The Relationships Between Healthcare Utilization, Health Outcomes, and Perception of Health in People with Schizophrenia

Abstract:Introduction Schizophrenia is a highly disabling serious mental illness that affects 1% of the general US population and is associated with an increased prevalence of comorbidities, resulting in inflated healthcare costs. Previous research has shown that while PwS have lower socioeconomic status and higher rates of unemployment, a majority have government health insurance. Despite high rates of insurance, health outcomes among PwS vary greatly. In contrast to health insurance, healthcare utilization may be a better target for improving health outcomes for PwS, but its relation to sociodemographic and health factors is not well known. Understanding these relationships will aid in identifying PwS most likely in need for increased healthcare services. The present study examines the relationships between healthcare utilization and various demographic factors, psychological qualities that affect health, and health outcomes.We hypothesized that greater healthcare utilization will be associated with: 1) older age, lower levels of education, and lower household income; 2) lower self-compassion and higher perceived stress; and 3) worse physical, mental, and cognitive health. Methods The sample included 259 people with schizophrenia (age 26-66 years) from San Diego. Assessments included self-compassion (Neff Self-Compassion Scale), perceived stress (Perceived Stress Scale), well being (36-Item Short Form Survey), depression (Calgary Depression Rating Scale), positive and negative symptoms (Scale for the Assessment of Positive/Negative Symptoms), physical comorbidities (Cumulative Illness Rating Scale), global cognition (Telephone Interview for Cognitive Status-modified), and executive functioning (Delis-Kaplan Executive Function System). Additionally, demographic information and clinical data (e.g., antipsychotic and non-psychotropic medications, and body mass index) were collected.Healthcare utilization was the self-reported number of doctor visits. Participants were grouped into subgroups of seeing a doctor 0-4 or 5+ times in the past year.We compared demographic factors, psychological factors, and health outcomes between the utilization subgroups using chi-squared and independent sample t-tests. Results The sample included 36.3% of PwS who saw a doctor 5+ times in the past year. Of the whole sample, 50% were female and 37.2% had a household income of less than $75,000/year, a mean age of 48.45, and an average of 13.63 years of education.Those with lower healthcare utilization were significantly more likely to have lower years of education (F=15.2, p<.001) and household income (χ²=5.38, p=0.02), but did not differ in sex and age.PwS who saw doctors 5+ times/year had lower self-compassion (F=9.51, p<.001) and higher perceived stress (F=1.15, p<.001).PwS who saw doctors 5+ times/year had better self-reported mental well-being (F=3.35, p<.001), more depressive symptoms (F=1.95, p=0.046), worse positive symptoms (F=0.055, p<.001), and worse worse negative symptoms (F=1.09, p<.001). Taking higher doses of antipsychotic medications (F=3.61, p<.001) and taking more non-psychotropic (F=16.7, p<.001) medications were also associated with higher healthcare utilization. Additionally, PwS who saw the doctor 5+/year had better self-reported physical well-being (F=8.892, p<.001), more physical comorbidities (F=2.18, p<.001), and higher BMI (F=3.107, p<.001).PwS who saw the doctor more frequently also had lower scores in global cognition (F=0.001, p<.001) and executive functioning (F=3.644, p<.001). Conclusions Our findings indicate that healthcare utilization in PwS is associated with lower socioeconomic factors and worse objective mental, physical, and cognitive outcomes; but with better self-perceived health. These findings are generally consistent with our hypotheses. Surprisingly, we did not find relationship between resource utilization and age, which may reflect survivor effects where healthier PwS are more likely to survive to middle age. Furthermore, higher healthcare utilization was associated with worse objective health markers, but better subjective health, which may reflect the heterogeneity of healthcare usage. The current analyses do not account for the type of doctor's visits (emergency versus regular outpatient care) which may relate differently with health outcomes. Future directions include longitudinal investigations to understand how health outcomes may be affected by trends in healthcare utilization and more nuanced assessment of healthcare utilization. Understanding how resource utilization is related to demographic and health outcomes in PwS may reveal future targets of intervention for preventive medicine.
