Abstract:Introduction Isolated, confined, and extreme (ICE) environments such as found at Antarctic, Arctic, and other remote research stations are considered space-analogs to study the long duration isolation aspects of operational space mission conditions. Methods We interviewed 24 sojourners that participated in different short/long duration missions in an Antarctic (Concordia, Halley VI, Rothera, Neumayer II) or non-Antarctic (e.g., MDRS, HI-SEAS) station or in polar treks, offering a unique insight based on first-hand information on the nature of demands by ICE-personnel at multiple levels of functioning. We conducted a qualitative thematic analysis to explore how sojourners were trained, prepared, how they experienced the ICE-impact in function of varieties in environment, provided trainings, station-culture, and type of mission. Results The ICE-environment shapes the impact of organizational, interpersonal, and individual working- and living systems, thus influencing the ICE-sojourners' functioning. Moreover, more specific training for operating in these settings would be beneficial. The identified pillars such as sensory deprivation, sleep, fatigue, group dynamics, displacement of negative emotions, gender-issues along with coping strategies such as positivity, salutogenic effects, job dedication and collectivistic thinking confirm previous literature. However, in this work, we applied a systemic perspective, assembling the multiple levels of functioning in ICE-environments. Discussion A systemic approach could serve as a guide to develop future preparatory ICE-training programs, including all the involved parties of the crew system (e.g., family, on-ground crew) with attention for the impact of organization- and station-related subcultures and the risk of unawareness about the impact of poor sleep, fatigue, and isolation on operational safety that may occur on location.

Abstract:This study reports findings from 21 in-depth interviews with oncology clinicians (social workers, physicians, and nurses). Interviews addressed clinicians' overall experiences of their work, including professional roles, teamwork, challenges and rewards of the work, and thoughts about whether oncology work changed their world-view. Researchers used an expedition metaphor to understand oncology clinicians' experiences. An expedition and oncology work have in common a clear division of labor, the necessity of collaboration, intense work, significant obstacles, great rewards, and work that the world at large cannot tolerate undertaking. Understanding the experiences of oncology health professionals is essential in attracting and retaining dedicated clinicians and in helping them mitigate the potential deleterious effects of their work, such as compassion fatigue. Additionally, the expedition metaphor itself is compelling; it highlights the breadth of experiences of oncology clinicians, particularly that successful multidisciplinary teamwork in itself can ameliorate compassion fatigue.

Abstract:Humans have developed strategies of varying complexity to augment their probability of survival and success in complex environments. In contemporary outdoor adventure scenarios, advances in knowledge, training and technology have been adopted to make us safer, or make us feel safer. Avalanche-prone terrain is one such complex environment where uncertainty is key. A small-sample pilot study was conducted during a 3-year trial of mandatory avalanche transceiver, shovel and probe use at Glenmore Lodge, sportscotland National Outdoor Training Centre. The perceived effects that the equipment might have on participants' risk perceptions and risk-taking behaviour were investigated through semi-structured interviews. Patterns of avalanche risk perception and relationship with the safety equipment varied strongly between expertise levels, and a discrepancy between declared risk behaviour and action, demonstrations of optimistic bias and defensive denial were evident. Wider implications are suggested regarding subjective relationships with both risk and technology in adventure settings and, in our leisure more generally.

Abstract:Historically, natural and manmade disasters create many victims and impose pressures on health-care infrastructure and staff; potentially hampering the provision of patient care and overloading clinician capacity. Throughout the course of history, clinicians have performed heroics to work well above their required duty, despite limitations, even putting their own health and safety at risk. In times when clinicians needed to either physically abandon patients or consider abandoning active treatment, we have seen extreme hesitancy to do so, fearing that they may be giving up too soon, that undue harm may come to patients, or even feeling unsure of legal or moral burdens that may ensue. In times when clinicians are placed in this unimaginable position, feeling isolated and overwhelmed, it is essential that they be supported and provided with resources to standardize decision-making.

