Abstract:Postoperative atrial fibrillation (poAF) remains a common and clinically significant complication following cardiac surgery, posing challenges to both patients and health care professionals. In their retrospective cohort study titled "Sex, Atrial Fibrillation, and Long-Term Mortality After Cardiac Surgery," 1 the authors shed light on the intricate interplay between sex, poAF, and long-term mortality after open heart surgery. By examining data from 2 tertiary care centers in Massachusetts and comprising more than 20 000 patients, the study provides valuable insights into the relationship between these variables. The findings of the study have significant implications for clinical practice, research, and policy. The authors report that while women were less likely to develop poAF compared with men following cardiac surgery, the women who did develop poAF faced a disproportionately higher risk of long-term mortality. This observed sex disparity in the association between poAF and long-term outcomes clarifies previous conflicting results reported in the literature and underscores the need for a deeper understanding of the underlying mechanisms driving these differences. 2 The higher risk of AF observed in men is multifactorial, and one factor that might partially explain this finding is the higher expression of repolarizing ion channel subunits that decrease repolarization time, shorten atrial refractoriness, and induce re-entry. 3 Additionally, the authors included the type of cardiac surgery in their model of long-term mortality, demonstrating an interaction between poAF and sex even after adjusting for this categorical variable. Knowing how sex affects the course of poAF and its association with future mortality is relevant for risk stratification. Precisely predicting poAF could enable timely preventive measures for susceptible individuals, potentially mitigating poAF-related complications. At the heart of this study lies the recognition that poAF, traditionally viewed as a largely self-limited event, may have long-term implications that extend beyond the immediate postoperative period. A previous meta-analysis of 2 458 010 patients 4 suggested a higher risk of early and long-term stroke and mortality for patients with poAF. New-onset poAF has been recognized as a risk factor for long-term persistent AF, 5 which might help to explain the association with adverse long-term outcomes. While the pathophysiological mechanisms underlying poAF are multifactorial, ranging from perioperative inflammation to autonomic dysregulation, the observed association between poAF and long-term mortality highlights the need to reevaluate our approach to postoperative care delivery. 2 Indeed, attributing the observed disparity solely to biological differences overlooks the potential role of variations in long-term postoperative care delivery, including disparities in surveillance, pharmacological management, and access to specialized cardiovascular care. 6 As evidenced by the findings of this study, such a one-size-fits-all approach is inadequate and may contribute to disparities in outcomes between men and women undergoing cardiac surgery. Moving forward, efforts to optimize postoperative care should prioritize a personalized, sex-specific approach that accounts for the unique biological and psychosocial factors influencing outcomes in women. 7 Furthermore, the observed association between poAF and long-term mortality highlights the importance of comprehensive, multidisciplinary care models prioritizing continuity of care. Beyond the immediate postoperative period, ongoing management and surveillance are critical to mitigating the long-term adverse events seen in individuals with poAF and optimizing outcomes for all patients. This necessitates a paradigm shift toward holistic, patient-centered care delivery models that prioritize risk stratification, early intervention, and shared decision-making. Although clinical models for predicting the risk of poAF have been created to identify high-risk patients, these models are significantly deficient due to their limited applicability, reliance on hard-to-obtain variables, and inclusion of information from variables occurring after surgery or even after poAF onset. Understanding the impact of sex and other fundamental demographic factors is essential for crafting a precise predictive model. In light of these findings, future research efforts should focus on elucidating the underlying mechanisms driving the observed disparities in poAF incidence and long-term mortality.
