Abstract:Background: This study assessed the prevalence of gastroesophageal reflux disease (GERD) in a general adult population in Iran. The association between GERD and various factors was also evaluated. Methods: We performed a cross-sectional study on 163,018 individuals aged over 35 who were enrolled in the PERSIAN cohort. GERD was defined as the occurrence of heartburn and/or regurgitation symptoms at least several days a month. Survey design analysis for pooled data was performed and multiple regression analysis was conducted to determine the independent risk factors for GERD. Results: The prevalence of GERD in our study was estimated at 21.86% (95% confidence interval:17.4%-36.4%). The mean age of the participants was 49.84 years±9.25 (35-70) and 44.75% of the participants were male. Symptoms of heartburn and regurgitation were reported in 18.65% (n: 29,170) and 6.06% (n: 9,717) of participants, respectively. In the multivariate analysis, several factors were found to be associated with a higher prevalence of GERD: female sex, age >50, current smoking, opium use, weekly consumption of fried foods, frequent consumption of hot tea, less than 6 hours of sleep per night, psychiatric disorders, usage of NSAIDs, and poor oral hygiene, were associated with a higher prevalence of GERD. Conversely, higher education levels and average physical activity were found to be less commonly associated with GERD. Conclusion: We found a relatively high prevalence of GERD (21.86%) in this population-based study in Iran. By identifying modifiable risk factors, this research offers opportunities for targeted interventions and lifestyle modifications to reduce the burden of GERD.
Prevalence and risk factors of gastroesophageal reflux disease symptoms in Mazandaran, North of Iran: A Tabari cohort study

Abstract:Background: Gastro-esophageal reflux disease (GERD) is a very common complaint. It is a major health concern and there is paucity of information about the epidemiology of the disease and its risk factors in Iran, especially Mazandaran province (North of Iran). This study aimed at investigating the prevalence of regurgitation and the factors associated with this condition in Tabari cohort study. Methods: This was a cross-sectional study that analyzed data from Tabari cohort study. Information including the presence and frequency of heartburn and regurgitation, demographic characteristics, socioeconomic status, occupational history, history of chronic illnesses, history of alcohol and cigarette consumption were recorded. Results: The prevalence of GERD symptoms were 27.6% (20.4% in men, and 32.4% in women, p=0.0001). The frequency of typical symptoms was significantly higher in women than that in men. The risk of developing GERD symptoms were 1.7 times higher in women (p=0.0001). The highest prevalence of GERD symptoms was found in urban areas (41.8%, p=0.0001), in people with low educational levels (48%, p=0.0001), and in participants with history of depression symptoms (36.2%, p=0.0001). The prevalence of GERD symptoms was significantly high in individuals with higher BMI (29.5%, p=0.006), greater waist to hip ratio (29.1%, p=0.0001, p=0.0001), and high waist circumference (31.7%, p=0.0001). Conclusion: This study showed gender, region of residence, educational level, and depression symptoms as the main risk factors for developing GERD symptoms.

Abstract:AIM:To analyze the spectrum and risk factors of gastroesophageal reflux disease (GERD) based on presenting symptoms and endoscopic findings. METHODS:A cross-sectional survey in a cluster random sample was conducted from November 2004 to June 2005 using a validated Chinese version Reflux Disease Questionnaire (RDQ) and other items recording the demographic characteristics and potential risk factors for GERD.Subjects were defined as having GERD symptoms according to the RDQ score (> 12).All subjects were endoscopied and the definition and severity of erosive esophagitis were evaluated by Los Angeles classification.The statistical analysis was performed with SPSS13.0 programs. RESULTS:Of 2231 recruited participants, 701 (31.40%) patients were diagnosed as having GERD while 464 (20.80%) patients had objective findings of reflux esophagitis (RE).Of those 464 patients, only 291 (13.00%) were reported as subjects with GERD symptoms.A total of 528 (23.70%) patients were found to have GERD symptoms, including 19.50% patients with grade A or B reflux esophagitis, 0.90% with grade C and 0.40% with grade D. On multivariate analysis, old age, male, moderate working burden, divorced/widowed and strong tea drinking remained as significant independent risk factors for erosive esophagitis.Meanwhile, routine usage of greasy food and constipation were considered as significant independent risk factors for non-erosive reflux disease (NERD).CONCLUSION: GERD is one of the common GI diseases with a high occurrence rate in China and its main associated factors include sex, anthropometrical variables and sociopsychological characteristics.

