Abstract:Psychometric properties of the Insomnia Severity Index (ISI) were analyzed in U.S. college samples. ISI items and total score with sleep and psychosocial questionnaires were examined in Experiment I. ISI diagnostic accuracy in a clinical sample with and without insomnia was assessed in Experiment II. ISI test-retest validity, confirmatory factor analysis (CFA), and Item Response Theory via Graded Response Model (GRM) were assessed in Experiment III. Results indicated analogous ISI and sleep diary items showed moderate correlations (r1 = .40; r2 = .45). The ISI total had weak to strong correlations with other indicators of sleep-related disturbance (rs = .25 - .62). The ISI had weak to moderate correlations with psychosocial measures commonly associated with insomnia (rs = .10 - .57). The diagnostic accuracy of the ISI was very high (area under the curve = .999). Sensitivity and specificity were maximized at a cutoff score ≥ 8. The ISI demonstrated good test-retest reliability (ICC = .87). CFA revealed a three-factor model for two study samples and GRM indicated better ability of the ISI to assess moderate (Sample III) and moderate to high (Sample I) levels of insomnia severity. The ISI demonstrated good psychometric properties and appears generally valid for screening insomnia disorder and assessing insomnia severity in college students. Overlap with psychological symptoms suggests caution while interpreting these constructs independently.

Abstract:Background: Insomnia is the sleep disorder with the highest incidence worldwide. It is estimated that this condition increases the risk of developing psychiatric, neurological, and cardiovascular problems. Due to this, it is important to have brief, reliable and valid psychometric instruments that allow health personnel their timely detection in first level health centers. Objectives: Analyze the psychometric properties of the Insomnia Severity Index (ISI) in its version adapted to Spanish in a sample of Mexican adults. Methods: The sample consisted of 310 adults, 223 (71.9%) women and 87 (28.1%) men. The comparison of four ISI measurement models of one, two and three factors and a bifactor model was carried out, its internal consistency was analyzed, an analysis of invariance by sex and correlation analysis with the Athens and Epworth scales. Results: The ISI bifactor model with a general factor (G) and a specific factor for insomnia impact (I) showed the best fit indices (χ2= 29.48, gl = 11, SRMR= 0.03, CFI= 0.98, TLI= 0.96, RMSEA= 0.07), and demonstrated configural, metric, and scalar invariance by sex. Adequate reliability was demonstrated by Omega coefficients (General: ωG= 0.86, Impact: ωI= 0.86) showed adequate reliability; the scale showed very strong correlations with the Athens scale (rAGoverall= 0.84; rAImpact=0.75) and weak to moderate correlations with the Epworth scale (rEGeneral= 0.39; rEImpact=0.44). Conclusions: The ISI bifactor version in Spanish presents adequate psychometric properties for the measurement of insomnia and, as it is a brief tool, it can be used at different levels of health care.

Abstract:Introduction The Insomnia Severity Index (ISI) is a self-administrated questionnaire most frequently used to assess insomnia in clinical and non-clinical populations. Objective To evaluate the psychometric properties of the Arabic ISI among patients diagnosed with chronic diseases. Methods A cross-sectional and descriptive correlational design was used. A total of 1,005 patients with chronic diseases completed the seven items of the Arabic ISI version. The scale was assessed in terms of acceptability, internal consistency, and validity. Construct validity was explored with the use of principal factor analysis and confirmatory factor analysis, to examine the dimensional structure of the ISI. Results The Cronbach's alpha coefficient for the Arabic ISI was 0.82, which shows good reliability. The total ISI score did not have floor or ceiling effects. There was evidence of discriminate validity. The Principal Component Analysis (PCA) indicated two factors (four items loading on Factor I and three items loading on Factor II). The construct validity of PCA in terms of two factors was explored by confirmatory factor analysis to examine the dimensional structure of the ISI. The confirmatory factor analysis showed an absolute fit for the two-factor model. Conclusion The results support the two-factor structure of ISI. The Arabic version of the ISI demonstrated good reliability and validity for assessing insomnia in patients diagnosed with chronic diseases.

