Abstract:Background: The United Nations has declared that humans have a right to clean air. Despite this, many deaths and disability-adjusted life years are attributed to air pollution exposure each year. We face both challenges to air quality and opportunities to improve, but several areas need to be addressed with urgency.Objective: This paper summarises the recent research presented at the Pacific Basin Consortium for Environment and Health Symposium and focuses on three key areas of air pollution that are important to human health and require more research.Findings and conclusion: Indoor spaces are commonly places of exposure to poor air quality and are difficult to monitor and regulate. Global climate change risks worsening air quality in a bi-directional fashion. The rising use of electric vehicles may offer opportunities to improve air quality, but it also presents new challenges. Government policies and initiatives could lead to improved air and environmental justice. Several populations, such as older people and children, face increased harm from air pollution and should become priority groups for action.

Abstract:Despite past improvements in air quality, very large parts of the population in urban areas breathe air that does not meet European standards let alone the health-based World Health Organisation Air Quality Guidelines. Over the last 10 years, there has been a substantial increase in findings that particulate matter (PM) air pollution is not only exerting a greater impact on established health endpoints, but is also associated with a broader number of disease outcomes. Data strongly suggest that effects have no threshold within the studied range of ambient concentrations, can occur at levels close to PM2.5 background concentrations and that they follow a mostly linear concentration–response function. Having firmly established this significant public health problem, there has been an enormous effort to identify what it is in ambient PM that affects health and to understand the underlying biological basis of toxicity by identifying mechanistic pathways—information that in turn will inform policy makers how best to legislate for cleaner air. Another intervention in moving towards a healthier environment depends upon the achieving the right public attitude and behaviour by the use of optimal air pollution monitoring, forecasting and reporting that exploits increasingly sophisticated information systems. Improving air quality is a considerable but not an intractable challenge. Translating the correct scientific evidence into bold, realistic and effective policies undisputedly has the potential to reduce air pollution so that it no longer poses a damaging and costly toll on public health.

Abstract:Exposure to air pollution has a detrimental effect on health and disproportionately affects people living in socio-economically disadvantaged areas. Engaging with communities to identify concerns and solutions could support organisations responsible for air quality control, improve environmental decision-making, and widen understanding of air quality issues associated with health. This scoping review aimed to provide an overview of approaches used to engage communities in addressing air quality and identify the outcomes that have been achieved.

Abstract:Ambient air pollution is a major public health concern and comprehensive new legislation is currently being considered to improve air quality in Europe. The European Respiratory Society (ERS), Health Effects Institute (HEI), and International Society for Environmental Epidemiology (ISEE) organised a joint meeting on May 24, 2023 in Brussels, Belgium, to review and critically evaluate the latest evidence on the health effects of air pollution and discuss ongoing revisions of the European Ambient Air Quality Directives (AAQDs). A multi-disciplinary expert group of air pollution and health researchers, patient and medical societies, and policy representatives participated. This report summarises key discussions at the meeting.

Abstract:Abstract The practice of participatory sensing for environment monitoring has rapidly evolved over the years. There has been a steady growth of citizen-based air quality monitoring projects that aim to build partnerships, knowledge-sharing platforms, awareness, and ultimately resilience to issues related to air quality. Whilst citizen science has reshaped air quality research by bringing a fresh perspective on democratizing science for the public good, there is little research about how citizen-generated data can be used for facilitating and improving evidence-based policymaking. To address the problem in a structured manner, we examine the existing literature related to citizen science, air quality, and policymaking to understand the existing gaps and opportunities. That is followed by a review of major grassroots and collaborative citizen science air quality monitoring initiatives in Asia, Africa, Europe, and Latin America. We explore the range of citizen science methods and applications to understand how they are creating opportunities for dialog between practitioners and policymakers, discuss the concerns about citizen-generated data, and see if the data is used for policy action. Finally, we propose a methodology for integrating data-based evidence into shaping policy. The methodology combines scientific evidence, participation, and deliberation to realize the full potential of citizen science in air quality monitoring.

Abstract:The amount of scientific findings linking air pollution with adverse health effects is continuously growing and indicates a need for action to improve air quality. On 21 September 2005, the European Commission published a new draft directive on air quality, as part of the Thematic Strategy on air pollution. This is a long-term plan on how to reduce air pollution in the European Union in the next 15 years. Immediately after its release, the Commission received criticism for not going far enough from various instances, such as research institutions and nongovernmental organizations concerned with health and environmental effects of air pollution. One policy argument for not taking full measures was the argument of scientific uncertainty. In light of this air quality strategy and the corresponding criticism which ensued, the present article discusses how the ambiguity of scientific uncertainty may contribute to impeding the process of translating scientific findings into concrete policy options. As complete certainty is likely to never be achieved, the question arises whether it is possible to determine and agree on clear and applicable definitions of certain levels of scientific certainty. The case referred to in this paper clearly demonstrates a situation with discordant views on the uncertainty of scientific findings. More discussion on how to define scientific uncertainty and how to deal with it would be beneficial for both the scientific and the political communities. Finally, it is important to recognize that scientific evidence is not the only driver influencing policy decisions.

