted endovascular treatment.After endovascular treatment,post-embolization syndromes appeared in 5 patients,among them 4 patients were found with partial renal infarct during following-up.There was no else major complication and periprocedural mortality.The rupture or reperfusion of RAA was not observed.Conclusion Endovascular treatment is safe and effective for RAA with high success rate.

Abstract:Objective To study the variation of renal vessels with retroperitoneal laparoscopy so as to increase the safety of retroperitoneal laparoscopic surgeries.Methods A total of 525 patients underwent retroperitoneal laparoscopic nephrectomy or partial nephrectomy at our hospital between January 2004 and June 2008.There were 316 males and 209 females with a mean age of (58 ± 13) years old.The procedures were as follows:(1) patients lay on one side with their waist up and the retroperitoneal cavity was established with our institutional method;(2) gerota's fascia was separated widely along the ventral surface of major psoas muscle;(3) the tissues around renal arteries and veins were isolated by ultrasonic scalpel.Careful observation was performed to explore if there were duplicated or accessory renal vessels;(4) renal vessels were cut by Endo-GIA/Hem-o-lok or blocked by bulldog clamps;(5) whole or partial kidney was finally resected (remaining procedures omitted).Results Among all patients,58 patients (11.0% ,58/525) had a variation of renal vessels.There were double renal arteries on one side (n = 18) ,double renal veins (n = 10),3 renal veins (n = 1) and double arteries and veins on one side (n =3).Twenty-five patients (4.8%,25/525) had one accessory renal artery on one side while 19 (76.0% ,19/25) accessory renal arteries went toward the upper kidney pole.The diameter of one patient's left spermatic vein was similar with that of renal vein and they were joined by lumber vein.Conclusion The variation of renal artery is more common than that of renal vein.The accessory renal arteries are common and usually go toward the upper kidney pole.The variation of renal vessels should be considered before and during a laparoscopic procedure.
An Uncommon Variant of Nutcracker Syndrome Secondary to Left Renal Vein Compression Between the Right Renal Artery and The Proper Hepatic Artery

Abstract:Nutcracker syndrome refers to the compression of the left renal vein between the abdominal aorta and the superior mesenteric artery. The subsequent venous congestion of the left kidney, when symptomatic, could be associated with left flank pain, hematuria, varicocele, dyspareunia, dysmenorrhea, and proteinuria. Here we describe a 42-year-old female patient with simultaneous Dunbar syndrome and a rare variant of nutcracker syndrome in which the left renal vein (LRV) compression is secondary to the unusual path of the vein between the right renal artery and the proper hepatic artery. For both the nutcracker syndrome and the Dunbar syndrome, open approach by median mini-laparotomic access for transposition of LRV, and resection of the diaphragmatic pillars and arcuate ligament was attempted. During the intervention, due to anatomical issues, the LRV transposition was converted to endovascular stenting of the LRV, moreover the implanted stent was transfixed with an external non-absorbable suture to avoid migration. At the 12 months follow-up the patient was asymptomatic, and the duplex scan confirmed the patency of the celiac trunk without re-stenosis and a correct position of the LRV stent with no proximal or distal migration.

Abstract:Background: The rationale behind left renal vein division (LRVD) is the assumption that adequate collateral draining channels will develop. Purpose: To describe computed tomography (CT) findings after LRVD during aortic surgery. Material and methods: Among 61 consecutive patients who underwent LRVD during aneurysm repair or revascularization for aortic occlusive disease between January 2003 and December 2017, 51 patients (40 men, mean age 71.4 ± 8.4 years) were enrolled. Contrast-enhanced CT images were analyzed to evaluate collateral drainage, patency, left renal vein diameter, and left renal parenchymal thickness. A total of 115 radiologic reports were reviewed to check whether these findings were accurately mentioned. Results: The median time period of the first postoperative follow-up CT was 36 days (range 7-1351 days). The gonadal vein (n = 47) was the most common collateral draining channel, followed by the retroperitoneal veins (n = 42) and adrenal vein (n = 33). Thrombosis occurred in five patients between postoperative days 7 and 17 in the remnant renal vein (n = 3), remnant renal vein plus gonadal and adrenal veins (n = 1), and gonadal vein (n = 1). There was a significant decrease in renal vein diameter (-0.48 ± 2.12 mm, P = 0.006). There was no significant difference in parenchymal thickness (-0.25 ± 1.27 mm, P = 0.193). Neither LRVD nor any associated findings were correctly stated on radiologic reports. Conclusion: Postoperative contrast-enhanced CT can delineate collateral draining channels and complications following LRVD. However, these findings tend to be either missed or misinterpreted.