Abstract:Objective The aim of this study was to explore the psychometric properties of the Insomnia Severity Index (ISI) based on modern test theory, such as item response theory (IRT) and Rasch analysis, with shortened versions of the ISI among the general population.Methods We conducted two studies to evaluate the reliability and validity of the shortened versions of the ISI in a Korean population. In Study I, conducted via online survey, we performed an exploratory factor analysis (n=400). In Study II, confirmatory factor analysis (CFA) was conducted (n=400). IRT and Rasch analysis were performed on all samples. Participants symptoms were rated using the ISI, Dysfunctional Beliefs and Attitudes about Sleep–16 items, Dysfunctional Beliefs about Sleep–2 items, Patient Health Questionnaire–9 items, and discrepancy between desired time in bed and desired total sleep time.Results CFA showed a good fit for the 2-factor model of the ISI (comparative fit index=0.994, Tucker–Lewis index=0.990, root-meansquare-error of approximation=0.039, and standardized root-mean-square residual=0.046). The 3-item versions also showed a good fit for the model. All scales showed good internal consistency reliability. The scale information curve of the 2-item scale was similar to that of the full-scale ISI. The Rasch analysis outputs suggested a good model fit.Conclusion The shortened 2-factor ISI is a reliable and valid model for assessing the severity of insomnia in the Korean population. The results are needed to be explored further among the clinical sample of insomnia.

Identifying the best sleep measure to screen clinical insomnia in a psychiatric population

Abstract:Background: Insomnia symptoms are highly prevalent among patients with psychiatric disorders, and this mandates the need to identify the best self-administered sleep measure to screen for clinical insomnia among them. Methods: A total of 400 psychiatric outpatients completed the Pittsburgh Sleep Quality Index, Insomnia Severity Index (ISI), Epworth Sleepiness Scale, Flinders Fatigue Scale, Functional Outcomes of Sleep Questionnaire, and Dysfunctional Beliefs and Attitudes about Sleep Scale in a cross-sectional study. The sensitivity, specificity, positive, and negative predictive values of these six sleep scales were assessed and compared in relation to both ICD-10 and DSM-5 insomnia disorder status established using the interviewer-administered Brief Insomnia Questionnaire. Results: Receiver operator characteristic curves with the area under the curve (AUC) revealed the ISI to be the most accurate measure to discriminate cases and non-cases on both ICD-10 (AUC = 0.88, 95% CI = 0.84-0.92) and DSM-5 (AUC = 0.82, 95% CI = 0.78-0.86) criteria with "good" accuracy. The cut-off scores of ≥14 and ≥ 11 for the ISI provided optimal sensitivity and specificity for the detection of ICD-10 and DSM-5 insomnia, respectively. Discussion: With the new calling from DSM-5 to treat sleep symptoms in the presence of a co-existing mental condition, early detection of psychiatric patients with clinically significantly insomnia using a simple but accurate self-report sleep measure becomes important. Our study suggests that the ISI could be used as a potential screening tool for comorbid insomnia disorder in patients with mental disorders.

Abstract:Abstract Purpose To examine the psychometric properties of the Sleep Condition Indicator (SCI) using different psychometric approaches [including classical test theory, Rasch models, and receiver operating characteristics (ROC) curve] among patients with advanced cancer. Methods Through convenience sampling, patients with cancer at stage III or IV ( n = 859; 511 males; mean ± SD age = 67.4 ± 7.5 years) were recruited from several oncology units of university hospitals in Iran. All the participants completed the SCI, Insomnia Severity Index (ISI), Pittsburgh Sleep Quality Index (PSQI), Epworth Sleepiness Scale (ESS), Hospital Anxiety and Depression Scale (HADS), General Health Questionnaire (GHQ), and Edmonton Symptom Assessment Scale (ESAS). In addition, 491 participants wore an actigraph device to capture objective sleep. Results Classical test theory [factor loadings from confirmatory factor analysis = 0.76–0.89; test–retest reliability = 0.80–0.93] and Rasch analysis [infit mean square (MnSq) = 0.63–1.31; outfit MnSq = 0.61–1.23] both support the construct validity of the SCI. The SCI had significant associations with ISI, PSQI, ESS, HADS, GHQ, and ESAS. In addition, the SCI has satisfactory area under ROC curve (0.92) when comparing a gold standard of insomnia diagnosis. Significant differences in the actigraphy measure were found between insomniacs and non-insomniacs based on the SCI score defined by ROC. Conclusion With the promising psychometric properties shown in the SCI, healthcare providers can use this simple assessment tool to target the patients with advanced cancer who are at risk of insomnia and subsequently provide personalized care efficiently.

