Abstract:In 1992, the Committee on Neurotoxicology and Models for Assessing Risk of the National Academy of Sciences in Washington DC focused with a scientific perspective on the identification of substances with neurotoxic potential, studies of exposed populations, risk assessment, and biologic markers of disease. This Committee recommended: " all physicians should be trained to take a thorough occupational-exposure history and to be aware of other possible sources of toxic exposure ". Although convened after several outbreaks of neurotoxic syndromes, clinical neurological considerations were lacking. After defining keys words, namely Environment, Neurotoxicology and Neurotoxicants, we present some demonstrative cases; e.g., the Epidemic Neuropathy in Cuba, Minamata disease, ALS/PDC on Guam, and the ALS hot spot in the French Alps. Always with a clinical and practical approach, we will then review the milieux that contain and convey potential neurotoxicants, the different exposure routes and the clinical presentations. Drawing lessons from clinical cases, we offer some thoughts concerning the future of Environmental Neurotoxicology (ENT). Pointing notably to the diffuse chemical contamination of ecosystems and living beings, including Homo sapiens , we question the real impact of agents with neurotoxic potential on the human brain, considering the effects, for example, of air pollution, endocrine disruptors and nanoparticles. Concern is expressed over the lack of knowledge of the non-monotonic kinetics of many of these chemicals, the major concern being related to mixtures and low-dose exposures, as well as the delayed appearance in clinical expression of prevalent neurodegenerative diseases.

Abstract:Over time it became clear that exposure to chemical substances is complex in nature. We have come to understand that chemicals are distributed between the gas phase, airborne particles, settled dust and exposed surfaces, in a manner dependent on their respective physical properties. In terms of deposition and its effect, whether a molecule enters the lung via the gas phase or the particle phase makes a significant difference. The oral intake of substances from house dust is strongly dependent on the occupant’s age and habit. The importance of the dermal intake pathway via the room air is now known and is the subject of several studies. Furthermore, we have also learned that secondary products from chemical reactions in the gas phase and on surfaces also contribute to exposure. Today, biomonitoring methods are available for many of the substances, which can be found in indoors. The identification of priority chemicals and the development of methods for these, with help from expert groups, is undoubtedly also a purposeful and seminal approach. With this contribution, which bases on a recent review paper, the authors hope to demonstrate that research addressing the sources, redistribution and fate of chemicals found indoors and human biomonitoring not only complement one another but also belong together within the field of exposure analysis. With the appropriate experimental design, external and internal exposure as well as the intake and uptake of a substance can be determined. A further important aspect is the post hoc analysis of house dust and urine samples, which are stored in environmental specimen banks. The identification and temporal tracking of so-called emerging chemicals is thereby enabled. This, in turn, creates the possibility of taking a virtual look into the future and acting preemptively, instead of constantly just looking back into the past and watching the decay of substances which have not been utilized for a considerable length of time.

Abstract:Endocrine disrupting chemicals are a group of pollutants that can affect the endocrine system and lead to diseases and dysfunctions across the lifespan of organisms. They are omnipresent. They are in the air we breathe, in the food we eat and in the water we drink. They can be found in our everyday lives through personal care products, household cleaning products, furniture and in children’s toys. Every year, hundreds of new chemicals are produced and released onto the market without being tested, and they reach our bodies through everyday products. Permanent exposure to those chemicals may intensify or even become the main cause for the development of diseases such as type 2 diabetes, obesity, cardiovascular diseases and certain types of cancer. In recent years, legislation and regulations have been implemented, which aim to control the release of potentially adverse endocrine disrupting chemicals, often invoking the precautionary principle. The objective of this review is to provide an overview of research on environmental aspects of endocrine disrupting chemicals and their effects on human health, based on evidence from animal and human studies. Emphasis is given to three ubiquitous and persistent groups of chemicals, polychlorinated biphenyls, polybrominated diphenyl ethers and organochlorine pesticides, and on two non-persistent, but ubiquitous, bisphenol A and phthalates. Some selected historical cases are also presented and successful cases of regulation and legislation described. These led to a decrease in exposure and consequent minimization of the effects of these compounds. Recommendations from experts on this field, World Health Organization, scientific reports and from the Endocrine Society are included.

