Abstract:The draft sequence of the human genome became available almost a decade ago but the encoded proteome is not being explored to its fullest. Our bibliometric analysis of several large protein families, including those known to be "druggable", reveals that, even today, most papers focus on proteins that were known prior to 2000. It is evident that one or more aspects of the biomedical research system severely limits the exploration of the proteins in the 'dark matter' of the proteome, despite unbiased genetic approaches that have pointed to their functional relevance. It is perhaps not surprising that relatively few genome-derived targets have led to approved drugs.

Abstract:Over the last decade, the study of genomes has rapidly advanced to the point that genomic research now serves as the basis for many medical decisions and public health initiatives. Genetic variation is likely to contribute substantially to the variation in drug response observed across human populations. Genomic tools such as sequence variation and, more specifically, personal genome sequencing incorporating genome-wide association studies and next-generation sequences enable the precise prediction and treatment of disease. At present, DNA-based risk assessment for common complex diseases, application of molecular signatures and dose selection of therapeutic drugs are the important issues in personalized medicine. In order to make personalized medicine effective, these genomic techniques must be standardized and integrated into health systems and clinical workflow for prediction of disease incidence, genetic and environmental information to optimize the medical care and outcomes for each patient. Technology continues to lead the field of personalized medicine since the interpretation of the human genome is progressing as the cost and duration of genomic sequencing continue to decrease sharply. This review aimed at understanding the technologies that reveal how the changes that occur within the genome can alter their functions and the genomic variations that constitute individual susceptibility to diseases and responses to therapy.

Abstract:A large proportion of biomedical research and the development of therapeutics is focused on a small fraction of the human genome. In a strategic effort to map the knowledge gaps around proteins encoded by the human genome and to promote the exploration of currently understudied, but potentially druggable, proteins, the US National Institutes of Health launched the Illuminating the Druggable Genome (IDG) initiative in 2014. In this article, we discuss how the systematic collection and processing of a wide array of genomic, proteomic, chemical and disease-related resource data by the IDG Knowledge Management Center have enabled the development of evidence-based criteria for tracking the target development level (TDL) of human proteins, which indicates a substantial knowledge deficit for approximately one out of three proteins in the human proteome. We then present spotlights on the TDL categories as well as key drug target classes, including G protein-coupled receptors, protein kinases and ion channels, which illustrate the nature of the unexplored opportunities for biomedical research and therapeutic development.

Abstract:Pharmacogenomics is defined as research into the relationship between inherited genetic variations in drug metabolizing enzymes, transporters and targets and individual variations in person’s response to drugs (fate of drug in human body, safety and efficacy). Personalized dosing is pharmacogenomics-based therapeutic regimen tailored to other individual characteristics. This article summarizes the progress in clinical application of personalized dosing from the perspective of pharmacogenomics of drug metabolizing enzymes and transporters, and proposes to draw attention to key scientific issues (e.g., the effect of multi-genes and non-genetic factors on drug effects, the integration of therapeutic drug monitoring and pharmacogenomics); meanwhile, bottle necks in the clinical application and corresponding strategies are proposed.

Abstract:This review explores genetic contributors to drug interactions, known as drug-gene and drug-drug-gene interactions (DGI and DDGI, respectively). This article is part of a mini-review issue led by the International Society for the Study of Xenobiotics (ISSX) New Investigators Group. Pharmacogenetics (PGx) is the study of the impact of genetic variation on pharmacokinetics (PK), pharmacodynamics (PD), and adverse drug reactions. Genetic variation in pharmacogenes, including drug metabolizing enzymes and drug transporters, is common and can increase the risk of adverse drug events or contribute to reduced efficacy. In this review, we summarize clinically actionable genetic variants, and touch on methodologies such as genotyping patient DNA to identify genetic variation in targeted genes, and deep mutational scanning as a high-throughput in vitro approach to study the impact of genetic variation on protein function and/or expression in vitro . We highlight the utility of physiologically based pharmacokinetic (PBPK) models to integrate genetic and chemical inhibitor and inducer data for more accurate human PK simulations. Additionally, we analyze the limitations of historical ethnic descriptors in pharmacogenomics research. Altogether, the work herein underscores the importance of identifying and understanding complex DGI and DDGIs with the intention to provide better treatment outcomes for patients. We also highlight current barriers to wide-scale implementation of PGx-guided dosing as standard or care in clinical settings.

