Abstract:Background and Aims: Malignant biliary obstruction is always caused by tumors which are unresectable so that palliative stent placement is conducted for drainage of bile duct tree. Recently, irradiation stent with 125I seeds has been used to improve the stent patency and survival time of patients. We conducted this meta-analysis to evaluate the therapeutic efficacy and safety of biliary stent placement with 125I seeds compared with stent placement alone in patients with malignant biliary obstruction. Methods: We searched Pubmed, Web of Science, ClinicalTrials.gov, Cochrane Library, Embase and CNKI databases for all relevant studies up to 1 May 2020. Patient survival, stent patency, and adverse events were the primary outcome measured. Also, Review Manager 5.3 and Stata/SE15.0 were used to perform the analysis. Results: Eleven randomized controlled trials with a total of 767 patients were included for meta-analysis. Stent combined with 125I seeds showed lower risk of stent occlusion at 3 month (Odds Ratios(OR) = 0.15; 95%CI: 0.05-0.49, P =0.002), 6 month (OR = 0.18; 95%CI: 0.08-0.44, P = 0.0001), 9 month (OR = 0.10; 95%CI: 0.05-0.20, P < 0.00001) and 1 year (OR = 0.15; 95%CI: 0.07-0.31, P < 0.00001) and better mean survival (MD = 125days; 95% CI 91-159 days; P < 0.00001) compared with stent placement alone. Also, reconstructed Kaplan-Meier data demonstrated improved survival in patients treated with stent plus 125I seeds (hazard ratio(HR)= 1.886; 95% CI: 1.609 to 2.210; P < 0.0001) Moreover, our analysis did not show significant difference between the two groups about the risk of adverse events including abdominal pain, hemobilia, pancreatitis, cholangitis and cholecystitis. Conclusion: 125I seeds combined with stent demonstrated superior stent patency and improved survival time compared to stent alone with acceptable complications.
Endoscopic Airway Stenting for Tracheobronchial Stenosis and Tracheoesophageal Fistula

Abstract:Objective To evaluate the therapeutic efficacy and complications of endoscopic placement of nickel titanium alloy stents for the treatment of airway stenosis and tracheoesophageal fistula. Methods Nickel titanium alloy stents were placed into the airway of 21 patients with tracheobronchial stenosis under direct view of bronchoscope. Stents coated with membrane were used in patients complicated with tracheoesophageal fistula. Four patients with severe stenosis underwent tracheobronchial dilatations before stenting. Stents bound with radioactive iodine seeds were inserted in 3 of the patients with bronchogenic carcinoma. Results Dyspnea was relieved in all of the patients immediately after stenting and dilatation. Disappearance of cough during food taking was found in 2 patients with tracheoesophageal fistula after placement of coated stent. Stents were inserted successfully after dilatation in 4 paitents with severe stenosis. Diminished tumor growth was found in all the 3 patients who received stents bound with radioactive iodine seeds 1 month later. Blood stained sputum occurred in 2 patients immediately after stenting. Granuloma occurred in 2 patients in 1 month after stenting. Conclusion Endoscopic placement of nickel titanium alloy stents is a safe and useful measure for the treatment of airway stenosis and tracheoesophageal fistula. Tracheobronchial dilatations can make stenting more safe and convenient. Stents bound with radioactive iodine seeds are useful in brachytherapy of bronchogenic carcinoma.
Hybrid stenting with silicone Y stents and metallic stents in the management of severe malignant airway stenosis and fistulas

