Abstract:Abstract Objective: To establish a comprehensive and accurate assessment model of periodontal alveolar bone loss based on panoramic images.Methods:A total of 640 panoramic images were included, and 3 experienced periodontal physicians marked the key points needed to calculate the degree of periodontal alveolar bone loss and the specific location and shape of the alveolar bone loss. A deep learning architecture based on UNet and YOLO-v4 was proposed to localize the tooth and key points, and the percentage and stageof periodontal alveolar bone loss were accurately calculated. The ability of the model to recognize these features was evaluated and compared with that of general dental practitioners.Results: The overall classification accuracy of the model was 0.77, and the performance of the model varied for different tooth positions and categories;model classification was generally more accurate than that of general practitioners.Conclusion: It is feasible to establish deep learning model forassessmentand staging radiographicperiodontal alveolar bone loss using two-stage architecture based on UNet and YOLO-v4.

Abstract:OBJECTIVES Periapical periodontitis and caries are common chronic oral diseases affecting most teenagers and adults worldwide. The purpose of this study was to develop an evaluation tool to automatically detect dental caries and periapical periodontitis on periapical radiographs using deep learning. METHODS A modified deep learning model was developed using a large dataset (4,129 images) with high-quality annotations to support the automatic detection of both dental caries and periapical periodontitis. The performance of the model was compared to the classification performance of dentists. RESULTS The deep learning model automatically distinguished dental caries with an F1-score of 0.829 and periapical periodontitis with an F1-score of 0.828. The comparison of model-only and expert-only detection performance showed that the accuracy of the fully automatic method was significantly higher than that of the junior dentists. With deep learning assistance, the experts not only reached a higher diagnostic accuracy with an average F1-score of 0.7844 for dental caries and 0.8208 for periapical periodontitis compared to expert-only scenarios but also increased interobserver agreement from 0.585/0.590 to 0.726/0.713 for dental caries and from 0.623/0.563 to 0.752/0.740 for periapical periodontitis. CONCLUSIONS Based on the experimental results, deep learning can improve the accuracy and consistency of evaluating dental caries and periapical periodontitis on periapical radiographs. CLINICAL SIGNIFICANCE Deep learning models can improve accuracy and consistency and reduce the workload of dentists, making AI a powerful tool for clinical practice.

Predicting case difficulty in endodontic microsurgery using machine learning algorithms

Abstract:Objectives: The study aimed to develop and validate machine learning models for case difficulty prediction in endodontic microsurgery, assisting clinicians in preoperative analysis. Methods: The cone-beam computed tomographic images were collected from 261 patients with 341 teeth and used for radiographic examination and measurement. Through linear regression (LR), support vector regression (SVR), and extreme gradient boosting (XGBoost) algorithms, four models were established according to different loss functions, including the L1-loss LR model, L2-loss LR model, SVR model and XGBoost model. Five-fold cross-validation was applied in model training and validation. Explained variance score (EVS), coefficient of determination (R2), mean absolute error (MAE), mean squared error (MSE) and median absolute error (MedAE) were calculated to evaluate the prediction performance. Results: The MAE, MSE and MedAE values of the XGBoost model were the lowest, which were 0.1010, 0.0391 and 0.0235, respectively. The EVS and R2 values of the XGBoost model were the highest, which were 0.7885 and 0.7967, respectively. The factors used to predict the case difficulty in endodontic microsurgery were ordered according to their relative importance, including lesion size, the distance between apex and adjacent important anatomical structures, root filling density, root apex diameter, root resorption, tooth type, tooth length, root filling length, root canal curvature and the number of root canals. Conclusions: The XGBoost model outperformed the LR and SVR models on all evaluation metrics, which can assist clinicians in preoperative analysis. The relative feature importance provides a reference to develop the scoring system for case difficulty assessment in endodontic microsurgery. Clinical significance: Preoperative case assessment is a crucial step to identify potential risks and make referral decisions. Machine learning models for case difficulty prediction in endodontic microsurgery can assist clinicians in preoperative analysis efficiently and accurately.

