Abstract:BackgroundWith the end of 35 years of the one-child policy in China, the second-child policy presents a challenge for maternity hospitals. We aimed to inform configuration of medical resources by investigating willingness to have a second child in women with risk factors for adverse pregnancy outcomes.MethodsWe did a cross-sectional study in two hospitals in Zhejiang province, China. We surveyed women aged 30–45 years with gynecological disease (6000 in a university hospital and 6000 in a county hospital) by questionnaire, between January and April, 2016. The survey included questions about age, location, educational degree, occupation, physical health, family economic situation, current marital status, composition of women's parents' family, influence of second child on family and work, and childbearing history. Patients with malignant gynecological disease were excluded. Written informed consent was obtained, and the study was approved by the hospitals' institutional review boards.FindingsFor university hospital patients, willingness to have a second child was more closely associated with age (OR 1·005, 95% CI 0·105–3·843, p=0·005) and location (1·041, 0·077–10·37, p=0·043) than in county hospital patients. University hospital patients, with mean age 33·93 years (SD 3·77), were more likely to have two children than county hospital patients, with mean age 34·57 years (SD 4·20; OR 5·85, 95% CI 1·20–28·51, p=0·014). Patients living in rural areas were more likely to be willing to have two children than those living in cities (57·61%, 1060 of 1840 vs 45·91%, 1910 of 4160; OR 1·028, 95% CI 0·51–4·38, p=0·008). For county hospital patients, willingness to have a second child was more closely associated with parents' family composition (OR 1·082, 95% CI 0·02–7·05, p=0·05) than for university hospital patients. In the university hospital, women from families in which both husband and wife were only children, or in which the woman was an only child, were more likely to have two children than from families in which both husband and wife were not only children (64·57%, 1270 of 1966 vs 35·43%, 1670 of 4714, OR 1·02, 95% CI 0·04–10·22, p=0·017.InterpretationRisk factors for adverse pregnancy outcomes are not associated with a reduced willingness to have a second child. In addition to strengthening the capacity and quality of maternal health delivered by hospitals, the Chinese government should increase training and financial support for hospitals to cope with adverse pregnancy outcomes.FundingNone.
[Short-Term Clinical Outcomes of Neonates of Secundiparous Mothers: a Single-Center Cohort Study].

Abstract:OBJECTIVE To investigate the short-term clinical outcomes of neonates of secundiparous mothers, and to provide a reference for the clinical practice after the change in birth policy. METHODS A cohort study was performed for the parturients and their neonates born in Yongkang Maternal and Child Health Care Hospital in Zhejiang, China between June 2015 and April 2016. According to the parity of the mother, the neonates were divided into primiparous group and secundiparous group. The short-term clinical outcomes of neonates were compared between the two groups. RESULTS A total of 4 091 neonates who met the inclusion criteria were enrolled, and there were 2 023 neonates in the primiparous group and 2 068 in the secundiparous group. In the secundiparous group, most mothers (57.16%) were aged 26-34 years, and 16.49% were aged above 35 years. Compared with the primiparous group, the secundiparous group had a significantly higher rate of births by cesarean section (42.55% vs 25.06%; P<0.05). The percentages of neonatal respiratory distress syndrome (NRDS; 8.6% vs 3.4%) and transient tachypnea of the newborn (TTN; 7.2% vs 2.6%; P<0.05) in hospitalized neonates from the secundiparous group were significantly higher than in those from the primiparous group. CONCLUSIONS Elderly parturient women are not the major population of secundiparous parturients. The neonates of secundiparous mothers have a higher rate of births by cesarean section, which might be associated with increased percentages of NRDS and TTN in hospitalized neonates from the secundiparous mothers.
Urinary incontinence, faecal incontinence and pelvic organ prolapse symptoms 20-26 years after childbirth: A longitudinal cohort study