Abstract:Background: Airway stenting is frequently used in the palliative treatment of patients with advanced tumor-induced airway stenosis and fistulas. However, there is paucity of studies regarding the use of airway stents in restoring patency. The aim of the study was to assess the efficacy and safety of hybrid silicon Y stents and covered self-expanding metal stents (SEMS) and in reestablishing patency in airway stenoses and fistulas. Methods: This retrospective study included 31 patients between January 2016 to December 2019 with inoperable complex malignant airway stenoses and fistulas, managed with Silicone Y stents, and covered SEMS. The clinical details, clinical outcomes and complications up to 6 months were extracted from medical records. The improvement of performance was assessed based on modified British Medical Research Council (mMRC) dyspnea scores (t=6.892, P<0.001), Karnofsky Performance Scores (KPS) (t=-11.653, P<0.001), and performance status (PS) (t=3.503, P<0.001). Result: A total of 31 silicon Y stents and 35 covered SEMSs were inserted. Of the 31 patients (M:F 20:11; age: 54.64±9.57), 25/31 (80.6%) experienced immediate relief of symptoms following stent placement. Patients' mMRC dyspnea scores, KPS, and PS showed significant improvement following stenting. The mean duration of stent placement was 146.3±47.7 days, and 17/31 (55%) patients were alive at 6 months. No major complications related to hybrid stenting were observed during the follow-up period. Conclusions: Hybrid stenting is a feasible and safe palliative treatment for malignant airway stenosis and fistulas to improve quality of life and can be performed without major complications.
Dosimetric Evaluation and Clinical Application of Radioactive Iodine-125 Brachytherapy Stent in the Treatment of Malignant Esophageal Obstruction

Abstract:Objective To evaluate the dosimetric characteristics and the clinical application of radioactive iodine-125 brachytherapy stent (RIBS) in malignant esophageal obstruction. Methods The dose distribution of RIBS with different seed spacing, diameter and length was studied by treatment planning system (TPS) calculation, thermoluminescence dosimeter (TLD) measurement and Monte Carlo (MC) data fitting. And the data of esophageal cancer patients who were treat with this type of RIBS was analyzed retrospectively. Results Doses around the RIBS calculated by the TPS lay between those measured by the TLDs and those simulated by the MC, and the differences between the three methods were significant (p<0.05), the overall absolute dose differences among the three methods were small. Dose coverage at 1.5 cm from the center was comprehensive when the activity reached 0.6 mCi. Both the conformability and the uniformity of isodose lines produced by a seed spacing of 1.0 cm were superior to those produced by a seed spacing of 1.5 cm. The data of 50 patients treated with RIBS was analyzed. They were followed up until February 2020 when all of the patients died. The overall improvement rate of dysphagia after RIBS implant was 90%. Moderate and severe complications with an incidence of more than 10% were hematemesis (28%), pain (20%), and lung infection (10%). Stent restenosis occurred in 4 patients at a median interval of 108 days from the procedure. The overall incidence of fatal complications was 38% (including hematemesis, infection and asphyxia). The median survival time of patients with and without a history of radiotherapy were 3.4 months and 6 months, respectively, the difference of which was significant (p=0.021). No other factors affecting survival were identified. For patients with and without a history of radiotherapy, the incidences of fatal complications were 51.7% and 19%, respectively (p=0.019). No correlation between dose and stent restenosis was found. Conclusion TPS calculations are suitable for clinical applications. RIBS can effectively alleviate obstructive symptoms for patients with malignant esophageal obstruction, but the incidence of fatal complications was high, care should be taken when choosing this treatment.

Abstract:Objective: To observe clinical results of expandable esophageal stent loaded with 125I seeds for advanced esophageal cancer.Materials and Methods: 30 patients who suffered from advanced esophageal cancer received expandable esophageal stent loaded with 125I seeds deployment.Clinical and imaging follow up were performed post procedure.Results: Stents,diameter,18-20mm;length,6-10mm,were successfully deployed in 30 patients.Barium meal study pre procedure showed all patients had esophageal stenoses greater than 80%.Stenotic lumina recovered to 90-100% of normal diameter after stent placement.Partial response(n=15),progressive disease(n=5) and stable disease(n=10) were found in 30 patients at the third follow up month by using CT angiography.Overall survival was 6-20.5 month,mean 9.9 month.All patients had improved dysphagia after stent deployment.25 patients had Karnofsky score ≥60 after procedure compared with 11 patients before operation.Conclusion: Expandable esophageal stent loaded with 125I seeds was an ideal therapeutic approach for patients with advanced esophageal cancer.