Abstract:Drug addiction remains a substantial health issue with limited treatment options currently available. Despite considerable advances in the understanding of human genetic architecture, the genetic underpinning of complex disorders remains elusive. On the basis of our current understanding of neurobiology, numerous candidate genes have been implicated in the etiology and response to treatment for different addictions. Genome-wide association (GWA) studies have also identified novel targets. However, replication of these studies is often lacking, and this complicates interpretation. The situation is expected to improve as issues such as phenotypic characterization, the apparent "missing heritability," the identification of functional variants, and possible gene-environment (G x E) interactions are addressed. In addition, there is growing evidence that genetic information can be useful in refining the choice of addiction treatment. As genetic testing becomes more common in the practice of medicine, a variety of ethical and practical challenges, some of which are unique to drug addiction, will also need to be considered.

Abstract:Predicting drug responses based on individual transcriptomic profiles holds promise for refining prognosis and advancing precision medicine. Although many studies have endeavored to predict the responses of known drugs to novel transcriptomic profiles, research into predicting responses for newly discovered drugs remains sparse. In this study, we introduce scDrug+, a comprehensive pipeline that seamlessly integrates single-cell analysis with drug-response prediction. Importantly, scDrug+ is equipped to predict the response of new drugs by analyzing their molecular structures. The open-source tool is available as a Docker container, ensuring ease of deployment and reproducibility. It can be accessed at https://github.com/ailabstw/scDrugplus.

Abstract:The recent publication of the human genome sequence provides an opportunity both to combat diseases that are presently considered as pharmaceutically intractable and also to improve current therapies for many common human diseases. The identification of every human gene by ongoing bioinformatic efforts has the potential, when combined with functional genomic approaches, to pinpoint the molecular basis of every human disease, and to discover appropriate intervention points. This exciting prospect is directly relevant to the successful development of effective therapeutics because the past record of drug discovery suggests that 30%–40% of experimental drugs fail because an inappropriate biological target was pursued. The major impact of genomic information may therefore be to reduce this biological failure rate by earlier definition of drug targets related to disease susceptibility or progression. This paper briefly reviews some of the approaches that can be used to identify biologically relevant drug targets.

Abstract:Abstract Leveraging genetics insights to promote drug repurposing has become a promising and active strategy in pharmacology. Indeed, among the 50 drugs approved by FDA in 2021, two-thirds have genetically supported evidence. In this regard, the increasing amount of widely available genome-wide association studies (GWAS) datasets have provided substantial opportunities for drug repurposing based on genetics discoveries. Here, we developed PharmGWAS, a comprehensive knowledgebase designed to identify candidate drugs through the integration of GWAS data. PharmGWAS focuses on novel connections between diseases and small-molecule compounds derived using a reverse relationship between the genetically-regulated expression signature and the drug-induced signature. Specifically, we collected and processed 1929 GWAS datasets across a diverse spectrum of diseases and 724 485 perturbation signatures pertaining to a substantial 33609 molecular compounds. To obtain reliable and robust predictions for the reverse connections, we implemented six distinct connectivity methods. In the current version, PharmGWAS deposits a total of 740 227 genetically-informed disease-drug pairs derived from drug-perturbation signatures, presenting a valuable and comprehensive catalog. Further equipped with its user-friendly web design, PharmGWAS is expected to greatly aid the discovery of novel drugs, the exploration of drug combination therapies and the identification of drug resistance or side effects. Computational Strategies in Drug Discovery: Leveraging Subtractive Genomic Analysis for Target Identification

Abstract:The utilization of In-silico subtractive genomic analysis has emerged as an important and essential method in modern drug discovery and development since it significantly improves the process of identifying and validating potential targets for discovering novel therapeutic compounds to treat severe infections caused by Antimicrobial-resistant (AMR) - pathogenic species. This review provides a complete overview of the methodology, advantages, disadvantages, and prospects, associated with subtractive genomic research in the context of drug discovery and development. The initial phase of analysis encompasses the retrieval of data, which serves as a foundation for the subsequent data mining process in Phase 1. After data mining, Phase 2 utilizes subtractive channels for the target's non-homology and essentiality analysis. Phase 3 of the study aims to provide a comprehensive understanding of prospective targets by their qualitative characterization. Further, Phase 4 of the study emphasizes on conducting structure-based analyses, which involves the determination, refinement, evaluation, and validation of three-dimensional structures of the target proteins, along with their active site prediction and selection of the novel therapeutic compounds against that active site on the obtained targets through virtual screening and docking studies by utilizing various databases and servers. The therapeutic compounds obtained can be then validated by in vitro and in vivo testing, thereby establishing a connection between the computational predictions and real applications.