Abstract:Objective: To investigate pelvic floor dysfunction (PFD; urinary incontinence (UI), faecal incontinence (FI) and prolapse) ≥20 years after childbirth and their association with delivery mode history and demographic characteristics. Design: Cohort study with long-term follow-up. Setting: Maternity units in Aberdeen and Birmingham (UK) and Dunedin (NZ). Population: Women giving birth in 1993/1994. Methods: Postal questionnaires at 20 (New Zealand) or 26 (United Kingdom) years after index birth (n = 6195). Regression analyses investigated associations between risk factors and UI, FI and prolapse symptoms. Main outcome measures: Prevalence of self-reported UI, FI, 'something coming down' from or in the vagina (SCD), and the Pelvic Organ Prolapse-Symptom Score, and relationships with delivery method. Results: Thirty-seven per cent (n = 2270) responded at 20/26 years, of whom 61% reported UI (59% of whom reported more severe UI), 22% FI and 17% prolapse symptoms. Having only caesarean section (CS) was associated with a significantly lower risk of UI (OR 0.63, 95% CI 0.46-0.85), FI (OR 0.63, 95% CI 0.42-0.96) and SCD (OR 0.44, 95% CI 0.27-0.74) compared to only spontaneous vaginal deliveries (SVDs). Having any forceps delivery was associated with reporting FI compared to only SVDs (OR 1.29, 95% CI 1.00-1.66), but there was no association for UI (OR 0.95, 95% CI 0.76-1.19) or SCD (OR 1.05, 95% CI 0.80-1.38). Higher current BMI was associated with all PFD outcomes. Conclusions: Prevalence of PFD continues to increase up to 26 years following index birth, and differences were observed according to delivery mode history. Exclusive CS was associated with less risk of UI, FI and any prolapse symptoms.
Effects of Using Sitting Position Versus Lithotomy Position During the Second Stage of Labour on Maternal and Neonatal Outcomes and the Childbirth Experience of Chinese Women: A Prospective Cohort Study

Abstract:Existing research concerning the effects of the sitting birth position during the second stage of labour on maternal and neonatal outcomes remains controversial, and there is a lack of studies to explore its effect on the childbirth experience. The objective of this study is to explore whether the sitting birth position would influence maternal and neonatal outcomes, as well as the childbirth experience. The prospective cohort design was conducted in the study from February to June 2023, a total of 222 women (including primiparous women and multiparous women) were enrolled in our study, and they were divided into the sitting position cohort (n = 106) or the lithotomy position cohort (n = 116). The pre-designed questionnaire and Childbirth Experience Questionnaire (CEQ) were used for data collection during hospitalisation. Chi-square, Fisher’s exact test, t-tests, or the Mann–Whitney U test were utilised to assess differences between groups. Multivariate linear regression and logistic regression were employed to control possible confounders. The study found that primiparous women in the sitting position cohort had a shorter duration of the second stage of labour, higher spontaneous vaginal birth rates, lower episiotomy rates, and a better childbirth experience (p < 0.01). After adjusting for confounding factors through multiple linear and logistic regression analyses, the results remained consistent with those reported above. No neonate in each cohort had Apgar scores at 1 min and 5 min postpartum less than 7 or a Cord artery pH less than 7.00, regardless of parity. Based on the findings, we recommend that women could take the sitting birth position into account when giving birth for a positive childbirth experience, especially for primiparous women. The study could also serve as a reference for healthcare providers in the management of childbirth positions and the development of high-quality maternal care.
Long Interpregnancy Interval and Adverse Perinatal Outcomes: a Retrospective Cohort Study

Abstract:We conducted a retrospective cohort study of 9,552 women experiencing their second delivery between 2014 and 2016 at the International Peace Maternity and Child Health Hospital to investigate the association between the interpregnancy interval(IPI)and adverse perinatal outcomes. With the 12–23-mon IPI as the reference category, logistic regression analyzes were used to examine associations between different IPIs(<12, 12–23, 24–59, 60–119, and ≥120 mon) and perinatal outcomes(gestational diabetes mellitus, premature membrane rupture, gestational hypertension, preterm birth, low birth weight, and macrosomia).Compared with the 12–23-mon IPI category, women with longer IPIs had a higher risk of adverse perinatal outcomes, and those with an IPI ≥120 mon had the highest risk of gestational diabetes mellitus and premature membrane rupture(adjusted odds ratio(OR) 1.76, 95% confidence interval(CI) 1.32–2.35 and adjusted OR 2.03, 95% CI 1.53–2.67, respectively). These results indicate that a longer IPI is associated with a higher risk of adverse perinatal outcomes and an IPI of ≥120 mon appears to be independently associated with a higher risk of gestational diabetes mellitus and premature membrane rupture.