Abstract:Exposure science plays an essential role in the U.S. Environmental Protection Agency's (U.S. EPA) mission to protect human health and the environment. The U.S. EPA's Center for Public Health and Environmental Assessment (CPHEA) within the Office of Research and Development (ORD) provides the exposure science needed to characterize the multifaceted relationships between people and their surroundings in support of national, regional, local and individual-level actions. Furthermore, exposure science research must position its enterprise to tackle the most pressing public health challenges in an ever-changing environment. These challenges include understanding and confronting complex human disease etiologies, disparities in the social environment, and system-level changes in the physical environment. Solutions will sustainably balance and optimize the health of people, animals, and ecosystems. Our objectives for this paper are to review the role of CPHEA exposure science research in various recent decision-making contexts, to present current challenges facing U.S. EPA and the larger exposure science field, and to provide illustrative case examples where CPHEA exposure science is demonstrating the latest methodologies at the intersection of these two motivations. This blueprint provides a foundation for applying exposomic tools and approaches to holistically understand real-world exposures so optimal environmental public health protective actions can be realized within the broader context of a One Health framework.

Endocrine-disrupting chemicals: Mainstream recognition of health effects and implications for the practicing internist

Abstract:Rapidly advancing evidence documents that a broad array of synthetic chemicals found ubiquitously in the environment contribute to disease and disability across the lifespan. Although the early literature focused on early life exposures, endocrine-disrupting chemicals (EDCs) are now understood to contribute substantially to chronic disease in adulthood, especially metabolic, cardiovascular, and reproductive consequences as well as endocrine cancers. The contribution to mortality is substantial, with over 90,000 deaths annually and at least $39 billion/year in lost economic productivity in the United States (US) due to exposure to certain phthalates that are used as plasticizers in food packaging. Importantly, exposures are disproportionately high in low-income and minoritized populations, driving disparities in these conditions. Though non-Hispanic Blacks and Mexican Americans comprise 12.6% and 13.5% of the US population, they bear 16.5% and 14.6% of the disease burden due to EDCs, respectively. Many of these exposures can be modified through safe and simple behavioral changes supported by proactive government action to both limit known hazardous exposures and to proactively screen new industrial chemicals prior to their use. Routine healthcare maintenance should include guidance to reduce EDC exposures, and a recent report by the Institute of Medicine suggests that testing be conducted, particularly in populations heavily exposed to perfluoroalkyl substances-chemicals used in nonstick coatings as well as oil- and water-resistant clothing.

Abstract:Climate change is anticipated to alter the production, use, release, and fate of environmental chemicals, likely leading to increased uncertainty in exposure and human health risk predictions. Exposure science provides a key connection between changes in climate and associated health outcomes. The theme of the 2015 Annual Meeting of the International Society of Exposure Science-Exposures in an Evolving Environment-brought this issue to the fore. By directing attention to questions that may affect society in profound ways, exposure scientists have an opportunity to conduct "consequential science"-doing science that matters, using our tools for the greater good and to answer key policy questions, and identifying causes leading to implementation of solutions. Understanding the implications of changing exposures on public health may be one of the most consequential areas of study in which exposure scientists could currently be engaged. In this paper, we use a series of case studies to identify exposure data gaps and research paths that will enable us to capture the information necessary for understanding climate change-related human exposures and consequent health impacts. We hope that paper will focus attention on under-developed areas of exposure science that will likely have broad implications for public health.

Abstract:Around two-thirds of chronic human disease is not explainable by genetics alone. The Lancet Commission on Pollution and Health estimates that 16% of global premature deaths are linked to pollution. Additionally, it is now thought that humankind has surpassed the safe planetary operating space for introducing human-made chemicals into the Earth System. Direct and indirect exposure to a myriad of chemicals, known and unknown, poses a significant threat to biodiversity and human health, from vaccine efficacy to the rise of antimicrobial resistance as well as autoimmune diseases, and mental health disorders. The exposome chemical space remains largely uncharted due to the sheer number of possible chemical structures, estimated at over 10^60 unique forms. Conventional methods have cataloged only a fraction of the exposome, overlooking transformation products and often yielding uncertain results. In this perspective we have reviewed the latest efforts in mapping the exposome chemical space and its sub-spaces. We also provide our view on how the integration of data driven approaches might be able to bridge the identified gaps.