Abstract:The Athens Insomnia Scale (AIS) can be regarded as a highly useful instrument in both clinical and research settings, except for when assessing the severity level. This study aims to determine the severity criteria for AIS by using the Insomnia Severity Index (ISI). A total of 1666 government employees aged 20 years or older were evaluated using the AIS and ISI, the Patient Health Questionnaire for depressive symptoms, the Epworth Sleepiness Scale for daytime sleepiness, and the Short Form Health Survey of the Medical Outcomes Study for health-related quality of life (QoL). A significant positive correlation (r) was found between the AIS and the ISI (r = 0.80, p < 0.001). As a result of describing receiver–operator curves, the severity criteria of the AIS are capable of categorizing insomnia severity as follows: absence of insomnia (0–5), mild insomnia (6–9), moderate insomnia (10–15), and severe insomnia (16–24). In addition, compared to all scales across groups categorized by AIS or ISI, it was revealed that similar results could be obtained (all p < 0.05). Therefore, the identification of the severity of AIS in this study is important in linking the findings of epidemiological studies with those of clinical studies.

Abstract:Objective: To identify the prevalence of indicators of the nursing diagnosis “insomnia” in older people through the Insomnia Severity Index (ISI) scale. Materials and methods: Cross-sectional epidemiological study involving older individuals from a community center. Data analysis was conducted between July and August 2021 using the R program. Univariate analysis was adopted for sociodemographic characterization. The χ2 test (Pearson’s chi-square) was employed to assess the association between the sociodemographic variables, nursing diagnosis indicators, and the ISI. Results: A total of 90 older people (mean age = 76.8 years, SD = 7.2 years) with at least one defining characteristic were included in this study. There was a higher prevalence of the diagnostic indicators “insufficient physical endurance” (75.6%; 95% CI: 65-83%) and “average daily physical activityis less than recommended for age and gender” (71.1%; 95% CI: 61-79%). In contrast, the diagnostic indicators “increased accidents” (6.7%; 95% CI: 3-13%) and “lifestyle incongruent with normal circadian rhythms” (2.2%; 95% CI: 92-99%) had a lower prevalence. A significant association was found between ISI, the defining characteristics (p = 0.009; < 0.001; 0.005; 0.002; 0.022; < 0.001; 0.004; < 0.001), and the related factors (p = 0.007; 0.004; < 0.001; 0.006; 0.049) of nursing diagnosis. Conclusions: The prevailing indicators of insomnia in community-dwelling older individuals were demonstrated in our sample and their association with a validated clinical assessment survey was established. These findings may contribute to nurses’ diagnostic accuracy and their abilityto conduct effective diagnostic screening in older populations.

Abstract:Background More than 80% of patients seeking metabolic and bariatric surgery (MBS) report disturbance in sleep function. No studies have assessed the psychometric properties of sleep measures in MBS samples. Objectives This study assessed the reliability and validity of the Pittsburgh Sleep Quality Index (PSQI) and the Insomnia Severity Index (ISI) in a large sample of patients seeking MBS. Setting Two academic medical centers. Methods Measures of sleep (PSQI & ISI) and psychological functioning, including the Quick Inventory of Depressive Symptomatology (QIDS), were administered pre-surgically. Internal consistency was assessed with mean inter-item correlations and item-total correlations. Construct validity was assessed using confirmatory factor analyses (CFAs) based on models identified in the literature and by measuring convergent and discriminant validity. Results Participants ( N = 939) were primarily female (83.4%) with a mean age of 41.7 ( SD= 11.5) years and mean BMI of 47.5 kg/m 2 ( SD = 8.5). CFAs indicated excellent fit for two-factor solutions for the PSQI and ISI. Internal consistency for the PSQI and ISI were acceptable. Convergent validity was demonstrated by large correlations between the PSQI and ISI ( r = .80, p < .001), and moderate correlation between the PSQI, ISI, and QIDS ( r s > .50, ps < .001). The PSQI and ISI were moderately correlated with QIDS items assessing sleep function ( p < .001), but correlations with items assessing appetite change were small ( r < .10), demonstrating good discriminant validity. Conclusions Results support the reliability and validity of the PSQI and ISI for patients undergoing MBS.