Abstract:Background: The treatment of recurrent tracheal adenoid cystic carcinoma (TACC), a rare pulmonary malignant tumor, typically involves bronchoscopic interventional therapy for patients ineligible for surgery or external radiotherapy. This report describes an innovative treatment approach for TACC, initially managed with interventional bronchoscopy and subsequently with a Y-shaped airway stent loaded with 125 I seeds, following recurrence after 2 years. Case presentation: A 50-year-old man presented with intermittent coughing for 2 months and was admitted to the hospital after the discovery of TACC a month earlier. Preoperative bronchial artery embolization was performed to reduce the risk of bleeding, followed by bronchoscopic tumor resection in January 2017 using an electric loop at the base, complemented by argon plasma coagulation (APC) and cryotherapy. Although short-term outcomes were favorable, the long-term prognosis remained suboptimal. Due to tumor recurrence, airway tumor resection and implantation of the 125 I seed stent were performed under bronchoscope guidance in early 2019. The stent was removed 5 months later. The patient tolerated the procedure well and exhibited no signs of relapse or progression during the 5-year postoperative follow-up period. This case illustrates the successful application of a stent loaded with 125 I seeds for treating unresectable TACC. Conclusions: Treatment of TACC with a covered airway stent loaded with 125 I seeds may be a safe, effective, and scalable method.

Abstract:Objective: To explore the efficacy and safety of the covered airway stent loaded with (125)I seeds for the treatment of tracheal adenoid cystic carcinoma (TACC). Methods: We retrospectively reviewed the clinical data from 8 patients with TACC who had received placement of the covered stent loaded with (125)I seeds between December 2014 and July 2017 in the endoscopic center of the Second Affiliated Hospital of Xiamen Medical College. We compared the difference in the dyspnea index, the diameter of the airway lumen, and the lesion surrounding the airway wall before and after treatment. The complications were also recorded during follow-up. Results: Eight patients underwent successful placement of a total of 11 radioactive stents (2 straight-type stents, 2 L-shape stents, and 7 Y-shape stents, all loaded a total of 243 radioactive particles). Displacement of stents took place within 2 weeks in 2 patients, who were managed with re-stenting and fixation. No further displacement occurred during follow-up. The median time to stent removal was 2.9(interquartile range: 2.3,3.0) months. After stent placement, the dyspnea index was significantly decreased compared with pre-treatment level (mean: 0.1 vs. 3.4, t=8.881, P<0.001). Bronchoscopic re-assessment showed that the residual tumor within the airway was detected in only one patient and that the tumor completely disappeared in the remaining 7 patients. Treatment with stents loaded with radioactive particles yielded smooth and pale airway mucosa with formation of partial scar formation. Chest computed tomography re-assessment demonstrated significantly larger luminal diameter than that before treatment (mean: 13.1 mm vs. 3.3 mm, t=-7.839, P<0.001). The airway wall thickness was notably reduced after treatment (mean: 4.3 mm vs. 14.4 mm, t=7.620, P<0.001). The lesions surrounding the airway wall completely disappeared in 7 patients and decreased for more than 50% in a single patient. The median duration of follow-up was 28.0(interquartile range: 24.8,31.5) months. Recurrence of tumor was documented in a single case within 2 years. Six patients did not experience recurrence within the 2-year follow-up period. No death or severe complications were recorded during follow-up. Conclusion: The (125)I radioactive stent is effective for dilating the stenotic airway and ameliorating the symptoms, and thus might be an effective and safe method for the treatment of TACC. Further studies that explore the efficacy of stents loaded with (125)I particles are needed.
Central airway obstruction treatment with self-expanding covered Y-carina nitinol stents: A single center retrospective analysis

