Abstract:Asthma is a common respiratory disease in modern society. However, people are rarely aware of the symptoms of asthma because the early stage of asthma is similar to that of the common cold (e.g., wheeze, cough, and shortness of breath). To tackle this challenge, we propose an asthma detection system, Ubi-Asthma, based on the smartwatch. Ubi-Asthma combines breathing signals and guttural sound (e.g., cough sound and throat-clearing sound) signals to realize passive and accurate asthma detection without interrupting the user. Not only can Ubi-Asthma extract breathing signals from the user even if the user is walking but also recognize guttural sound signals when the user is engaged in voice communication without being disturbed by noise. The features of breathing and guttural sound are combined to improve the accuracy of asthma detection. We have implemented a fully functional prototype using an off-the-shelf smartwatch. Fifty volunteers participate in an extensive experiment of up to 150 h to train Ubi-Asthma. As a result, Ubi-Asthma can reach a detection accuracy of 98.4%, which is higher than breath- or guttural-based asthma detection systems. Ubi-Asthma is expected to provide a potential solution for smart-home applications in the future.

Abstract:This paper presents a cloud-connected indoor air quality sensor system that can be deployed to patients' homes to study personal microenvironmental exposure for asthma research and management. The system consists of multiple compact sensor units that can measure residential NO2, ozone, humidity, and temperature at one minute resolution and a cloud based informatic system that acquires, stores, and visualizes the microenvironmental data in real time. The sensor hardware can measure NO2 as low as 10 ppb and ozone at 15 ppb. The cloud informatic system is implemented using open-source software on Amazon Web Service for easy deployment and scalability. This system was successfully deployed to pediatric asthma patients' homes in a pilot study. In this study, we discovered that some families can have short term NO2 exposure higher than EPA's one hour exposure limit (100 ppb), and NO2 micropollution episodes often arise from natural gas appliance usage such as gas stove burning during cooking. By combining the personalized air pollutant exposure measurements with the physiological responses from a patient diary and medical record, this system can enable novel asthma research and personalized asthma management.

Abstract:The electronic nose (e-Nose) has more and more promising applications in home healthcare. In this paper, we introduce a portable e-Nose intended to satisfy the home healthcare applications. It employs semiconductor and electrochemical sensors to compose an array in a chamber to conduct breath analysis with pre-concentration and multiple desorption procedures. ON e-Nose II supplies a smart and interactive graphical user interface by a colourful TFT LCD and USB host interface. A package of supporting software on personal computer (PC) is also developed for data acquisition and pattern recognition. The results indicate that using multiple desorption methods, our e-Nose system may distinguish lung benign diseases, like pneumonia and pneumoconiosis, especially, healthy controls and lung cancer patients were separated with high accuracy through the detection of volatile organic compounds (VOCs) in breath. Our e-Nose instrument demonstrates excellent potential for assist in quick monitoring of respiratory diseases for family members at home in the near future.

Abstract:Global industrialization and urbanization have led to increased levels of air pollution. Those with respiratory diseases, such as asthma, are at the highest risk for adverse health effects and reduced quality of life. Studying the relationship between pollutants and symptoms is usually achieved with data from government air quality monitoring stations, but these fail to report the spatial and temporal resolution required to track a person's true exposure, especially when the majority of their time is spent indoors. We develop and build eight wrist-worn wearable devices, weighing only 64 g, to measure known asthma symptom triggers: ozone, total volatile organic compounds, temperature, humidity, and activity level. The devices use commercial off-the-shelf components, costing under $150 each to build. This report focuses on the design, calibration, and testing of the devices. Emphasis is placed on the calibration of a metal-oxide-semiconductor gas sensor for detecting ozone, which is a difficult task because of the large variations in ambient temperature and humidity found when using a wearable device. Examples of testing the devices in four real environments are also discussed: 11 days inside a well-ventilated laboratory, ten days outdoors during the summer, alternating the devices between indoor and outdoor environments to examine their response to quickly changing environments, and a field test where scripted activities are performed for a full day. The work demonstrates a wearable device for environmental health studies and addresses the challenges of existing sensors for real-world applications.