Abstract:As a newly appointed junior staff nurse in a city Accident & Emergency (A & E) department, the author took part in an orientation programme which included joining a paramedic crew for a 12-hour shift as a nurse observer. During this shift the crew and observer were involved in a major incident, when a coach carrying a party of pensioners overturned into a water-filled culvert, with a loss of 13 lives. This paper offers a personal reflection on the holistic application of multidisciplinary teamwork, so often rehearsed but, thankfully, rarely required. Over 1 year later these reflections may serve as a respectful reminder of the decreased, and through that, an appreciation of the arduous tasks faced by emergency workers both front-line and within the A & E department. Also discussed is the need for access and the opportunity for counselling if required, thereby possibly averting the development of Post Traumatic Stress Disorder, or at least recognising its early stages and beginning to work with it.

Abstract:On 19 June 1989, the cruise liner Maksim Gorkiy hit an ice floe southwest of Svalbard. The passengers and parts of her crew abandoned the ship. In a massive rescue operation 953 crew and passengers were rescued, in addition to the ship. This is only one of several examples of serious ship accidents in polar waters.The aim of the article is to study coping strategies for resilience among a group of crew members and passengers taking part in the SARex2 exercise, coping strategies that may contribute to a group's survival following a ship accident and the subsequent evacuation of the ship.The empirical findings stem from participant observation during SARex2 in Svalbard in 2017, a review of the requirements specified in the International Code for Ships Operating in Polar Waters (IMO, 2014) and a literature review on the special challenges of operating in these waters. The theoretical lenses are teamwork, sense making and resilience.The article starts with a presentation of the challenges of operating in polar waters, as well as some relevant parts of the Polar Code. Then the conceptual framework of the study is presented, followed by information on the SARex2 exercise and methodology. Then follows a discussion of coping strategies for resilience following a ship accident in these waters. Finally, some concluding remarks are presented, on how to transfer a group of crew members and passengers into a team required for survival after a ship accident and the subsequent evacuation of the ship in polar waters.

Abstract:Efforts to increase inclusion in the geosciences have progressed much in the past few decades. As inclusion grows our procedures and methods for accommodating individuals' needs must adapt. Responsible leaders must grow to meet challenges in a way that mirrors modern approaches to dealing with hardships individuals face. Generalized Anxiety Disorder (GAD) and related conditions are a growing problem among students. Often compounded by the trying nature of field work, individuals with anxiety disorders encounter challenges that can leave the person debilitated and unresponsive. Simple measures can provide support allowing more individuals to maximize their field experiences. While leaders always carry first aid kits and a basic knowledge of treatment of minor wounds, many people are unprepared for aiding a colleague experiencing anxiety-related symptoms such as Acute Panic Attacks (APAs) or induced mutism. This paper describes simple methods for identifying and managing acute symptoms as well as determining when professional medical intervention is necessary. Our aim is to provide Earth scientists of all education levels a basic overview of the acute symptoms of APAs and provide a guide for preventing and aiding affected colleagues in a field setting.

Abstract:Counter-terrorism (CT) operations may involve intelligence-gathering, reconnaissance, surveillance, or direct action (e.g. arrest) to disrupt terrorist activities, and can take place in domestic settings and on international deployments. They involve defence, security, and law enforcement personnel, who often enter risky and dangerous settings, exposing themselves to diverse physical, psychological, and social stressors. The extent to which a person adjusts and adapts to these stressors has implications for safety, performance, health, and well-being. Despite their importance, the psychological aspects of CT operations have received scant research attention. This paper explores what we might learn from the research on performance and health in other extreme environments to support the work of CT professionals operating in hazardous settings. Guided by a model of stress and coping in extreme environments, we review potential physical, psychological, and social stressors and discuss mitigating factors and coping strategies. A systematic, evidence-based understanding of stress and coping in dangerous settings will be of value to those involved in planning CT operations, enhancing understanding of the impact that stressors and constraints could have on the safety, performance, and health of operational personnel, and providing pointers to effective mitigation.