Abstract:BACKGROUND: With the comprehensive implementation of the second child policy in China, the proportion of multiparous women has increased dramatically in the past few years. As nearly half of them have a history of previous cesarean delivery, there is widespread concern regarding complications of their pregnancy.METHODS: The aim of this study was to evaluate the effect of the previous delivery mode on subsequent pregnancy outcomes in the real world based on data from a Chinese birth registry. Birth registry data from July 1, 2016 to June 30, 2017 among a Chinese population were collected and analyzed retrospectively. This study included 14 representative hospitals from 10 provinces of China. All delivery data were collected by an internet-based system using a birth registration platform. The study enrolled 36,355 multiparas. Information extracted for analysis included basic demographic characteristics, previous delivery mode, current delivery mode, major maternal complications, and neonatal outcomes. Pregnancy outcomes of women with previous cesarean delivery (PCS group, n=14,774) were compared with the outcomes of women with previous vaginal delivery (PVD group, n=21,581).RESULTS: There were statistically significant differences in the major pregnancy outcomes between the PCS group and the PVD group. The PCS group had a higher incidence of cesarean section (CS), placenta previa, postpartum hemorrhage, uterus rupture, hysterectomy, gestational diabetes, gestational hypertension, delivery before 37 weeks of gestation, low birth weight, and Apgar Score at 5 min ≤3.CONCLUSIONS: Women with previous cesarean delivery had poorer pregnancy outcomes than women with previous vaginal delivery. Avoiding unnecessary CS, especially in primiparas is essential to improving maternal and neonatal outcomes in later pregnancies.

Effects of delivery mode on postpartum fecal incontinence and urinary incontinence in Chinese women

Abstract:Objective To investigate the prevalence and associated factors of fecal incontinence (FI) and urinary incontinence (UI) in postpartum Chinese women.Methods Questionnaires about FI and UI symptoms were completed via telephone interviews conducted within six months after delivery.Multi-variant Logistic analysis was applied for relation between delivery mode and FI or UI.Results (1) Two thousand and twelve postpartum women were admitted into this study,among which 14 (0.7％) had FI within 6 months after delivery.Logistic regression analysis showed that FI was significantly associated with forceps delivery (OR =20.09,95 ％ CI:3.64-110.90,P =0.000),and mediolateral episiotomy (OR=6.11,95％ CI:1.29-28.80,P=0.024).(2) Among the 2012 women,the prevalence of UI,stress urinary incontinence (SUI),urgent urinary incontinence (UUI)and mixed urinary incontinence (MUD was 10.04％ (n=202),8.15％ (n=164),0.94％ (n=19)and 0.94 ％ (n =19),respectively.Logistic regression analysis found that SUI prevalence was related to maternal age (OR =1.07,95％ CI:1.04-1.11,P =0.000),maternal weight before delivery (OR=1.04,95％ CI:1.02-1.06,P=0.001),neonatal head circumference (OR=1.20,95％ CI:1.05-1.39,P =0.010),mediolateral episiotomy (OR =4.96,95 ％ CI:3.05-8.07,P =0.0005 ),spontaneous vaginal delivery (OR=5.22,95％ CI:2.53-10.76,P=0.000) and forceps delivery (OR=9.20,95％ CI:4.07-20.79,P=0.000).UUI was related to maternal weight before delivery (OR=1.51,95％ CI:1.12-2.05,P=0.008).MUI was related to maternal weight before delivery (OR=1.06,95％ CI:1.00-1.11,P=0.049),duration of second stage of labor (OR=1.01,95％ CI:1.00-1.03,P =0.010),mediolateral episiotomy (OR =7.76,95％ CI:1.42-42.52,P=0.017) and forceps delivery (OR=15.21,95％ CI:1.61-143.44,P=0.018).(3) The prevalence of SUI was higher at 4 days and 42 days after delivery (7.95％ and 9.10％).Conclusions (1) F1 and UI prevalence is lower in this study than in other reports.(2) Vaginal delivery is a risk factor for women's FI and UI,especially forceps delivery and mediolateral episiotomy.(3) Maternal age,pre-delivery weight,newborn head circumference,spontaneous vaginal delivery,forceps delivery and mediolateral episiotomy might increase the risk of UI.
Patterns and Associated Factors of Caesarean Delivery Intention among Expectant Mothers in China: Implications from the Implementation of China’s New National Two-Child Policy