Abstract:Non-invasive continuous alcohol monitoring has potential applications in both population research and in clinical management of acute alcohol intoxication or chronic alcoholism. Current wearable monitors based on transdermal alcohol content (TAC) sensing are relatively bulky and have limited quantification accuracy. Here we describe the development of a discreet wearable transdermal alcohol (TAC) sensor in the form of a wristband or armband. This novel sensor can detect vapor-phase alcohol in perspiration from 0.09 ppm (equivalent to 0.09 mg/dL sweat alcohol concentration at 25 °C under Henry's Law equilibrium) to over 500 ppm at one-minute time resolution. The TAC sensor is powered by a 110 mAh lithium battery that lasts for over 7 days. In addition, the sensor can function as a medical "internet-of-things" (IoT) device by connecting to an Android smartphone gateway via Bluetooth Low Energy (BLE) and upload data to a cloud informatics system. Such wearable IoT sensors may enable large-scale alcohol-related research and personalized management. We also present evidence suggesting a hypothesis that perspiration rate is the dominant factor leading to TAC measurement variabilities, which may inform more reproducible and accurate TAC sensor designs in the future.

Abstract:A novel portable wireless volatile organic compound (VOC) monitoring device with disposable sensors is presented. The device is miniaturized, light, easy-to-use, and cost-effective. Different field tests have been carried out to identify the operational, analytical, and functional performance of the device and its sensors. The device was compared to a commercial photo-ionization detector, gas chromatography-mass spectrometry, and carbon monoxide detector. In addition, environmental operational conditions, such as barometric change, temperature change and wind conditions were also tested to evaluate the device performance. The multiple comparisons and tests indicate that the proposed VOC device is adequate to characterize personal exposure in many real-world scenarios and is applicable for personal daily use.

Abstract:This paper presents the research and development of an Internet of Things- (IoT-) based remote health monitoring system for asthmatic patients. Asthma is an inflammatory disease. Asthma causes the lungs to swell and get narrower, making it difficult to carry air in and out of the lungs. This situation makes breathing very difficult. Remote patient monitoring (RPM) is a method of collecting health-related data from patients who are in a remote location and electronically transmitting it to healthcare providers for evaluation and consultation. The aim of this study is to design a monitoring system that allows doctors to monitor asthmatic patients from a remote area. The proposed system will allow patients to measure oxygen saturation (SpO2), heart rate, body temperature, humidity, volatile gases, room temperature, and electrocardiogram (ECG) using various sensors, which will be displayed in an application. This data is then sent to the doctor to monitor the patient’s condition and suggest appropriate actions. Overall, the system consists of an Android application, a website, and various sensors. The Android studio and Java programming language were used to develop the application. For the frontend, the website was built using Hypertext Markup Language (HTML), Cascading Style Sheets (CSS), JavaScript, and jQuery. The system also uses Django, a Python-based open-source web framework, for the backend. The system developed the various sensors using an ESP8266 microcontroller compatible with the Arduino Integrated Development Environment (IDE). The system uses a MAX30100 pulse oximeter and heart rate sensor, a GY-906 MLX90614 noncontact precision thermometer, a DHT11 humidity and temperature sensor, a MQ-135 gas and air quality sensor, and an AD8232 ECG sensor for collecting various parameters that may trigger asthma attacks. Finally, the system developed the Asthma Tracker app and the Asthma Tracker website for remote health monitoring. The system was initially tested on demo patients and later deployed and tested on seven real human test subjects. Overall, the monitoring system produced satisfactory results. The data acquired by the sensors has a high level of accuracy. The system also maintained user-friendliness and low cost.

Abstract:Blood sampling, despite its historical significance in clinical diagnostics, poses challenges such as invasiveness, infection risks, and limited temporal fidelity for continuous monitoring. In contrast, exhaled breath offers a non-invasive, pain-free, and continuous sampling method, carrying biochemical information through volatile compounds like ammonia (NH ). NH in exhaled breath, influenced by kidney function, emerges as a promising biomarker for renal health assessment, particularly in resource-limited settings lacking extensive healthcare infrastructure. Current analytical methods for breath ammonia, though effective, often face practical limitations. In this work, we introduce a low-cost, internet-connected, paper-based wearable device for measuring exhaled ammonia, designed for early detection of kidney dysfunction at the point-of-need. The device, which attaches to disposable facemasks, utilizes a disposable paper-based sensor array housed in a biodegradable plastic enclosure to mitigate high relative humidity (RH) issues in breath analysis. We validated our technology using a laboratory setup and human subjects who consumed ammonium chloride-containing candy to simulate elevated breath ammonia. Our wearable sensor offers a promising solution for rapid, point-of-need kidney dysfunction screening, particularly valuable in resource-limited settings. This approach has potential applications beyond kidney health monitoring, including chemical industry safety and environmental sensing, paving the way for accessible, continuous health monitoring.

