Abstract:Odontogenic infections often occur with dental caries and periodontal disease and commonly have local and systemic implications. Diagnosing a nonodontogenic orofacial infection related to an odontogenic infection is difficult. Both odontogenic and nonodontogenic infections may extend to potential fascial spaces in the orofacial area or deep in the head and neck. The latter complication is often life threatening. Here, we report a case in which a 54-year-old male with diabetes mellitus (DM) had painful swelling of the right cheek. No odontogenic infection source was found after oral examination. Cellulitis was diagnosed, and the infection was determined to be in the right buccal, submandibular, and submental space. After sufficient antibiotic treatment, pus was drained from the right oral commissure, and Klebsiella pneumonia was determined. The patient's history showed that he had DM and suffered a scratch to his oral right commissure as a result of an accident, which explained the nonodongenic cellulitis.
Odontogenic Submandibular and Descending Necrotising Fasciitis or Myositis - A Case Report on a Clinical Dilemma

Abstract:The rationale: Odontogenic space infection has the potential to spread uncontrollably into adjoining fascial spaces. Prompt diagnosis and management are mandatory to prevent gross morbidity and death. Patient concerns: We present a case report of odontogenic origin-submandibular necrotising fasciitis and myositis in a lactating female. Diagnosis: Computed tomography scan revealed an ill-defined fluid collection with air foci within it in the left submandibular region with evidence of extension along the left sternocleidomastoid and along the strap muscles of the neck. Histopathology of the necrotic tissue confirmed necrotising fasciitis. Treatment: Extensive surgical debridement and tooth extraction of left mandibular second molar under antibiotic cover. Outcomes: Uneventful post-surgical wound healing and general medical condition of the mother and child were satisfactory at 13th-month follow-up. Take-away lessons: Prompt diagnosis and management of odontogenic fascial space infection in a lactating mother to prevent the spread of infection in both lactating mother and the child.
Prompt and aggressive treatment of deep neck infection in neglected diabetic patient: A case report and literature review

Abstract:Background: Deep neck infection (DNI) is an infection that occurs in the deep neck fascia and spaces commonly found in diabetic patients. Impaired immune system due to hyperglycemic condition in diabetic patients leads to different clinical presentations, prognosis, and management and therapy in this group of patients. Case presentation: We reported a deep neck infection and abscess in a diabetic patient that resulted in acute kidney injury and airway obstruction. We have done CT-scan imaging that supported the diagnosis of a submandibular abscess. Prompt and aggressive treatment of DNI with antibiotics, blood glucose control, and surgical incision has exhibited a favorable outcome. Conclusion: Diabetes mellitus is the most common comorbidity among patients with DNI. Studies showed that hyperglycemia impaired bactericidal functions of neutrophils, cellular immunity, and complement activation. Aggressive treatment, including early incision and drainage of abscess, dental surgery to eradicate the source of infection, prompt empirical antibiotic administration, and intensive blood glucose regulation will result in favorable results without a prolonged hospital stay.

Abstract:Background: Necrotising fasciitis is a devastating infection characterised by rapidly progressing necrotising infection of the superficial fascia with secondary necrosis of the overlying skin. Aims: To describe the pathophysiology, differential diagnosis, and outcome in a rare case of periorbital necrotising fasciitis caused by group A β-haemolytic Streptococcus. Methods: A 60-year-old female with insulin-dependent diabetes presented with pyrexia and bilateral peri-orbital swelling, progressing to left periorbital necrotising fasciitis. It was caused by dual infection with group A β-haemolytic Streptococcus and Herpes Simplex Virus 1. Results: A combination of intravenous antibiotics and surgical debridement and subsequent skin grafting resulted in a beneficial outcome in our patient. Conclusions: Differentiating cellulitis and necrotising fasciitis can be difficult when presenting signs and symptoms are non-specific. If not treated quickly with antibiotics and debridement of the infected tissue, the patient may develop septic shock within hours.