Abstract:Abstract Background Gastroesophageal reflux disease (GERD), which leads to acid reflux into the esophagus, is a common gastrointestinal disorder. Several studies have shown the prevalence of GERD in Iranian population, but their evidence is contradictory. Therefore, the present study was conducted to investigate the epidemiology of GERD in Iran. Methods The entire steps of this systematic review and meta-analysis were based on the MOOSE protocol, and the results were reported accordance with the PRISMA guideline. This review is registered on PROSPERO (registration number: CRD42020142861). To find potentially relevant published articles, comprehensive search was done on international online databases Scopus, Science Direct, EMBASE, PubMed/Medline, CINAHL, EBSCO, Cochrane Library, Web of Science, Iranian online databases and the Google Scholar search engine in June 2019. Cochran test and I 2 index were used to assess the heterogeneity of the studies. Data were analyzed using Comprehensive Meta-Analysis software ver. 2. The significance level of the test was considered to be P < 0.05. Results The daily, weekly, monthly, and overall prevalence of GERD symptoms in Iranian population was 5.64% (95%CI [confidence interval]: 3.77–8.35%; N = 66,398), 12.50% (95%CI: 9.63–16.08%; N = 110,388), 18.62% (95%CI: 12.90–26.12%; N = 70,749) and 43.07% (95%CI: 35.00–51.53%; N = 73,189), respectively. The daily, weekly, monthly, and overall prevalence of heartburn in Iranian population was 2.46% (95%CI: 0.93–6.39%; N = 18,774), 9.52% (95%CI: 6.16–14.41%; N = 54,125), 8.19% (95%CI: 2.42–24.30%; N = 19,363) and 23.20% (95%CI: 13.56–36.79%; N = 26,543), respectively. The daily, weekly, monthly, and overall prevalence of regurgitation in Iranian population was 4.00% (95%CI: 1.88–8.32%; N = 18,774), 9.79% (95%CI: 5.99–15.60%; N = 41,140), 13.76% (95%CI: 6.18–44.31%; N = 19,363) and 36.53% (95%CI: 19.30–58.08%; N = 21,174), respectively. The sensitivity analysis for prevalence of all types GERD, heartburn and regurgitation symptoms by removing a study showed that the overall estimate is still robust. Conclusion The present meta-analysis provides comprehensive and useful information on the epidemiology of GERD in Iran for policy-makers and health care providers. This study showed a high prevalence of GERD in Iran. Therefore, effective measures on GERD-related factors such as lifestyle can be among the health policies of Iran.

Abstract:Background and Objectives: Gastroesophageal Reflux Disease (GERD) is a condition characterized by heartburn and acid regurgitation without signs of oesophagal mucosal injury on one hand, and erosive oesophagitis and Barrett's oesophagus consequences like oesophagal cancer on the other. The aim of this study was to explore the involvement of different lifestyle-related factors in the aetiology of symptomatic GERD. Methods: In this cross-sectional study, 79 patients (28 men and 51 women) aged be-tween 20-68 years old were recruited randomly through a direct interview between January to October 2021, and they were prescribed Proton Pump Inhibitors. Prior to starting this study, the approvals had been granted by the ethics committee and oral consent was gained from the participants. A questionnaire was designed and consisted of demographic and clinical characteristics regarding GERD. Statistical analysis was done using SPSS version 25 for describing frequencies and percentages, followed by Chi-square and Fisher’s Exact tests as inferential statistical analysis for finding associations between variables. Results: Among the total of 79 patients with gastroesophageal reflux disease, the findings indicated that more than half of the study sample (58.2%) were overweight and obese (26.6% and 31.6% respectively), and the heavy smokers made the highest percentage among smokers (19%). There was a statistically significant association between gender and smoking and BMI among GERD patients (P-value = 0.001 and < 0.001 respectively). There was a very highly significant association between BMI and heartburn and nausea and vomiting (P-value = 0.001 for both), while there was a non-significant association between BMI and chest pain, cough, and sleep pattern disturbance (P-value = 0.324, 0.558, and 0.907 respectively). Conclusion: A higher BMI and smoking are associated with a higher likelihood of GERD symptoms

