Abstract:Recently, several studies have reported on the fine-tuning of foundation models for image-text modeling in the field of medicine, utilizing images from online data sources such as Twitter and PubMed. Foundation models are large, deep artificial neural networks capable of learning the context of a specific domain through training on exceptionally extensive datasets. Through validation, we have observed that the representations generated by such models exhibit inferior performance in retrieval tasks within digital pathology when compared to those generated by significantly smaller, conventional deep networks.

Abstract:Recent advances in artificial intelligence have witnessed the emergence of large-scale deep learning models capable of interpreting and generating both textual and imaging data. Such models, typically referred to as foundation models, are trained on extensive corpora of unlabeled data and demonstrate high performance across various tasks. Foundation models have recently received extensive attention from academic, industry, and regulatory bodies. Given the potentially transformative impact that foundation models can have on the field of radiology, this review aims to establish a standardized terminology concerning foundation models, with a specific focus on the requirements of training data, model training paradigms, model capabilities, and evaluation strategies. We further outline potential pathways to facilitate the training of radiology-specific foundation models, with a critical emphasis on elucidating both the benefits and challenges associated with such models. Overall, we envision that this review can unify technical advances and clinical needs in the training of foundation models for radiology in a safe and responsible manner, for ultimately benefiting patients, providers, and radiologists.

Abstract:The growing demand for accurate and equitable AI models in digital dermatology faces a significant challenge: the lack of diverse, high-quality labeled data. In this work, we investigate the potential of domain-specific foundation models for dermatology in addressing this challenge. We utilize self-supervised learning (SSL) techniques to pre-train models on a dataset of over 240,000 dermatological images from public and private collections. Our study considers several SSL methods and compares the resulting foundation models against domain-agnostic models like those pre-trained on ImageNet and state-of-the-art models such as MONET across 12 downstream tasks. Unlike previous research, we emphasize the development of smaller models that are more suitable for resource-limited clinical settings, facilitating easier adaptation to a broad range of use cases. Results show that models pre-trained in this work not only outperform general-purpose models but also approach the performance of models 50 times larger on clinically relevant diagnostic tasks. To promote further research in this direction, we publicly release both the training code and the foundation models, which can benefit clinicians in dermatological applications.

Do Histopathological Foundation Models Eliminate Batch Effects? A Comparative Study

Abstract:Deep learning has led to remarkable advancements in computational histopathology, e.g., in diagnostics, biomarker prediction, and outcome prognosis. Yet, the lack of annotated data and the impact of batch effects, e.g., systematic technical data differences across hospitals, hamper model robustness and generalization. Recent histopathological foundation models -- pretrained on millions to billions of images -- have been reported to improve generalization performances on various downstream tasks. However, it has not been systematically assessed whether they fully eliminate batch effects. In this study, we empirically show that the feature embeddings of the foundation models still contain distinct hospital signatures that can lead to biased predictions and misclassifications. We further find that the signatures are not removed by stain normalization methods, dominate distances in feature space, and are evident across various principal components. Our work provides a novel perspective on the evaluation of medical foundation models, paving the way for more robust pretraining strategies and downstream predictors.

Abstract:Artificial intelligence has started to transform histopathology impacting clinical diagnostics and biomedical research. However, while many computational pathology approaches have been proposed, most current AI models are limited with respect to generalization, application variety, and handling rare diseases. Recent efforts introduced self-supervised foundation models to address these challenges, yet existing approaches do not leverage pathologist knowledge by design. In this study, we present a novel approach to designing foundation models for computational pathology, incorporating pathologist expertise, semi-automated data curation, and a diverse dataset from over 15 laboratories, including 58 tissue types, and encompassing 129 different histochemical and immunohistochemical staining modalities. We demonstrate that our model "RudolfV" surpasses existing state-of-the-art foundation models across different benchmarks focused on tumor microenvironment profiling, biomarker evaluation, and reference case search while exhibiting favorable robustness properties. Our study shows how domain-specific knowledge can increase the efficiency and performance of pathology foundation models and enable novel application areas.

Abstract:Foundation models in deep learning are characterized by a single large-scale model trained on vast amounts of data serving as the foundation for various downstream tasks. Foundation models are generally trained using self-supervised learning and excel in reducing the demand for training samples in downstream applications. This is especially important in medicine, where large labelled datasets are often scarce. Here, we developed a foundation model for cancer imaging biomarker discovery by training a convolutional encoder through self-supervised learning using a comprehensive dataset of 11,467 radiographic lesions. The foundation model was evaluated in distinct and clinically relevant applications of cancer imaging-based biomarkers. We found that it facilitated better and more efficient learning of imaging biomarkers and yielded task-specific models that significantly outperformed conventional supervised and other state-of-the-art pretrained implementations on downstream tasks, especially when training dataset sizes were very limited. Furthermore, the foundation model was more stable to input variations and showed strong associations with underlying biology. Our results demonstrate the tremendous potential of foundation models in discovering new imaging biomarkers that may extend to other clinical use cases and can accelerate the widespread translation of imaging biomarkers into clinical settings.