Abstract:Wearable sweat sensors are reshaping healthcare monitoring, providing real-time data on hydration and electrolyte levels with user-friendly, noninvasive devices. This paper introduces a highly portable two-channel microfluidic device for simultaneous sweat sampling and the real-time detection of volatile organic compound (VOC) biomarkers. This innovative wearable microfluidic system is tailored for monitoring diabetes through the continuous and noninvasive tracking of acetone and ammonia VOCs, and it seamlessly integrates with smartphones for easy data management. The core of this system lies in the utilization of carbon polymer dots (CPDs) and carbon dots (CDs) derived from monomers such as catechol, resorcinol, o-phenylenediamine, urea, and citric acid. These dots are seamlessly integrated into hydrogels made from gelatin and poly(vinyl alcohol), resulting in an advanced solid-state fluorometric sensor coating on a cellulose paper substrate. These sensors exhibit exceptional performance, offering linear detection ranges of 0.05-0.15 ppm for acetone and 0.25-0.37 ppm for ammonia, with notably low detection limits of 0.01 and 0.08 ppm, respectively. Rigorous optimization of operational parameters, encompassing the temperature, sample volume, and assay time, has been undertaken to maximize device performance. Furthermore, these sensors demonstrate impressive selectivity, effectively discerning between biologically similar substances and other potential compounds commonly present in sweat. As this field matures, the prospect of cost-effective, continuous, personalized health monitoring through wearable VOC sensors holds significant potential for overcoming barriers to comprehensive medical care in underserved regions. This highlights the transformative capacity of wearable VOC sweat sensing in ensuring equitable access to advanced healthcare diagnostics, particularly in remote or geographically isolated areas.

Abstract:Monitoring respiration is vital for personal diagnosis of chronic diseases. However, the existing respiratory sensors have severe lim-itations, such as single function, finite detection parameters, and lack of smart signal analysis. Here, we present an integrated wearable and low-cost smart respiratory monitoring sensor (RMS) system with artificial intelligence (AI)-assisted diagnosis of respiratory abnormality by detecting multi-parameters of human respiration. Coupling with intelligent analysis and data mining algorithms embedded in a phone app, the lighter system of 7.3 g can acquire real-time self-calibrated parameters, including breathing frequency, apnea hypopnea index (AHI), vital capacity (VC), peak expiratory flow (PEF), and other respiratory indexes with an accuracy >95.21%. The data can be wirelessly transferred to the user's data cloud terminal. The RMS system enables comprehensive multi -phys-iological parameters analysis for auxiliary diagnosing and classifying diseases, including sleep apnea, rhinitis, and chronic lung diseases, as well as rehabilitation of COVID-19, and exhibits advantages of portable healthcare.
Continuous Critical Respiratory Parameter Measurements Using a Single Low-Cost Relative Humidity Sensor: Evaluation Study

Abstract:Background: Accurate and portable respiratory parameter measurements are critical for properly managing chronic obstructive pulmonary diseases (COPDs) such as asthma or sleep apnea, as well as controlling ventilation for patients in intensive care units, during surgical procedures, or when using a positive airway pressure device for sleep apnea. Objective: The purpose of this research is to develop a new nonprescription portable measurement device that utilizes relative humidity sensors (RHS) to accurately measure key respiratory parameters at a cost that is approximately 10 times less than the industry standard. Methods: We present the development, implementation, and assessment of a wearable respiratory measurement device using the commercial Bosch BME280 RHS. In the initial stage, the RHS was connected to the pneumotach (PNT) gold standard device via its external connector to gather breathing metrics. Data collection was facilitated using the Arduino platform with a Bluetooth Low Energy connection, and all measurements were taken in real time without any additional data processing. The device's efficacy was tested with 7 participants (5 men and 2 women), all in good health. In the subsequent phase, we specifically focused on comparing breathing cycle and respiratory rate measurements and determining the tidal volume by calculating the region between inhalation and exhalation peaks. Each participant's data were recorded over a span of 15 minutes. After the experiment, detailed statistical analysis was conducted using ANOVA and Bland-Altman to examine the accuracy and efficiency of our wearable device compared with the traditional methods. Results: The perfused air measured with the respiratory monitor enables clinicians to evaluate the absolute value of the tidal volume during ventilation of a patient. In contrast, directly connecting our RHS device to the surgical mask facilitates continuous lung volume monitoring. The results of the 1-way ANOVA showed high P values of .68 for respiratory volume and .89 for respiratory rate, which indicate that the group averages with the PNT standard are equivalent to those with our RHS platform, within the error margins of a typical instrument. Furthermore, analysis utilizing the Bland-Altman statistical method revealed a small bias of 0.03 with limits of agreement (LoAs) of -0.25 and 0.33. The RR bias was 0.018, and the LoAs were -1.89 and 1.89. Conclusions: Based on the encouraging results, we conclude that our proposed design can be a viable, low-cost wearable medical device for pulmonary parametric measurement to prevent and predict the progression of pulmonary diseases. We believe that this will encourage the research community to investigate the application of RHS for monitoring the pulmonary health of individuals.

