Blastocyst euploidy rates in low-prognosis patients according to the POSEIDON criteria: a retrospective analysis of 3016 embryos

Abstract:Research QuestionDo euploidy rates of embryos differs in four groups of women with low prognosis stratified by POSEIDON criteria?DesignThis was a retrospective cohort study of low-prognosis patients who met the POSEIDON criteria and underwent genetic testing for aneuploidy (PGT-A) from January 2013 to June 2020 in Center for Reproductive Medicine, Shandong University. 3, 016 blastocysts from 1, 269 PGT-A cycles were enrolled in the study. The primary outcome was the euploidy rate of the blastocysts. Within each group, regression analyses were performed to quantitatively describe the relationship between maternal age and embryo euploidy rate.ResultsThe euploidy rate of embryos in POR patients was 39.1% totally. The numbers of blastocysts were 727, 1052, 275 and 962 in groups 1, 2, 3, and 4, respectively, and the euploidy rates of embryos were 57.2%, 34.9%, 52.4% and 26.2% accordingly (p<0.001). Within each group, the euploidy rate decreased with age, especially in women aged ≥35 years old (i.e. groups 2 and 4).Conclusion(s)The euploidy rates favored in group 1 and 3 with young age, which re-emphasized that oocyte quality was the primary determine factor for embryo euploidy rate. Our findings demonstrated the reasonability of the categorization of patients with POR by POSEIDON criteria depending on the female ages and ovarian reserve biomarkers. These results also provide clues for POR patients in different subgroups with proper counselling on the possible prognosis.