Abstract:AbstratThe coronavirus disease (COVID-19) pandemic further revealed the barriers to accelerated discovery and development of transformative medicines for life threatening diseases. To effectively and efficiently respond to unmet medical needs, efforts should be directed towards revolutionizing the predictive capability of non-clinical surrogates that inform drug discovery and development programs. I developed this mini special issue amidst the COVID-19 pandemic to evaluate recent advancements and opportunities for four main subthemes that support drug discovery and development including prediction of metabolic pathways, translational pharmacokinetic and pharmacodynamic studies, pharmacogenomics, and trends in bioanalysis. Scientific papers in these areas were covered by investigators from the International Society for the Study of Xenobiotics New Investigator Group and other investigators. Advancement in the predictive capability of in silico, in vitro, and in vivo models used to determine the absorption, distribution, metabolism, excretion, and toxicity profile of investigational drugs can help offset the cost of unexpected safety and/or efficacy issues during clinical studies. Likewise, extensive application of pharmacogenomics in drug development and clinical care can help direct therapeutic benefits to the appropriate patient population with the overall goal of accelerating drug development and mitigating failed drug cost. Finally, I hope that the scientific contributions in this mini special issue will stimulate practical advancements across all aspects of basic science research that support drug discovery and development to help unlock the door to the next generation of drug discovery and development that features reduced failure rates and accelerated development.

Artificial Intelligence in the Life Sciences

Abstract:A rising amount of research demonstrates that artificial intelligence and machine learning approaches can provide an essential basis for the drug design and discovery process. Deep learning algorithms are being developed in response to recent advances in computer technology as part of the creation of therapeutically relevant medications for the treatment of a variety of ailments. In this review, we focus on the most recent advances in the areas of drug design and discovery research employing generative deep learning methodologies such as generative adversarial network (GAN) frameworks. To begin, we examine drug design and discovery studies that use several GAN methodologies to evaluate one key application, such as molecular de novo design in drug design and discovery. Furthermore, we discuss many GAN models for dimension reduction of single-cell data at the preclinical stage of the drug development pipeline. We also show various experiments in de novo peptide and protein creation utilizing GAN frameworks. Furthermore, we discuss the limits of past drug design and discovery research employing GAN models. Finally, we give a discussion on future research prospects and obstacles.

Abstract:Drug design is a complex pharmaceutical science with a long history. Many achievements have been made in the field of drug design since the end of 19th century, when Emil Fisher suggested that the drug–receptor interaction resembles the key and lock interplay. Gradually, drug design has been transformed into a coherent and well-organized science with a solid theoretical background and practical applications. Now, drug design is the most advanced approach for drug discovery. It utilizes the innovations in science and technology and includes them in its wide-ranging arsenal of methods and tools in order to achieve the main goal: discovery of effective, specific, non-toxic, safe and well-tolerated drugs. Drug design is one of the most intensively developing modern sciences and its progress is accelerated by the implication of artificial intelligence. The present review aims to capture some of the most important milestones in the development of drug design, to outline some of the most used current methods and to sketch the future perspective according to the author’s point of view. Without pretending to cover fully the wide range of drug design topics, the review introduces the reader to the content of Molecules’ Special Issue “Drug Design—Science and Practice”.

Expert Opinion on Drug Discovery

Abstract:Introduction: DNA-encoded chemical libraries (DELs) have come of age and emerged to become a powerful technology platform for ligand discovery in biomedical research and drug discovery. Today, DELs have been widely adopted in the pharmaceutical industry and employed in drug discovery programs worldwide. DELs are capable of interrogating drug targets with an extremely large number of compounds highly efficiently. Area covered: In this review, the authors introduce the history of DELs and provide an overview of the major technological components, including encoding methods, library synthesis, chemistry, selection methods, hit deconvolution strategy, and post-selection data analysis. A brief update on the hit compounds recently discovered from DEL selections against drug targets is also provided. Finally, the authors discuss their views on the present challenges and future directions for the development and application of DELs in drug discovery. Expert opinion: DELs have provided great opportunities for lead compound discovery at an unprecedented scale and efficiency in drug discovery. The key to the future success of DELs as true discovery modalities, rather than just 'a way to make many compounds,' is to go beyond physical binding to functional or even phenotypic assays with the capability to probe the biological system.