Abstract:Background: Central airway obstruction (CAO) is one of the most challenging, potentially lethal complications in malignant and benign respiratory diseases. Worsening dyspnea is also a relevant cause for reduced quality of life in such patients. Here, we present our data on the application of covered, self-expanding Y-carina nitinol stents due to benign and malignant diseases. Methods: We retrospectively identified 27 patients who had undergone 31 rigid bronchoscopies with implantation of covered Y-carina nitinol stents over a period of 10 years in order to evaluate indication, clinical course, and outcome. Results: Short-term survival of successfully stented patients with palliative and curative treatment goal did not differ, allowing for diagnosis independent indication. With respect to overall survival, patients with endoluminal obstruction benefited most compared to patients with fistula and/or external compression. Granulation tissue formation (61.3%) and mucus plugging (80.6%) were the most frequent complications. Material defect (6.5%) and migration (3.2%) were rare complications that could be handled by revisional rigid bronchoscopy and stent exchange in some cases. Conclusions: Implantation of self-expanding covered Y-carina nitinol stents via rigid bronchoscopy is a feasible and safe treatment option for benign and malignant central airway obstruction. Especially in palliative, malignant airway stenosis, stenting might facilitate additional treatment options and optimize dyspnea and eventually quality of life.
Different irradiation dosages of ~(125)I seeds in the management of malignant airway stenosis

Abstract:Objective:To evaluate the effectiveness and safety of two different irradiation dosages of endobronchial ~ 125I seeds for malignant airway stenosis induced by central lung cancer. Methods: Endobronchial brachytherapy was performed by implanting and suspending one (Group A) or two rows (Group B) of ~ 125I seeds in airway via cricothyroid membrane puncture. Radiotherapy lasted three months. Results:Of 22 cases,airway stenosis was ameliorated in 63% patients in group A and 81% patients in group B (complete response rate+part response rate). There was no significant difference between the two groups(P> 0.05). The respiratory symptoms and chest pain were relieved in both groups with no significant difference(P>0.05). But the percentage of peak expiratory flow (PEF%) was significantly different between the two groups(P<0.05). Conclusion:Suspension of ~ 125I seeds in airway is effective in management of malignant airway stenosis. No significant difference was found between the two irradiation dosages of ~ 125I seeds. Implantation of two rows of ~ 125I seeds was more helpful to improve lung function.
Computed Tomography Fluoroscopy-Guided Percutaneous 125I Seed Implantation for Safe, Effective and Real-Time Monitoring Radiotherapy of Inoperable Stage T1-3N0M0 Non-Small-cell Lung Cancer.

Abstract:The management of inoperable lung cancer remains a challenge. It has been proven that computed tomography (CT)-guided iodine-125 (125I) seed implantation is a safe and efficient method for treating lung cancer. Computed tomographic fluoroscopy (CTF) is superior to traditional CT for percutaneous management of lung lesions, due to the real-time guidance and accurate localization of the lesions. The aim of the present prospective study was to evaluate the feasibility, safety and efficacy of CTF-guided percutaneous permanent implantation of 125I seeds for the treatment of selected patients with inoperable stage T1-3N0M0 non-small-cell lung cancer (NSCLC). A total of 24 patients with resectable but inoperable stage T1-3N0 NSCLC, with a total of 28 lesions, underwent CTF-guided percutaneous implantation of radioactive 125I seeds. A prescription dose of 100-120 Gy was delivered to each lesion. The complications and local tumor control rates were documented. Survival was estimated using the Kaplan-Meier method. All the patients successfully completed the procedure, with a mean procedure duration of 45.7 min (range, 30-75 min). No severe complications occurred. Small asymptomatic pneumothorax with lung volume compression of <10% and minor hemorrhage along the needle track without hemoptysis occurred immediately after the procedure in 3 (12.5%) and 4 (16.7%) of the 24 patients, respectively. At a median follow-up of 31.5 months (range, 8-46 months), the local control rate (LCR) of the lesions was 78.6% (22/28). The 1-, 2- and 3-year overall survival rate was 95.8, 78 and 55%, respectively. In conclusion, CTF is the favourable imaging guidance method for the percutaneous implantation of 125I seeds. CTF-guided brachytherapy with implantation of 125I seeds is a safe, feasible and effective modality for the treatment of inoperable early-stage NSCLC and may be considered an alternative option in selected patients with medically inoperable NSCLC.
Clinical effect and safety of 125I particle stent on treatment of postoperative esophageal stricture for patients with advanced esophageal cancer