Abstract:Objectives: Detecting caries lesions is challenging for dentists, and deep learning models may help practitioners to increase accuracy and reliability. We aimed to systematically review deep learning studies on caries detection. Data: We selected diagnostic accuracy studies that used deep learning models on dental imagery (including radiographs, photographs, optical coherence tomography images, near-infrared light transillumination images). The latest version of the quality assessment tool for diagnostic accuracy studies (QUADAS-2) tool was used for risk of bias assessment. Meta-analysis was not performed due to heterogeneity in the studies methods and their performance measurements. Sources: Databases (Medline via PubMed, Google Scholar, Scopus, Embase) and a repository (ArXiv) were screened for publications published after 2010, without any limitation on language. Study selection: From 252 potentially eligible references, 48 studies were assessed full-text and 42 included, using classification (n = 26), object detection (n = 6), or segmentation models (n = 10). A wide range of performance metrics was used; image, object or pixel accuracy ranged between 68%-99%. The minority of studies (n = 11) showed a low risk of biases in all domains, and 13 studies (31.0%) low risk for concerns regarding applicability. The accuracy of caries classification models varied, i.e. 71% to 96% on intra-oral photographs, 82% to 99.2% on peri-apical radiographs, 87.6% to 95.4% on bitewing radiographs, 68.0% to 78.0% on near-infrared transillumination images, 88.7% to 95.2% on optical coherence tomography images, and 86.1% to 96.1% on panoramic radiographs. Pooled diagnostic odds ratios varied from 2.27 to 32,767. For detection and segmentation models, heterogeneity in reporting did not allow useful pooling. Conclusion: An increasing number of studies investigated caries detection using deep learning, with a diverse types of architectures being employed. Reported accuracy seems promising, while study and reporting quality are currently low. Clinical significance: Deep learning models can be considered as an assistant for decisions regarding the presence or absence of carious lesions.
Dental Caries Detection and Classification in CBCT Images Using Deep Learning

Abstract:Objectives: This study aimed to investigate the accuracy of deep learning algorithms to diagnose tooth caries and classify the extension and location of dental caries in cone beam computed tomography (CBCT) images. To the best of our knowledge, this is the first study to evaluate the application of deep learning for dental caries in CBCT images. Methods: The CBCT image dataset comprised 382 molar teeth with caries and 403 noncarious molar cases. The dataset was divided into a development set for training and validation and test set. Three images were obtained for each case, including axial, sagittal, and coronal. The test dataset was provided to a multiple-input convolutional neural network (CNN). The network made predictions regarding the presence or absence of dental decay and classified the lesions according to their depths and types for the provided samples. Accuracy, sensitivity, specificity, and F1 score values were measured for dental caries detection and classification. Results: The diagnostic accuracy, sensitivity, specificity, and F1 score for caries detection in carious molar teeth were 95.3%, 92.1%, 96.3%, and 93.2%, respectively, and for noncarious molar teeth were 94.8%, 94.3%, 95.8%, and 94.6%. The CNN network showed high sensitivity, specificity, and accuracy in classifying caries extensions and locations. Conclusions: This research demonstrates that deep learning models can accurately identify dental caries and classify their depths and types with high accuracy, sensitivity, and specificity. The successful application of deep learning in this field will undoubtedly assist dental practitioners and patients in improving diagnostic and treatment planning in dentistry. Clinical significance: This study showed that deep learning can accurately detect and classify dental caries. Deep learning can provide dental caries detection accurately. Considering the shortage of dentists in certain areas, using CNNs can lead to broader geographic coverage in detecting dental caries.

Abstract:ABSTRACT Background: Dentists begin the diagnosis by identifying and enumerating teeth. Panoramic radiographs are widely used for tooth identification due to their large field of view and low exposure dose. The automatic numbering of teeth in panoramic radiographs can assist clinicians in avoiding errors. Deep learning has emerged as a promising tool for automating tasks. Our goal is to evaluate the accuracy of a two-step deep learning method for tooth identification and enumeration in panoramic radiographs. Materials and Methods: In this retrospective observational study, 1007 panoramic radiographs were labeled by three experienced dentists. It involved drawing bounding boxes in two distinct ways: one for teeth and one for quadrants. All images were preprocessed using the contrast-limited adaptive histogram equalization method. First, panoramic images were allocated to a quadrant detection model, and the outputs of this model were provided to the tooth numbering models. A faster region-based convolutional neural network model was used in each step. Results: Average precision (AP) was calculated in different intersection-over-union thresholds. The AP50 of quadrant detection and tooth enumeration was 100% and 95%, respectively. Conclusion: We have obtained promising results with a high level of AP using our two-step deep learning framework for automatic tooth enumeration on panoramic radiographs. Further research should be conducted on diverse datasets and real-life situations.
Deep learning for tooth identification and numbering on dental radiography: a systematic review and meta-analysis