Abstract:INTRODUCTION Diabetic foot can be dened as an infection involving the foot due to chronic or acute injury to the soft tissues of the foot with evidence of pre existing sensory neuropathy, micro vasculopathy and immunodeciency. Angiopathy and neuropathy are relatively common complications of diabetes mellitus (DM), causing diabetic foot, which brings enormous social and economic pressure to the patients. Diabetic foot ulcers combined with necrotizing fasciitis has rapid onset, involves a wide range of lesions. Necrotizing fasciitis is a rapidly progressive life-threatening infection. The risk factors for necrotizing fasciitis are diabetes, immunosuppression, hypertension, chronic renal failure. It causes systemic toxemia, septic shock and multiple organ dysfunctions increasing the mortality rate to 15% to 35%.

Abstract:Necrotizing fasciitis is an uncommon yet highly dangerous bacterial infection characterized by rapid spread along the fascial planes and subcutaneous tissue, leading to extensive tissue necrosis and often resulting in death. The swift progression of necrosis can induce systemic sepsis, toxic shock syndrome, and multi-organ failure. While necrotizing fasciitis of the neck is rare, it typically originates from dental or pharyngeal sources. Successful treatment hinges on early diagnosis, appropriate antibiotic therapy, and surgical intervention for tissue debridement. This article presents the case of a 40-year-old individual with necrotizing fasciitis of the neck. We herein review the clinical features, pathogenesis, and treatment approach for the case. Rapid recovery necessitated comprehensive medical treatment targeting the underlying cause with aggressive supportive measures. Surgical intervention involved thorough debridement to remove necrotic tissue, irrigation with antiseptic solutions, and early application of topical antimicrobials.

Abstract:This case scenario shows the value of applying relevant imaging and the selection of appropriate antibiotic via culture and sensitivity before prescribing especially when dealing with a patient with type 1 Diabetes Mellitus. When confronted with a diagnosis of acute osteomyelitis, it is always better to refer the patient to a hospital for admission where the management and any other intervention is easily accomplished. Admission to the hospital, culture and sensitivity, and appropriate intravenous antibiotics may have limited the progress of the infection and ultimately may have prevented the sequestration in this patient, limiting the degree of morbidity. Monitoring and control of blood glucose levels is an important part of the management in a patient from this scenario.

Abstract:Introduction: Cervical necrotizing fasciitis is a fatal and rapid multi- bacteria infection causing extensive necrosis of the subcutaneous tissues and fascial planes with resultant skin gangrene and associated systemic manifestation. The aim of the present study is to report four cases of cervical necrotizing fascitis highlighting their source. Method: The case notes of the patients were retrieved and reviewed, literature search was done using Medline, journals available and various texts. Results: Most often the cause of cervical necrotizing fascitis is of dental origin. The diagnosis depends mainly on clinical features and a high index of suspicion because the clinical features maybe innocuous at the early stage. Conclusion: Despite aggressive management with liberal wound debridement, intravenous antibiotics, and nutritional support, the mortality was still high due to late presentation.

Abstract:Descending necrotizing mediastinitis is a potential consequence of cervicofacial infection, which can promptly progress into a life-threatening disease, with the sequelae of severe sepsis, organ failure and death. This article highlights the importance of this pathology and discusses key signs and symptoms that may be identified when performing an assessment on a patient with a large cervicofacial swelling. This article also describes the case of a young and healthy patient who experienced this pathology from an odontogenic source. CPD/Clinical Relevance: Descending necrotizing mediastinitis, although rare, is a potentially life threatening consequence of odontogenic infection and awareness of it is imperative for dental professionals.

Abstract:Osteomyelitis is inflammation of the bone which begins as an infection of the medullary cavity with rapid involvement of the haversian systems and extension to the periosteum. Various etiological factors are involved in origin of the disease, among them fungal origin is rare. Specific feature in fungal osteomyelitis is involvement of maxillary sinus. Maxillary osteomyelitis can be classified based on the following causes: traumatic, rhinogenic, and odontogenic. I hereby report a case of fungal osteomyelitis involving maxilla in a 50-yearold man with uncontrolled diabetes mellitus. This specific case was necrosed bone with analysed using Energy dispersive Xray and Scanning electron microscope analysis. This case report highlights the need of oral physician to rule out fungal origin in osteomyelitis.