Abstract:Study objective Cognitive Behavioral Insomnia Therapy (CBT-I) is the gold standard insomnia treatment and the Insomnia Severity Index (ISI) is a frequently used treatment outcome measure. The ISI has strong psychometric properties and is purported to measure perceived insomnia symptom severity. However, little is known about the factors that drive insomnia severity perception and the psychometric properties of the ISI post-CBT-I. Methods Participants were treatment-seeking adults meeting DSM-5 Insomnia Disorder criteria (n = 203, ages 18–77, M = 45.95 years). Participants completed sleep and mood questionnaires, including the ISI, pre- and post-CBT-I. They completed daily Consensus Sleep Diaries each morning throughout two pre-treatment weeks, eight weeks of treatment and two weeks post-treatment. A hierarchical regression analysis examined what predicted post-CBT-I ISI scores and Cronbach's alpha was computed to examine post-treatment reliability of the ISI. Results The regression analysis revealed that lower post-treatment ISI scores were associated with lower pre-treatment ISI, and greater decreases in fatigue and generalized anxiety symptoms. The model did not significantly improve when pre-treatment sleep effort or changes in sleep diary indices were added. The post-treatment ISI Cronbach's alpha was .88. Conclusions Although the ISI has been shown to have sound psychometric properties, clinicians should consider that post CBT-I ISI scores are not related to their sleep improvements. Instead, they seem to be related to whether patients perceive themselves as poor sleepers pre-treatment and whether they felt less tired and anxious after CBT-I. Researchers should consider the impact of factors other than sleep when using the ISI at post-treatment. Patients are telling us that CBT-I should focus on addressing symptoms of fatigue and general anxiety; perhaps CBT-I could be improved further to address these concerns more effectively. Summary This investigation shows that when individuals are rating their symptom severity after CBT-I, they are also integrating how they felt before treatment and whether they experienced a change in their fatigue or anxiety.

Abstract:Abstract Objective: Insomnia severity index (ISI) is a widely used scale in various demographic groups but its psychometric properties have not been established in a sample of Indian subjects. This study was conducted as a preliminary study to extend and confirm the applicability of ISI to an Indian population of university students and to establish its correlation with the Pittsburgh Sleep Quality Index (PSQI). Materials and methods: The study was conducted among the student population of Jamia Millia Islamia, New Delhi, India, with due approval by the institutional ethical committee. Twenty-five poor sleepers with a mean age of 25.24±7.04 years and a BMI of 24.2±2.5 kg/m 2 were recruited for the study from the university population. They were administered ISI and PSQI on test day and the same was repeated after 1 week. Results: The test-retest reliability for ISI (intra-class correlation coefficient, ICC 2,1 −0.84) was excellent. It was further established by the Bland-Altman graph and scatter plot. The results also showed that ISI (Pearson’s coefficient r− 0.45) had a strong positive correlation with PSQI. Internal consistency for the ISI (Cronbach’s α–0.84) was excellent. Conclusion: The study findings suggest that ISI has excellent internal consistency, test-retest reliability and validity for the university population of poor sleepers in India. Hence, it can prove to be a good tool for screening insomnia in the current population.

