Although surgical release of upper extremity nerve compression syndromes is highly effective, persistence or recurrence of symptoms and signs may occur. Thorough investigation is necessary in this situation before treatment is recommended. If the symptoms cannot be explained by other pathology than compression of the affected nerve and if conservative management has not provided improvement, reoperation may be considered. This review provides an overview of the diagnostic and surgical considerations in the revision of carpal tunnel syndrome, cubital tunnel syndrome and thoracic outlet syndrome.Level of evidence: V

Abstract:Within the past decade, there have been multiple innovations in the field of nerve surgery. In this review, we highlight critical changes and innovations that have helped advance the field and present opportunities for further study. This includes the modification and clarification of the classification schema for nerve injuries which informs prognosis and treatment, and a refined understanding and application of electrodiagnostic studies to guide patient selection. We provide indications for operative intervention based on this nerve injury classification and propose strategies best contoured for varying injury presentations at differing time points. Abstract:Background: Carpal Tunnel Syndrome (CTS) is one of the most common peripheral neuropathies. The typical symptoms are tingling and numbness in the median nerve distribution of the hand. Current treatment for CTS includes general conservative treatment and surgical treatment. Surgical treatment plays a crucial role in the management of CTS, but little bibliometric analysis has been conducted on it. Therefore, this study aimed to map the literature co-citation network using CiteSpace (6.1 R4) software. Research frontiers and trends were identified by retrieving subject headings with significant changing word frequency trends, which can be used to predict future research advances in the surgical treatment of CTS. Methods: Publications on the surgical treatment of CTS in the Web of Science database were collected between 2003 and 2022. CiteSpace software was applied to visualize and analyze publications, countries, institutions, journals, authors, references, and keywords. Results: A total of 336 articles were collected, with the USA being the major publishing power in all countries/regions. JOURNAL OF HAND SURGERY AMERICAN VOLUME was the journal with the most published and co-cited articles. Based on keyword and reference co-citation analysis, keywords such as CTS, surgery, release, median nerve, and diagnosis were the focus of the study. Conclusion: The results of this bibliometric study provide clinical research advances and trends in the surgical treatment of patients with CTS over the past 20 years, which may help researchers to identify hot topics and explore new directions for future research in the field.

Abstract:This focus issue highlights state-of-the-art techniques, equipment, and practices in the modern era of spine surgery while providing a glimpse into the next generation of patient care. A broad range of topics are presented to cover the full spectrum of the field. Degenerative diseases are discussed in a series of 3 articles on (1) pathophysiology, management, and surgical approaches to degenerative cervical myelopathy; (2) novel approaches to degenerative thoracolumbar disease (eg, interspinous process spacers, minimally invasive/endoscopic approaches); and (3) animal models and emerging therapeutics in degenerative disk disease. Also included is a unique study aiming to establish the critically important cost-benefit relationship for spine procedures with perspectives on how value is defined and how to address variability.Primary and metastatic spine oncology are reviewed with a focus on upcoming targeted biologics, subspecialized radiotherapy (eg, proton-beam, carbon-ion, stereotactic radiosurgery), genetic profiling to stratify risk, and morbidity-reducing surgical approaches (eg, minimally invasive/endoscopic resections, percutaneous instrumentation). Trauma is discussed in 2 high-quality papers on controversies in spinal trauma and neuroprotective/neuroregenerative interventions for traumatic spinal cord injury. A stimulating article on cervical, thoracolumbar, and pediatric deformity highlights the rapid evolution of deformity surgery with a look at innovative tools (eg, high-fidelity 3-dimensional reconstructions, magnetically controlled growing rods) and their impact on quality of life. Additionally, a must-read article on surgical site infections discusses key risk factors and evidence-based preventative techniques to remain aware of. Finally, cutting-edge technologies, including computer-assisted navigation, shared-control robotics, neuromodulation, novel osteobiologics, and biomaterials, are covered in detail in a series of 3 fascinating papers on the next generation of the field.Each section intends to highlight the salient literature and afford insights from multiple key thought leaders in an effort to minimize bias and provide varied perspectives. Overall, we hope this issue provides high-quality, evidence-based data relevant to trainees and practicing surgeons while also stimulating excitement about the future of spine surgery.

Abstract:Journal of Hand Surgery (European Volume), Ahead of Print. Nerve grafting, tendon transfer and joint fusion are routinely used to improve the upper limb function in patients with brachial plexus palsies. Newer techniques have been developed that provide additional options for reconstruction. Nerve transfer is a tool for restoring upper limb function in total root avulsions where nerve grafting is not possible. In partial brachial plexus injuries, nerve transfers can greatly improve shoulder, elbow, wrist and hand function. Intraoperative electrical stimulation can be used to diagnose precisely which nerve is injured and to choose which nerve fascicles should be transferred. Finally, measuring the postoperative outcome can improve the evaluation of our techniques. The aim of this article was to present the current techniques used to treat patients with brachial plexus injury.

Abstract:The clinical and operative findings are reviewed in 47 patients with intractable hemifacial spasm. The syndrome was classical in its features in 45 patients and atypical in two. Mechanical compression distortion of the root exit zone of the facial nerve was noted in all 47 patients. In 46 the abnormality was vascular cross-compression, usually by an arterial loop. In one patient, a small cholesteatoma was discovered and removed. Morbidity and postoperative results are discussed.