Abstract:Objective To investigate clinical effect and safety of the 125I particle stent for treatment of postoperative esophageal stricture on patients with advanced esophageal cancer.Methods Sixty patients with postoperative esophageal stricture after esophageal cancer operation were randomly assigned to receive treatment with a conventional self-expanding stent (control group) or a stent loaded with 125I seeds (irradiation group).After stent implantation,the outcomes were compared in terms of the success rate of stent implantation,relief of dysphagia,survival time,and complications related to the procedure.Dysphagia was assigned a grade 0-4.The t test and the x2 test were used to make statistical analysis.Results The success rate of stents placement of two groups were 100％ (30/30).The results of preoperative dysphagia score showed that the patients of control group with 0,1,2,3,4 degree were 0,2,3,22 and 3 patients ; the patients of radiation group with 0,1,2,3,4 degree were 0,2,2,23 and 3 patients; in postoperative 1 week,the control group turned to 1,17,6,6 and 0 patients,the radiation group turned to 4,21,3,2 and 0 patients ; the difference between the 2 groups was not statistically significant (x2 =1.214,P =0.675) ; in postoperative 2 months,the patients with the above degree of dysphagia score of control group were 25,3,2,0,0; and the radiation group were 27,2,1,0,0,the difference between the 2 groups was not statistically significant (x2 =1.043,P =0.894).Followed up for 6 months,1 patient gastrointestinal bleeding,4 patients gastrointestinal pain and 8 patients esophageal restenosis were occurred in control group,corresponding in irradiated group were 0,5,and 3 patients respectively,the incidence of postoperative esophageal restenosis of irradiated group was lower than control group,and the difference was statistically significant (x2 =4.384,P =0.036).After follow-up for 18 months,10 patients were survival among the 30 patients of control group,the live time of the 20 patients dead patients were 2 to 14 months,control group of 30 patients,10 patients of survival of 20 deaths of patients from 2 to 14 months,the average value was (4.8± 1.7)months ; there were 15 survival patients among irradiation group,the survival time of the 15 deaths was 4 to 15 months,and the average value was (9.8 ± 2.9) months.By the end of follow-up,the average survival time for all patients of irradiated group was longer than the control group,the difference was statistically significant (t =8.917,P =0.002).Conclusion In patients with postoperative esophageal stricture and advanced esophageal cancer,stent loaded with 125I stent has potential benefit in that it allows a longer relief of dysphagia and a longer survival time. Treating cancerous large airway stenosis with staging radioactive particle implantation guided by computed tomography and fiber bronchoscopy: a clinical study

Abstract:Background: The purpose of this study is to investigate the clinical effectiveness of staging radioactive particle implantation guided by computed tomography (CT) and fiber-optic bronchoscopy in treating cancerous large airway stenosis. Methods: A total of 102 patients were included; 57 had undergone staging radioactive particle implantation guided by CT and fiber bronchoscopy and 45 did not. Patients were evaluated by CT and fiber-optic bronchoscopy to determine the feasibility of the implantation of radioactive seeds for the treatment of cancerous large airway stenosis. The treatment planning system (TPS) was used to plan the doses. Radioactive seeds were implanted using fiber-optic bronchoscopy. One week later, CT-guided implantation of radioactive seeds was performed. Results: The clinical evaluation showed complete, partial, slight, and non-response in 38, 14, 5, and 0 patients, respectively. None of the patients were found with serious complications. The diameter of the affected airway, Karnofsky score, dyspnea index, survival, and quality of life of the patients in both groups was significantly higher and significantly different after the treatment (P < 0.05). The dyspnea index was significantly lower in the treatment group as compared with the control group (P < 0.001). Conclusion: CT- and fiber bronchoscopy-guided staging radioactive particle implantation has definite treatment effectiveness in treating cancerous large airway stenosis. It should be widely used in clinical practices.