Abstract:Introduction Gastroesophageal reflux disease (GERD) is marked by the frequent occurrence of stomach acid flowing back into the esophagus, causing symptoms such as heartburn and acid regurgitation at least once a week. When reflux leads to troublesome symptoms and esophageal damage and adversely affects quality of life, it is diagnosed as GERD. Age, gender, ethnicity, genetic predispositions, and aspects of diet and lifestyle, including factors like obesity and smoking, are associated with GERD. Methodology This cross-sectional study was conducted within the Departments of General Medicine, Surgery, and Gastroenterology at Khyber Teaching Hospital (KTH) in Peshawar, spanning from January 2024 to June 2024. Patients who visited these departments or the Outpatient Department within the specified period with GERD were included in the study. A non-probability purposive sampling technique was used. For the analysis, we utilized IBM SPSS Statistics version 21. Results This study consists of 280 participants. The mean age of the participants in this study was 44.60 years. GERD has a significant association with obesity (69.99) and lack of exercise (80%), and a negative association was found between other gastrointestinal conditions (55.71%) and smoking (64.28). Common symptoms among GERD patients were swallowing difficulty, regurgitation, heartburn, and chest pain. Conclusion Our study is the first to examine the relationship between lifestyle factors and GERD among Pakistani patients. Our findings highlight significant associations between GERD and several factors, including gender, BMI, dietary habits, and lack of exercise. Notably, cultural and regional differences appear to influence GERD prevalence and its risk factors, as demonstrated by the minimal impact of alcohol consumption in our study population.

Abstract:Gastroesophageal reflux disease (GERD) is a common functional gastrointestinal disorder with significant effects on the quality of life. The burden of GERD is soaring in Asia. Preventing symptom relapse is a therapeutic goal in GERD patients. Since proton pump inhibitors (PPI) are the first-line treatment of GERD, drug failure has become a major problem in the treatment procedure. We reviewed the literature in order to find articles related to comorbidities and symptoms affecting GERD from 1980 to 2015 via PubMed and Google Scholar using keywords such as 'Gastroesophageal reflux disease', 'Gastrointestinal symptoms' and Boolean operators (such as AND, OR, NOT). Due to the cost of PPI therapy and the high rate of GERD relapse after PPI therapy, demand for continuing this type of treatment is decreasing. Thus, we need to discover new approaches to treat the disease and also investigate the relationship between the treatment of GERD and its comorbidities and symptoms such as functional constipation.

Abstract:Gastroesophageal reflux disease (GERD) is one of the most common causes of health care seeking at the primary care level in many countries. At an epidemiological level, GERD has been shown to be associated with a number of risk factors: obesity, tobacco smoking, alcohol abuse, certain patterns of eating behaviour, and the use of several medications. GERD is now regarded as a heterogeneous disease and includes different phenotypes (erosive reflux disease, non-erosive reflux disease, hypersensitive oesophagus, functional heartburn), the proper diagnosis of which improves the effectiveness of therapy in patients with heartburn symptoms. Daily impedance–pH monitoring is known to be an integral part of the diagnostic algorithm for GERD and is a functional diagnostic method to record all types of refluxes entering the oesophagus regardless of pH, to assess their association with symptoms, and to determine whether patients with heartburn symptoms belong to a particular phenotype. Esophageal manometry plays a key role in the evaluation of patients with heartburn symptoms, as it helps to rule out other conditions that may mimic GERD: achalasia cardia and scleroderma esophagus. This technique is used to assess thoracic esophageal motility and sphincter function and in the assessment of patients prior to antireflux surgery or in the refractory course of GERD. The article describes in detail GERD risk factors (triggers of heartburn), as well as diagnostic aspects, taking into account a differentiated approach to patients with heartburn based on daily impedance–pH monitoring data in accordance with the current guidelines and recommendations.