Abstract:Psychological resilience is the ability to use personal qualities to withstand pressure, consisting of the interaction between the individual and the environment over time. It is essential when operating in extreme environments which are typically characterised by a complex combination of stressors with increased elements of risk and adversity. Psychological resilience has never been investigated "live" (e.g., in the moment) throughout the duration of an extreme endurance challenge, despite anecdotal accounts of the need for resilience to successfully function in such environments. The aim of the study was to explore psychological resilience with challenge team members (n = 4, mean age = 46.0 years) involved in a 25-day extreme endurance challenge. The object of the challenge was to 'TAB' (Tactical Advance to Battle, fast marching with weighted packs) 100 peaks in the UK in 25 days and complete long-distance bike rides between base camps. A mixed-methods approach with a focus on qualitative methods was utilised. Specifically, individual reflective video diaries (n = 47) and focus groups (n = 4) were completed and analysed using interpretative phenomenological analysis (IPA). At the same time, the 10-item Connor Davidson Resilience Scale was employed to measure resilience, which highlighted the individualised and dynamic nature of resilience. Two superordinate themes were identified from the video diaries and focus groups, namely, the identification of the stressors within extreme environments and strategies to maintain functioning. Stressors were split into subordinate themes of significant and every day, and collectively, they created a cluster effect which contributed to pressure associated with operating in these environments. Challenge team members employed various strategies to maintain functioning, including using a challenge mindset to positively appraise pressure as a challenging learning experience. Further research should continue to develop an understanding of how participants completing challenges within extreme environments utilise and develop personal qualities to maintain functioning.

Abstract:Emergency services personnel are a high stress occupation, being frequently confronted with highly consequential stressors and expected to perform: without fault; under high pressure; and in unpredictable circumstances. Research often invokes similarities between the experiences of emergency services personnel and elite athletes, opening up the possibility of transferring learnings between these contexts. Both roles involve genuine risks to emotional wellbeing because their occupations involve significant stress. Similarly, both roles face obstacles and injury, and their "success" is dependent on high-quality execution of their skills under pressure. As such, both occupations are required to have resilience and effective coping abilities to ensure psychological well-being. Researchers suggest emotional awareness may be a key variable in the management and maintenance of resilience. This study: (1) explored the experiences of emergency services personnel; (2) characterised connections between emotional awareness and resilience; and (3) reflected on the ways these findings can be extrapolated to elite athletes. We analysed 11 interviews with emergency services personnel. Participants identified resilience as crucial when coping with stress, however, many defined resilience as remaining unaffected by stress rather than, for example, managing and responding to it. Participants defined emotional awareness as understanding their emotions, and they recognised associated benefits for coping, resilience, and burnout. Nevertheless, most participants did not engage in practices to improve their emotional awareness. Barriers, such as maladaptive beliefs and help-seeking stigma, interfered with participants' ability to cultivate emotional awareness, to promote resilience. In contrast, some participants described profound improvements in resilience and coping following the cultivation of emotional awareness. This finding illustrates that systemic change must target the individual, team, and organisation to correct misperceptions about resilience, emotional awareness, and psychological help-seeking. Developing emotional awareness may help emergency services personnel and other high stress occupations like elite athletes process difficult experiences and enhance their resilience, promoting well-being, and career longevity.

ObjectiveThis paper surveys the existing literature surrounding problem-solving and team dynamics in complex and unpredictable scenarios, and evaluates the applicability of studying Earth-based construction teams to identify training needs for Lunar construction crews.BackgroundLunar and other space exploration construction crews will work in extreme environments and face unpredictable challenges, necessitating real-time problem-solving to address unexpected contingencies. This work will require coordination with Mission Control and autonomous assistants, so crew training must account for multi-agent, distributed teamwork.MethodA narrative literature review identified processes, attributes, and skills necessary for the success of Lunar construction teams. We summarized relevant frameworks and synthesized collective findings into over-arching trends and remaining research gaps.ResultsWhile significant literature exists surrounding team performance, very little systematic inquiry has been done with a focus on Lunar construction crews and operations, particularly with respect to dynamic problem-solving and team-based decision-making. Established and standardized metrics for evaluating team performance are lacking, resulting in significant variation in reported outcomes between studies.ConclusionLunar and other space exploration construction teams will need training that focuses on developing the right approach to team-based problem-solving, rather than on preparing response execution for known contingencies. An investigation of successful Earth-based construction crews may facilitate the development of relevant metrics for training future Lunar construction crews.ApplicationMetrics and team training protocols developed for future Lunar construction teams may be adaptable and applicable to a wide range of extreme teams facing uncertain challenges, such as aircrews, surgical teams, first responders, and construction crews.