Abstract:OBJECTIVE This study aimed to investigate the clinical efficacy and feasibility of the treatment of advanced esophageal cancer with a combination of a 125I particle-integrated esophageal covered stent and hyperbaric oxygen. METHODS 45 patients with advanced esophageal cancer were enrolled and were randomly divided into two groups: a treatment group and a control group. Patients in the treatment group were treated with a 125I particle-integrated esophageal covered stent and hyperbaric oxygen, while patients in the control group were treated with a 125I particle-integrated esophageal covered stent, and the clinical effects and long-term survival time of the two groups were observed. RESULTS In the treatment group, the complete remission (CR) rate and partial remission (PR) rate of local lesions were 19.2% and 61.5% respectively, and the total effective rate was 80.7%. In the control group, the CR rate and PR rate of local lesions were 10.5% and 52.6%, respectively, and the total effective rate was 63.1%. The total effective rate was higher in the treatment group than in the control group, and the difference was statistically significant (P < 0.05). CONCLUSION The combination of a 125I particle-integrated esophageal covered stent and hyperbaric oxygen shows good short- and long-term efficacy in the treatment of advanced esophageal cancer.

Abstract:Background: Post-intubation subglottic stenosis (PI-SGS) is a fatal disease which could result in partial or complete narrowing of the airway. Although airway stenting is commonly used as an alternative treatment for PI-SGS patients unsuitable for surgery, complications including stent migration and excessive granulation tissue formation are frequently encountered. Additionally, tracheotomy is necessary in patients undergoing T-tube placement. Therefore, it is necessary to further enhance the effectiveness of airway stenting in refractory PI-SGS. In this study, we aimed to evaluate the safety and effectiveness of utilizing short bronchial Dumon (BD) stents in managing refractory PI-SGS. Methods: PI-SGS patients who were not suitable for surgery and in whom previous interventional treatments had proven ineffective were enrolled. Short BD stents were inserted via rigid bronchoscopy under general anesthesia. Complications and outcomes were assessed by follow-ups. Results: Fourteen patients were included and successful stent insertion was achieved in all cases. The median diameter and length of stents was 12 (0.25) and 33.5 (5) mm, respectively. During the 6-month follow-up period, complications were reported in five patients. Granulation tissue formation was the most frequently observed complication (4 in 14 patients, 28.57%), followed by stent migration (2 in 14 patients, 14.29%). Out of the total participants, 11 patients (78.57%) demonstrated good tolerance to the stent, while 3 (21.43%) required stent removal. Among these three patients, two finally underwent subsequent T-tube insertion after the removal. Twelve patients (85.71%) avoided the tracheotomy and T-tube insertion. Conclusions: The utilization of short BD stents appears to be a safe and effective approach for managing refractory PI-SGS. The complications and tolerability are acceptable.
Clinical Study of Airway Stent Implantation in the Treatment of Patients with Malignant Central Airway Obstruction

