Abstract:Aims: To determine the pregnancy outcome of early and late onset preeclampsia. Methods: This was a cross sectional analytical study conducted in the department of Obstetrics and Gynaecology, Manipal Teaching Hospital from July to October 2021. All cases of preeclampsia diagnosed according to International Society of Study of Hypertension in Pregnancy 2018, were included. Early and late onset preeclampsia cut-off used was 34 weeks. Preeclampsia diagnosed before 34 were classified as early onset and after 34 weeks as late onset preeclampsia. Maternal and perinatal outcomes were analyzed using Statistical Package for Social Sciences version 21. Results: The prevalence of early onset and late onset preeclampsia were 4.3% and 8.3% of all deliveries. Early to late onset preeclampsia were 1:2. Renal involvement, placental abruption, IUGR, low birth weight, low Apgar and perinatal morbidity were significantly more in early onset; pre-term labor and use of MgSO4 and antihypertensives were also more in early onset. Conclusion: Late onset preeclampsia was more common than early onset preeclampsia but the maternal and perinatal outcome were poor for early onset as compared to late onset preeclampsia.

Abstract:Aim: To determine the circulatory procalcitonin levels, lipid profile and uric acid levels in patients presenting with preeclampsia and to correlate these with clinical severity of the disease. Methodology: This double blinded case-control study was conducted for two years in department of Obstetrics and Gynecology in collaboration with the department of biochemistry, Government Institute of Medical Sciences, Greater Noida, UP. This study included 80 subjects out of 40 were in group I (Control) and other 40 were in group II (PE patients) admitted in the hospital in third trimester of pregnancy. General demographic and clinical data such as age, gestational week, number of gestation, and BMI were recorded, and the results of routine blood cell counts, PCT and other indicators were also obtained. The PE patients were women younger than 40 years old who met the diagnostic criteria for PE in the guidelines for diagnosis and treatment of hypertensive diseases in pregnancy-2014. The healthy pregnant women were between 24 and 40 years old, with a single fetus, normal fetal development, normal liver and kidney function, and without eclampsia, gestational diabetes mellitus, fetal growth restriction, or history of other adverse pregnancy conditions.
Umbilical and middle cerebral arteries Doppler velocimetry in early and late onset pre-eclampsia

Abstract:Background: Pre-eclampsia (PE) is a multisytemic disorder originating from the placenta with a high prevalence in sub-Saharan Africa. Early ( 34 weeks) – onset PE have different maternal and perinatal outcomes with overlapping clinical features. Differences in Doppler velocimetry pattern in these subgroups appears unsettled.Methods: In a prospective cohort study, 110 pregnant women with singleton pregnancy diagnosed with PE were recruited and had umbilical and middle cerebral arteries (MCA) Doppler velocimetry done. The pregnancies were followed up to delivery and outcome recorded and analysed using Microsoft excel 2013. Student t-test and Chi-squared test were used for continuous and categorical variables respectively. Level of significance was set at less than 0.05.Results: There were 53 (49.1%) women with early – onset preeclampsia and 55 (50.9%) with late-onset PE. The mean age of women was 30.30±5.2 years. The mean umbilical artery, middle cerebral artery pulsatility indices (PI) and cerebroplacental ratio (CPR) were 1.3±0.5, 1.1±0.5 and 1.1±1.2 respectively. There was statistical significant difference between the umbilical artery PI (p˂0.001), middle cerebral artery PI (p˂0.05) and CPR (p˂0.001) between early onset and late onset preeclamptic women. The resistance index and systolic diastolic ratio of both the umbilical and middle cerebral arteries were similar between the two groups (p>0.05).Conclusions: This study showed that umbilical and middle cerebral artery PI and CPR may be the most important Doppler parameters to watch-out for in monitoring women with PE.