Abstract:Artificial chemical products are widely used and ubiquitous worldwide and pose a threat to the environment and human health. Accumulating epidemiological and toxicological evidence has elucidated the contributions of environmental chemical contaminants to the incidence and development of chronic diseases that have a negative impact on quality of life or may be life-threatening. However, the pathways of exposure to these chemicals and their involvements in chronic diseases remain unclear. We comprehensively reviewed the research progress on the exposure risks of humans to environmental contaminants, their body burden as indicated by blood monitoring, and the correlation of blood chemical contaminants with chronic diseases. After entering the human body through various routes of exposure, environmental contaminants are transported to target organs through blood circulation . The application of the modern analytical techniques based on human plasma or serum specimens is promising for determining the body burden of environmental contaminants, including legacy persistent organic pollutants , emerging pollutants, and inorganic elements. Furthermore, their body burden, as indicated by blood monitoring correlates with the incidence and development of metabolic syndromes, cancers, chronic nervous system diseases, cardiovascular diseases, and reproductive disorders. On this basis, we highlight the urgent need for further research on environmental pollution causing health problems in humans.

Abstract:The chemical burden on the environment and human population is increasing. Consequently, regulatory risk assessment must keep pace to manage, reduce, and prevent adverse impacts on human and environmental health associated with hazardous chemicals. Surveillance of chemicals of known, emerging, or potential future concern, entering the environment-food-human continuum is needed to document the reality of risks posed by chemicals on ecosystem and human health from a one health perspective, feed into early warning systems and support public policies for exposure mitigation provisions and safe and sustainable by design strategies. The use of less-conventional sampling strategies and integration of full-scan, high-resolution mass spectrometry and effect-directed analysis in environmental and human monitoring programmes have the potential to enhance the screening and identification of a wider range of chemicals of known, emerging or potential future concern. Here, we outline the key needs and recommendations identified within the European Partnership for Assessment of Risks from Chemicals (PARC) project for leveraging these innovative methodologies to support the development of next-generation chemical risk assessment.

Abstract:Chinese children are exposed to broad environmental risks ranging from well-known hazards, such as pesticides and heavy metals, to emerging threats including many new man-made chemicals. Although anecdotal evidence suggests that the exposure levels in Chinese children are substantially higher than those of children in developed countries, a systematic assessment is lacking. Further, while these exposures have been linked to a variety of childhood diseases, such as respiratory, endocrine, neurological, behavioral, and malignant disorders, the magnitude of the associations is often unclear. This review provides a current epidemiologic overview of commonly reported environmental contaminants and their potential impact on children's health in China. We found that despite a large volume of studies on various topics, there is a need for more high-quality research and better-coordinated regional and national data collection. Moreover, prevention of such diseases will depend not only on training of environmental health professionals and enhanced research programs, but also on public education, legislation, and networking.