Abstract:Background: Airway stenting is a therapeutic option for malignant central airway obstructions (MCAO), including both intraluminal and extraluminal obstructions. The objective of this study is to investigate the clinical features and results of long-term improved prognosis for MCAO patients after airway stent implantation. Methods: Ninety-eight MCAO patients who underwent stent placement in our hospital from January 2013 to April 2020 were included in this study. The data included baseline data, clinical characteristics, laboratory test data, stent implantation data, and treatment as well as survival after stent implantation. The survival rates among individuals were compared via log-rank tests. Potential prognostic factors were identified using multivariate cox hazard regression models. Results: A retrospective analysis of these patients was generated. MCAO was mainly caused by lung cancer (53/98, 54.08%), esophageal cancer (22/98, 22.45%), and thyroid cancer (3/98, 3.06%). The median survival time of participants was 5.5 months. Univariate analysis indicated that the survival rate was related to primary disease, ECOG PS score, stent site, hemoglobin (Hb), albumin (ALB), and serum lactate dehydrogenase (LDH) (P < 0.05). The cox risk regression model showed that the survival rate was significantly influenced by ECOG PS score (OR = 3.468, 95%CI = 1.426-8.432, P = 0.006) and stent site (OR = 1.544, 95%CI = 1.057-2.255, P = 0.025). Conclusions: Compared with the site of stent placement, the ECOG PS score is the primary factor in the survival rate of MCAO patients after airway stenting.

Abstract:Background: Airway stent placement has been proven as a satisfactory treatment of advanced malignant airway lesions. Flowever, stent-related complications could impact the therapeutic effect of this treatment. This study aimed to assess the application of airway stents, including the clinical effects, and to explore high-risk factors for stent-related complications. Methods: A retrospective cohort study of lung cancer (LC) patients and esophageal cancer (EC) patients was conducted at our center. Patients who received stent placement for malignant airway lesions from January 2014 to July 2017 were included in the study. Patient clinical status and stent-related complications, including granulation tissue formation, restenosis, atelectasis, migration, mucous plugging and infection were comprehensively analyzed. Results: Fifty-six symptomatic patients who underwent 66 stent procedures were included. There was an immediate relief of symptoms after stent placement, and the Karnofsky Performance Status (KPS) scores significantly improved (56.67 +/- 23.52 versus 79.05 +/- 20.71; P<0.001). Among all the patients, general anesthesia (P=0.038) and pre-stent non-surgical therapy (P=0.048) were risk factors for granulation. A Charlson comorbidity index (CCI) <3 (P=0.008) and a procedure duration time >110 min (P=0.005) were associated with an increased risk of restenosis. Stent placement only in the main trachea (P=0.049) increased the risk of stent migration. Stent placement in the carina or upper airways (P=0.041) increased the risk of mucous plugging. The stent length (>60 mm) had a direct correlation with both mucous plugging in LC patients (P=0.003) and granulation tissue formation in EC patients (P=0.019). Conclusions: Airway stent placement immediately and significantly improved the clinical symptoms for patients with advanced malignant airway lesions. High-risk factors for different stent-related complications were identified, which provided evidence for further clinical improvement with airway stents.

Abstract:Background Self-expanding metallic stent (SEMS) is a palliative therapy for patients with malignant central airway obstruction (CAO) or tracheoesophageal fistula (TEF). Despite this, many patients experience death shortly after SEMS placement. Aims We aimed to investigate the effect of SEMS on the palliative treatment between malignant CAO and malignant TEF patients and investigate the associated prognostic factors of the 3-month survival. Methods We performed a single-center, retrospective study of malignant CAO or TEF patients receiving SEMS placement. Clinical data were collected using the standardized data abstraction forms. Data were analyzed using SPSS 22.0. A two-sided P -value <0.05 was statistically significant. Results 106 malignant patients (82 CAO and 24 TEF) receiving SEMS placement were included. The body mass index (BMI), hemoglobin levels, and albumin levels in the malignant TEF group were lower than in the malignant CAO group (all P < 0.05). The procalcitonin levels, C-reactive protein levels, and the proportion of inflammatory lesions were higher in the malignant TEF group than in the malignant CAO group (all P < 0.05). The proportion of symptomatic improvement after the SEMS placement was 97.6% in the malignant CAO group, whereas 50.0% in the malignant TEF group, with a significant difference ( P = 0.000). Three months after SEMS placement, the survival rate at was 67.0%, significantly lower in the malignant TEF group than in the malignant CAO group (45.8% vs. 73.2%, P = 0.013). Multivariate analysis revealed that BMI [odds ratio (OR) = 1.841, 95% certificated interval (CI) (1.155-2.935), P = 0.010] and neutrophil percentage [OR = 0.936, 95% CI (0.883–0.993), P = 0.027] were the independent risk factors for patients who survived three months after SEMS placement. Conclusions We observed symptom improvement in malignant CAO and TEF patients after SEMS placement. The survival rate in malignant TEF patients after SEMS placement was low, probably due to aspiration pneumonitis and malnutrition. Therefore, we recommend more aggressive treatment modalities in patients with malignant TEF, such as strong antibiotics, nutrition support, and strategic ventilation. More studies are needed to investigate the prognostic factors in patients with malignant airway disorders receiving SEMS placement.