Moreover, the article explores innovative legal approaches and policy recommendations designed to tackle the complexities of modern-day air pollution. Case studies and comparative analyses of successful legal interventions provide insights into promising strategies for policymakers, legal practitioners, and environmental advocates. By critically evaluating existing legal mechanisms and proposing adaptive strategies, this research seeks to contribute to a more comprehensive understanding of the role of law in mitigating smog and pollution. Ultimately, it advocates for a collaborative and proactive approach that harnesses the power of legal frameworks to safeguard air quality and promote a healthier, sustainable future for all. Abstract:Air pollution contributes to the premature deaths of millions of people each year around the world, and air quality problems are growing in many developing nations. While past policy efforts have succeeded in reducing particulate matter and trace gases in North America and Europe, adverse health effects are found at even these lower levels of air pollution. Future policy actions will benefit from improved understanding of the interactions and health effects of different chemical species and source categories. Achieving this new understanding requires air pollution scientists and engineers to work increasingly closely with health scientists. In particular, research is needed to better understand the chemical and physical properties of complex air pollutant mixtures, and to use new observations provided by satellites, advanced in situ measurement techniques, and distributed micro monitoring networks, coupled with models, to better characterize air pollution exposure for epidemiological and toxicological research, and to better quantify the effects of specific source sectors and mitigation strategies.

Abstract:The need to address the impact of air pollution on health is reinforced by recent scientific evidence and the 2021 WHO Air Quality Guidelines (AQG). Air pollution is an avoidable risk factor causing a high burden for society with elevated deaths, health disorders, disabilities and huge socio-economic costs, especially in low- and middle-income countries. We have evaluated recent evidence from international reports, systematic reviews and official websites of international agencies. Growing evidence shows a causal relationship between air pollution exposure and acute lower respiratory infections, chronic obstructive pulmonary disease, asthma and lung cancer. Exposure to air pollution in both the short- and long-term has a serious impact on respiratory health. Harmful effects occur even at very low pollutant concentration levels, and there are no detectable thresholds below which exposure may be considered safe. The adverse respiratory health effects of air pollutants, even at low levels, are confirmed by recent epidemiological studies. Scientific respiratory societies and patient associations, along with other stakeholders in the health sector, should increase their engagement and advocacy to raise awareness of clean air policies and the latest WHO AQG.

Abstract:Deprived communities in many cities are exposed to higher levels of outdoor air pollution, and there is increasing evidence of similar disparities for indoor air pollution exposure. There is a need to understand the drivers for this exposure disparity in order to develop effective interventions aimed at improving population health and reducing health inequities. With a focus on London, UK, this paper assembles evidence to examine why indoor exposure to PM2.5, NOx and CO may disproportionately impact low-income groups. In particular, five factors are explored, namely: housing location and ambient outdoor levels of pollution; housing characteristics, including ventilation properties and internal sources of pollution; occupant behaviours; time spent indoors; and underlying health conditions. Evidence is drawn from various sources, including building physics models, modelled outdoor air pollution levels, time-activity surveys, housing stock surveys, geographical data, and peer-reviewed research. A systems framework is then proposed to integrate these factors, highlighting how exposure to high levels of indoor air pollution in low-income homes is in large part due to factors beyond the control of occupants, and is therefore an area of systemic inequality.POLICY RELEVANCE: There is increasing public and political awareness of the impact of air pollution on public health. Strong scientific evidence links exposure to air pollution with morbidity and mortality. Deprived communities may be more affected, however, with limited evidence on how deprivation may influence their personal exposure to air pollution, both outdoors and indoors. This paper describes different factors that may lead to low-income households being exposed to higher levels of indoor air pollution than the general population, using available data and models for London (i.e. living in areas of higher outdoor air pollution, in poor-quality housing, undertaking more pollution-generating activities indoors and spending more time indoors). A systems approach is used to show how these factors lead to systemic exposure inequalities, with low-income households having limited opportunities to improve their indoor air quality. This paper can inform actions and public policies to reduce environmental health inequalities, considering both indoor and outdoor air.