Abstract:Objective: This study explores the basic demographic characteristics of expectant mothers in the context of their intentions regarding mode of delivery, in particular, the preference for caesarean delivery, and analyzes the social and psychological factors that influence delivery preference. Method: A cross-sectional survey of pregnant women was conducted during June to August in 2015. This study adopted a stratified sampling method, and 16 representative hospitals in five provinces of China were included. Results: 1755 and 590 of expectant mothers in their first and second pregnancies, respectively, were enrolled in this study. 354 (15.10%) intended to deliver by caesarean section and 585 (24.95%) participants were uncertain prior to delivery. 156 (8.89%) of expectant mothers in their first pregnancy and 198 (33.56%) expectant mothers in their second pregnancy intended to deliver by caesarean section. Ordinal logistic regression analysis found that nationality, parity, trimester of pregnancy, and advanced maternal age were factors associated with intention to deliver by caesarean (ordered logistic regression/three-level caesarean delivery intention criterion; odds ratios p < 0.05). Conclusions: 8.89% of first pregnancy expectant mothers and 33.56% of second pregnancy expectant mothers intended to deliver by caesarean section. Any intervention program to reduce the rate of Caesarean delivery should focus on the Han population, older pregnant women, and expectant mothers in their second pregnancy, at an early gestation.
Association between primiparas'willingness of a second child and cesarean section

Abstract:ObjectiveTo explore the impact of primiparas’ intention to have a second child on their delivery mode.MethodsDuring March 1st， 2019 and November 30th， 2020， the enrolled pregnant women were investigated by questionnaires at two points， first trimester pregnancy registration and post-natal visit at maternal and child healthcare community centers of Xuhui District. Logistic regression analysis was used to examine the association of willingness of a second birth and their mode of delivery.Results2 000 questionnaires were distributed and 1 664 valid questionnaires were analyzed. The overall cesarean delivery rate in nulliparous women was 40.08% （667/1 664）， with 8.95% （149/1 664） of non-medical indication cesarean delivery. Multiple Logistic regression analysis of overall delivery rate showed that age， willingness to have more children in the future， plan of vaginal delivery at first questionnaire， and complications during pregnancy were associated with overall cesarean delivery rate， and women without the willingness to have more children in the future were more likely to undergo cesarean delivery （OR=1.350， 95%CI： 1.052-1.732）. Age， whether to have a second child in the future， plan of vaginal delivery at first questionnaire， and complications during pregnancy were all factors associated with increased risks of non-medical indicated cesarean delivery， while women with no plan of a second child in the future had nearly 2 times of risk of non-medical indicated cesarean delivery （OR=1.909， 95%CI： 1.117-3.262）.ConclusionThe mode of delivery is affected by many factors， among which willingness to have more children in the future is an important factor associated with increased risks of overall cesarean delivery and non-medical indicated cesarean delivery.
Factors Associated with Intention to Have Caesarean Delivery in Pregnant Women in China: a Cross-Sectional Analysis

Abstract:BackgroundThe proportion of caesarean deliveries significantly increased from 2·0% in 1978––1985 to 36·6% in 2006–2010 among Chinese pregnant women. The rate reached 55% in 2014. In this study we explored the basic demographic characteristics and preferred method of delivery of pregnant women and analysed the associated social and psychological factors.MethodWe did a cross-sectional analysis among pregnant women during June to August, 2015, in 16 hospitals from five provinces of mainland China. Data were collected by interview survey. Intended method of delivery was categorised as “vaginal delivery”, “unsure before delivery”, or “caesarean delivery”. The study protocol was approved by the Ethics Committee of Chongqing Medical University (record number 2015008). All participants gave written informed consent.Findings1755 (75%) women in their first pregnancy and 590 (25%) in their second pregnancy were enrolled in this study. Of these, 354 (156 in first pregnancy and 198 in second pregnancy) intended to undergo caesarean delivery, 585 (25%) participants were unsure prior to delivery, and 1406 (60·0%) intended to have vaginal delivery. Ordinal logistic regression analysis found that women from ethnic minorities (n=8, 8·6%) were less likely to intend to have caesarean delivery than women from the Han Chinese population (n=346, 15·4%; β −0·4863, 95% CI −0·9588 to −0·0398), and women in mid-pregnancy (n=97, 13·8%) were less likely to intend to have caesarean delivery than women in early pregnancy (n=42, 14·3%; −0·2726, −0·5215 to −0·0209). Women in their second pregnancy (n=198, 33·6%) were more likely to intend to have caesarean delivery than those in their first pregnancy (n=156, 8·9%; 0·8526, 0·4593 to 1·0308), and those with advanced maternal age (aged ≥34 years, n=91, 37·5%) were more likely to intend to have caesarean delivery than younger mothers (263, 13·5%; 0·7454, 0·6467 to 1·0584).InterpretationIntervention programmes should focus on women from the Han population, older pregnant women, women in the middle stages of pregnancy, and women in their first pregnancy to reduce the rate of unnecessary caesarean delivery. Quantitative Analysis of Breastfeeding & Maternal and Infant Health Care Conditions in Different Fetal Times of Multiparas under the Two-child Policy