Abstract:Abstract The purpose of this clinical study was to investigate the efficacy and safety of airway stent implantation and transarterial infusion chemotherapy in the sequential treatment of severe airway stenosis caused by esophageal cancer. Data of patients with advanced esophageal cancer complicated by severe airway stenosis treated with airway stent implantation and transarterial infusion chemotherapy were retrospectively analyzed. Furthermore, dyspnea, clinical efficacy, adverse reactions, and survival of patients were evaluated. 71 patients were included in this study. There were 28 patients with grade III dyspnea and 43 patients with grade IV dyspnea before airway stent implantation, and 34 patients with grade I dyspnea, 35 patients with grade II dyspnea and 2 patients with grade III dyspnea after airway stent implantation. After airway stent implantation and 1–3 courses of transarterial infusion chemotherapy, 11, 41 and 19 patients had complete response, partial response and stable response respectively. Total disease control rate (DCR) and objective response rate (ORR) were 100.0% and 73.2%, respectively. During the follow-up, 32 patients died of organ failure, 24 patients died of tumor-related respiratory failure, and 10 patients died of gastrointestinal bleeding. The median survival time of all patients was 8 months, and the 1-year survival rate was 40.8%. Airway stent implantation combined with arterial infusion chemotherapy is safe and effective for sequential treatment of esophageal cancer with severe airway stenosis.

Abstract:This study assessed the efficacy and safety of radioactive iodine-125 seed ablation brachytherapy (RSABT) in comparison to microwave ablation therapy (MWAT) for treating inoperable stage I non-small cell lung cancer (NSCLC). We conducted a retrospective analysis of data from stage I NSCLC patients who underwent CT-guided RSABT or MWAT. The primary outcomes measured were progression-free survival (PFS), overall survival (OS), and the occurrence of adverse events. Of the patients included in the study, 71 underwent RSABT and 105 received MWAT. The median follow-up time for these groups was 47.4 months and 60 months, respectively. The PFS rates at 1-year, 3-year, and 5-year for the RSABT group were 87.3%, 72.6%, and 65.8%, while for the MWAT group, they were 89.5%, 69.3%, and 43.7%, respectively (P = 0.011). The OS rates at 1-year, 3-year, and 5-year for the RSABT group were 97.2%, 78.1%, and 66.1%, and for the MWAT group, they were 99%, 75.8%, and 55%, respectively (P = 0.112). Upon multivariate analysis, the treatment modality was identified as an independent predictor of PFS (P = 0.008). Additionally, both sex and T stage were found to be independent predictors of both PFS and OS (P < 0.05). Adverse events, such as pneumothorax, occurred in 50% of the MWAT group and 39% of the RSABT group (P = 0.313). The incidence of pleural effusion was 44% in the MWAT group compared to 14% in the RSABT group (P < 0.001). Needle bleeding was observed in 32% of the RSABT group and 5% of the MWAT group (P < 0.001). We conclude RSABT demonstrates promising efficacy and safety in the treatment of stage I NSCLC. However, further studies are essential to validate these preliminary findings.