Abstract:Global CO2 emissions have been an essential topic of the environmental discussion. Still, empirical data is needed to support arguments that high-quality government actions could reduce these emissions. By analyzing data from 137 nations from 2000 to 2020, we offer strong evidence that state policies focused on promoting healthy ecosystems, sustainable economic growth, and transcendent legislative changes are capable of decreasing CO2 emissions. Based on our findings, there are essentially three critical institutional factors that need to be improved for environmental policies to be efficient: the concept of law, which protects citizens' intellectual property rights; citizens' speech, which allows them to participate in elections and represent themselves freely, and the management of corruption. Policies aimed at promoting economic growth, lowering oil and gas use, enhancing the usage of green energy by the public and private sectors, and enhancing such institutional factors are all necessary components of a climate-friendly financial strategy.

Abstract:Tackling climate change at the global level is central to a growing field of scientific research on topics such as environmental health, disease burden, and its resulting economic impacts. At the local level, cities constitute an important hub of atmospheric pollution due to the large amount of pollutants that they emit. As the world population shifts to urban centers, cities will increasingly concentrate more exposed populations. Yet, there is still significant progress to be made in understanding the contribution of urban pollutants other than CO2, such as vehicle emissions, to global climate change. It is therefore particularly important to study how local governments are managing urban air pollution. This paper presents an overview of local air pollution control policies and programs that aim to reduce air pollution levels in megacities. It also presents evidence measuring their efficacy. The paper argues that local air pollution policies are not only beneficial for cities but are also important for mitigating and adapting to global climate change. The results systematize several policy approaches used around the world and suggest the need for more in-depth cross-city studies with the potential to highlight best practices both locally and globally. Finally, it calls for the inclusion of a more human rights-based approach as a mean of guaranteeing of clean air for all and reducing factors that exacerbate climate change.

Reducing greenhouse gas emissions and improving air quality: Two global challenges

Abstract:There are many good reasons to promote sustainable development and reduce greenhouse gas emissions and other combustion emissions. The air quality in many urban environments is causing many premature deaths because of asthma, cardiovascular disease, chronic obstructive pulmonary disease, lung cancer, and dementia associated with combustion emissions. The global social cost of air pollution is at least $3 trillion/year; particulates, nitrogen oxides and ozone associated with combustion emissions are very costly pollutants. Better air quality in urban environments is one of the reasons for countries to work together to reduce greenhouse gas emissions through the Paris Agreement on Climate Change. There are many potential benefits associated with limiting climate change. In the recent past, the concentrations of greenhouse gases in the atmosphere have been increasing and the number of weather and climate disasters with costs over $1 billion has been increasing. The average global temperature set new record highs in 2014, 2015, and 2016. To reduce greenhouse gas emissions, the transition to electric vehicles and electricity generation using renewable energy must take place in accord with the goals of the Paris Agreement on Climate Change. This work reviews progress and identifies some of the health benefits associated with reducing combustion emissions.

Abstract:As evidence of adverse health effects due to air pollution continues to increase, the World Health Organization (WHO) recently published its latest edition of the global air quality guidelines (World Health Organization, 2021). Although not legally binding, the guidelines aim to provide a framework in which policymakers can combat air pollution by formulating evidence-based air quality management strategies. In the light of this, the European Union has stated its intent to revise the current ambient air quality directive (2008/50/EC) to more closely resemble the newly published WHO guidelines (European Commission, 2020). This article provides an informed opinion on selected features of the air quality directive that we believe would benefit from a reassessment. The selected features include discussion about (1) air quality sensors as a part of a hierarchical observation network, (2) the number of minimum sampling points and their siting criteria, and (3) new target air pollution parameters for future consideration.

Abstract:Recent Global Burden of Disease studies have provided quantitative evidence of the significant risk of ambient pollution to human health globally, but particularly in Asia. In the mega cities of emerging Asian economies, the rapidly growing fleet of vehicles - not necessarily low emitters - contribute to overall air pollution. On a continental scale, power generation, with billions of tonnes of coal burned annually, household emissions, trans-boundary pollution, and resuspended desert and exposed soil dust, all are the constituents of the ambient air pollution which is particularly acute in cities. By 2050, the world population is expected to grow to 9.3 billion, with 6.3 billion living in urban areas. China is at the forefront of this trend with 89 cities in 2010 exceeding one million of habitants, out of 442 such cities worldwide. The economic burden of the impact of air pollution on human life is evident around the world by the cost of health care. In addition to human health, it has been conclusively established that the release of the products of fossil fuel combustion into the air, which has accelerated exponentially over the past decades, has significant and long term consequences on the entire Earth system. The emissions originate from industrial, transport and energy generation sources, and have been studied and quantified in terms of carbon dioxide (CO2), as well as other greenhouse gases, including methane, nitrogen oxides and ozone. Based on this, predictions have been made about the future climate, particularly in terms of temperature increase, which in turn drives the evolution of the Earth as a whole. While the pressing need for lowering the emissions and significant improvement of air quality has been identified, the complexity of the problem requires input from a diverse mix of science, technology, innovation, health disciplines and policy. Further, to accelerate progress in cleaning the air, cross-disciplinary and applied research, and most importantly, international collaboration is of crucial significance. In this context, it is our pleasure to present the STOTEN Special Issue (SI) focused on: Air Quality in China: Current and Emerging Challenges.