Abstract:Background: Various studies have shown that insomnia is associated with computer use. The Athens Insomnia Scale (AIS) is an 8-item tool that has been widely used for screening insomnia. No studies have investigated the psychometric validity of AIS in occupational computer users. Objective: the current research aimed to test the psychometric properties of the AIS among occupational computer users. Materials and Methods: a sample of four hundred and twenty-four occupational computer users (age: 20–65 years and body mass index: 21.6 ± 3.5 kg/m2) completed an AIS and a socio-demographic questionnaire in this cross-sectional study. Results: a confirmatory factor analysis demonstrated that the three-factor model had an adequate fit (the goodness of fit index (0.95), incremental fit index (0.90) and χ2/df (2.61)). Evidence was found for configural, scalar and metric invariance of the 3-factor model across gender groups. A moderate level of internal consistency was implied by a Cronbach’s alpha of 0.66. Conclusion: the findings of the present research support the validity of AIS for screening insomnia, as demonstrated by the scale’s psychometric properties; its internal consistency, internal homogeneity, item discrimination, and factorial validity.

Development and Validation of the Insomnia Daytime Symptoms and Impacts Questionnaire (IDSIQ)

Abstract:Background and objective: Chronic insomnia has major consequences for daytime functioning, yet no fully validated patient-reported outcome instrument for once-daily assessments is available to measure these consequences. This study describes the development and psychometric evaluation of the Insomnia Daytime Symptoms and Impacts Questionnaire (IDSIQ). Methods: The Daytime Insomnia Symptom Scale (DISS), an existing 20-item instrument for assessing daytime functioning, was modified to give an 18-item version of the IDSIQ (IDSIQ-18) based on iterative qualitative interviews with 54 subjects with insomnia and expert input. The construct validity and other psychometric properties of the IDSIQ-18 were analyzed based on an interventional study (NCT03056053) in which subjects with insomnia received zolpidem (5 or 10 mg) daily for 2 weeks and an observational study among subjects with no diagnosis of insomnia (good sleepers). Participants in both studies completed the IDSIQ-18 daily for 2 weeks. Exit interviews were conducted with a sample of subjects who completed the interventional study to elicit concepts defining the experience of insomnia, to assess understanding of the response scales, and to determine meaningful change thresholds. Exploratory factor analysis and Rasch analysis were conducted to further assess the structure and latent model for the scoring of the final IDSIQ instrument. Further psychometric evaluation of the final IDSIQ was then conducted. Results: Subjects in both the interventional study (N = 114) and observational study (N = 103) were predominantly female (65% for subjects with insomnia and 60% for good sleepers). Mean age was 51 years for subjects with insomnia and 45 years for good sleepers. Subjects in the exit interviews (N = 41) demonstrated a good understanding of the IDSIQ-18 response scales. Day 1 mean scores were higher (worse) in subjects with insomnia compared with good sleepers. Based on inter-item correlation, exploratory factor, and Rasch analyses and review of the qualitative data, four items were removed. This yielded the final IDSIQ, with 14 items comprising three domains: Alert/Cognition, Mood, and Sleepiness. The domain structure was determined in a confirmatory factor analysis. Evidence of internal consistency reliability was strong: day 1 Cronbach's alpha was 0.917 for IDSIQ total score and 0.806-0.918 for the domains. Test-retest reliability, assessed for subjects with insomnia with no change on the Patient Global Assessment of Disease Severity scale between day 1 and day 8, was also good (intra-class correlation coefficient 0.856-0.911). Meaningful change thresholds derived for this sample using anchor-based approaches were 20 for IDSIQ total score, 9 for the Alert/Cognition domain, 4 for the Mood domain, and 4 for the Sleepiness domain. Conclusions: These studies, which closely followed Food and Drug Administration Guidance for Industry on patient-reported outcome measures, support use of the IDSIQ as a fit-for-purpose measure for deriving valid and reliable endpoints in insomnia clinical research trials and real-world studies.
Psychometric Analysis of a Chinese Version of the Sleep Hygiene Index in Nursing Students in China: a Cross-Sectional Study