Abstract:Abstract The global epidemiological landscape has undergone a significant transformation, witnessing a shift towards non-communicable diseases (NCDs) as the predominant causes of mortality and morbidity, displacing infectious diseases. This transition is evident both in Africa and worldwide, arising from a multifaceted interplay of genetic and environmental factors. Notably, exposure to a diverse array of environmental hazards (encompassing contaminated water sources, air pollution, pesticides, agrochemicals, heavy metals, and industrial chemicals), contributes substantially to the escalating incidence of disease. Of particular concern within this milieu is the presence of endocrine disrupting chemicals (EDCs), which disrupt hormonal function and exacerbate susceptibility to diseases across various life stages. EDCs pervade ecosystems through various pathways, including contamination of the food chain and leaching from food contact materials, underscoring the imperative to understand their collective impact on human, animal, and environmental health. Particularly pertinent are contemporary insecticides, notably those employed in malaria vector control, which exhibit endocrine disrupting properties. Despite awareness of their adverse effects on endocrine function and persistence in the environment, certain nations, such as South Africa, continue to rely on insecticides like DDT (1,1,1-trichloro-2,2-bis(chlorodiphenyl)ethane) for indoor residual spraying (IRS). Our objective is to synthesize the findings from several of our studies examining the effects of pesticides on human health. These studies evaluate outcomes such as semen parameters, sperm DNA damage, and epigenetic changes. Additionally, we investigate the impact of exposure to other environmental EDCs on reproductive health in animals. Exposures to insecticides, primarily occurring through low-level environmental or occupational contact, are associated with adverse male reproductive health due to their EDC activity. Our research specifically delves into the endocrine disrupting properties of insecticides used in malaria vector control in Africa. These compounds accumulating in the environment and posing risks to both wildlife and human fertility and health. Our investigations unveil a troubling trend of sustained DDT exposure over a 13-year period, despite a decline in exposure levels. This persistent exposure adversely impacts seminal parameters and chromatin integrity, suggesting potential reproductive health implications, particularly in malaria-prone areas, where synergistic and additive effects with known EDCs may exacerbate the situation. Furthermore, new findings confirmed paternal exposure to DDT correlates with adverse sperm health, potentially affecting offspring health through epigenetic modifications linked to fertility, neurodevelopment, and hormone regulation. Analyses of fresh produce from South African markets highlight the presence of multiple pesticide residues, some of which exhibit estrogenic activity, potentially contributing to adverse health effects. Similar concerns arise from assessments of bottled water and food contact materials, which contain compounds like phthalates, bisphenol A (BPA), and nonylphenol, associated with human health risks. These findings underscore the need for comprehensive health risk assessments of complex EDC mixtures within the food chain, considering potential additive and synergistic effects of multiple contaminants, including pesticides, agrochemicals, industrial chemicals, personal care products, and emerging environmental contaminants. Exposure can occur in various settings, whether it's in the workplace or at home, highlighting the importance of educational initiatives and the critical need to mitigate such exposure.

Abstract:Technological, agricultural, and medical advances have improved the lifestyle of humankind. However, these advances have caused new problems that affect the environment and future generations. Emerging contaminants display properties such as low degradation potential and environmental persistence. In addition, most contaminants are lipophilic, which culminates in high bioaccumulation. The disposal of pharmaceuticals and personal care products into the environment underlies microbial and bacterial resistance. Plasticizers change several characteristics of industrialized materials, such as flexibility, but they are potentially carcinogenic and disrupt the endocrine system. Pesticides prevent the propagation of numerous kinds of pests; nevertheless, they exert neurotoxic and mutagenic effects, and they impact the environment negatively. Addition of flame retardants to a number of materials prevents flame propagation; however, after their release into the environment, these chemicals may bioaccumulate in organisms and disrupt the endocrine system, too. Surfactants can change the surface and interfacial properties of liquids, but their presence in the environment can interfere with countless enzymes and can even impair the endocrine system of various organisms and induce the feminization of species. Hence, gaining knowledge about emerging contaminants is increasingly important to minimize future damage and enable proper monitoring of each class of compounds in the environment which will help to improve legislation on this matter.

Abstract:Humans are exposed to a large number of environmental chemicals: Some of these may be toxic, and many others have unknown or poorly characterized health effects. There is intense interest in determining the impact of exposure to environmental chemical mixtures on human health. As the study of mixtures continues to evolve in the field of environmental epidemiology, it is imperative that we understand the methodologic challenges of this research and the types of questions we can address using epidemiological data. In this article, we summarize some of the unique challenges in exposure assessment, statistical methods, and methodology that epidemiologists face in addressing chemical mixtures. We propose three broad questions that epidemiological studies can address: a) What are the potential health impacts of individual chemical agents? b) What is the interaction among agents? And c) what are the health effects of cumulative exposure to multiple agents? As the field of mixtures research grows, we can use these three questions as a basis for defining our research questions and for developing methods that will help us better understand the effect of chemical exposures on human disease and well-being.

Abstract:Daily use of chemicals is an essential part of modern life. Endocrine-disrupting chemicals (EDCs) are a heterogeneous group of exogenous chemicals or chemical mixtures that interfere with the action of hormones and consequently cause adverse effects to humans and wildlife. The number of EDCs has markedly increased over the past 60 years. Humans are constantly exposed to hundreds of EDCs mainly through air, water, and food. Exposure to EDCs (in utero or lifetime) may be a significant component of the environmental origin of several medical conditions. The developing fetus and neonate are more sensitive than adults to perturbation by EDCs. The prenatal damage can cause adverse consequences later in life (developmental origins of adult disease). In many cases, the damage is irreversible. There is also a possibility of transgenerational effects. By interfering with hormonal functions, EDCs can contribute to a variety of dysfunctions and diseases including obesity, diabetes, reproductive disorders, and cancers. Information on long-term effects of chronic, low-dose exposure to EDCs is relatively limited. EDCs represent a global threat for human health and cause a high cost for the society. Promoting public knowledge and initiating preventive measures will help minimizing the health and economic consequences of EDCs for future generations.
Exposure information in environmental health research: Current opportunities and future directions for particulate matter, ozone, and toxic air pollutants