Abstract:This article discusses the opportunities, applications and future directions of large-scale pre-trained models, i.e., foundation models, for analyzing medical images. Medical foundation models have immense potential in solving a wide range of downstream tasks, as they can help to accelerate the development of accurate and robust models, reduce the large amounts of required labeled data, preserve the privacy and confidentiality of patient data. Specifically, we illustrate the "spectrum" of medical foundation models, ranging from general vision models, modality-specific models, to organ/task-specific models, highlighting their challenges, opportunities and applications. We also discuss how foundation models can be leveraged in downstream medical tasks to enhance the accuracy and efficiency of medical image analysis, leading to more precise diagnosis and treatment decisions.

Abstract:Purpose of review: Last year marked the development of the first foundation model in ophthalmology, RETFound, setting the stage for generalizable medical artificial intelligence (GMAI) that can adapt to novel tasks. Additionally, rapid advancements in large language model (LLM) technology, including models such as GPT-4 and Gemini, have been tailored for medical specialization and evaluated on clinical scenarios with promising results. This review explores the opportunities and challenges for further advancements in these technologies. Recent findings: RETFound outperforms traditional deep learning models in specific tasks, even when only fine-tuned on small datasets. Additionally, LMMs like Med-Gemini and Medprompt GPT-4 perform better than out-of-the-box models for ophthalmology tasks. However, there is still a significant deficiency in ophthalmology-specific multimodal models. This gap is primarily due to the substantial computational resources required to train these models and the limitations of high-quality ophthalmology datasets. Summary: Overall, foundation models in ophthalmology present promising opportunities but face challenges, particularly the need for high-quality, standardized datasets for training and specialization. Although development has primarily focused on large language and vision models, the greatest opportunities lie in advancing large multimodal models, which can more closely mimic the capabilities of clinicians.

Abstract:Abstract Our knowledge of cancer biology has advanced with the characterization of distinct cell types and cell states within heterogeneous tumor environments. Many methods for imaging and sorting tumor cells require biomarker labels that alter cell characteristics and create a selection bias. The use of label-free single-cell methods should further improve cancer studies involving viable, unperturbed cells for downstream assays such as RNA-Seq, cell culture expansion, and functional assays. To eliminate biomarkers and enable broader assessment of cells, we developed REM-I, a platform that characterizes and sorts unlabeled single cells based on high-dimensional morphology. Cells are captured with brightfield imaging and processed in real-time by self-supervised deep learning models to generate quantitative AI embeddings representative of cell morphology. A significant technical challenge in building REM-I was developing an AI model based on extracted features from cell images without prior knowledge of cell types, cell preparation, or other application-specific knowledge. Accordingly, we developed the Human Foundation Model (HFM), a hybrid architecture that combines self-supervised learning (SSL) and morphometrics (computer vision) to extract 115 dimensional embeddings representing cell morphology from high-resolution REM-I cell images. SSL produces a foundation model with high generalization capacity that enables hypothesis-free sample exploration and efficient generation of application-specific models. Meanwhile, computer vision extracts features that represent measurable and interpretable concepts (e.g., cell size, shape, texture, intensity). The training process for the HFM self-supervised backbone model utilizes the discriminatory power of supervised tasks. Using synthetic cells and three cancer cell lines, we trained and then validated the reproducibility and generalization capabilities of the resulting model. Results show combining deep learning and morphometrics improve interpretability of data and enable rapid characterization and classification of tumor cells with high accuracy. We also report on the Deepcell® Axon data suite, a tool to analyze data and customize reusable workflows. This includes the ability to store and manage data, visualize high dimensional data as low-dimensional projections, and train classifiers to identify and sort cell populations. To enable compatibility with user-preferred downstream analysis pipelines, Axon provides data export options for images, plots, and embeddings. Our approach allows users of all skill levels to access and interpret AI-enabled morphological profiling. Applications of REM-I include hypothesis-free evaluation of heterogeneous tumor samples, label-free cancer cell enrichment, characterization of distinct cell states, and multi-omic integration. Abstract:Pathological scarring resulting from traumas and wounds, such as hypertrophic scars and keloids, pose significant aesthetic, functional and psychological challenges. This study provides a comprehensive transcriptomic analysis of these conditions, aiming to illuminate underlying molecular mechanisms and potential therapeutic targets. We employed a co-expression and module analysis tool to identify significant gene clusters associated with distinct pathophysiological processes and mechanisms, notably lipid metabolism, sebum production, cellular energy metabolism and skin barrier function. This examination yielded critical insights into several skin conditions including folliculitis, skin fibrosis, fibrosarcoma and congenital ichthyosis. Particular attention was paid to Module Cluster (MCluster) 3, encompassing genes like BLK, TRPV1 and GABRD, all displaying high expression and potential implications in immune modulation. Preliminary immunohistochemistry validation supported these findings, showing elevated expression of these genes in non-fibrotic samples rich in immune activity. The complex interplay of different cell types in scar formation, such as fibroblasts, myofibroblasts, keratinocytes and mast cells, was also explored, revealing promising therapeutic strategies. This study underscores the promise of targeted gene therapy for pathological scars, paving the way for more personalised therapeutic approaches. The results necessitate further research to fully ascertain the roles of these identified genes and pathways in skin disease pathogenesis and potential therapeutics. Nonetheless, our work forms a strong foundation for a new era of personalised medicine for patients suffering from pathological scarring.