Abstract:Hypertensive disorders in gestation are categorized as hypertension in pregnancy, chronic hypertension, eclampsia and pre-eclampsia. The World Health Organization states: “There are 529,000 maternal deaths annually worldwide due to pre-eclampsia and eclampsia. This study will help in determining pregnancy outcomes in early vs late-onset pr-eclampsia. Aim: To access the pregnancy outcomes of early and late onset pre-eclampsia. Study Design: A retrospective, descriptive and cross-sectional study Place and Duration: In the Obstetrics and Gynecology department of Allama Iqbal Memorial Teaching Hospital Sialkot and Islamic International Medical College, Rawalpindi for one-year duration form January 2021 to December 2021. Methods: A total 130 were admitted to the gynae ward with pre-diagnosis of pre-eclampsia were included. The cut-off value of the early and late onset PE was thirty-four weeks. Pre-eclampsia analysed < 34 weeks was labeled as early-onset pre-eclampsia and if diagnosed after 34 weeks will be labeled as late-onset pre-eclampsia. Obstetric and perinatal outcomes were assessed with version 21 of the Social Sciences Statistical Package. Results: A total 130 were admitted to the gynae ward with pre-diagnosis of pre-eclampsia were included. The severe pre-eclampsia was observed in 48(36.9%) pregnant women and 82 women (63.1%) had no symptoms of severe pre-eclampsia. In 38 (29.2%) cases; early-onset pre-eclampsia was detected and late-onset PE in 92 (70.7%) cases. The rate of severe PE in early-onset PE has been documented in 22 (57.8%) of 38 cases, and severe PE has been reported in late-onset PE in 29 (31.5%) of 92 cases. Adverse maternal outcomes occurred in 20 cases, including 7 cases of placenta abruptio and 3 cases of eclampsia with seizure disorders. Conclusions: The results indicate that in addition to intensive fetal monitoring in women with early-onset pre-eclampsia, attention should be paid to neurological, cardiopulmonary, and haematological parameters. The Risk of Persistent Hypertension and Chronic Kidney Disease in Early- and Late-Onset Preeclampsia: A Report From Developing Country

Abstract:BACKGROUND AND OBJECTIVE: Preeclampsia (PE) has been disproportionately prevalent in developing countries and constitutes a leading cause of maternal mortality, and also has long-term impacts, including renal consequences. This study aimed to explore the risk of persistent hypertension and kidney failure in early-onset PE (EOP) and late-onset PE (LOP) in the five years after delivery.METHODS: This retrospective cohort study included women with a prior history of severe PE or normotensive pregnancy admitted to tertiary hospitals in Indonesia. The blood pressure, body mass index (BMI), urea, creatinine serum, and protein urine were analyzed, and the risk of chronic kidney disease (CKD) after five years was performed using the Kidney Disease Improvement Global Outcomes (KDIGO) classification.RESULTS: Twenty-seven EOP, 35 LOP, and 30 normotensive cases were included. Mean blood pressure after five years was recorded as 115.6 ± 14.25 mmHg in the normotensive group, 131.82 ± 19.34 mmHg in the LOP group, and 154.96 ± 23.48 mmHg in the EOP group. According to the KDIGO classification, the normotensive group had an average 10% risk of CKD, but severe PE had a risk of CKD greater than 90%. In the severe PE group, the risk of CKD was 20.94 times higher compared to normotensive women (OR 20.94; 95% CI 2.67-163.72, p = 0.004). The risk of CKD in the EOP group was 6.75 times higher than in the LOP group (OR 6.75; 95% CI 2.19-20.76, p = 0.001), whereas persistent hypertension in the EOP group was 5.78 times higher than in the LOP group (OR 5.78; 95% CI 1.91-17.395, p = 0.002).CONCLUSIONS: PE women have a higher risk of CKD than normotensive women. Women with a history of EOP are more likely to develop persistent hypertension and CKD than women with a prior LOP history.

Abstract:Background: Preeclampsia, a pregnancy-specific complication occurring in the second half of human pregnancy and one of the leading causes for perinatal mortality and morbidity, is characterized by the onset of hypertension and proteinuria. We measured circulating adipocyte fatty acid binding protein (AFABP) and retinol binding protein-4 (RBP-4) in patients with preeclampsia.Methods: Twenty-seven healthy non-pregnant women, 27 healthy pregnant women at third trimester and 26 women with preeclampsia were recruited and blood samples were taken. Concentrations of serum AFABP and RBP-4 were measured with ELISA.Results: There were significant differences in serum AFABP (median: 0.99, 0.93 and 1.81 ng/ml, respectively, P < 0.001) and RBP-4 (mean: 3.16, 2.65 and 4.27 ng/ml, respectively, P = 0.022) among non-pregnancy, normal pregnancy and preeclampsia. Serum AFABP was significantly higher in preeclampsia than non-pregnancy (P < 0.001) and normal pregnancy (P = 0.001). Serum RBP-4 was significantly higher in preeclampsia than normal pregnancy (P = 0.007) but not non-pregnancy (P = 0.061). There were no significant differences in serum RBP-4 and AFABP between non-pregnancy and normal pregnancy. Serum RBP-4 was significantly higher in severe than mild preeclampsia (mean: 5.15 vs 2.84 ng/ml, P = 0.046). There was no significant difference in serum AFABP between mild and severe preeclampsia.Conclusion: Increased circulating AFABP and RBP-4 concentrations were demonstrated, suggesting it be an important pathophysiology of preeclampsia. Diet and Pre-eclampsia: A Prospective Multicentre Case-Control Study in Ethiopia