Abstract:Understanding and quantifying outdoor and indoor sources of human exposure are essential but often not adequately addressed in health effect studies for air pollution. Air pollution epidemiology, risk assessment, health tracking, and accountability assessments are examples of health effect studies that require but often lack adequate exposure information. Recent advances in exposure modeling along with better information on time–activity and exposure factor data provide us with unique opportunities to improve the assignment of exposures for both future and ongoing studies linking air pollution to health impacts. In September 2006, scientists from the US Environmental Protection Agency and the Centers for Disease Control and Prevention along with scientists from the academic community and state health departments convened a symposium on air pollution exposure and health to identify, evaluate, and improve current approaches for linking air pollution exposures to disease. This manuscript presents the key issues, challenges, and recommendations identified by the exposure working group, who used case studies of particulate matter, ozone, and toxic air-pollutant exposure to evaluate health effects for air pollution. One of the overarching lessons of this workshop is that obtaining better exposure information for these different health effect studies requires both goal setting for what is needed and mapping out the transition pathway from current capabilities for meeting these goals. Meeting our long-term goals requires definition of incremental steps that provide useful information for the interim and move us toward our long-term goals. Another overarching theme among the three different pollutants and the different health study approaches is the need for integration among alternate exposure-assessment approaches. For example, different groups may advocate exposure indicators, biomonitoring, mapping methods (GIS), modeling, environmental media monitoring, and/or personal exposure modeling. However, emerging research reveals that the greatest progress comes from integration among two or more of these efforts.

Environment and Health: Endocrine-Disrupting Chemicals

Abstract:Endocrine-disrupting chemicals (EDCs) increasingly have been a subject of concern and study in the past few decades. These chemicals can interfere directly or indirectly with normal physiology of endocrine system organs or organs under the influence of hormones. EDCs have been shown to cause a range of adverse effects, such as developmental abnormalities, abnormal growth patterns in children, reproductive abnormalities, hormone-sensitive cancers, and alterations in immune function. Some have been shown to contribute to obesity. Most EDCs are synthetically developed compounds that are ubiquitous in food packaging, consumer products, and the environment. They are found in pesticides, herbicides, plastics, solvents, flame retardants, and in the environment as industrial byproducts and waste. These substances are poorly regulated and exposures are not tracked. It is virtually impossible to avoid contact with EDCs in everyday life. Populations at greatest risk of adverse health effects include fetuses, newborns, and pregnant individuals. Comprehensive preconception and prenatal care can help family members and caregivers identify sources of and minimize EDC exposure in newborns and infants. For individuals, the clinical significance of these exposures is unknown and there is no current role for testing. For patients with possible exposure, a pertinent history should be taken and counseling provided to help minimize exposure.

Abstract:There is an ongoing probing of the role of chemicals in the indoor environment. The majority of potential target substances are so-called very volatile, volatile, and semi-volatile organic compounds (VVOCs, VOCs, and SVOCs). Depending on their physical properties and the mass transfer conditions, they are distributed in or between the gas phase, particle phase, settled house dust, surface films, clothing, and other fabrics as well as the exposed skin and hair of the occupants themselves. Therefore, inhalation, ingestion, and dermal uptake all must be considered as relevant pathways for exposure assessment in human habitats. Exposure to VVOCs, VOCs, and SVOCs can be estimated by measuring their concentrations in relevant indoor compartments or by determining the amounts of the target compounds and/or their metabolites in urine and blood. Assessing the various routes of exposure often requires a combination of sophisticated and interdisciplinary theoretical background and experimental techniques. Consequently, close communication and collaboration between chemical and exposure scientists are needed to achieve a better understanding of human exposure to chemical substances in various indoor environments. Embedded in the toxicological context, this is the basis for assessing the corresponding health risks and for determining control strategies or approaches to limit such risks.