Abstract:Necrotizing fasciitis (NF) of the face is a rare yet serious condition requiring prompt and comprehensive management. This approach typically involves input from various medical specialties such as infectious disease specialists, critical care physicians, and surgeons. The primary goals are early recognition, aggressive surgical debridement, appropriate antibiotic therapy, and supportive care. Prompt diagnosis is crucial, based on symptoms like severe pain, rapidly spreading erythema, and systemic signs of infection. Broad-spectrum antibiotics are initiated empirically, and adjusted based on culture results. Urgent surgical debridement is crucial, removing all necrotic tissue. Careful consideration must be given to preserve vital structures. Close monitoring and intensive care may be necessary, especially for severe cases. Soft tissue reconstruction may follow once the infection is controlled, aiming to restore function and aesthetics. Long-term follow-up is essential to observe for complications and recurrence.

Abstract:Fournier's gangrene (FG) is a relatively rare yet profoundly severe disease. It predominantly affects males; however, mortality rates are comparatively elevated in females. It is a rapidly spreading, life-threatening necrotizing fasciitis that can affect all parts of the body but primarily targets the genital region and the perineum. The clinical presentation is highly characteristic of the disease and is often sufficient for reaching a definitive diagnosis. Common risk factors for the development of this condition include diabetes mellitus (DM), obesity, trauma, alcoholism, smoking, arterial hypertension (which predisposes to obstructive endarteritis), and immunosuppressive disorders, such as HIV and cancer. Prompt diagnosis and treatment are imperative for the prognosis and survival of patients. Herein, we present a case of a 33-year-old woman with a medical history of type 1 diabetes mellitus (treated with insulin), arterial hypertension, and obesity. She presented with pain and swelling in the external genitalia (right labia majora), which later progressed to severe necrotizing fasciitis. The patient underwent surgical debridement and drainage, along with intensive medical therapy.

Abstract:Background: Pantoea agglomerans is a gram negative, aerobic/facultative anaerobic, rod shaped bacilli commonly isolated from plants, soil, food and faeces.(1) It is a rare cause of opportunistic infections in humans acquired mainly via two major routes being, wound infection or hospital acquired. Case report: Here, we encountered a landmark, first of its kind, head and neck manifestation of a cervical soft tissue abscess with Pantoea agglomerans being the miscreant. The patient presented with complaints of a left sided neck swelling, which was radiologically suggestive of a cold abscess, however clinical suscpicion encouraged us to perform an incision and drainage, culture of which revealed this notorious phytogenic bacterium. Discussion: Commonly encountered Pantoea infected cases documented in literature have shown a clinical picture of endophthalmitis, acute unilateral dacryocystitis, periostitis, endocarditis, osteomyelitis and a tumour like muscle cyst of the thigh with many of them eventually leading to septicemia while a few also resolved with targeted antibiotics.(2) Remarkably, no ENT or head and neck presentations have been reported in literature till date. History of trauma by brushing against a mango tree was confirmed retrospectively, which was found to be the missing piece of the puzzle.

Abstract:Mucormycosis (zygomycosis) is a severe and often fatal mycotic infection affecting primarily immunocompromised individuals. A 61-year-old female with type 2 diabetes mellitus and end-stage renal disease developed septic shock in association with mucormycosis. Despite antifungal treatment with liposomal amphotericin B, the patient's condition rapidly deteriorated, leading to death within 48 hours. This case underscores the aggressive nature of mucormycosis, highlighting the necessity for early diagnosis using advanced diagnostic tools and prompt treatment to improve patient outcomes.

Abstract:Diabetes-associated focal myonecrosis is a rare complication seen in individuals with long-standing uncontrolled diabetes, characterized by inflammation and necrosis of a single or group of muscles. The exact cause of this condition is not well understood, but it is believed to be due to focal muscle infarction secondary to arteriosclerosis and diabetic microangiopathy. Diagnosis is challenging and often requires clinical examination, lab investigations, imaging, and EMG. Treatment is mainly supportive with pain control and tight glycemic control, and surgical intervention is rarely needed. The clinical presentation includes a sudden onset of localized pain and swelling in the affected muscle, which may be accompanied by fever, malaise, and weight loss. Diabetic myonecrosis exhibits a slightly higher prevalence in females and commonly manifests at an early stage. While the short-term prognosis is good, the recurrence rate is high, often affecting the opposite limb within six months. Our case describes a 35-year-old young male with uncontrolled diabetes mellitus, diagnosed one year ago, who presented with medial thigh pain and tenderness for the last two days. Due to his early disease, focal myonecrosis was not our first differential diagnosis. A CT scan with contrast revealed findings consistent with either focal myositis or infarction. We ruled out other causes, including infections, autoimmune disease, trauma, and medications, and in combination with the patient's uncontrolled diabetes mellites, a diagnosis of diabetes-associated focal myonecrosis was made. The patient improved with blood sugar control and supportive care, including nonsteroidal anti-inflammatory drugs and muscle relaxants.
Actinomycosis of periodontium, mimicking gingival enlargement - A case report and its management