Abstract:Introduction: Pre-eclampsia is one of the most commonly encountered hypertensive disorders of pregnancy that accounts for 20-80% of maternal mortality in developing countries, including Ethiopia. For many years, diet has been suggested to play a role in pre-eclampsia. However, the hypotheses have been diverse with inconsistent results across studies, and this has not been studied in Ethiopia. Objectives: The objective of this study was to determine the effect of dietary habits on the incidence of pre-eclampsia in Bahir Dar, Ethiopia Methods: A prospective multicentre unmatched case-control study was conducted among 453 (151 cases and 302 controls) pregnant women attending antepartum or intrapartum care in public health facilities of Bahir Dar City from June to September 2014. The interviewer conducted a face-to-face interview, measured the mid-upper arm circumference (MUAC) and collected the mid-pregnancy haemoglobin level from clinical notes using a standardized and pretested questionnaire. Epi Info 3.5.3 was used for data entry and cleaning, while IBM SPSS Statistics 20 was used for data analysis. Backward stepwise unconditional logistic regression analysis was employed to determine the strength of association of predictive variables with the outcome variable and to control for the effect of confounding variables. A P-value ≤0.05 was considered statistically significant. Result: For every 1-cm increase of MUAC, there was an increase in the incidence rate of pre-eclampsia by a factor of 1.35 (adjusted odds ratio (AOR)=1.35, 95% confidence interval (CI): 1.21, 1.51). A higher incidence of pre-eclampsia was found in women who reported to have consumed coffee daily during pregnancy (AOR=1.78, 95% CI: 1.20, 3.05). Similarly, for women who had anaemia during the first trimester, the incidence of pre-eclampsia was 2.5 times higher than their counterparts (AOR=2.47, 95% CI: 1.12, 7.61). This study also revealed consumption of fruit or vegetables at least three times a week during pregnancy to be protective against pre-eclampsia (AOR=0.51, 95% CI: 0.29, 0.91; AOR=0.46, 95% CI: 0.24, 0.90, respectively). In addition, compliance with folate intake during pregnancy has shown a significant independent effect on the prevention of pre-eclampsia in this study (AOR=0.16, 95% CI: 0.08, 0.29). Conclusion and recommendation: Adequate vegetable and fruit consumption and compliance with folate intake during pregnancy are independent protective factors against pre-eclampsia. On the other hand, higher MUAC, anaemia and daily coffee intake during pregnancy are risk factors for the development of pre-eclampsia. Audience-specific education and promotion of the use of the protective factors identified in this study should be prioritized. The risk factors identified can be used for prediction and early diagnoses of pre-eclampsia allowing timely interventions to be performed to minimize deaths associated with severe pre-eclampsia/eclampsia.

Linking EDC-laden food consumption and modern lifestyle habits with preeclampsia: A non-animal approach to identifying early diagnostic biomarkers through biochemical alterations