Abstract:Background Poor air quality is associated with poor health. Little attention is given to the complex array of environmental exposures and air pollutants that affect mental health during the life course. Aims We gather interdisciplinary expertise and knowledge across the air pollution and mental health fields. We seek to propose future research priorities and how to address them. Method Through a rapid narrative review, we summarise the key scientific findings, knowledge gaps and methodological challenges. Results There is emerging evidence of associations between poor air quality, both indoors and outdoors, and poor mental health more generally, as well as specific mental disorders. Furthermore, pre-existing long-term conditions appear to deteriorate, requiring more healthcare. Evidence of critical periods for exposure among children and adolescents highlights the need for more longitudinal data as the basis of early preventive actions and policies. Particulate matter, including bioaerosols, are implicated, but form part of a complex exposome influenced by geography, deprivation, socioeconomic conditions and biological and individual vulnerabilities. Critical knowledge gaps need to be addressed to design interventions for mitigation and prevention, reflecting ever-changing sources of air pollution. The evidence base can inform and motivate multi-sector and interdisciplinary efforts of researchers, practitioners, policy makers, industry, community groups and campaigners to take informed action. Conclusions There are knowledge gaps and a need for more research, for example, around bioaerosols exposure, indoor and outdoor pollution, urban design and impact on mental health over the life course.

Abstract:Abstract Because of its serious large-scale effects on ecosystems and its transboundary nature, acid rain received for a few decades at the end of the last century wide scientific and public interest, leading to coordinated policy actions in Europe and North America. Through these actions, in particular those under the UNECE Convention on Long-range Transboundary Air Pollution, air emissions were substantially reduced, and ecosystem impacts decreased. Widespread scientific research, long-term monitoring, and integrated assessment modelling formed the basis for the policy agreements. In this paper, which is based on an international symposium organised to commemorate 50 years of successful integration of air pollution research and policy, we briefly describe the scientific findings that provided the foundation for the policy development. We also discuss important characteristics of the science–policy interactions, such as the critical loads concept and the large-scale ecosystem field studies. Finally, acid rain and air pollution are set in the context of future societal developments and needs, e.g. the UN’s Sustainable Development Goals. We also highlight the need to maintain and develop supporting scientific infrastructures.

Abstract:While ambitious carbon reduction policies are needed to avoid dangerous levels of climate change, the costs of these policies can be balanced by wide ranging health benefits for local communities. Cities, responsible for ~70% of the world's greenhouse gas (GHG) emissions and home to a growing majority of the world's population, offer enormous opportunities for both climate action and health improvement. We aim to review the current state of knowledge on key pathways leading from carbon mitigation to human health benefits, and to evaluate our current ability to quantify health benefits for cities around the world. For example, because GHGs and air pollutants are both released during fuel combustion, reducing fuel burning can reduce both GHGs and air pollutants, leading to direct health benefits. Air quality improvements may be particularly important for city-scale climate action planning because the benefits occur locally and relatively immediately, compared with the global and long-term (typically, decades to centuries) benefits for the climate system. In addition to improved air quality, actions that promote active transport in cities via improved cycling and pedestrian infrastructure can reap large cardiovascular health benefits via increased physical activity. Exposure to green space has been associated with beneficial health outcomes in a growing number of epidemiological studies and meta-analyses conducted around the world. Finally, noise is an underappreciated environmental risk factor in cities which can be addressed through actions to reduce motor vehicle traffic and other noise sources. All of these environmental health pathways are supported by well-conducted epidemiological studies in multiple locales, providing quantitative exposure–response data that can be used as inputs to health impact assessments (HIAs). However, most epidemiologic evidence derives from studies in high-income countries. It is unclear to what extent such evidence is directly transferable for policies in low- and middle-income countries (LMICs). This gap calls for a future focus on building the evidence based in LMIC cities. Finally, the literature suggests that policies are likely to be most effective when they are developed by multidisciplinary teams that include policy makers, researchers, and representatives from affected communities.