Abstract:Objectives: Improved tools based on deep learning can be used to accurately number and identify teeth. This study aims to review the use of deep learning in tooth numbering and identification. Methods: An electronic search was performed through October 2023 on PubMed, Scopus, Cochrane, Google Scholar, IEEE, arXiv, and medRxiv. Studies that used deep learning models with segmentation, object detection, or classification tasks for teeth identification and numbering of human dental radiographs were included. For risk of bias assessment, included studies were critically analysed using quality assessment of diagnostic accuracy studies (QUADAS-2). To generate plots for meta-analysis, MetaDiSc and STATA 17 (StataCorp LP, College Station, TX, USA) were used. Pooled outcome diagnostic odds ratios (DORs) were determined through calculation. Results: The initial search yielded 1618 studies, of which 29 were eligible based on the inclusion criteria. Five studies were found to have low bias across all domains of the QUADAS-2 tool. Deep learning has been reported to have an accuracy range of 81.8%-99% in tooth identification and numbering and a precision range of 84.5%-99.94%. Furthermore, sensitivity was reported as 82.7%-98% and F1-scores ranged from 87% to 98%. Sensitivity was 75.5%-98% and specificity was 79.9%-99%. Only 6 studies found the deep learning model to be less than 90% accurate. The average DOR of the pooled data set was 1612, the sensitivity was 89%, the specificity was 99%, and the area under the curve was 96%. Conclusion: Deep learning models successfully can detect, identify, and number teeth on dental radiographs. Deep learning-powered tooth numbering systems can enhance complex automated processes, such as accurately reporting which teeth have caries, thus aiding clinicians in making informed decisions during clinical practice.

Abstract:Periapical radiolucencies, which can be detected on panoramic radiographs, are one of the most common radiographic findings in dentistry and have a differential diagnosis including infections, granuloma, cysts and tumors. In this study, we seek to investigate the ability with which 24 oral and maxillofacial (OMF) surgeons assess the presence of periapical lucencies on panoramic radiographs, and we compare these findings to the performance of a predictive deep learning algorithm that we have developed using a curated data set of 2902 de-identified panoramic radiographs. The mean diagnostic positive predictive value (PPV) of OMF surgeons based on their assessment of panoramic radiographic images was 0.69 (±0.13), indicating that dentists on average falsely diagnose 31% of cases as radiolucencies. However, the mean diagnostic true positive rate (TPR) was 0.51 (±0.14), indicating that on average 49% of all radiolucencies were missed. We demonstrate that the deep learning algorithm achieves a better performance than 14 of 24 OMF surgeons within the cohort, exhibiting an average precision of 0.60 (±0.04), and an F1 score of 0.58 (±0.04) corresponding to a PPV of 0.67 (±0.05) and TPR of 0.51 (±0.05). The algorithm, trained on limited data and evaluated on clinically validated ground truth, has potential to assist OMF surgeons in detecting periapical lucencies on panoramic radiographs.

Abstract:Periapical radiographs are routinely used in dental practice for diagnosis and treatment planning purposes. However, they often suffer from artifacts, distortions, and superimpositions, which can lead to potential misinterpretations. Thus, an automated detection system is required to overcome these challenges. Artificial intelligence (AI) has been revolutionizing various fields, including medicine and dentistry, by facilitating the development of intelligent systems that can aid in performing complex tasks such as diagnosis and treatment planning. The purpose of the present study was to verify the diagnostic performance of an AI system for the automatic detection of teeth, caries, implants, restorations, and fixed prosthesis on periapical radiographs. A dataset comprising 1000 periapical radiographs collected from 500 adult patients was analyzed by an AI system and compared with annotations provided by two oral and maxillofacial radiologists. A strong correlation (R > 0.5) was observed between AI perception and observers 1 and 2 in carious teeth (0.7-0.73), implants (0.97-0.98), restored teeth (0.85-0.89), teeth with fixed prosthesis (0.92-0.94), and missing teeth (0.82-0.85). The automatic detection by the AI system was comparable to the oral radiologists and may be useful for automatic identification in periapical radiographs.