Abstract:Objective: The aim of the study had two stages. One was to examine the psychometric quality of the Sleep Hygiene Index (SHI) in Chinese version and its predicted function for the prevalence of insomnia. The other was to describe the prevalence of poor sleep hygiene habits and associated factors of sleep hygiene habits in Chinese nursing students. Method: According to Brislin translation model, the English version of SHI was translated into Chinese. And a pilot-survey was carried out to measure psychometric quality of the Chinese version of SHI with 260 nursing students by convenient sampling. Then a cross-sectional survey was conducted. 659 un-dergraduates were recruited by simple random sampling in a medical university in China. Data collection instruments consisted of a demographic questionnaire, the Sleep Hygiene Index (SHI), the Insomnia Severity Index (ISI) and the Brief Insomnia Questionnaire (BIQ). Data were analyzed by SPSS 24.0 and Amos 24.0 with P = 0.05 as the significant test value. Results: The internal consistency reliability of SHI in Chinese version was more than 0.60 (alpha = 0.62, omega = 0.63). The concurrent validity presented significantly (r = 0.25, P < 0.001). Exploratory factor analysis found that a six component model explained 63.06% of total variance and confirmatory factor analysis showed good fitness (chi(2)/df = 2:14; RMSEA = 0.04). ROC analysis showed that the cut-off value pre-dicting for insomnia was 5.50 (52.90% sensitivity and 75.80% specificity). The area under the ROC curve was 0.66 (95% confidence interval = 0.61-0.71). 199 (30.20%) participant had poor sleep hygiene habits, especially in the aspects of staying too longer in bed (65.25%) and irregular sleep schedule. Multiple linear regression analysis showed health condition, academic difficulties and gender were more common associated factors of sleep hygiene. Conclusions: The Chinese version of the Sleep Hygiene Index demonstrates satisfactory psychometric qualities and has higher sensitivity and specificity to predict for insomnia. So SHI could be used in Chinese nursing students and detect high levels of insomnia. The status of sleep hygiene of nursing students in China should be concerned. Sleep hygiene education should be carried out in nursing stu-dents with different gender, health condition, and academic performance. (C) 2021 Elsevier B.V. All rights reserved.

Abstract:Study objectives The comorbidity of insomnia and obstructive sleep apnea (COMISA) is common and associated with adverse clinical consequences. While insomnia is often underdiagnosed among individuals with OSA, the identification of insomnia among these individuals may lead to improved medical care. Our objective was to assess the performance of two simplified tools (insomnia severity index-2 (ISI-2) and ISI-3) to identify insomnia among individuals with OSA. We hypothesized that the ISI-2 and ISI-3 perform well to diagnose insomnia among individuals with OSA. Methods Four different cohorts of individuals with sleep disorders were studied. Cohorts 1,2 and 3 included individuals with OSA. Cohort 4 included individuals with insomnia only. The performance of ISI-2 and ISI-3 was compared to ISI-7-based diagnosis of insomnia using receiving operating characteristic curve analysis. Results Cohorts 1, 2, 3 and 4 included 490, 124, 39 and 118 individuals, respectively. Individuals from cohorts 1, 2 and 3 were middle-aged, obese and had severe OSA on average. Individuals from cohort 2 were predominantly male while from cohorts 3 and 4 were predominantly female. Individuals from cohort 4 were slightly younger, and eutrophic on average. Cohorts 3 and 4 had higher insomnia severity (ISI-7) than cohorts 1 and 2. Using cohort 1, the best cut-offs for ISI-2 and ISI-3 were ≥6 and ≥8, respectively. Area under the curve (AUC) for ISI-2 was .900, .951 and .893 among cohorts 1, 2 and 4, respectively. AUC for ISI-3 was .924, .961 and .936 among cohorts 1, 2 and 4, respectively. Conclusion ISI-2 and ISI-3 are accurate screening tools for insomnia among individuals with sleep disorders. Easy recognition of insomnia among individuals with COMISA may improve clinical outcomes.