Abstract:Objective To investigate the prevalence of gastroesophageal reflux disease(GERD)in outpatients who visited department of internal medicine in hospitals of Zhejiang province and its risk fac- tors were analyzed.Methods The epidemiologic study was carried out on outpatients with reflex diagnos- tic questionnaire(RDQ).The data was analyzed with Logistic regression methods.Results The preva- lence of GERD in those patients was 7.28%(95% CI:6.87%-7.69%)in preliminary screening.There was significant correlation among gender,age,degree of labour intensity,status of marriage and the oc currence of GERD(P<0.05).The prevelence of GERD was increased in patients with habits of over eat- ing,drinking carbonate beverage and strong tea,eating greasy and acridity food and sweetmeat,smoking, constipation(P<0.01).Single-factor Logistic regression analysis indicated that gender,age,degree of labour intensity,on night or shift,status of marriage were risk factors of GERD.Multi-factors Logistic regression analysis indicated that age,on night or shift,degree of labour intensity,status of marriage, eating greasy food and sweetmeat,drinking strong tea,constipation were the risk factors of GERD. Conclusions RDQ questionnaire is a simple and effective method in clinic diagnosis of GERD.Many risk factors are involved in pathogeneses of GERD,and more attention should be pay on it.

Abstract:Objective To explore the risk factors of gastrointestinal esophageal reflux disease(GERD) and its clinical characteristics. Methods We analyzed the Reflux Disease Questionnaire(RDQ) and other related data from GI out-patient department,People's hospital,Peking University during March 2004 to May 2004,with follow-up call made in May of 2011. Results 1045(99.3%) out of 1052 RDQ questionnaires were available.GERD,noted in 16.3% patients,was not seasonal.The mean course of disease took for 6.45 years.Old age [odds ratio(OR)=1.02;95% confidence interval(CI)=1.01～1.04],physical work(OR=1.79,95% CI:1.13～2.86) and drinking(OR=2.63,95% CI:1.17～5.92) increased the risk of GERD.Gender,smoking,body mass index,as well as blood type were not associated with GERD.Among GERD cases,29.6% had pharyngeal and respiratory symptoms,significantly higher than non-GERD cases(χ2=37.6,P0.01).26.3% GERD cases took antacid drugs,9.3% of which took proton pump inhibitors,and 40% cases were not satisfied with the treatment.7 years later,307 cases received follow-up 7 years later.44.0% of the GERD cases still suffered from significant symptoms. Conclusion GERD is common in GI department of comprehensive hospitals.Olde age,physical work and excessive drinking are risk factors of GERD.Symptoms of GERD are hard to control and mostly combining with extra-esophageal manifestations,and would last for years in half of the cases.

Abstract:Gastroesophageal reflux disease (GERD) is one of the most common diseases of the upper gastrointestinal tract. The most characteristic symptom of the disease is heartburn, which occurs at least once a week. The prevalence of the disease varies and, depending on the region of the world, it may affect from a few to over 30% of an adult population. It is estimated that in Poland this disease may affect up to 35.5% of adults reporting abdominal ailments. If untreated, the disease can lead to serious complications including precancerous conditions and esophageal adenocarcinoma. Pharmacotherapy is considered as the first-line treatment in GERD patients but lifestyle modifications, including diet changes, are an important element supporting the treatment of the disease. Many factors may contribute to the development of the disease. Among them, there are non-modifiable factors such as age, sex or genetic factors and modifiable factors, e.g. lifestyle, diet, excessive body weight. This review focuses on GERD risk factors related to lifestyle and nutrition that include both dietary components and nutritional behaviour. Lifestyle risk factors that may contribute to GERD symptoms include excessive body weight, particularly obesity, moderate/high alcohol consumption, smoking, postprandial and vigorous physical activity, as well as lack of regular physical activity. Many studies indicate fatty, fried, sour, spicy food/products, orange and grapefruit juice, tomatoes and tomato preserves, chocolate, coffee/tea, carbonated beverages, alcohol as triggers for GERD symptoms. Eating habits such as irregular meal pattern, large volume of meals, eating meals just before bedtime may correlate with the symptoms of GERD. The role of lifestyle, diet and eating habits as risk factors for GERD is not clearly understood, and the results of the available studies are often contradictory. Determination of modifiable risk factors for this disease and its symptoms is important for effective dietary prevention and diet therapy of GERD.