Abstract:Preeclampsia (PE), a pregnancy complication characterized by new-onset hypertension with or without proteinuria and/or end-organ damage, and it may be influenced by exposure to endocrine-disrupting chemicals present in processed foods and modern lifestyles. This study explores the potential link using a non-animal approach to identify early diagnostic biomarkers for preeclampsia. Seventy pregnant women aged 21-41 years participated, and completed questionnaires assessing socio-demographic factors, Suboptimal Health Status Questionnaire scores for fatigue, digestive, cardiovascular, immune, and mental health issues, and exposure to endocrine-disrupting chemicals from processed food consumption and daily product use. Peripheral blood samples were analyzed for hormone profiles, complete blood count, and liver function tests (LFT). Statistical analysis revealed that mothers above 27 years old, with a Body Mass Index exceeding 32.59 Kg/m2, and a Mean Arterial Pressure of 108.5 mmHg exhibited a potential obesogenic effect on preeclampsia development. Socio-demographic factors like, lower economic class, housewife status, primiparous pregnancy, non-graduate education, and rural residence were significantly associated with results. Analysis of biochemical parameters revealed that serum creatinine, blood urea, total protein, platelet count, blood urea nitrogen, bilirubin profile, LFT profile, and thyroid profile showed potential detrimental effects on kidney, liver, muscle, and thyroid function in preeclampsia patients. Notably, PC, serum urea, bilirubin, total protein, serum glutamic-oxaloacetic transaminase (SGOT), alkaline phosphatase (ALP), and thyroid stimulating hormone (TSH) levels were significantly associated with preeclampsia in individuals reporting higher exposure to endocrine disrupting chemicals (EDCs). Minor biochemical alterations were also observed with dairy product consumption. SHS-25 analysis indicated a significant increase in fatigue, and digestive, cardiovascular, immune, and mental health-related issues in patients. Probably, biochemical alterations due to EDC exposure from processed foods and modern lifestyle habits contribute to organ dysfunction in preeclampsia. Identifying these potential biomarkers may pave the way for the development of non-invasive, early diagnostic tools for improved preeclampsia management. This research emphasizes the importance of non-animal testing methods for assessing EDC-related health risks in pregnancy and contributes to the advancement of early PE diagnosis strategies.

Abstract:Preeclampsia and eclampsia have the greatest impact on the morbidity and mortality of pregnant women and newborns. Management of patients with manifestations of preeclampsia requires a well-formulated plan that takes into account maternal risk factors, clinical, laboratory, and imaging (X-ray) findings, and time of onset gestational age, and delivery. The purpose of this study was to determine the relationship between body mass index (BMI), albumin levels, mean arterial pressure (MAP), and food intake on the incidence of preeclampsia. This prospective cohort study was conducted at the Community Health Centers working area of Central Jakarta from October 2021 to May 2022. A sample of 167 pregnant women was selected using consecutive sampling with inclusion criteria of 13-16 weeks gestation and was willing to participate in the research program. Samples who had communication disorders were on routine medication, and had a history of other diseases, except for hypertension, were excluded from the study. Data processing uses the SPSS 22.0 program with independent t-test and chi-square test analysis. The results showed that 8.4% of pregnant women with preeclampsia were positive, there was a significant relationship between age (p = 0.042; RR = 2.99; 95% CI: 1.00-9.16); spacing of pregnancies (p= 0.008; RR= 3.97; 95% CI: 1.46-10.78); maternal history of hypertension (p= 0.002; RR= 5.18; 95% CI: 1.98-13.59); BMI in early pregnancy (p= 0.001; RR= 6.02; 95% CI: 1.98-18.28); albumin level (p= 0.001; RR= 13.48; 95% CI: 1.80-100.69); MAP (p< 0.001; RR= 10.93; 95%CI: 4.47-26.70); animal protein intake (p= 0.024; RR= 3.13; 95%CI: 1.10-8.91); and vegetable intake (p= 0.038; RR= 2.79; 95%CI: 1.02-7.64) with preeclampsia. Preventing the occurrence of preeclampsia can be done by early examination of albumin levels and maintaining food intake.

Abstract:PURPOSE: We sought to examine associations of first-trimester intake of calcium, n-3 and n-6 fatty acids, trans fatty acids, magnesium, folate, and vitamins C, D, and E with preeclampsia (PE) and gestational hypertension (GH).METHODS: We studied associations of diet with PE or GH among 1718 women in the prospective cohort study Project Viva, using logistic regression and adjusting for maternal age, prepregnancy body mass index, first trimester systolic blood pressure, race/ethnicity, education, and parity. We assessed first-trimester diet using a validated semiquantitative food frequency questionnaire.RESULTS: A total of 59 (3%) women developed PE, and 119 developed (7%) GH. We found a somewhat-lower risk of PE associated with higher intake of the elongated n-3 fatty acids docosahexacnoic and eicosapentaenoic acids (odds ratio [OR] 0.84, 95% confidence interval [95% CI]: 0.69-1.03 per 100 mg/day), fish (OR 0.91, 95% CI 0.75-1.09 per serving/day), and the ratio of docosahexaenoic + eicosapentaenoic to arachadonic acid (OR 0.82,95% CI 0.66-1.01). We did not observe a lower risk of GH or PE with a greater intake of calcium; vitamin C, D, or E; milk; magnesium; folate; or with lower intake of n-6 or trans fatty acids.CONCLUSIONS: Our results support a potential benefit for elongated n-3 fatty acids in preventing preeclampsia.