Abstract:Abstract Background The objective of the present study was to translate and linguistically validate in Moroccan dialect the original English version of the Insomnia Severity Index (ISI). The translation and adaptation of this scale are obtained by using the process forward translations and backward translations. The participants filled out the final questionnaire two times with an interval of 10 days. Cronbach’s alpha coefficient assesses the acceptability and internal reliability. As for the test-retest reproducibility, we used the inter-class correlation coefficient (ICC) (95% confidence interval (CI)) and the kappa coefficient, as well as the convergent validity for each scale. Results The sample included 120 subjects, 49 men (40.8%) and 71 women (59.2%). The mean age of participants was 29.23 ± 6.69 years. The translated version was quite acceptable. The Cronbach’s alpha coefficient was 0.83. The CCI (95% CI) was 0.99 (0.989, 0.995), while the kappa coefficient was 0.76. The correlations between the items of each questionnaire and its score showed good to strong positive correlations. Conclusions These results concluded that psychometric properties are adequate compared to the original version. Thus, this translated version had to be used for the sleep assessment in the Moroccan population.

Abstract:For patients with cancer, sleep disturbance is commonplace. Using classical test theory and Rasch analyses, the present study compared two commonly used psychometric instruments for insomnia – Athens Insomnia Scale and Insomnia Severity Index – among patients with advanced cancer. Through convenience sampling, patients with cancer at stage III or IV (n = 573; 326 males; mean age = 61.3 years; SD = 10.7) from eight oncology units of university hospitals in Iran participated in the study. All the participants completed the Athens Insomnia Scale, Insomnia Severity Index, Edmonton Symptom Assessment Scale, Hospital Anxiety and Depression Scale, General Health Questionnaire‐12, Epworth Sleepiness Scale and Pittsburgh Sleep Quality Index. Additionally, 433 participants wore an Actigraph device for two continuous weekdays. Classical test theory and Rasch analysis both supported the construct validity for Athens Insomnia Scale (factor loadings from confirmatory factor analysis = 0.61–0.87; test–retest reliability = 0.72–0.82; infit mean square = 0.81–1.17; outfit MnSq = 0.79–1.14) and for Insomnia Severity Index (factor loadings from confirmatory factor analysis = 0.61–0.81; test–retest reliability = 0.72–0.82; infit mean square = 0.72–1.14; outfit mean square = 0.76–1.11). Both Athens Insomnia Scale and Insomnia Severity Index had significant associations with Edmonton Symptom Assessment Scale, Hospital Anxiety and Depression Scale, General Health Questionnaire‐12, Epworth Sleepiness Scale and Pittsburgh Sleep Quality Index, as well as having good sensitivity and specificity. Significant differences in the actigraphy measure were found between insomniacs and non‐insomniacs based on Athens Insomnia Scale or Insomnia Severity Index score. With promising results, healthcare providers can use either Athens Insomnia Scale or Insomnia Severity Index to understand the insomnia of patients with advanced cancer.

Abstract:STUDY OBJECTIVES Nurses are a group at high risk for nightmares, yet little is known about the rate of nightmare disorder and associated psychosocial factors in this group in part attributable to the lack of a self-report questionnaire to assess DSM-5 criteria for nightmare disorder. Aims of the current study were to 1) report on development and initial validity of a self-report measure of DSM-5 nightmare disorder, and 2) examine the rate and associated factors of nightmare disorder among nurses. METHODS Nurses (N = 460) completed baseline measures online including Nightmare Disorder Index (NDI), psychosocial and demographic questionnaires. A subset (n = 400) completed 14 days of sleep diaries and actigraphy. RESULTS NDI demonstrated satisfactory psychometric characteristics as indicated by good internal consistency (α = .80), medium inter-item correlations (r = 0.50), medium to large item-total (r = 0.55 - 0.85) and convergent correlations (0.32 - 0.45), and small to medium discriminant correlations (-0.12 - 0.33). Per NDI, 48.7% of nurses reported no nightmares in the past month, 43.9% met partial/subthreshold criteria and 7.4% met full criteria for probable nightmare disorder. Nurses with nightmare disorder demonstrated significantly poorer psychosocial functioning (i.e., posttraumatic stress, depression, anxiety, stress) than those with subthreshold nightmare symptoms, who had poorer functioning than those with no nightmares. CONCLUSIONS NDI is an efficient and valid self-report assessment of nightmare disorder. Nurses have high rates of nightmares and nightmare disorder which are associated with poorer psychosocial functioning. We recommend increased nightmare screening particularly for high-risk populations such as healthcare workers.