Abstract:Gingival abscess is a localized, acute inflammatory lesion which is caused by various factors that include microbial plaque infection, foreign body impaction, and trauma. Management includes a simple incision and draining of the purulent exudate along with systemic antibiotics in most of the cases. This article reports a case of a 16-year-old female patient, with a chief complaint of painful and swollen gums in her lower right front tooth region of the jaw along with fever for the past 5 days. Emergency treatment of abscess drainage was done on day 1. On 5th day of review, the case presented with persistent signs of gingival inflammation in relation to 43 to 41. Considering the provisional diagnosis as dentigerous cyst with persistent gingival abscess, definitive treatment was performed by transalveolar extraction of the impacted canine and debridement of the osseous lesion. The histologic analysis of the lesion exhibited foci of eosinophilic coagulum-like areas with epicentric and radiating filamentous appearance at the periphery, which is suggestive of an inflamed dentigerous cyst with actinomycosis infection of periodontium.
Preseptal cellulitis and cerebral venous sinus thrombosis complication in a patient with diabetes mellitus

Abstract:This was a rare case of preseptal cellulitis with an unexpected complication by cerebral venous sinus thrombosis. A 73-year-old woman with poorly controlled diabetes mellitus presented with a week history of swelling and redness on the left upper eyelid and right forehead, associated with poor oral intake, lethargy, and fever. She was generally lethargic with poor verbal response. She had cellulitis of the left upper eyelid and right forehead with the left upper eyelid necrosis. Computed tomography venography of the brain revealed thrombosis of the right transverse sinus, right sigmoid sinus, and right internal jugular vein. She later developed left upper eyelid and right forehead abscesses. Incision, drainage, and wound debridement were performed. She was treated with intravenous antibiotics and anticoagulant. After 1 week of treatment, the preseptal and forehead cellulitis had resolved. However, she passed away due to aspiration pneumonia with respiratory failure on day-13 of hospitalization.

Abstract:Salmonella infections commonly cause gastroenteritis and enteric fever but can also result in extraintestinal infections, especially in immunocompromised individuals. Although rare, Salmonella infection in the head and neck region was found to be more common in diabetics and patients with malignancy. We present a unique case of a 52-year-old immunocompromised man with uncontrolled diabetes who developed a submandibular abscess due to Salmonella Paratyphi B. The patient initially presented with pain, swelling, and difficulty swallowing, which worsened over a week. Diagnostic imaging revealed a well-defined abscess with lymphadenopathy. Management included incision and drainage, followed by identification of Salmonella Paratyphi B and targeted antibiotic therapy based on culture and sensitivity analysis. This case highlights the importance of early recognition, appropriate diagnostic imaging, and targeted antimicrobial therapy in managing uncommon manifestations of Salmonella infection in the head and neck. Continued vigilance and multidisciplinary management are essential for favorable patient outcomes in such cases.

Abstract:Journal of International Medical Research, Volume 52, Issue 8, August 2024. Mouth floor cellulitis is a type of diffuse cellulitis involving the submandibular, submental, and sublingual spaces. This condition may cause asphyxia due to elevation and posterior deviation of the tissues of the floor of the mouth. The severity of submandibular gland infection often escalates in the presence of underlying comorbidities. Advanced age, hyperglycemia, and an immunocompromised status often lead to the rapid development of infection, resulting in complications such as acute upper airway obstruction. These complications increase treatment difficulty and the risk of mortality. We herein report a case involving an older adult with diabetes who developed mouth floor cellulitis secondary to a submandibular gland infection. Despite the severity of the submandibular gland infection, a timely, effective, and multidisciplinary approach improved the patient's prognosis.