Abstract:Background: The detection and early diagnosis of root fractures can be challenging; this difficulty applies particularly to newly qualified dentists. Aside from clinical examination, diagnosis often requires radiographic assessment. Nonetheless, human fallibility can introduce errors due to a lack of experience. Aim: The proposed system aimed to assist in detecting root fractures through the integration of artificial intelligence techniques into the diagnosis process as a step for automating dental diagnosis and decision-making processes. Materials and method: A total of 400 radiographic images of fractured and unfractured teeth were obtained for the present research. Data handling techniques were implemented to balance the distribution of the samples. The AI-based system used the voting technique for five different pretrained models namely, VGG16, VGG19, ResNet50. DenseNet121, and DenseNet169 to perform the analysis. The parameters used for the analysis of the models are loss and accuracy curves. Results: VGG16 exhibited notable success with low training and validation losses (0.09% and 0.18%, respectively), high specificity, sensitivity, and positive predictive value (PPV). VGG19 showed potential overfitting concerns, while ResNet50 displayed progress in minimizing loss but exhibited bias toward unfractured cases. DenseNet121 effectively addressed overfitting and noise issues, achieving balanced metrics and impressive PPVs for both fractured and unfractured cases (0.933 and 0.898 respectively). With increased depth, DenseNet169 demonstrated enhanced generalization capability. Conclusion: The proposed AI- based system demonstrated high precision and sensitivity for detecting root fractures in endodontically treated teeth by utilizing the voting method.
Classification of mandibular molar furcation involvement in periapical radiographs by deep learning

Abstract:Purpose: The purpose of this study was to classify mandibular molar furcation involvement (FI) in periapical radiographs using a deep learning algorithm. Materials and methods: Full mouth series taken at East Carolina University School of Dental Medicine from 2011-2023 were screened. Diagnostic-quality mandibular premolar and molar periapical radiographs with healthy or FI mandibular molars were included. The radiographs were cropped into individual molar images, annotated as " healthy" or " FI," and divided into training, validation, and testing datasets. The images were preprocessed by PyTorch transformations. ResNet-18, a convolutional neural network model, was refined using the PyTorch deep learning framework for the specific imaging classification task. CrossEntropyLoss and the AdamW optimizer were employed for loss function training and optimizing the learning rate, respectively. The images were loaded by PyTorch DataLoader for efficiency. The performance of ResNet-18 algorithm was evaluated with multiple metrics, including training and validation losses, confusion matrix, accuracy, sensitivity, specificity, the receiver operating characteristic (ROC) curve, and the area under the ROC curve. Results: After adequate training, ResNet-18 classified healthy vs. FI molars in the testing set with an accuracy of 96.47%, indicating its suitability for image classification. Conclusion: The deep learning algorithm developed in this study was shown to be promising for classifying mandibular molar FI. It could serve as a valuable supplemental tool for detecting and managing periodontal diseases.

Abstract:Objectives: Several factors such as unavailability of specialists, dental phobia, and financial difficulties may lead to a delay between receiving an oral radiology report and consulting a dentist. The primary aim of this study was to distinguish between high-risk and low-risk oral lesions according to the radiologist's reports of cone beam computed tomography (CBCT) images. Such a facility may be employed by dentist or his/her assistant to make the patient aware of the severity and the grade of the oral lesion and referral for immediate treatment or other follow-up care. Methods: A total number of 1134 CBCT radiography reports owned by Shiraz University of Medical Sciences were collected. The severity level of each sample was specified by three experts, and an annotation was carried out accordingly. After preprocessing the data, a deep learning model, referred to as CNN-LSTM, was developed, which aims to detect the degree of severity of the problem based on analysis of the radiologist's report. Unlike traditional models which usually use a simple collection of words, the proposed deep model uses words embedded in dense vector representations, which empowers it to effectively capture semantic similarities. Results: The results indicated that the proposed model outperformed its counterparts in terms of precision, recall, and F1 criteria. This suggests its potential as a reliable tool for early estimation of the severity of oral lesions. Conclusions: This study shows the effectiveness of deep learning in the analysis of textual reports and accurately distinguishing between high-risk and low-risk lesions. Employing the proposed model which can Provide timely warnings about the need for follow-up and prompt treatment can shield the patient from the risks associated with delays. Clinical significance: Our collaboratively collected and expert-annotated dataset serves as a valuable resource for exploratory research. The results demonstrate the pivotal role of our deep learning model could play in assessing the severity of oral lesions in dental reports.