Abstract:Objective To analyze the different conditions of breastfeeding and implementations of maternal and infant health care services between the first and second fetal time of multiparas,in order to explore the methods to promote breastfeeding & maternal and infant health care conditions under the two-child policy.Methods From January to March 2017,using the method of multistage stratified sampling,11 cities of Zhejiang Province was divided into 3 layers by economic levels ranking from high to low.This study randomly selected one city every layer,2 districts in each selected city,and one community health service center in each selected district.In all selected community health service centers,healthy mothers who had registered in community health service centers with no organic diseases and had a second baby aging from 6 months to 3 years old without birth defects were selected.The self-administered questionnaire was used and it contained basis information of respondents,main family members who took care of babies and pregnancy assessment conditions.This study compared and analyzed the different feeding patterns within 6 months of the first and second fetal times,different exclusive breastfeeding duration,different weaning reasons,whether having experienced breastfeeding difficulties and the source of the most effective help,different participation rates of pregnancy schools and postpartum visit situations between the first and second fetal times.Results There were 41 multiparas in the economic development class Ⅰ,73 multiparas in the economic development class Ⅱ and 54 in the economic development class There were significant differences of pregnancy assessment rates before the second pregnancy among different gestational ages (x2 =7.816,P ＜ 0.01).The consistency in feeding patterns of different fetal times was poor (Kappa =0.292,P ＜ 0.01),while there were statistically significant differences of feeding patterns in two fetal times (W =24.997,P =0.035).Exclusive breastfeeding duration and the whole breastfeeding duration of the second fetal time were longer than the first fetal time (P ＜ 0.05).The consistency in breastfeeding difficulties of different fetal times was general (Kappa =0.499,P ＜ 0.01) with statistical differences (x2 =4.45,P =0.034).'Experienced relatives or friends' and'searching by myself'were the two most effective ways for help when meeting breastfeeding difficulties.The participation rates of pregnancy schools were about 50.0％ in two fetal times with general consistency (Kappa =0.502,P ＜ 0.01) and no significant difference was found (W =1.097,P =0.778).There were 81 (48.2％) multiparas required the educations and instructions about breastfeeding skills after the second delivery.The consistency in postpartum visit situations was general in two fetal times (Kappa =0.431,P ＜0.01) with statistically significant differences (W =18.964,P ＜ 0.01).Conclusion The exclusive breastfeeding rate is raised in the second fetal time and the whole breastfeeding duration of the second fetal time is longer than the first fetal time.The participation rates of pregnancy schools in two fetal times are lower than 50.0％.There are gaps of times and contents of postpartum visit between the implementation and national requirements.General practitioners could offer relevant guidance to the elderly residents who take care of babies within general practitioners contract system in order to promote the breastfeeding situation.
Delivery Mode and the Pelvic Floor Function of Primiparous Women at Early Postpartum: An Observational Retrospective Cohort Study