Safety in Extreme Environments

Abstract:Abstract Coordinating complex crisis responses generally requires comprehensive skills. When operating in the High Arctic, extreme environments make this task even more challenging. Training for these situations is thus of utmost importance, but such training requires extensive resources for full-scale exercises. Simulation is an alternative that can yield an effective learning process and learning outcomes. In this study, we explored how simulator-based accident training in high-risk, highly sensitive environments can be optimised. To do this, Exercise Isfjord was designed for students in the master’s Preparedness and Emergency Management programme at NORDLAB at Nord University. This exercise involved a grounded cruise ship from which passengers needed to be evacuated, in addition to a severe oil spill. Interviews with students, mentors, facilitators, and academic staff were conducted, and thematic analysis was used for analysing the data. Our findings indicate that a pedagogical wheel consisting of the six factors preparations, psychological safety, mentors, learning objectives, context realism, and evaluations promotes an optimal learning process in simulated-accident exercises in high-risk and highly sensitive environments. Thus, we recommend tailor-made simulated exercises for crisis management and emergency preparedness.

Abstract:Disruptive domestic and global events can directly affect health care workers' ability to deliver safe, quality care. Health care workers should have an awareness and understanding of the disaster continuum and how their actions throughout each phase can affect the crisis response. Robust, intentional education and training for all team members is essential to uphold safe clinical environments and delivery of high‐quality care during crises. This article explores key elements of providing education during a crisis response, including launching a command center, forming a structured team with strong leadership, establishing organized communication channels, conducting educational needs assessments, and developing and implementing education plans to assist with mitigating any issues related to knowledge deficits. In addition, recommendations for evaluating the effectiveness of the education at producing the desired learning outcomes are provided, as well as the implications of such a program for perioperative educators, frontline staff members, and leaders.

Abstract:Nature based tourism (NBT) is becoming increasingly popular, particularly following the COVID-19 pandemic as people began to sought outdoor activities. Accompanying the projected rise in NBT demand in a post COVID-19 era are increasing challenges associated with climate change, particularly in mountain regions. However, there is limited local knowledge documented to date from those who are intricately involved in mountain NBT activities and have experienced the impacts of climate change first hand. Using an online survey ( n = 169), this research is the first to present the intimate knowledge of mountain guides in Canada, offering novel insight into climate change risks and opportunities for NBT in mountain regions, including strategies to contend with risk and adaptation. From this survey, 99% of guides indicated that they have experienced change in the mountain environment throughout the course of their career and due to the adaptive nature of guides, many have already implemented strategies to adapt to the impacts of climate change. While findings presented in this paper offer practical knowledge to plan for a future threatened with rapid climatic change, further research is required to explore effectiveness of adaptation strategies, scope of adaptive capacity, changes in natural infrastructure, and guides' roles as educators.

A New Pedagogy to Enhance the Safety and Resilience of Journalists in Dangerous Environments Globally

Abstract:Risks to journalists are rising with disasters, epidemics, physical, mental and digital harassment all increasing globally. Some 1382 journalists have been killed since 1992 and 246 are imprisoned. However, the threat type has been changing, with the majority of journalists killed annually being ones working in their own country, often who are targeted for assassination. In response, UNESCO and others have called for research into best practice for safety education to halt this and the consequential decline in global media freedom. This five-year award winning project, A Holistic Humanitarian Approach to Enhance the Safety and Resilience of Journalists Globally, tested the hypothesis that a new pedagogy based on a ‘holistic humanitarian’ philosophy would be more effective in protecting journalists working in dangerous domains globally than existing provisions. The little-changed 30-year-old dominant international provision, the ‘military battlefield’ pedagogy, is used by the world’s major news organizations like BBC, CNN and the New York Times. This new pedagogy adapted and customized best practice from other professions and used Taylor’s 2020 Competencies for Disaster Healthcare professionals. A new program was devised and the two international cohorts who took it in 2018 and 2019 judged that it ‘very significantly’ enhanced their resilience and safety skills. Its concentration on group and individual physical and mental resilience building, risk mitigation, psychology, communication, self-defence, and digital security skill acquisition was a paradigm shift in training internationally for news professionals in dangerous environments. The research, thus, proved the study’s hypotheses.