Abstract:Hemifacial spasm (HFS) is a common cranial nerve disease. In HFS research, we conducted a bibliometric analysis to examine the development and research trends. A retrieval of HFS studies published between 2011 and 2022 was performed from the Web of Science Core Collection in September 2022. Two scientometric tools were used to perform bibliometric and visualization-based analyses: VOSviewer and CiteSpace. Bibliometric analysis of 1461 studies published between 2011 and 2022 was carried out using data from 444 journals, 6021 authors, 1732 institutions, and 76 countries/regions. China, the USA, Japan, and South Korea were four key contributors to this study. Shanghai Jiaotong University was the major institution with the larger number of publications. Li Shiting was the most prolific author. Jannetta PJ was the most co-cited author. World Neurosurgery was the top prolific journal. Journal of Neurosurgery was the top co-cited journal. The top five keywords were hemifacial spasm, microvascular decompression, trigeminal neuralgia, surgery, and neurovascular compression. This study examines the research trends in global scientific research on HFS over the last decade. Researchers interested in learning more about current trends and novel research frontiers in this area can benefit from the study.

Improving Surgeon Well-Being: Ergonomics in Neurosurgery

Abstract:Background: Musculoskeletal disorders are common among surgeons, and affect most neurosurgeons over the course of their career. Although all subspecialist neurosurgeons may be affected by physical strain, spine surgeons and skull base surgeons have a high propensity for workplace injury as a result of long procedures with repetitive movements in strained physical positions. Methods: In this review, the prevalence of musculoskeletal disorders in neurosurgery, the state of innovation to improve ergonomics in the operating room for neurosurgeons, and potential limitations in advancing technology with the goal of maximizing neurosurgeon longevity are discussed. Results: Innovations such as robotics, the exoscope, and handheld devices with more degrees of freedom have allowed surgeons to maneuver instruments without exerting excessive effort, all while maintaining neutral body positioning, avoiding joint and muscle strain. Conclusions: As new technology and innovation in the operating room develop, there has been a larger emphasis placed on maximizing surgeon comfort and neutral positioning, by minimizing force exertion and fatigue.
Surgical and Conservative Treatments of Complete Spontaneous Posterior Interosseous Nerve Palsy with Hourglass-Like Fascicular Constrictions: a Retrospective Study of 41 Cases.

Abstract:BACKGROUND:The surgical treatment of spontaneous posterior interosseous nerve (PIN) palsy with hourglass-like fascicular constriction (HLFC) remains controversial.OBJECTIVE:To review 41 patients with complete spontaneous PIN palsy with HLFC to clarify the necessity and choice of surgery.METHODS:Interfascicular neurolysis (NY), neurorrhaphy, and autografting were performed on 10, 8, and 6 patients, respectively. The thinning extent of a nerve fasciculus ≤0.25, 0.25 to 0.75, and ≥0.75 was defined as mild, moderate, and severe constriction, respectively. Final British Medical Research Council muscle power grade ≥4 was defined as good recovery.RESULTS:Ultrasound showed the number, location, and thinning extent of HLFC of PIN well, with results that were highly consistent with intraoperative measurements. Of the 17 conservatively treated patients, 13 recovered well. Of the 24 surgically treated patients, 20 recovered well. For NY, 8 patients with mild to moderate PIN constriction recovered well, but 2 patients with severe PIN constriction recovered poorly. For 16 patients with severe HLFC, 12 of 14 patients who underwent neurorrhaphy or autografting recovered well; the surgical effects were much better than those of 2 patients who had undergone NY.CONCLUSION:Ultrasound is a helpful diagnostic technique for spontaneous PIN palsy with HLFC. Surgery is necessary for PIN constriction if conservative treatments fail. Surgical choices depend largely on the thinning extent of the PIN constriction and the age of the patients. The outcomes of patients aged ≥50 years were much worse. We suggest NY for mild to moderate, and neurorrhaphy or autografting for severe PIN constriction.

Abstract:Editorial Almost a hundred years has passed since neurosurgery became an official surgical specialty of medicine. Forged from the field of general surgery, the founders of modern neurosurgery were able to progress the field despite having rudimentary methods of anesthesia, electrocautery, understanding of post-operative hematomas, and ways to monitor intracranial pressure. The combination of increased understanding of antiseptic techniques, advancements in surgical technologies, and early experimental successes lead to the formation of the first organized group called the Society of Neurological Surgeons in the United States in 1919. Later leading to the formation of other American societies and groups, such as The Harvey Cushing Society in 1932 [1]. On the international front the promotion of neurosurgery was also accelerating during this time. The first journal solely devoted to neurosurgery was established in Berlin in 1936 [2]. Also around this time the Journal of Neurosurgery was created in 1944 by the American Association of Neurological Surgeons receiving submissions from all over the world [3]. Today there are nearly two hundred neurosurgery related journals around the world and more than ten neurosurgery related journals in China [4]. Although there are many international neurosurgery journals, there has never been an English language based journal originating from China. Unfortunately, this often hindered the exchange of original neurosurgical research and information from China to the rest of the world. Since there are more than ten thousand practicing Chinese neurosurgeons, there is a great need for scientific communication in the Chinese neurosurgery community.