Abstract:Background: Different modes of delivery are strongly associated with postpartum pelvic floor muscle (PFM) injury and postpartum pelvic floor dysfunction. This study used Glazer PFM surface electromyography (sEMG) to objectively assess postpartum PFM function to determine the effects of different modes of delivery on pelvic floor function in the early postpartum period in primiparous women. Methods: There were 1286 cases of cesarean delivery (CD) and 2099 cases of vaginal delivery (VD). The vaginal delivery group was further divided into four subgroups (A: intact perineum without laceration and first-degree laceration; B: second-degree laceration; C: mediolateral episiotomy; D: forceps delivery). Pelvic floor sEMG indices of the subjects were analyzed at 6–8 weeks postpartum. Results: The results showed that the mean peak amplitude of phasic (flick) contractions and the mean amplitude of tonic contractions were both significantly higher in CD than in VD (p < 0.01). In contrast, the mean amplitude variability of tonic contractions was lower in CD than in VD (p < 0.01). The mean peak amplitude of phasic (flick) contractions and the mean amplitude of tonic contractions were statistically lower in the forceps group than in the other vaginal delivery groups (p < 0.05). The mean amplitude variability of tonic contractions was greater in the forceps group than in groups A, B and C (p < 0.01). Conclusions: Vaginal delivery, especially forceps delivery, may result in impaired pelvic floor muscle function in the early postpartum period compared to cesarean delivery.

Association of pre‐pregnancy body mass index and gestational weight gain on postpartum pelvic floor muscle morphology and function in Chinese primiparous women: A retrospective cohort study

Abstract:Objective Our study aimed to investigate the association between maternal pre‐pregnancy body mass index (BMI), gestational weight gain (GWG), and impaired pelvic floor muscle (PFM) morphology and function during the early postpartum period. Methods This retrospective cohort study was conducted at Shanghai First Maternity and Infant Hospital from December 2020 to December 2022. A total of 1118 primiparous women with singleton pregnancies who underwent vaginal deliveries and participated in postpartum PFM assessments were included. Maternal pre‐pregnancy BMI and GWG were considered as exposures. PFM morphology and function impairment were the primary outcomes. PFM morphology impairment, defined as levator ani muscle avulsion, was assessed using transperineal ultrasound. PFM function impairment, manifested as diminished PFM fiber strength, was assessed through vaginal manometry. Multivariable logistic regression analysis was employed to calculate adjusted odds ratios (aOR) with 95% confidence intervals (CI). Restricted cubic spline models were used to validate and visualize the relationship. Results Women with lower pre‐pregnancy BMI were at an increased risk of levator ani muscle avulsion (aOR = 1.73, 95% CI: 1.10–2.70, P = 0.017), particularly when combined with excessive GWG during pregnancy (aOR = 3.20, 95% CI: 1.15–8.97, P = 0.027). Lower pre‐pregnancy BMI was also identified as an independent predictor of PFM weakness (aOR = 1.53, 95% CI: 1.08–2.16, P = 0.017 for type I fiber injuries). Notably, regardless of the avulsion status, both underweight and overweight/obese women faced an elevated risk of reduced PFM strength (aOR = 1.74, 95% CI: 1.17–2.59, P = 0.006 for underweight women with type I fiber injuries; aOR = 1.67, 95% CI: 1.06–2.64, P = 0.027; and aOR = 1.73, 95% CI: 1.09–2.76, P = 0.021 for overweight/obese women with type I and type II fibers injuries, respectively). Conclusions Both lower and higher pre‐pregnancy BMI, as well as excessive GWG, were strongly associated with PFM impairments. These findings highlighted the critical importance of comprehensive weight management throughout pregnancy to effectively promote women's pelvic health.

Abstract:Using the immunoperoxidase technique in conjunction with specific antisera to alpha-atrial natriuretic polypeptide (alpha-ANP), it was shown that immunoreactive cell bodies and varicose fibers are widely distributed throughout the rat brain. The highest concentrations of alpha-ANP-containing neuronal cell bodies and fibers were found in the hypothalamus and septum. This result confirms the radioimmunological determination of alpha-ANP immunoreactivity in the rat brain.

