Lymphoproliferative disorders in renal transplant recipients: Virologic aspects

Abstract:Most cases of post‐transplant lymphoproliferative disorders are linked to infection with Epstein‐Barr virus (EBV). We examined the relationship between the use of polyclonal and monoclonal anti‐lymphocyte globulins, pretransplant EBV serology and PTLD in cadaveric renal transplant recipients. We found that EBV‐seronegative patients were at significantly increased risk of developing PTLD and this risk was further amplified by the use of anti‐lymphocyte globulins, particularly when they are used for both induction and rejection therapy. The results of a prospective study of quantitative oropharyngeal EBV shedding in renal and cardiac transplant recipients is reported. The relationship between EBV shedding, immunosuppressive regimens, antiviral drugs, serologic responses and the risk of PTLD is examined.