Abstract:Gastroesophageal reflux disease (GERD) is a common gastrointestinal disorder with an increasing prevalence. GERD develops when the reflux of stomach contents causes troublesome typical and atypical symptoms and/or complications. Several risk factors of GERD have been identified and evaluated over the years, including a considerable amount of genetic factors. Multiple mechanisms are involved in the pathogenesis of GERD including: (1) motor abnormalities, such as impaired lower esophageal sphincter (LES) resting tone, transient LES relaxations, impaired esophageal acid clearance and delayed gastric emptying; and (2) anatomical factors, such as hiatal hernia and obesity. Genetic contribution seems to play a major role in GERD and GERD- related disorders development such Barrett's esophagus and esophageal adenocarcinoma. Twin and family studies have revealed an about 31% heritability of the disease. Numerous single-nucleotide polymorphisms in various genes like FOXF1, MHC, CCND1, anti-inflammatory cytokine and DNA repair genes have been strongly associated with increased GERD risk. GERD, Barrett's esophagus and esophageal adenocarcinoma share several genetic loci. Despite GERD polygenic basis, specific genetic loci such as rs10419226 on chromosome 19, rs2687201 on chromosome 3, rs10852151 on chromosome 15 and rs520525 on the paired related homeobox 1 gene have been mentioned as potential risk factors. Further investigation on the risk genes may elucidate their exact function and role and demonstrate new therapeutic approaches to this increasingly common disease.

Abstract:INTRODUCTION: Gastroesophageal reflux disease (GERD) is one of the most common chronic gastrointestinal disorders in adults, it develops when the stomach contents reflux and rise up into the esophagus as a result from lower esophageal sphincter dysfunction. Stomach acid that touches the lining of the esophagus causes symptoms and complications. The classical symptoms of GERD include heartburn, usually after eating, chest pain, and regurgitation.AIM: To measure the prevalence of gastroesophageal reflux disease and determine its risk factors among the students of Shaqra University.METHODS: A cross-sectional study was conducted using a structured questionnaire distributed among Shaqra University students after multistage stratification and random sampling technique to stratify students according to gender and the three main colleges in Shaqra city, Saudi Arabia. The sample size was determined to be 435 with the precision of ± 5% and a 95% confidence interval (CI). The questionnaire included demographic data like age, gender, height and weight, lifestyle, and dietary habits. Statistical data were analyzed using the Statistical Package for Social Sciences (SPSS) version 16. Results with a P value of <0.05 were considered statistically significant.RESULTS: A total of 400 [227 (56%) male and 173 (43%) female] participants were evaluated. And 95 participants got a gastroesophageal reflux disease questionnaire score of ≥8 thus determining the prevalence of GERD to be 23.8%. Univariate analysis revealed that gender, smoking, familial history of GERD, high body mass index (>25 kg/m2), fast food, tea, carbonated beverages consumption, quick eating, and sleeping within 1 hour of dinner are associated with symptomatic GERD (P < 0.05).CONCLUSION: The results show a high prevalence of GERD in Shaqra university students and the presence of many modifiable risk factors which merits the conduction of public health campaigns to raise awareness about the disease and its risk factors.
A Mendelian Randomization Study to Assess the Genetic Liability of Gastroesophageal Reflux Disease for Cardiovascular Diseases and Risk Factors.

Abstract:Observational studies have reported that gastroesophageal reflux disease (GERD) is a risk factor for cardiovascular diseases (CVD); however, the causal inferences between them remain unknown. We conducted a Mendelian randomization (MR) study to estimate the causal associations between GERD and 10 CVD outcomes, as well as 14 cardiovascular risk factors. We used summary statistics from genome-wide association studies for GERD and the FinnGen consortium for CVD. We further investigated whether GERD correlated with cardiovascular risk factors and performed multivariable MR and mediation analyses to estimate the mediating effects of these risk factors on GERD-CVD progression. Sensitivity analyses and replication analyses were also performed. Our results indicated that GERD was positively associated with seven CVD outcomes with odds ratios of 1.26 [95% confidence interval (CI), 1.15, 1.37] for coronary artery disease, 1.41 (95% CI, 1.28, 1.57) for myocardial infarction, 1.34 (95% CI, 1.19, 1.51) for atrial fibrillation, 1.34 (95% CI, 1.21, 1.50) for heart failure, 1.30 (95% CI, 1.18, 1.43) for any stroke, 1.19 (95% CI, 1.06, 1.34) for ischemic stroke and 1.29 (95% CI, 1.16, 1.44) for venous thromboembolism. Furthermore, GERD was associated with nine cardiovascular risk factors and major depressive disorder demonstrated significant mediation effects on the causal pathway linking GERD and any stroke. This study demonstrates that GERD is associated with seven CVD outcomes and nine cardiovascular risk factors. Importantly, GERD treatment may help prevent common CVD events.