Abstract:OBJECTIVE:To evaluate the association of route of delivery in primiparae with prevalence of urinary incontinence.METHODS:One thousand primiparae who had birth deliveries from March 2001 to March 2002 in our hospital were telephone interviewed through a questionnaire about symptoms of urinary incontinence before and during pregnancy, and after delivery. Totally 548 women answered the questionnaire based on International Consultant on Incontinence completely. The incidence and degrees of different kinds of urinary incontinence in connection with different ways of delivery and the associated factors were evaluated using SPSS software.RESULTS:(1) Among 548 cases, urinary incontinence was diagnosed in 167 (30.5%). (2) According to logistic regression, cesarean section and neonatal weight were associated factors with incontinence. If the odds ratio (OR) were taken as 1.0 for the incontinence women who had a normal vaginal delivery, then OR for cesarean section would be 0.326. OR for neonatal weight would be 1.633. Neonatal weight was the only high risk factor related with incontinence in women with vaginal delivery (P = 0.013, OR = 2.081). (3) The incidence of urinary incontinence in spontaneous vaginal delivery, forceps and cesarean section was 38.6% (105/272), 43.8% (21/48) and 18.0% (41/228) respectively. There was no difference in the incidence of urinary incontinence between spontaneous vaginal delivery and forceps (P > 0.05). A lower incidence of urinary incontinence was found in cesarean section than in other ways of delivery (P < 0.05). (4) The incidences of stress urinary incontinence, mixed incontinence and urgent incontinence were 33.8%, 4.4% and 0.4% respectively in spontaneous vaginal delivery; 35.4%, 8.3% and 0% in forceps delivery; and 14.5%, 3.1% and 0.4% in cesarean section. The incidence of stress urinary incontinence was higher in vaginal delivery women than in cesarean section women (P < 0.05). Severe cases of stress urinary incontinence were more in spontaneous vaginal delivery than in cesarean section (P < 0.05). (5) 1.2% (2/167) women reported urinary incontinence before pregnancy; 7.8% (13/167) during pregnancy; 44% (75/167) within one year after delivery; 40.7% (68/167) persisting for more than one year after delivery; and 2.4% (4/167) in recent time.CONCLUSIONS:(1) Cesarean section may decrease the risk of urinary incontinence compared with vaginal delivery. (2) Most patients have the symptoms of incontinence within one year after delivery, with some lasting for more than one year. (3) Heavy fetal weight increases the risk of stress incontinence.
Pelvic floor dysfunction after intervention, compared with expectant management, in prolonged second stage of labour: A population‐based questionnaire and cohort study

Abstract:Objective To investigate the effect of vacuum extraction (VE) or caesarean section (CS), compared with expectant management, on pelvic floor dysfunction (PFD) 1–2 years postpartum in primiparous women with a prolonged second stage of labour. Design A population‐based questionnaire and cohort study. Setting Stockholm, Sweden. Population A cohort of 1302 primiparous women with a second stage duration of ≥3 h, delivering from December 2017 to November 2018. Methods The 1‐year follow‐up questionnaire from the Swedish National Perineal Laceration Register was distributed 12–24 months postpartum. Exposure was VE or CS at 3–4 h or 4–5 h, compared with expectant management. Main outcome measures Pelvic floor dysfunction was defined as at least weekly symptoms of urinary incontinence, pelvic organ prolapse or a Wexner score of ≥4. The risk of PFD was calculated using Poisson regression with robust variance estimation, presented as crude and adjusted relative risks (RRs and aRRs) with 95% confidence intervals (95% CIs). The implication of obstetric anal sphincter injury (OASI) on pelvic floor disorders was investigated through mediation analysis. Results In total, 35.1% of women reported PFD. Compared with expectant management, the risk of PFD was increased after VE at 3–4 h (aRR 1.33, 95% CI 1.06–1.65) and 4–5 h (aRR 1.34, 95% CI 1.05–1.70), but remained unchanged after CS. The increased risk after VE was not mediated by OASI. Conclusions Pelvic floor dysfunction was common in primiparous women after a prolonged second stage, and the risk of PFD increased after VE but was unaffected by CS, compared with expectant management. If a spontaneous vaginal delivery eventually occurred, allowing an extended duration of labour did not increase the risk of PFD.