Here, we present a liquid metal-based flexible electrode coupled with SnS2 nanomaterials as a wearable gas-sensing device, with added Bluetooth capabilities for remote respiratory monitoring and diagnoses. The flexible epidermal device exhibits superior skin compatibility and high responsiveness (1092%/ppm), ultralow detection limits (1.32 ppb), and a good selectivity of NO gas at ppb-level concentrations. Taking advantage of the fast recovery kinetics of SnS2 responding to H2O molecules, it is possible to accurately distinguish between different respiratory patterns based on the amount of water vapor in the exhaled air. Furthermore, based on the different redox types of H2O and NO molecules, the electric signal is reversed once the exhaled NO concentration exceeds a certain threshold that may indicate the onset of conditions like asthma, thus providing an early warning system for potential lung diseases. Finally, by integrating the wearable device into a wireless cloud-based multichannel interface, we provide a proof-of-concept that our device could be used for the simultaneous remote monitoring of several patients with respiratory diseases, a crucial field in future digital healthcare management.

Abstract:Fossil fuel internal combustion engines generate and release a huge amount of nitrogen dioxide, leading to respiratory and allergic diseases such as asthma, pneumonia, and possibly tuberculosis. Here we develop an alveolus-inspired membrane sensor (AIMS) for self-powered wearable nitrogen dioxide detection and personal physiological assessment. The bionic AIMS exhibits an excellent sensitivity up to 452.44%, a good linearity of 0.976, and superior selectivity under a NO2 concentration of 50 ppm. Furthermore, the AIMS can also be employed to diagnose human breath behaviors for breath analysis. The fundamental sensing mechanism is established using a combination of thermodynamic analysis, finite-element analysis, and phase-field simulations. It is found that the depolarization field inside the sensitive materials plays a crucial role in the self-powered gas-sensing performance. This work not only provides an efficient, low-cost, portable, and environmentally friendly means for active environmental assessment and personal biomonitoring but also provides a deep understanding of the gas-sensing mechanisms.

Abstract:A portable chemical sensing system that integrates sample preconcentration, separation and detection as well as wireless communication functionalities into a compact, wearable format can provide continuous and real-time monitoring of volatile organic compounds in the environment. The sensing modality relies on tuning forks coated with molecularly imprinted polymers that, in conjunction with sample preconcentration, offer selective detection down to parts-per-billion levels. The use of capillary columns allows individual components of complex mixtures to be detected at these highly sensitive levels even in the presence of interferents. The wireless capability facilitates the utilization of a paired smartphone as the user interface as well as a vehicle for additional processing and storage of the measured data. This integrated approach offers a cost-effective and reliable platform for personal exposure assessment.

Finally, by integrating the wearable device into a wireless cloud-based multichannel interface, we provide a proof-of-concept that our device could be used for the simultaneous remote monitoring of several patients with respiratory diseases, a crucial field in future digital healthcare management.

Abstract:An integrated volatile organic toxicants sensor with a Bluetooth device interface has been developed. The device is based on novel tuning fork sensor platform along with a wireless communication/interface technology taken in an integrated system approach. It features high sensitivity and selectivity. The sensitivity and selectivity are accomplished through the use of novel tuning fork sensor modified by design (molecularly imprinted) polymers and selective filtering. Experiments have shown that the device can detect toxic volatile organic compounds (VOCs) under high concentrations of common interferents from flavors and fragrances. Applications of the device for detection of BTEX in real-world situations such as outdoor and gas station VOCs have also been demonstrated. All these features make the device a promising candidate to be deployed in real-world applications, particularly in environmental health and air pollution studies.