Abstract:Despite the high prevalence of typical symptoms of gastroesophageal reflux disease (GERD), approximately 30% of patients have functional esophageal disorders (FED) on ambulatory reflux monitoring, which may include reflux hypersensitivity (RH; defined as physiologic acid exposure but temporally correlated symptoms of reflux), or functional heartburn (FH; defined as physiologic acid exposure and negative symptom correlation). There are limited epidemiological data characterizing these conditions. We investigated demographic and socioeconomic factors as well as medical comorbidities which may predispose to FED versus pathologic GERD.

Abstract:Background: Gastroesophageal Reflux Disease (GERD) as troublesome symptoms, it results from the retrograde flow of gastric contents into the esophagus, oropharynx, and/or respiratory tract, it sufficient to impair an individual’s quality of life. Aim: Was to assess quality of life among patients with gastroesophageal reflux disease. Research design: Descriptive research design was utilized to conduct this study. Setting: The present study was conducted at Medicine Outpatient Clinic in Benha University Hospital. Sampling: Simple random sample was used for 313 patients at the previously mentioned setting. Tools of data collection: Three tools were used. I: A structured interviewing questionnaire which consisted of four parts to assess socio-demographic characteristics of patient, medical history, knowledge of patient regarding GERD and Patient reported practices regarding GERD. II: Frequency Scale for the Symptoms of GERD. III: Quality of life of patient with GERD. Results: 48.6% of studied patients had poor total knowledge level regarding gastroesophageal reflux disease, 64.9% of studied patients had unsatisfactory total reported practices level regarding esophageal reflux, 54.6% of studied patients had high recurrence of gastroesophageal reflux symptoms and 54.3% of studied patients had poor total QoL level regarding esophageal reflux. Conclusion: There was no statistically significant difference between total patients QoL and their socio-demographic characteristics except for their sex and marital status, there were highly statistically significant relation between total knowledge scores and total practices scores of studied patients. There was highly statistically significant relation between total practices scores and total quality of life scores among studied of patient. Recommendations: Develop and implement educational program for studied patients to improve their knowledge and practices toward coping with GERD.
Risk Factors for Refractory Gastroesophageal Reflux Disease

Abstract:Objective:This study aims to collect clinical informations,gastrocopy features and 24 h ambulatory esophageal impedance-pH index of patients with gastroesophageal reflux disease(GERD),and to compare the reflux conditions between refractory GERD and non-refractory GERD,and to provide evidence for the pre-diagnosis of refractory GERD.Methods:From October 2008 to December 2012,74 patients complaining of suspected reflux symptoms or extra-esophageal manifestations were treated in Gastroenterology Department of Zhongshan Hospital.The age,sex,height,weight and the result of endoscopy were recorded.All patients underwent 24 h ambulatory esophageal impedance-pH examination.The definition of refractory GERD:patients with GERD whose deMeester score 50%improvement in the chief complaint after giving once daily for at least 4weeks proton pump inhibitors(PPIs).Patients with GERD whose deMeester score 50%improvement in the chief complaint after giving PPIs once daily for at least 4 weeks were not refractory GERD.Patients with normal endoscopy results or 24 h ambulatory esophageal impedance-pH were not GERD.SPSS 17.0 was used to compare the basic conditions and reflux characteristics among GERD,non-GERD,and refractory GERD patients.Results:(1)The times of acid reflux and total reflux of refractory GERD and non-refractory GERD patients had no statistical difference.The deMeester score and long acid reflux times of refractory GERD patients were higher than those of non-refractory GERD patients(P = 0.032,P = 0.008);(2) Refractory GERD had positive correlation with total acid reflux longer than 5 min and NERD.When one acid reflux longer than 5 min happened,the risk of refractory GERD increased by 36%.The GERD risk of NERD patients was as 4.54 times as that of reflux esophagitis(RE) patients;(3)The reflux characteristics of different GERD:The total proximal reflux of NERD was more than RE and Barrett's esophagus,and the percentage of proximal reflux was more than RE and Barrett's esophagus.Conclusions:(l)Acid reflux time longer than 5 min and NERD were the independent risk factors for refractory GERD;(2)The increase of proximal reflux percentage of NERD may have relations with its response to PPI.