Detection of the separated endodontic instrument on periapical radiographs using a deep learning-based convolutional neural network algorithm

Abstract:The study evaluated the diagnostic performance of an artificial intelligence system to detect separated endodontic instruments on periapical radiograph radiographs. Three hundred seven periapical radiographs were collected and divided into 222 for training and 85 for testing to be fed to the Mask R-CNN model. Periapical radiographs were assigned to the training and test set and labelled on the DentiAssist labeling platform. Labelled polygonal objects had their bounding boxes automatically generated by the DentiAssist system. Fractured instruments were classified and segmented. As a result of the proposed method, the mean average precision (mAP) metric was 98.809%, the precision value was 95.238, while the recall reached 98.765 and the f1 score 96.969%. The threshold value of 80% was chosen for the bounding boxes working with the Intersection over Union (IoU) technique. The Mask R-CNN distinguished separated endodontic instruments on periapical radiographs.

Abstract:Background/purpose Artificial Intelligence (AI) can optimize treatment approaches in dental healthcare due to its high level of accuracy and wide range of applications. This study seeks to propose a new deep learning (DL) ensemble model based on deep Convolutional Neural Network (CNN) algorithms to predict tooth position, detect shape, detect remaining interproximal bone level, and detect radiographic bone loss (RBL) using periapical and bitewing radiographs. Materials and methods 270 patients from January 2015 to December 2020, and all images were deidentified without private information for this study. A total of 8000 periapical radiographs with 27,964 teeth were included for our model. AI algorithms utilizing the YOLOv5 model and VIA labeling platform, including VGG-16 and U-Net architecture, were created as a novel ensemble model. Results of AI analysis were compared with clinicians' assessments. Results DL-trained ensemble model accuracy was approximately 90% for periapical radiographs. Accuracy for tooth position detection was 88.8%, tooth shape detection 86.3%, periodontal bone level detection 92.61% and radiographic bone loss detection 97.0%. AI models were superior to mean accuracy values from 76% to 78% when detection was performed by dentists. Conclusion The proposed DL-trained ensemble model provides a critical cornerstone for radiographic detection and a valuable adjunct to periodontal diagnosis. High accuracy and reliability indicate model's strong potential to enhance clinical professional performance and build more efficient dental health services.

Abstract:ObjectivesThe aim of this study was to develop an assessment tool for automatic detection of dental caries in periapical radiographs using convolutional neural network (CNN) architecture.MethodsA novel diagnostic model named ResNet + SAM was established using numerous periapical radiographs (4278 images) annotated by medical experts to automatically detect dental caries. The performance of the model was compared to the traditional CNNs (VGG19, ResNet-50), and the dentists. The Gradient-weighted Class Activation Mapping (Grad-CAM) technique shows the region of interest in the image for the CNNs.ResultsResNet + SAM demonstrated significantly improved performance compared to the modified ResNet-50 model, with an average F1 score of 0.886 (95% CI 0.855–0.918), accuracy of 0.885 (95% CI 0.862–0.901) and AUC of 0.954 (95% CI 0.924–0.980). The comparison between the performance of the model and the dentists revealed that the model achieved higher accuracy than that of the junior dentists. With the assist of the tool, the dentists achieved superior metrics with a mean F1 score of 0.827 and the interobserver agreement for dental caries is enhanced from 0.592/0.610 to 0.706/0.723.ConclusionsAccording to the results obtained from the experiments, the automatic assessment tool using the ResNet + SAM model shows remarkable performance and has excellent possibilities in identifying dental caries. The use of the assessment tool in clinical practice can be of great benefit as a clinical decision-making support in dentistry and reduce the workload of dentists.

