Abstract:We report a 38-year-old man who presented with bilateral conjunctival congestion, hoarseness, and progressively growing pruritic, infiltrated skin lesions that had first begun over the face and neck, and later spread to the trunk and the limbs in 4 months. The clinical appearance of the lesions mimics granulomatous rosacea, acne vulgaris, or pityrosporum folliculitis. Histopathologic examination of the lesions from the face and chest both revealed dense dermal nodular lymphohistiocytic infiltrates which were positive for CD68 and S-100, but negative for CD1a. A systemic work-up for him detected no lymphadenopathy or other systemic involvement. A diagnosis of extranodal Rosai-Dorfman disease was made, and the patient received systemic glucocorticoids, with considerable improvement after 4 months of therapy.

Abstract:Acne fulminans (AF), a severe acne variant primarily evident in adolescent males, is characterized by the sudden onset of severe and often ulcerating acne with fever and polyarthritis. A case of a 14-year-old initially treated with clindamycin and surgical debridement, highlights the complexity of AF, including challenges in diagnosis, treatment, and the importance of early dermatological consultation. Successful management was achieved through systemic therapy with retinoids and corticosteroids, resulting in significant improvement. This case underscores the necessity of a coordinated effort among dermatologists, endocrinologists, and rheumatologists for effective AF treatment, illustrating the critical role of timely diagnosis and comprehensive care in managing this rare and challenging condition.

Abstract:Acne fulminans (AF) is a severe form of inflammatory acne commonly associated with adolescents. It is characterized by an abrupt onset of painful nodules and plaques and can progress to suppurative, ulcerative, and hemorrhagic lesions. AF can be associated with systemic symptoms such as fever, arthralgia, and bone pain. The etiology of AF is unknown but it has been linked to the use of certain medications and has been rarely found in autoinflammatory syndromes. In previous years, there have been reports of <200 cases in the literature; however, AF may be more common in clinical practice than reported. The most common presentation of AF is seen in adolescents starting isotretinoin therapy. Diagnosis of AF is determined based on its clinical findings. The main purpose of this article is to provide clinicians with a practical approach to treating AF. Current evidence for its treatment is limited to case reports and case series. The mainstay treatment of AF is a combination of prednisone and isotretinoin. It is important to taper or discontinue any exacerbating or precipitating medications such as isotretinoin, antibiotics, or androgens when AF is identified. Along with treatment of AF, it is important to treat associated scarring. Early identification and treatment of AF in adolescents is crucial to minimize both acute symptoms and long-term scarring, and further research is needed to determine optimal management.

Abstract:Background: Acne fulminans (AF) is a severe variant of inflammatory acne. It typically manifests as an explosive worsening and ulceration of skin lesions, and can be associated with systemic symptoms. However, there is a paucity of evidence-based information and no clear guidelines concerning the classification and treatment of AF. Objective: To better define the spectrum of AF and its variants, devise optimal therapeutic approaches, and identify areas of future research. Methods: A panel of physicians with expertise in severe acne vulgaris was convened after a comprehensive literature review of severe acne variants. Priority topics were reviewed and presented by each panelist at a 5-hour conference. Following review of the audiotape and scribed notes from the conference, surveys were utilized to address points of controversy and to clarify consensus recommendations. Results: Appropriate clinical case presentations and consensus survey questions were utilized to create final recommendations based on both the literature and the expert consensus. Limitations: Limited evidenced-based data and prospective studies in the literature concerning the treatment of AF is available. Conclusion: These guidelines better characterize AF and provide health care practitioners approaches to the classification, treatment, and prevention of AF and its variants.

Abstract:Abstract Acne vulgaris continues to result in a significant psychological and physical burden on patients, mostly occurring at an important time of psychosocial development. We would like to highlight the novel case of a 19-year-old male patient who attended the dermatology department for treatment of nodulocystic acne. On presentation, he had large cystic lesions affecting the forehead, cheeks and neck and abutting the left medial canthus. He had a background history of aplastic anaemia, treated with eltrombopag, antithymocyte globulin and ciclosporin. Unfortunately, the patient developed idiopathic intracranial hypertension (IIH) secondary to treatment with ciclosporin. Due to the severity of his IIH, treatment with isotretinoin was contraindicated. He was commenced on erythromycin 500 mg twice daily along with adapalene gel topically but had a suboptimal therapeutic response. During his care, he was admitted under the haematology team with diffuse abdominal pain and a raised lactate dehydrogenase. He was subsequently diagnosed with paroxysmal nocturnal haemoglobinuria (PNH) and was commenced on biweekly eculizumab infusions. Within 1 month, he noticed a significant improvement in his acne. He was reviewed 3 months later, and his acne was clear, with only a few scattered pustules. The patient continues to report that scattered papules and pustules appear 1–2 days before his biweekly infusions; however, these quickly resolve after infusion. Eculizumab was first approved for treating PNH in 2007 and is a recombinant humanized monoclonal antibody inhibiting complement component C5. In vitro studies investigating the role of the complement pathway were carried out in the 1970s, at which time it was thought that colonizing bacteria can activate the complement pathway through stimulation of the reticuloendothelial system. Both acne and hidradenitis suppurativa (HS) are disorders of follicular occlusion; 23–70% of patients with HS report concomitant acne. Phase IIa/IIb clinical trials of C5a inhibition have shown promise in the treatment of HS. Although the pathogeneses of both acne and HS remain distinct, there have been studies showing positive response to C5 inhibition in patients with acne, supporting the theory that perhaps complement does pay a role in acne development. The authors present this case to highlight the complement pathway as a possible alternative therapeutic target in severe nodulocystic acne.

Diagnosis and Treatment of Rosacea Fulminans: A Comprehensive Review

Abstract:Rosacea fulminans is a rare inflammatory condition of the central face marked by the abrupt onset of erythematous coalescing papules, pustules, nodules, and draining sinuses. Due to infrequent reporting in the literature, the pathophysiology, classification, and nomenclature of this condition remain controversial. This comprehensive review evaluated a total of 135 cases of rosacea fulminans for clinical and histopathologic features and reported treatment strategies. Patients were 91% female with an average age of onset of 31.3 years. Only 19% of cases reported duration of symptoms longer than 3 months, and reports of recurrence were uncommon. A majority of patients had history of rosacea or flushing, and common triggers included hormonal shifts, emotional stress, and medications. Extrafacial or systemic involvement was rare. Though oral and topical antibiotics were frequently utilized to treat rosacea fulminans, there was a clear shift in reported treatments for rosacea fulminans following the introduction of isotretinoin use in 1987, marked by increased reliance on isotretinoin in addition to topical and systemic corticosteroids. Newer treatments were associated with superior improvement compared with antibiotic monotherapy, most notably dramatically reduced rates of scarring, though reduced rates of disease recurrence were not evident. Several patterns revealed through this review reinforce the classification of rosacea fulminans as a severe yet distinct variant of rosacea and highlight key distinguishing clinical features and treatment options for optimal management.