Abstract:The use of self-supervised learning (SSL) to train pathology foundation models has increased substantially in the past few years. Notably, several models trained on large quantities of clinical data have been made publicly available in recent months. This will significantly enhance scientific research in computational pathology and help bridge the gap between research and clinical deployment. With the increase in availability of public foundation models of different sizes, trained using different algorithms on different datasets, it becomes important to establish a benchmark to compare the performance of such models on a variety of clinically relevant tasks spanning multiple organs and diseases. In this work, we present a collection of pathology datasets comprising clinical slides associated with clinically relevant endpoints including cancer diagnoses and a variety of biomarkers generated during standard hospital operation from two medical centers. We leverage these datasets to systematically assess the performance of public pathology foundation models and provide insights into best practices for training new foundation models and selecting appropriate pretrained models.

Abstract:Robust medical Machine Learning (ML) models have the potential to revolutionize healthcare by accelerating clinical research, improving workflows and outcomes, and producing novel insights or capabilities. Developing such ML models from scratch is cost prohibitive and requires substantial compute, data, and time (e.g., expert labeling). To address these challenges, we introduce Health AI Developer Foundations (HAI-DEF), a suite of pre-trained, domain-specific foundation models, tools, and recipes to accelerate building ML for health applications. The models cover various modalities and domains, including radiology (X-rays and computed tomography), histopathology, dermatological imaging, and audio. These models provide domain specific embeddings that facilitate AI development with less labeled data, shorter training times, and reduced computational costs compared to traditional approaches. In addition, we utilize a common interface and style across these models, and prioritize usability to enable developers to integrate HAI-DEF efficiently. We present model evaluations across various tasks and conclude with a discussion of their application and evaluation, covering the importance of ensuring efficacy, fairness, and equity. Finally, while HAI-DEF and specifically the foundation models lower the barrier to entry for ML in healthcare, we emphasize the importance of validation with problem- and population-specific data for each desired usage setting. This technical report will be updated over time as more modalities and features are added.

A General-Purpose Multimodal Foundation Model for Dermatology

Abstract:Diagnosing and treating skin diseases require advanced visual skills across multiple domains and the ability to synthesize information from various imaging modalities. Current deep learning models, while effective at specific tasks such as diagnosing skin cancer from dermoscopic images, fall short in addressing the complex, multimodal demands of clinical practice. Here, we introduce PanDerm, a multimodal dermatology foundation model pretrained through self-supervised learning on a dataset of over 2 million real-world images of skin diseases, sourced from 11 clinical institutions across 4 imaging modalities. We evaluated PanDerm on 28 diverse datasets covering a range of clinical tasks, including skin cancer screening, phenotype assessment and risk stratification, diagnosis of neoplastic and inflammatory skin diseases, skin lesion segmentation, change monitoring, and metastasis prediction and prognosis. PanDerm achieved state-of-the-art performance across all evaluated tasks, often outperforming existing models even when using only 5-10% of labeled data. PanDerm's clinical utility was demonstrated through reader studies in real-world clinical settings across multiple imaging modalities. It outperformed clinicians by 10.2% in early-stage melanoma detection accuracy and enhanced clinicians' multiclass skin cancer diagnostic accuracy by 11% in a collaborative human-AI setting. Additionally, PanDerm demonstrated robust performance across diverse demographic factors, including different body locations, age groups, genders, and skin tones. The strong results in benchmark evaluations and real-world clinical scenarios suggest that PanDerm could enhance the management of skin diseases and serve as a model for developing multimodal foundation models in other medical specialties, potentially accelerating the integration of AI support in healthcare.

Abstract:Foundation models, large-scale, pre-trained deep-learning models adapted to a wide range of downstream tasks have gained significant interest lately in various deep-learning problems undergoing a paradigm shift with the rise of these models. Trained on large-scale dataset to bridge the gap between different modalities, foundation models facilitate contextual reasoning, generalization, and prompt capabilities at test time. The predictions of these models can be adjusted for new tasks by augmenting the model input with task-specific hints called prompts without requiring extensive labeled data and retraining. Capitalizing on the advances in computer vision, medical imaging has also marked a growing interest in these models. To assist researchers in navigating this direction, this survey intends to provide a comprehensive overview of foundation models in the domain of medical imaging. Specifically, we initiate our exploration by providing an exposition of the fundamental concepts forming the basis of foundation models. Subsequently, we offer a methodical taxonomy of foundation models within the medical domain, proposing a classification system primarily structured around training strategies, while also incorporating additional facets such as application domains, imaging modalities, specific organs of interest, and the algorithms integral to these models.