Statistical validation of a deep learning algorithm for dental anomaly detection in intraoral radiographs using paired data

Abstract:This article describes the clinical validation study setup, statistical analysis and results for a deep learning algorithm which detects dental anomalies in intraoral radiographic images, more specifically caries, apical lesions, root canal treatment defects, marginal defects at crown restorations, periodontal bone loss and calculus. The study compares the detection performance of dentists using the deep learning algorithm to the prior performance of these dentists evaluating the images without algorithmic assistance. Calculating the marginal profit and loss of performance from the annotated paired image data allows for a quantification of the hypothesized change in sensitivity and specificity. The statistical significance of these results is extensively proven using both McNemar's test and the binomial hypothesis test. The average sensitivity increases from $60.7\%$ to $85.9\%$, while the average specificity slightly decreases from $94.5\%$ to $92.7\%$. We prove that the increase of the area under the localization ROC curve (AUC) is significant (from $0.60$ to $0.86$ on average), while the average AUC is bounded by the $95\%$ confidence intervals ${[}0.54, 0.65{]}$ and ${[}0.82, 0.90{]}$. When using the deep learning algorithm for diagnostic guidance, the dentist can be $95\%$ confident that the average true population sensitivity is bounded by the range $79.6\%$ to $91.9\%$. The proposed paired data setup and statistical analysis can be used as a blueprint to thoroughly test the effect of a modality change, like a deep learning based detection and/or segmentation, on radiographic images.

Abstract:AIM: The goal was to use a deep convolutional neural network to measure the radiographic alveolar bone level to aid periodontal diagnosis.MATERIALS AND METHODS: A deep learning (DL) model was developed by integrating three segmentation networks (bone area, tooth, cemento-enamel junction) and image analysis to measure the radiographic bone level and assign radiographic bone loss (RBL) stages. The percentage of RBL was calculated to determine the stage of RBL for each tooth. A provisional periodontal diagnosis was assigned using the 2018 periodontitis classification. RBL percentage, staging, and presumptive diagnosis were compared with the measurements and diagnoses made by the independent examiners.RESULTS: The average Dice Similarity Coefficient (DSC) for segmentation was over 0.91. There was no significant difference in the RBL percentage measurements determined by DL and examiners ( <math><mi>p</mi><mo>=</mo><mn>.65</mn></math> ). The area under the receiver operating characteristics curve of RBL stage assignment for stages I, II, and III was 0.89, 0.90, and 0.90, respectively. The accuracy of the case diagnosis was 0.85.CONCLUSIONS: The proposed DL model provides reliable RBL measurements and image-based periodontal diagnosis using periapical radiographic images. However, this model has to be further optimized and validated by a larger number of images to facilitate its application.

Abstract:Background Dental caries is one of the most prevalent conditions in dentistry worldwide. Early identification and classification of dental caries are essential for effective prevention and treatment. Panoramic dental radiographs are commonly used to screen for overall oral health, including dental caries and tooth anomalies. However, manual interpretation of these radiographs can be time-consuming and prone to human error. Therefore, an automated classification system could help streamline diagnostic workflows and provide timely insights for clinicians. Methods This article presents a deep learning-based, custom-built model for the binary classification of panoramic dental radiographs. The use of histogram equalization and filtering methods as preprocessing techniques effectively addresses issues related to irregular illumination and contrast in dental radiographs, enhancing overall image quality. By incorporating three separate panoramic dental radiograph datasets, the model benefits from a diverse dataset that improves its training and evaluation process across a wide range of caries and abnormalities. Results The dental radiograph analysis model is designed for binary classification to detect the presence of dental caries, restorations, and periapical region abnormalities, achieving accuracies of 97.01%, 81.63%, and 77.53%, respectively. Conclusions The proposed algorithm extracts discriminative features from dental radiographs, detecting subtle patterns indicative of tooth caries, restorations, and region-based abnormalities. Automating this classification could assist dentists in the early detection of caries and anomalies, aid in treatment planning, and enhance the monitoring of dental diseases, ultimately improving and promoting patients' oral healthcare.