Abstract:Bulky and hyphenated laboratory-based analytical instrumentation such as gas chromatography/mass spectrometry is still required to trace breath biomarkers in the low ppbV level. Innovative sensor-based technologies could provide on-site and point-of-care (POC) detection of volatile biomarkers such as breath aldehydes related to oxidative stress and cancer. An electrochemical sensor system was developed for direct detection of the total abundance of aldehydes in exhaled breath in the ppbV level and for simultaneous determination of the airway inflammation markers carbon monoxide (CO) and nitric oxide (NO). The sensor system was tested in vitro with gaseous standard mixtures and in vivo in spontaneously breathing patients and under mechanical ventilation in an animal model. The sensor system provided in vitro and in vivo detection of trace levels of aldehydes, CO and NO. Inertness of the tubing system was important for reliable results. Sensitivity of the aldehyde sensor increased with humidity. Response time for analysis of breath samples was about 22 s and relative standard deviations of sensor amplitudes were <5%. Detection limits in the low ppbV range and a linear range of more than two orders of magnitude could be achieved for volatile aldehydes. Cross sensitivities were moderate for alcohols such as ethanol or isopropanol and negligible for other typical breath volatile organic compounds such as acetone, isoprene or propofol. In proof of concept analyses in patients suffering from lung cancer and diabetes, aldehyde and CO sensor signals differed between the groups. Elevated CO levels indicated previous smoking. In a mechanically ventilated pig, continuous monitoring of breath aldehyde concentrations in the low ppbV was realized. Cumulative aldehyde measurements may add interesting and complementary information to the conventional parameters used in clinical breath research. POC applicability, easy handling and low cost of sensors facilitate measurements in large patient cohorts.

Abstract:To improve our understanding of indoor and outdoor personal exposures to common environmental toxicants released into the environment, new technologies that can monitor and quantify the toxicants anytime anywhere are needed. This paper presents a wearable sensor to provide such capabilities. The sensor can communicate with a common smart phone and provides accurate measurement of volatile organic compound concentration at a personal level in real-time, providing environmental toxicants data every three minutes. The sensor has high specificity and sensitivity to aromatic, alkyl, and chlorinated hydrocarbons with a resolution as low as 4 parts-per-billion (ppb), with a detection range of 4 ppb–1000 ppm (parts-per-million). The sensor’s performance was validated using Gas Chromatography and Selected Ion Flow Tube – Mass Spectrometry reference methods in a variety of environments and activities with overall accuracy higher than 81% (r2 > 0.9). Field tests examined personal exposure in various scenarios including: indoor and outdoor environments, traffic exposure in different cities which vary from 0 to 50 ppmC (part-per-million carbon from hydrocarbons), and pollutants near the 2010 Deepwater Horizon’s oil spill. These field tests not only validated the performance but also demonstrated unprecedented high temporal and spatial toxicant information provided by the new technology.

Abstract:The urgent need for real-time and noninvasive monitoring of health-associated biochemical parameters has motivated the development of wearable sweat sensors. Existing electrochemical sensors show promise in real-time analysis of various chemical biomarkers. These sensors often rely on labels and redox probes to generate and amplify the signals for the detection and quantification of analytes with limited sensitivity. In this study, we introduce a molecularly imprinted polymer (MIP)-based biochemical sensor to quantify a molecular biomarker in sweat using electrochemical impedance spectroscopy, which eliminates the need for labels or redox probes. The molecularly imprinted biosensor can achieve sensitive and specific detection of cortisol at concentrations as low as 1 pM, 1000-fold lower than previously reported MIP cortisol sensors. We integrated multimodal electrochemical sensors with an iontophoresis sweat extraction module and paper microfluidics for real-time sweat analysis. Several parameters can be simultaneously quantified, including sweat volume, secretion rate, sodium ion, and cortisol concentration. Paper microfluidic modules not only quantify sweat volume and secretion rate but also facilitate continuous sweat analysis without user intervention. While we focus on cortisol sensing as a proof-of-concept, the molecularly imprinted wearable sensors can be extended to real-time detection of other biochemicals, such as protein biomarkers and therapeutic drugs.