Abstract:This study was conducted to evaluate the anti-diabe tic properties of ethanolic extract of two plants commo nly used in folk medicine, Mormodica charantia (bitter melon) and T rigonella foenum-graecum (fenugreek). The study was performed on STZinduced diabetic rats (DM type-I). Plant extracts o f these two plants were given to STZ diabetic rats at the concentratio n of 500 mg/kg body weight ,50 mg/kg body weight respectively. Cid ophage® (metformin HCl) were administered to another group to support the results at a dose of 500 mg/kg body weight, the eth anolic extracts and Cidophage administered orally once a day for four w eeks using a stomach tube and; serum samples were obtained for b iochemical analysis. The extracts caused significant decreases in glucose levels compared with diabetic control rats. Insulin secret ions were increased after 4 weeks of treatment with Cidophage ® compared with the control non-diabetic rats. Levels of AST and ALT li ver enzymes were normalized by all treatments. Decreases in liver ch olesterol, triglycerides, and LDL in diabetic rats were observ ed with all treatments. HDL levels were increased by the treatm n s in the following order: bitter melon, Cidophage , and fenugreek. Creatinine levels were reduced by all treatments. Serum nitric oxide and malonaldehyde levels were reduced by all extracts. GSH levels were increased by all extracts. Extravasation as measure d by the Evans Blue test increased significantly in STZ-induced di abetic animals. This effect was reversed by ethanolic extracts of b itter melon or fenugreek. Infant's Sex, Birth Control Policy and Postpartum Well-Being: a Prospective Cohort Study in Shanghai, China

Abstract:OBJECTIVES:The majority of Chinese families were under either one-child or two-child birth control policy status from 2001 to 2015. We explore the association between an infant's sex and the mother's postpartum well-being, which may be moderated by birth control policy status in China. SETTING AND PARTICIPANTS:We conducted a prospective cohort study in Shanghai City, one of the largest cities in China. A total number of 1730 childbearing women from eight obstetric hospitals across Shanghai were included in the study at baseline, with 1503 women completing the survey 7 days postpartum in 2013. MEASURES:The General Well-Being Schedule (GWBS) was used to assess maternal well-being at baseline and follow-up investigation in the study. The women's demographic, clinical characteristics, and well-being were measured at baseline. Maternal satisfaction and postpartum well-being were assessed in the follow-up survey. RESULTS:Multivariable linear regression analyses showed that women who gave birth to male infants were positively associated with the total score of maternal well-being, when the participating hospitals, maternal well-being at baseline, sociodemographic characteristics, and maternal and infant health outcomes were added to the adjustments (β=1.462, p<0.05). The association disappeared when the two-child policy status was added to the adjustments (p>0.05). The results of a multiple logistic regression model showed that having a male infant was a risk factor of 'severe distress' (OR=1.607, p<0.05), which was moderated by the two-child policy status (p>0.05). CONCLUSIONS:Our results emphasise the importance of conducting interventions to increase maternal general well-being, especially for those with a female infant in a society such as China where preference is for a son, and enhance the necessity of sustainability of a newly relaxed two-child policy which allows more couples to have a second child in China.
Does Epidural Anesthesia Influence Pelvic Floor Muscle Endurance and Strength and the Prevalence of Urinary Incontinence 6 Weeks Postpartum?

Abstract:Introduction and hypothesis With the increasingly extensive application of epidural analgesia, its effect on pelvic floor function outcomes has received growing attention. The aim of the study is to determine the possible effect of epidural analgesia on pelvic floor muscle (PFM) endurance and strength and the prevalence of urinary incontinence (UI) and stress urinary incontinence (SUI) at 6 weeks postpartum. Methods This is a retrospective cohort study of 333 primiparous women after vaginal delivery. At 6 weeks postpartum, a vaginal balloon connected to a high-precision pressure transducer was used to measure PFM strength and endurance. SUI/UI was determined using the verified Chinese International Classification of Urinary Incontinence Short Form (ICIQ-UI-SF) questionnaire. Statistical analysis was performed using binary logistic regression and multiple linear regression analysis. Results Women in the epidural analgesia group experienced longer first and second stages of labor (p < 0.05). There were no statistically significant differences in the rates of perineal lacerations, forceps assistance or episiotomy between women with or without epidural analgesia (p > 0.05). No statistically significant differences were found in PFM endurance (B: 0.933, 95% CI confidence interval: -1.413 to 3.278, p: 0.435) or PFM strength (B: 0.044, 95% CI: -3.204 to 3.291, p:0.979) between these two groups. In addition, the prevalence of UI (30.77% vs. 26.87%) and SUI (21.54% vs. 16.42%) in women with or without epidural analgesia was not statistically significant (p > 0.05). Conclusions PFM function and UI prevalence at 6 weeks postpartum are not significantly affected by epidural analgesia.
